RESUMO

The authors compare the results of valgisation tibial osteotomies in osteoarthrosis of the knee (146 cases) and osteoarthrosis of the knee with chondrocalcinosis (94 cases). The two groups were identical in terms of age, but differed by sex ratio (female predominance in CCA) and the initial radiological stage (more advanced forms in the CCA group). Results differed after the second year and reached significantly different values at 5 years (73% in knee osteoarthrosis as against 34% good clinical results in chondrocalcinosis). Study of two groups of patients paired for age, sex, weight and radiological stage showed that results at 5 years were significantly less good in the CCA group. These results are discussed and compared with those in the 1990 literature.

The ten published cases of septic arthritis due to Campylobacter fetus and two more recent cases, one of which was a delayed septic complication of a total hip replacement, are discussed. In 83% of cases, febrile monoarthritis occurred which in 7 out of the 12 cases involved an already diseased joint in a subject over 70 years of age (half of the patients), who was frequently alcoholic and presented either cancer or hepatic cirrhosis and/or diabetes. Hyperleukocytosis of the blood was variable (55% of cases). The joint fluid, which contained a high level of deteriorated polynuclear cells, was routinely cultured on an enriched medium in an anaerobic or microerophilic atmosphere and in 10 out of 12 cases Campylobacter fetus was isolated within 15 days, even in the absence of previous antibiotic therapy. The arthritis, which showed little sign radiographically of destructiveness, was cured, leaving no sequellae in most cases. Treatment usually involved ampicillin or the tetracyclines, either alone or in association with the aminoglycosides.

RESUMO

The authors describe 5 cases of bone sarcoma appearing between 9 and 29 years after radiation therapy. One patient was still surviving 21 years after above knee amputation, two died rapidly and two were lost to follow up. It is concluded that the incidence of post-radiation sarcoma is low but also that non-malignant tumours such as giant-cell tumours should never be treated by more than 1500 rads.

RESUMO

The authors have observed 9 cases of benign chondroblastoma of bones. After a review of the literature they recall that this is a rare condition (1% of bone tumors). That it is twice more frequent in males than in females. Aged 10-20, at the level of growing epiphysis shoulder and knee are the more common sites. The X-rays pictures are described, as well as the pathological findings. Pathological features close to those noted in aneurismal bone cysts were often encountered. The ultra-structure of these tumours was also studied. This type of tumour is essentially benign, however the association of aneurism-like pictures increase the incidence of recurrences.

RESUMO

Bone formation by the sarcoma is exclusive, without any cartilaginous component; the pattern of the tumour being sometimes quite similar to dysplasia lesional tissue. It seems to be a tissular transformation rather than an unicellular sarcomatous change. Among the etiological factors, except the part of the radiotherapy, periostal changes enhanced by a surgical procedure with acrylic cement in the femoral shaft are perhaps contributing to the development of a sarcoma whose pattern is quite particular.

Concerning these six cases, the authors recall the rarity of osseous localization of Hydatidiform disease, isolated as a rule, arising in the adult and preferentially involving the ilium, spinal column, femur, humerus and scapula. Being slowly progressive, the condition is revealed by pain, swelling and above all spontaneous fractures (long bones), or signs of root or medullary compression (spinal column), followed sometimes by fistula formation. Radiological diagnosis is often difficult with an osteolytic appearance without appreciable reactional osteogenesis. As for laboratory investigations, they are only of value in orientation; immunofluorescence being the weakest. The extent of osseous lysis frequently demands extensive surgery. From the anatomical standpoint, the hydatid variety of Echinococcosis has a specific intra-osseous development: beginning in the spongy bone, the architecture is intermediary between the hydatid appearance and the alveolar variety. Some cases however comprise a unilocular appearance with a suspicion of adventitial reaction. Extension into the soft parts frequently adopts a macroscopic appearance identical with that of visceral forms. Bony lysis is dependant on osteoclastic resorption or foci of necrosis while reactional osteogenesis is reduced. Medullary cellular reactions are determinant in the anatomical progress: osteitis or histiocyte and plasmodial reaction to the cuticle fragments.
