BACKGROUND: Electroconvulsive therapy (ECT) has been suggested as a treatment option for refractory status epilepticus (RSE) and super-refractory status epilepticus (SRSE). OBJECTIVE: The objective of this scoping review was to conduct an extensive literature review on the role of ECT as a treatment option for RSE and SRSE. METHODS: We searched Ovid MEDLINE and Scopus for journal articles from database inception until February 2024. Articles were then selected based on predetermined inclusion and exclusion criteria. RESULTS: We identified five retrospective case series with 28 adult patients receiving ECT for RSE or SRSE. ECT was administered within 3-70 days (mean 20 days) after the development of SE, and the mean number of ECT courses ranged from 1 to 12 sessions for each patient. ECT was administered in fixed or titrated doses. A total of 20 out of 28 patients (71%) showed clinical improvement, with two (7%) having complete cessation of seizures. It is essential to note that given the lack of control, there could be overreporting of clinical improvement in these studies. 11 patients (39%) were reported as deceased due to causes that were not directly related to ECT treatment. Four patients (14%) reported adverse effects of ECT, including memory, concentration, and/or cognitive impairment. CONCLUSIONS: There are level-4 Oxford Centre for Evidence-Based Medicine evidence and low-level Grading of Recommendations Assessment Development and Education evidence that suggest ECT as a treatment option for RSE and SRSE. In light of the limitations of the existing evidence, clinicians should carefully consider individual patients' clinical contexts when deciding on the appropriateness of ECT as a treatment option. Further research, including prospective studies with controlled designs, is needed to elucidate the efficacy, safety, and optimal regime of ECT in the management of RSE and SRSE.

Multitasking typically leads to interference. However, responding to attentionally demanding targets in a continuous task paradoxically enhances memory for concurrently presented images, known as the "attentional boost effect" (ABE). Previous research has attributed the ABE to a temporal orienting response induced by the release of norepinephrine from the locus coeruleus when a stimulus is classified as a target. In this study, we tested whether target classification and response decisions act in an all-or-none manner on the ABE, or whether the processes leading up to these decisions also modulate the ABE. Participants encoded objects into memory while monitoring a stream of letters and digits, pressing a key for target letters. To change the process leading to target classification, we asked participants to respond either to a specific target letter or an entire category of letters. To change the process leading to response, we asked participants to either respond immediately to the target or withhold the response until the appearance of the next stimulus. Despite successfully identifying the target and responding to it in all conditions, participants benefited less from target detection in category search than in exact search and less from delayed response than immediate response. These findings suggest that target and response decisions do not act in an all-or-none manner. Instead, the ABE and the temporal orienting response is sensitive to the speed of reaching a perceptual or response decision. (PsycInfo Database Record (c) 2024 APA, all rights reserved).

Familiarity and face inversion not only affect face recognition and memory but also influence attention. Face processing is less attention-demanding for familiar than for unfamiliar faces and for upright than for inverted faces. The automaticity raises the question of how face processing interacts with an increase in attention. Using a dual-task paradigm, we tested the interaction between attention and face familiarity and orientation. Participants encoded a series of faces to memory while simultaneously monitoring a stream of colored squares, pressing the space bar for target-colored squares and making no response to distractor-colored squares. Replicating previous findings of the attentional boost effect (ABE), we found that faces encoded with target squares were better remembered than faces encoded with distractor squares. If the automatic nature of familiar (or upright) face processing makes attention unnecessary, then the attentional boost should be attenuated for familiar relative to unfamiliar faces and for upright relative to inverted faces. Data from three experiments showed, however, that the ABE was the same for all types of faces. These results suggest that target detection did not simply elevate attention in an early encoding phase. Rather, selecting targets and rejecting distractors in the color task may have led to yoked temporal selection of target-concurrent faces for entry into memory.

Attention is tuned towards locations that frequently contain a visual search target (location probability learning; LPL). Peripheral vision, covering a larger field than the fovea, often receives information about the target. Yet what is the role of peripheral vision in attentional learning? Using gaze-contingent eye tracking, we examined the impact of simulated peripheral vision loss on location probability learning. Participants searched for a target T among distractor Ls. Unbeknownst to them, the T appeared disproportionately often in one quadrant. Participants searched with either intact vision or "tunnel vision," restricting the visible search items to the central 6.7º (in diameter) of the current gaze. When trained with tunnel vision, participants in Experiment 1 acquired LPL, but only if they became explicitly aware of the target's location probability. The unaware participants were not faster finding the target in high-probability than in low-probability locations. When trained with intact vision, participants in Experiment 2 successfully acquired LPL, regardless of whether they were aware of the target's location probability. Thus, whereas explicit learning may proceed with central vision alone, implicit LPL is strengthened by peripheral vision. Consistent with Guided Search (Wolfe, 2021), peripheral vision supports a nonselective pathway to guide visual search.

BACKGROUND: Lesion resolution is often observed in children with myelin-oligodendrocyte glycoprotein antibody-associated disease (MOGAD), and asymptomatic lesions are less commonly reported in MOGAD than in multiple sclerosis (MS). OBJECTIVE: We aimed to evaluate brain MRI changes over time in paediatric MOGAD. METHODS: Retrospective study in eight UK paediatric neuroscience centres. Acute brain MRI and available follow-up MRIs were reviewed. Predictors for lesion dynamic were evaluated using multivariable regression and Kaplan-Meier survival analyses were used to predict risk of relapse, disability and MOG-Ab status. RESULTS: 200 children were included (MOGAD 97; MS 103). At first MRI post attack, new symptomatic and asymptomatic lesions were seen more often in MS versus MOGAD (52/103 vs 28/97; p=0.002 and 37/103 vs 11/97; p<0.001); 83% of patients with MOGAD showed at least one lesion's resolution at first follow-up scan, and 23% had normal MRI. Only 1 patient with MS had single lesion resolution; none had normal MRI. Disappearing lesions in MOGAD were seen in 40% after the second attack, 21% after third attack and none after the fourth attack.New lesions at first follow-up scan were associated with increased likelihood of relapse (p=0.02) and persistent MOG-Ab serostatus (p=0.0016) compared with those with no new lesions. Plasma exchange was associated with increased likelihood of lesion resolution (p=0.01). Longer time from symptom onset to steroids was associated with increased likelihood of new lesions; 50% increase at 20 days (p=0.01). CONCLUSIONS: These striking differences in lesion dynamics between MOGAD and MS suggest greater potential to repair. Early treatment with steroids and plasma exchange is associated with reduced likelihood of new lesions.

BACKGROUND AND OBJECTIVES: We characterize clinical and neuroimaging features of SARS-CoV-2-related acute necrotizing encephalopathy (ANE). METHODS: Systematic review of English language publications in PubMed and reference lists between January 1, 2020, and June 30, 2023, in accordance with PRISMA guidelines. Patients with SARS-CoV-2 infection who fulfilled diagnostic criteria for sporadic and genetic ANE were included. RESULTS: From 899 articles, 20 cases (17 single case reports and 3 additional cases) were curated for review (50% female; 8 were children). Associated COVID-19 illnesses were febrile upper respiratory tract infections in children while adults had pneumonia (45.6%) and myocarditis (8.2%). Children had early neurologic deterioration (median day 2 in children vs day 4 in adults), seizures (5 (62.5%) children vs 3 of 9 (33.3%) adults), and motor abnormalities (6 of 7 (85.7%) children vs 3 of 7 (42.9%) adults). Eight of 12 (66.7%) adults and 4 (50.0%) children had high-risk ANE scores. Five (62.5%) children and 12 (66.7%) adults had brain lesions bilaterally and symmetrically in the putamina, external capsules, insula cortex, or medial temporal lobes, in addition to typical thalamic lesions of ANE. Hypotension was only seen in adults (30%). Hematologic derangements were common: lymphopenia (66.7%), coagulopathy (60.0%), or elevated D-dimers (100%), C-reactive protein (91.7%), and ferritin (62.5%). A pathogenic heterozygous c/.1754 C>T variant in RANBP2 was present in 2 children: one known to have this before SARS-CoV-2 infection, and a patient tested because the SARS-CoV-2 infection was the second encephalopathic illness. Three other children with no prior encephalopathy or family history of encephalopathy were negative for this variant. Fifteen (75%) received immunotherapy (with IV methylprednisolone, immunoglobulins, tocilizumab, or plasma exchange): 6 (40.0%) with monotherapy and 9 (60.0%) had combination therapy. Deaths were in 8 of 17 with data (47.1%): a 2-month-old male infant and 7 adults (87.5%) of median age 56 years (33-70 years), 4 of whom did not receive immunotherapy. DISCUSSION: Children and adults with SARS-CoV-2 ANE have similar clinical features and neuroimaging characteristics. Mortality is high, predominantly in patients not receiving immunotherapy and at the extremes of age.

Throughout prolonged tasks, visual attention fluctuates temporally in response to the present stimuli, task demands, and changes in available attentional resources. This temporal fluctuation has downstream effects on memory for stimuli presented during the task. Researchers have established that detection of a target (e.g., a square of a color to which participants are instructed to respond with a button press) within a rapid serial visual presentation (RSVP) stream leads to better memory for concurrently presented stimuli than for stimuli presented along with an RSVP distractor (e.g., a square of a color to which participants are instructed to withhold response). Although debates have arisen regarding whether this memory difference, termed the attentional boost effect, results from target-induced enhancement, distractor-induced impairment, or a combination of the two, researchers have largely come to focus on explanations that consider only target-induced memory enhancement. In the present study, we show across three large-sampled experiments a consistent appearance of both target-induced memory enhancement and distractor-induced memory impairment relative to a baseline. In each experiment, participants responded with a spacebar press to squares of one color in an RSVP stream while withholding response to squares of another color and trials with no square (baseline trials). They simultaneously memorized concurrently presented objects. The presence of both enhancement and impairment in these experiments invites the development of new dual-task research that considers distractor-induced memory impairment and the control of temporal selection across tasks. INTRODUCTION: Youth in a digital world face challenges to the healthy development of their sexuality from exposure to sexting, pornography, and other sexually explicit media. School-based sexual health education does not cover digital sexual content and its impact on sexuality and sexual relationships. Youth often avoid discussing sexual health with parents or other trusted adults, thus often relying on peers, making them ill-prepared to navigate the complexities (e.g., socially, emotionally, and romantically) of sexually explicit content. METHOD: A review of emerging evidence for application into clinical practice. RESULTS: This paper provides nurse practitioners with resources and information to incorporate this re-envisioned approach to sexual health care into practice to provide current, timely, and holistic youth sexual health care. DISCUSSION: Youth sexual health care must be re-envisioned. Nurse practitioners can improve youth sexual health by incorporating pornography and sexual media literacy, safe sexting, and concepts of consent into care using a sex-positive lens.

Increasing evidence has shown that implicit learning shapes visuospatial attention, yet how such learning interacts with top-down, goal-driven attention remains unclear. This study investigated the relationship between task goals and selection history using a location probability learning (LPL) paradigm. We tested whether a top-down spatial cue facilitates or interferes with the acquisition of implicit LPL. In a visual search task, participants were asked to give precedence to one of four, spatially cued, quadrants of the screen. Unbeknownst to them, there was an underlying uneven spatial probability in which the target appeared disproportionately often in the cued quadrant (37.5%) and a second, uncued quadrant (37.5%). To assess what participants had learned, neutral, uncued testing trials with an equal target location probability (25%) were used. Results revealed faster search times in the cued and the uncued high-probability quadrants compared to the two low-probability quadrants and these fast search times remained prevalent in the neutral testing blocks. Importantly, LPL was comparable between the cued and uncued locations in the testing blocks, suggesting that the spatial cue neither facilitated nor interfered with LPL. These results support the dual-system view of attention, revealing parallel systems supporting both goal-driven and experience-guided attention. Daily activities often occur in familiar environments, affording us an opportunity to learn. Laboratory studies have shown that people readily acquire an implicit spatial preference for locations that frequently contained a search target in the past. These studies, however, have focused on group characteristics, downplaying the significance of individual differences. In a pre-registered study, we examined the stability of individual differences in two variants of an implicit location probability learning (LPL) task. We tested the possibility that individual differences were stable in variants that shared the same search process, but not in variants involving different search processes. In Experiment 1, participants performed alternating blocks of T-among-Ls and 5-among-2s search tasks. Unbeknownst to them, the search target appeared disproportionately often in one region of space; the high-probability regions differed between the two tasks. LPL transferred between the two tasks. In addition, individuals who showed greater LPL in the T-task also did so in the 5-task and vice versa. In Experiment 2, participants searched for either a camouflaged-T against background noise or a well-segmented T among well-segmented Ls. These two tasks produced task-specific learning that did not transfer between tasks. Moreover, individual differences in learning did not correlate between tasks. Thus, LPL is associated with stable individual differences across variants, but only when the variants share common search processes.

The coronavirus disease 2019 (COVID-19) pandemic has unveiled a wide array of clinical biomarkers, and neurological manifestations in affected patients, necessitating further exploration. Methods: This single-center retrospective study evaluated clinical and neurological sequelae, demographics, as well as laboratory markers, in hospitalized COVID-19 patients from January to September 2020. Results: Among 1248 inpatients (median age: 68 years; 651 women), 387 (31%) were admitted to the ICU. Central nervous system (CNS) manifestations were present in 521 (41.74%) patients, while peripheral nervous system manifestations were observed in 84 (6.73%). COVID-19-related mortality occurred in 314 (25.16%) cases. ICU-admitted patients were predominantly male (P<0.0001), older (age≥60; P=0.037) and had more comorbidities such as diabetes (P=0.001), hyperlipidemia (P=0.043), and coronary artery disease (P=0.015). ICU patients exhibited more CNS manifestations (P=0.001), including impaired consciousness (P<0.0001) and acute cerebrovascular disease (P=0.023). Biomarkers linked to admission to the ICU included elevated white blood cell count, ferritin, lactate dehydrogenase, creatine kinase, blood urea nitrogen, creatinine, and acute phase reactants (e.g. erythrocyte sedimentation rate and C-reactive protein). ICU patients demonstrated lower lymphocyte and platelet counts compared to non-ICU patients. Those with CNS involvement in the ICU often exhibited elevated blood urea nitrogen, creatinine, and creatine kinase levels. Higher mortality from COVID-19 was observed in ICU patients (P<0.0001). Conclusions: Multiple serum biomarkers, comorbidities, and neurological manifestations in COVID-19 patients have been consistently documented and may be linked to increased morbidity, ICU admission, and mortality. Recognizing and addressing these clinical and laboratory markers is essential for effective COVID-19 management.

In the midst of the coronavirus disease 2019 (COVID-19) pandemic, intensive care units (ICUs) around the world have been pushed to their limits as they grapple with the effects of the severe acute respiratory syndrome coronavirus 2 virus. Identifying prognostic factors that influence mortality in COVID-19 patients admitted to the ICU could offer valuable insights for clinicians seeking to prevent disease progression. A retrospective analysis was conducted on COVID-19 patients admitted to the ICU between January and September 2020. The analysis considered patient demographics, comorbidities, neurological and non-neurological symptoms, as well as laboratory markers. The multivariate logistic regression analysis aims to uncover associations between these factors and patient outcomes. Of the 387 patients included in this study, nearly half (48.5%) of the ICU patients succumbed to COVID-19. Factors that contributed to increased mortality included being 60 years of age or older, impaired consciousness, lung disease, elevated international normalized ratio (INR), and elevated blood urea nitrogen (BUN) levels. Surprisingly, symptoms such as dizziness/lightheadedness, myalgia, and headache were associated with a higher likelihood of survival. In addition, elevated D-dimer and aspartate aminotransferase (AST) levels, as well as lymphopenia, were more commonly observed in deceased patients. The study concluded that those who died in the ICU tended to be older, white, and burdened with more comorbidities and impaired consciousness. With the intriguing link between specific symptoms and survival, further research is essential to uncover the underlying pathophysiological mechanisms that influence ICU patient outcomes in the context of COVID-19.

INTRODUCTION: Australia's population is steadily growing older, with older persons expected to make up over 20% of the population by 2066. Ageing is strongly associated with a significant drop in cognitive ability, ranging from mild cognitive impairment to severe cognitive impairment (dementia). This study examined the association between cognitive impairment and health-related quality of life (HRQoL) in older Australians. METHODS: Two waves of longitudinal data from the nationally representative Household, Income and Labour Dynamics in Australia (HILDA) survey were utilised, with the age cut-off for older Australians defined as above 50. The final analysis included 10,737 person-year observations from 6892 unique individuals between 2012 and 2016. This study utilised the Backwards Digit Span (BDS) test and Symbol Digit Modalities test (SDMT) to assess cognitive function. HRQoL was measured using the physical and mental component summary scores of the SF-36 Health Survey (PCS and MCS). Additionally, HRQoL was measured using health state utility values (SF-6D score). A longitudinal random-effects GLS regression model was used to analyse the association between cognitive impairment and HRQoL. RESULTS: This study found that approximately 89% of Australian adults aged 50 or older had no cognitive impairment, 10.16% had moderate cognitive impairment, and 0.72% had severe cognitive impairment. This study also found that moderate and severe cognitive impairment were both negatively associated with HRQoL. Older Australians with moderate cognitive impairment scored worse on the PCS (ß = - 1.765, SE = 0.317), MCS (ß = - 1.612, SE = 0.326), and SF-6D (ß = - 0.024, SE = 0.004) than peers without cognitive impairment given other covariates reference categories remain constant. Older adults experiencing severe cognitive had lower PCS (ß = - 3.560, SE = 1.103), and SF-6D (ß = - 0.034, SE = 0.012) scores compared to their counterparts with no cognitive impairment given other covariates reference categories remain constant. CONCLUSION: We found evidence that HRQoL is negatively associated with cognitive impairment. Our findings will be beneficial for the future cost-effectiveness intervention targeted at reducing cognitive impairment since it provides information on the disutility associated with moderate and severe cognitive impairment.

Growing evidence has shown that attention can be habit-like, unconsciously and persistently directed toward locations that have frequently contained search targets in the past. The attentional preference typically arises when the eye gaze aligns with the attended location. Here we tested whether this spatial alignment is necessary for the acquisition of a search habit. To divert eye movements away from an attended location, we used gaze-contingent eye tracking, restricting the visible portion of the screen to an area opposite to the current gaze. Participants searched for a T target amidst a circular array of L distractors. Unbeknownst to them, the target appeared more frequently in one screen quadrant. Despite fixating on a location diametrically opposite to the visible, attended region, participants acquired probability cuing, producing quicker responses when the target appeared in the high-probability quadrant. They also showed a speed advantage in the diagonal quadrant. The attentional preference for the high-probability quadrant persisted during a testing phase in which the target's location was unbiased, but only when participants continued to search with the restricted view. These results indicate that a search habit can be acquired even when participants are required to look away from the high-probability locations. The finding suggests that the learned search habit is not solely a result of oculomotor learning.

Transition metal dysregulation is associated with a host of pathologies, many of which are therapeutically targeted using chelators and ionophores. Chelators and ionophores are used as therapeutic metal-binding compounds which impart biological effects by sequestering or trafficking endogenous metal ions in an effort to restore homeostasis. Many current therapies take inspiration or derive directly from small molecules and peptides found in plants. This review focuses on plant-derived small molecule and peptide chelators and ionophores that can affect metabolic disease states. Understanding the coordination chemistry, bioavailability, and bioactivity of such molecules provides the tools to further research applications of plant-based chelators and ionophores.

BACKGROUND: Despite the high incidence of chronic obstructive pulmonary disease (COPD) in Aboriginal communities in Australia, Aboriginal Health Workers (AHWs) have limited knowledge about effective management. AIM: To evaluate an online education program, co-designed with AHWs and exercise physiologists (EPs) or physiotherapists (PTs), to increase knowledge about COPD and its management. METHODS: AHWs and EPs from four Aboriginal Community Controlled Health Services (ACCHS) were recruited. An Aboriginal researcher and a physiotherapist experienced in COPD management and pulmonary rehabilitation (PR) delivered seven online education sessions. These sessions used co-design principles and an Aboriginal pedagogy framework '8 Ways of learning', which incorporates Aboriginal protocols and perspectives to realign teaching techniques and strengthen learning outcomes. Topics covered were: How the lungs work; What is COPD; Medications and how to use inhalers and COPD Action Plans; Why exercise is important; Managing breathlessness; Healthy eating; Managing anxiety and depression. After each session, AHWs with support from EPs, co-designed education 'yarning' resources using Aboriginal ways of learning to ensure topics were culturally safe for the local Aboriginal community and practiced delivering this at the following session. At the end of the program participants completed an anonymous online survey (5-point Likert scale) to assess satisfaction, and a semi-structured interview about their experience of the online education. RESULTS: Of the 12 participants, 11 completed the survey (7 AHWs, 4 EPs). Most (90%) participants strongly agreed or agreed that the online sessions increased knowledge and skills they needed to support Aboriginal patients with COPD. All (100%) participants felt: their cultural perspectives and opinions were valued and that they were encouraged to include cultural knowledge. Most (91%) reported that delivering their own co-designed yarning scripts during the online sessions improved their understanding of the topics. Eleven participants completed semi-structured interviews about participating in online education to co-design Aboriginal 'yarning' resources. Themes identified were: revealing the Aboriginal lung health landscape; participating in online learning; structuring the online education sessions; co-designing with the facilitators. CONCLUSIONS: Online education using co-design and 8 Ways of learning was rated highly by AHWs and EPs for improving COPD knowledge and valuing cultural perspectives. The use of co-design principles supported the cultural adaptation of COPD resources for Aboriginal people with COPD. TRIAL REGISTRATION: PROSPERO (registration number: CRD42019111405).

This case series compared clinical variables and various combinations of immunotherapy received with outcomes of patients with severe acute necrotizing encephalopathy (ANE). We performed a retrospective review of clinical variables, immunotherapy received, and outcomes (based on the modified Rankin Scale) in Malaysia between February 2019 and January 2020. Twenty-seven children (12 male), aged 7 months to 14 years (mean 4 years) at diagnosis were included. Of these, 23 had an ANE severity score of 5 to 9 out of 9 (high risk). Eleven patients received tocilizumab (four in combination with methylprednisolone [MTP], seven with MTP + intravenous immunoglobulin [IVIG]) and 16 did not (two received MTP alone, 14 received MTP + IVIG). Nine died. Among the survivors, six had good outcomes (modified Rankin Score 0-2) at 6 months follow-up. All patients who received tocilizumab in combination with MTP + IVIG survived. Twenty children received first immunotherapy within 48 hours of admission. No significant association was found between the timing of first immunotherapy with outcomes. Those with brainstem dysfunction (p = 0.016) were observed to have poorer outcomes. This study showed a trend towards better survival when those with severe ANE were treated with tocilizumab in combination with MTP + IVIG. However, larger studies will be needed to determine the effect of this regime on the long-term outcomes.

Dual-task interference often arises when people respond to an incoming stimulus according to an arbitrary rule, such as choosing between the gas pedal and the brake when driving. Severe interference from response selection yields a brief "Psychological Refractory Period," during which a concurrent task is put on hold. Here, we show that response selection in one task does not always hamper the processing of a secondary task. Responding to a target may paradoxically enhance the processing of secondary tasks, even when the target requires complex response selection. In three experiments, participants encoded pictures of common objects to memory while simultaneously monitoring a rapid serial visual presentation (RSVP) of characters or colours. Some of the RSVP stimuli were targets, requiring participants to press one of the two buttons to report their identity; others were distractors that participants ignored. Despite the increased response selection demands on target trials, pictures encoded with the RSVP targets were better remembered than those encoded with the RSVP distractors. Contrary to previous reports and predictions from dual-task interference, the attentional boost from target detection overcomes increased interference from response selection.

The development of bioluminescence-based tools has seen steady growth in the field of chemical biology over the past few decades ranging in uses from reporter genes to assay development and targeted imaging. More recently, coelenterazine-utilizing luciferases such as Gaussia, Renilla, and the engineered nano-luciferases have been utilized due to their intense luminescence relative to firefly luciferin/luciferase. The emerging importance of these systems warrants investigations into the components that affect their light production. Previous work has reported that one marine luciferase, Gaussia, is potently inhibited by copper salt. The mechanism for inhibition was not elucidated but was hypothesized to occur via binding to the enzyme. In this study, we provide the first report of a group of nonhomologous marine luciferases also exhibiting marked decreases in light emission in the presence of copper (II). We investigate the mechanism of action behind this inhibition and demonstrate that the observed copper inhibition does not stem from a luciferase interaction but rather the chemical oxidation of imidazopyrazinone luciferins generating inert, dehydrated luciferins.
