RESUMO

Aims: To obtain real-world data on ramucirumab use and effectiveness for the treatment of advanced gastric cancer (AGC) or gastroesophageal junction adenocarcinoma (GEJ). Methods: Observational, retrospective study carried out in 20 Spanish hospitals, in patients who started ramucirumab treatment between December 2015 and December 2018. Descriptive analysis was conducted for patient characteristics, treatment patterns and effectiveness outcomes. Results: Three hundred seventeen patients were included (93.7% treated with ramucirumab-paclitaxel and 6.3% with ramucirumab); age 62.5 (11.3) years; 66.9% male. Median progression-free survival and overall survival were 3.9 months (95% CI: 3.4-4.3) and 7.4 (95% CI: 6.4-8.9) in combination regimen and 2.0 (1.1-2.8) and 4.3 (95% CI: 1.9-7.3) in monotherapy, respectively. Conclusion: The study findings were consistent with available real-world studies and randomized clinical trials.

Patients with HIV are susceptible to mycobacterium infection. In the case of fast-growing mycobacteria, the group to which Mycobacterium fortuitum (M. fortuitum) belongs, infections have been described in the skin, lungs, lymph nodes and disseminated disease. We present the case of a 43-year-old male patient with pre-diagnosis of HIV in antiretroviral therapy, which comes as a fever, asthenia, weight loss and chronic diarrhea. Abdominal tomography is performed and hepatosplenomegaly is evidenced with nodular lesions in the spleen. The splenic culture was finally positive for M. Fortuitum, with positive PCR to Mycobacterium tuberculosis. The current treatment protocols for this type of infection are based on the susceptibility shown in the cultures performed. With regard to coinfections between M. Fortuitum and Mycobacterium tuberculosis, in HIV positive patients, there are even less information.


Los pacientes con VIH son susceptibles a la infección por micobacterias. En el caso de las micobacterias de crecimiento rápido, grupo al que pertenece el Mycobacterium fortuitum (M. fortuitum), se han descrito infecciones en la piel, pulmones, ganglios linfáticos y enfermedad diseminada. Presentamos el caso de un paciente varón de 43 años, con diagnóstico previo de VIH en tratamiento antirretroviral, que acude por fiebre, astenia, pérdida de peso y diarrea crónica. Se realiza tomografía abdominal y se evidencia hepatoesplenomegalia con lesiones nodulares en el bazo. El cultivo esplénico, finalmente, fue positivo para M. fortuitum, con PCR positivo a Mycobacterium tuberculosis. Los protocolos actuales de tratamiento para este tipo de infecciones se basan en la susceptibilidad mostrada en los cultivos realizados. En lo que respecta a coinfecciones entre M. fortuitum y Mycobacterium tuberculosis, en pacientes VIH positivos, la información es aun menor.

Employees can help to improve organizational performance by sharing ideas, suggestions, or concerns about practices, but sometimes they keep silent because of the experience of negative affect. Drawing and expanding on this stream of research, this article builds a theoretical rationale based on core affect and cognitive appraisal theories to describe how differences in affect activation and boundary conditions associated with cognitive rumination and cognitive problem-solving demands can explain employee silence. Results of a diary study conducted with professionals from diverse organizations indicated that within-person low-activated negative core affect increased employee silence when, as an invariant factor, cognitive rumination was high. Furthermore, within-person high-activated negative core affect decreased employee silence when, as an invariant factor, cognitive problem-solving demand was high. Thus, organizations should manage conditions to reduce experiences of low-activated negative core affect because these feelings increase silence in individuals high in rumination. In turn, effective management of experiences of high-activated negative core affect can reduce silence for individuals working under high problem-solving demand situations.

INTRODUCTION: The treatment of brain dead donors with combined hormonal resuscitation protocols, including methylprednisolone (MP) and triiodothyronine (T3), among others, was developed to increase the viability and function of transplanted organs, primarily heart and lung. Even when it has regarded successful results in term of donors and organs recovery, its effects over specific parameters in organs like the liver are unknown. MATERIAL AND METHODS: Male Sprague-Dawley rats were pretreated with MP (0.34 mg/kg) and/or T3 (0.05 mg/kg) or their vehicles, and then subjected to partial hepatectomy of 70%. Three experimental groups and their respective controls were conformed: a. T3; b. NaOH; c. MP; d. vMP; e. MP+T3 and f. vMP+NaOH. The groups were evaluated at 0, 16, 24, 72 and 120 h post surgery. The effects of this protocol on regeneration, liver mass recovery, liver injury, oxidative stress and liver function were analyzed. RESULTS: MP+T3 pretreatment does not deleteriously affect liver regeneration after partial hepatectomy, as shown in the curve of total mass recovery, Ki67 staining and mitosis counting, and does not alter liver function. In addition, the treatment modestly decreases oxidative stress and liver injury, as evidenced by transaminases levels, histological analysis and oxidized proteins content. CONCLUSION: These preclinical results indicate that MP+T3 is harmless for liver tissue regeneration post hepatectomy and additionally exhibits anti-inflammatory and antioxidant effects; therefore, it would not be contraindicated for the treatment of multiorgan donors in brain death and particularly, if the occurrence of small for size syndrome is suspected.

This study assessed the extent to which two kinds of emotional labor (surface and deep acting) can lead to emotional exhaustion, reducing one's overall life satisfaction. Based on Self-Determination theory, the importance of perceived autonomy was also studied in relation to how it moderates the relationship between emotional labor and emotional exhaustion. Data collected online from 241 staff employed at a university in central United States revealed that the relationship between surface acting and emotional exhaustion was stronger among people with lower perception of autonomy, which had an impact on overall life satisfaction. No significant relationship between deep acting and emotional exhaustion was found. Future directions should include studying the model on other samples and using a longitudinal design.


Este estudio evaluó el grado en el cual dos clases de labores emocionales (acción profunda y superficial) pueden conducir a la exhaustividad emocional, reduciendo en general la satisfacción de vida. Basados en la teoría de la Auto-Determinación, la importancia de la autonomía percibida puede ser estudiada en relación a cómo es moderada la relación entre labor emocional y exhaustividad emocional. Los datos se recogieron via web desde 241 empleados en una universidad en la zona centro de Estados Unidos y revelaron que la relación entre acción superficial y exhaustividad emocional fue más fuente entre personas con bajos niveles de percepción de autonomía, la cual podría tener un impacto sobre la satisfacción de vida. No se encontraron relaciones significativas entre las acciones profundas y la exhaustividad emocional. Futuras investigaciones deberían incluir en el estudio del modelo utilizar otras muestras y utilizar un diseño longitudinal.

Este estudio examina las expectativas de roles futuros de 522 jóvenes universitarios en Santiago de Chile. Para esto, se utilizó la adaptación de un instrumento que evalúo las expectativas personales de hombres y mujeres sobre distintos roles que pueden ejercer en la vida: laboral, marital, parental y cuidado del hogar (The Life Role Salience Scales [LRSS]; Amatea, Cross, Clark & Bobby, 1986). Se evaluaron el nivel de Conservadurismo Valóri-co (Contreras & Plaza, 2004) y los datos sociodemográficos. Los análisis de regresión mostraron una preferencia masculina mayor en roles laboral y marital en comparación con las mujeres. El Conservadurismo Valórico influyó en expectativas de roles marital, parental y cuidado del hogar, mas no en el rol laboral.


This study reviews the future roles expectations in a sample of 522 young undergraduate students in Santiago, Chile. In order to do that it was administered the adaptation of the instrument (The Life Role Salience Scales [LRSS]; Amatea, Cross, Clark & Bobby, 1986) that assessed men's and women's personal expectations concerning life roles: occupational, marital, parental and homecare roles. As well the level of Valoric Conservatism (VC) was measure (Contreras & Plaza, 2004), and socio-demographic data. Regression analysis showed a male's bigger preference for work and marital roles than women. VC influenced the expectations for marital, parental and home-care roles, but it didn't influenced work role.

Se presenta el caso de un paciente procedente de la selva de Perú con diagnóstico de infección por el Virus de Inmunodeficiencia Humana (VIH) con cuadro respiratorio alto, fiebre y que presentó una radiografía con patrón miliar. Por ser la Tuberculosis una enfermedad prevalente en nuestro país e infección mas frecuente en pacientes con VIH recibió tratamiento anti tuberculoso. Sin embargo, el dato histórico de haber estado en zona endémica de micosis en algún momento de su vida, asociada a exámenes de laboratorio, permitió el diagnóstico de Histoplasmosis diseminada, como producto de la reactivación de una infección latente que se manifiesta cuando el nivel de inmunidad se deprime como ocurre en los pacientes infectados por el VIH. La historia de enfermedades, viajes y residencia previas es información fundamental para establecer una buena aproximación diagnóstico en pacientes con VIH.

Desde Noviembre 1984 - Agosto 1986, se realizó un estudio abierto y prospectivo en 16 pacientes con cuadro de Loxoscelismo cutáneo, las cuales recibieron como terapia única Diamino Definil Sulfona (DDS). La dosis administrada fue de 100 mg/día vía oral; 10/16 pacientes completaron 10 días; 2/16, 15 días; 2/16, 20 días; 1/16, 5 días; 1/16, 5 días (doble de la dosis). La evolución de 10/16 pacientes con lesión predominantemente del tipo placa liveloide, fue como sigue: 1/10 curó sin secuela alguna; 8/10 formaron crosta sin úlcera; 1/10 persistió con úlcera inicial. Además en estos 10 pacientes 3/10 tuvieron leve necrosis inicial asociada, y una vez terminada la terapia con DDS, desapareció la necrosis sin progrear a la úlcera, en 5/16 pacientes con lesión violácea predominantement, la evolución post-terapia fue: 2/5 curó sin secuela alguna; 3/5 hicieron costra que luego cayó sin dejar úlcera 1/15 de los pacientes tuvo además leve necrosis inicial que con el tratamiento no progresó y desapareció. El único paciente (1/16) con gran edema y eritema, evolucionó bien con el tratamiento, sin dejar secuela alguna. No se tuvo ningún efecto secundario de la DDS durante el tratamiento, que nos obligara a descontinuarla. Sólo al finalizar la terapia un paciente (1/16) presentó una erupción dérmica leve en las palmas de las manos, que tratada con cremas corticoides y antihistamínicos orales evolucionó favorablemente
