RESUMO

OBJECTIVE: On January 2020, the French College of Gynecologists and Obstetricians (CNGOF) issued new Clinical Practice Guidelines (CPG) "Breech Presentation". Since then, it is recommended to use a tocolytic agent to improve the success rate of External Cephalic Version (ECV). The aim of this study, one year after these CPG, is to compare ECV without (before CPG) and with (after CPG) tocolysis in a type III maternity hospital. We intend to assess its effects on immediate success rate of ECV and obstetrical and neonatal outcomes. MATERIALS AND METHODS: This is a single-center retrospective study conducted in Nantes University Hospital. We collected patient characteristics, immediate success rate, and maternal and neonatal outcomes at delivery of all ECV over two periods: the first one during 2019 (before CPG) and the second one from June 2020 to June 2021 (after CPG). RESULTS: We included 253 patients: 126 in the first period and 127 in the second period. Immediate success rate of ECV was significantly higher since the use of tocolysis: 38.6 % (period 2) vs 23.8 % (period 1) (P=0.011). However, there was not significant difference found for cephalic presentation at birth, mode of delivery or obstetrical and neonatal outcomes. CONCLUSION: The immediate success rate is significantly improved with the widespread use of tocolysis during ECV, with no change in obstetrical and neonatal outcomes.

Nineteen clinical stage I adenocarcinoma of the uterus with favourable histological prognosis factors (low grade, no myometrial extension, and no pelvic node involvement) were diagnosed using a pre-operative hysteroscopy. During the laparotomy, peritoneal cytology was performed systematically. The frequency of positive peritoneal washings was abnormally high (7 cases) with cytologic findings showing grouped cells in large clusters. However, these patients have not experienced peritoneal recurrences. The endoscopic procedures may have facilitated the transtubal malignant cell dissemination and are questionable in endometrial carcinoma.

Cellular material found in urinary sediment stems essentially from exfoliated cells of the vesical triangle whose morphology is under the influence of the sex hormones. The study of cellular modifications forms the basis of a cytological procedure--the urocytogram. The urocytograms of 103 newborns, both premature and full term, made in the intensive care unit of the Department of Neonatal Medicine, Port-Royal Hospital, Paris, showed that marked changes in hormonal impregnation occur as a function of gestational age and permitted the establishment of a maturation index for the epithelium of the vesical triangle. In boys, the percentage of basal cells varied from 17.30 at 28 weeks' gestational age to 31.64 at term; while in girls the variation were less marked: 0.20 at 28 weeks to 1.37 at term. The oestrogen index (OI) was higher in a girl at 28 weeks (OI = 45.20) than in a girl born at term (OI = 13.50). The lower the gestational age the higher the pyknotic index (PI = 52.80 at 28 weeks), and the latter decreased as gestational age rose (PI = 23.12 at term). In boys, the oestrogen and pyknotic indices showed less variation (18.50 at 28 weeks and 15.29 at term). It thus appears that the urocytogram, a totally nontraumatic examination of urine samples obtained by simple miction, has its place among the tests of fetal maturity.

The cellular material studied in the urinary sediment mainly obtained from exfoliated cells of the bladder trigone was submitted to hormonal stimulation. These cell changes constitute the basis of a cytological method called urocytogram. The urinary sediment was examined in twenty seven girls with Turner's syndrome. The authors consider the interest of repeat examinations in the follow-up and treatment of these patients.

RESUMO

The authors studied at the "Centre Anticancéreux de Rennes" the value of fine needle aspiration cytology in the valuation of recurrences in breast cancers. The cytologic procedures were made on breast scars. The cytologic investigation interested 32 cases of carcinoma recurrences. The high rate of correct cytology diagnosis and the very low of wrong results prove the value and interest of the fine needle aspiration cytology for recurrences in breast cancers.

RESUMO

The progestomimetic, contraceptive activities of quingestanol acetate are compared to those of five molecules (chlormadinone acetate, norgestrel, progesterone, norethisterone acetate, lynestrol). In the opinion of the authors, the therapeutic efficacity of contraceptive pills cannot be judged solely according to the weight of their various ingredients. An appreciation of the value of the various substances is based on a comparison between their physiological activity and their weight. The concept of a mini-dosage pill should therefore be considered, and due consideration paid to the physiological activity of each ingredient. In the experiments described here, quingestanol acetate shows marked statistical differences to the other five molecules.

RESUMO

The urocytogramm is a method of cytohormonal diagnosis carried out on urinary sediment cells, coming from the vesical trigone. This trigone epithelium is a target of sex hormons. This work done among cryptorchids adolescents always shows a non explained hormonal stimulation, with an intermediary type urocytogramm ; wether some aged boys, the urocytogramm is atrophic as for normal man. On the otherhand, adults received estrogens as a treatment for prostate carcinome have a superficial type urocytogramm : the more important is the hormonal response, the better is the pronostic.

The cellular material studied in the urinary sediment mainly obtained from exfoliated cells of the bladder trigone was submitted to hormonal stimulation. The urocytogram has proved a useful and even necessary examination to establish the hormonal check-up of a child suffering from early puberty. This simple, painless method, without any psychological consequences is also indicated in pediatric gynecology, and in certain sexual ambiguity cases. This examination, undergone by a child premature or not, contributes to the early diagnosis of congenital adrenal hyperplasia. In the course of childhood, it helps to investigate pseudohermaphroditisms and various malformations. This examination has proved an important help in the diagnosis of all the cases of sexual ambiguity.

In the urinary sediment, the cellular material mainly originates from cells of the bladdertrigone. These cells are submitted to hormonal stimulation and their study constitutes the base of a cytological method called "urocytogram". The urinary sediment was examined in thirty-five young girls with backward puberty. The repeated examinations contribute to differential diagnosis and help in etiologic diagnosis. Whenever it is necessary to investigate adolescent's sex hormones, an urocytogram, a simple and painless method, is indicated.

The cellular material studied in the urinary sediment mainly obtained from exfolied alls the bladder trigone was submitted to hormonal stimulation. These cell changes constitute the basis of a cytologic method called the urocytogram. The urocytogram was followed up in six children with early puberty and ten young girls with backward puberty. These repeated examinations contribute to the diagnosis and help in the differential diagnosis. Whenever it is necessary to investigate a child' sex hormones the urocytogram, simple painless method is indicated.
