INTRODUCTION: Congenital Zika syndrome (CZS) is a recently described disease. Our main objective was to evaluate and monitor, over 3 years, the ophthalmoscopic findings in children exposed to zika virus (ZIKV) during gestation. METHODS: This prospective observational study was conducted in Rio de Janeiro, Brazil, between April 2016 and May 2019. We evaluated two groups with exanthema serving as a proxy for viremia: (i) children whose mothers had exanthema during pregnancy and (ii) children who had microcephaly without maternal exanthema during pregnancy. We performed indirect ophthalmoscopy at recruitment and every 6 months thereafter. We also tested the association between ocular findings with maternal exanthema, microcephaly, CZS and maternal infection confirmed by reverse transcriptase quantitative polymerase chain reaction and gender. RESULTS: Of the 72 children included, 16 (22.2%) had optic nerve and/or retinal lesions. All 16 had CZS and 15 (93.7%) had microcephaly (14 at birth and 1 postnatally). The child with postnatally acquired microcephaly was born to a mother without exanthema during pregnancy. Fifty-six (77.8%) of the 72 children were followed for a median time of 24 months and none exhibited differences between admission and follow-up examinations. After logistic regression, only microcephaly at birth was associated with eye abnormalities (odds ratio, 77.015; 95% confidence interval, 8.85-670.38; p < 0.001). CONCLUSION: We observed that there was no progression of the lesions over the follow-up period. We also showed that the eye findings were associated only with microcephaly at birth. Attention should be paid to all children born during a ZIKV epidemic, regardless of maternal exanthema and/or microcephaly at birth.

RESUMO Objetivo: Comparar a acurácia entre a acuidade visual com melhor correção óptica em pacientes com retinopatia diabética e acuidade visual com correção óptica menor ou igual a 20/200 em ambos os olhos mensurada pela tabela de Snellen e pela tabela ETDRS, além da avaliação do tempo de leitura com tabela de leitura de Jaeger. Métodos: Estudo observacional e transversal. ormação de Grupos de Pesquisa: o Grupo I foi formado por pacientes com acuidade visual de 20/200 e o Grupo II, por pacientes com acuidade visual maior de 20/200. Os pacientes de ambos os grupos foram subdivididos em aqueles com retinopatia diabética não proliferativa e os com retinopatia diabética proliferativa. As variáveis pesquisadas foram sexo, idade, tabela ETDRS nos subgrupos (igual a 20/200, igual a 20/400 e maior de 20/200 e 20/400) e tempo de leitura da tabela de Jaeger para perto. O índice de significância estatística deste estudo foi de 5%. Resultados: Participaram do estudo 62 pacientes diabéticos, sendo 33 com retinopatia diabética não proliferativa e 29 com retinopatia diabética proliferativa. Dentre os pacientes com retinopatia diabética não proliferativa, 19 tinham acuidade visual com melhor correção óptica de 20/200 (30,6% da amostra), e 13 pacientes eram portadores de retinopatia diabética proliferativa com acuidade visual com melhor correção óptica de 20/200 (20,9% da amostra). A correlação entre a tabela Snellen e a tabela ETDRS revelou que 16 pacientes (25,8% da amostra) com cegueira legal apresentaram acuidade visual entre 20/180 e 20/120 segundo a tabela ETDRS. Para os pacientes portadores de retinopatia diabética não proliferativa com acuidade visual de 20/200 pela tabela ETDRS, houve tempo médio de leitura para perto da tabela de Jaeger de 23,6±1,4 segundos. Na análise estatística da acuidade visual mensurada com a Tabela de Snellen em comparação com a Tabela de Leitura para perto (Jaeger), mensurando tempo de leitura, ahouve significância estatística (p=0,04). Conclusão: Pacientes diabéticos com cegueira legal segundo a tabela de Snellen apresentam sua real capacidade visual residual subavaliada. É importante levar em consideração o tempo de leitura e o uso da tabela ETDRS para melhor acurácia.
ABSTRACT Purpose: To compare the accuracy of best-corrected visual acuity in patients with diabetic retinopathy to corrected visual acuity ≤ 20/200 in both eyes, measured by Snellen and ETDRS charts, in addition to evaluation of reading time by Jaeger chart. Methods: An observational and cross-sectional study. Study Groups: Group I, patients with visual acuity 20/200, and Group II, patients with visual acuity >20/200. Patients were subdivided into those with non-proliferative diabetic retinopathy and with proliferative diabetic retinopathy. The study variables were sex, age, ETDRS chart in the subgroups (equal to 20/200; equal to 20/400 and greater than 20/200 and 20/400), and reading time by Jaeger chart on the near vision. The statistical significance was set at 5%. Results: A total of 62 diabetic patients participated in the study; in that, 33 with non-proliferative diabetic retinopathy and 29 with proliferative diabetic retinopathy. Among patients with non-proliferative diabetic retinopathy, 19 had best-corrected visual acuity 20/200 (30.6% of sample), and 13 patients had proliferative diabetic retinopathy with best-corrected visual acuity 20/200 (20.9% of sample). The correlation between the Snellen and ETDRS charts revealed 16 patients (25.8% of sample) with legal blindness presented visual acuity between 20/180 and 20/120, as per the ETDRS chart. For patients with non-proliferative diabetic retinopathy with visual acuity 20/200 measured by the ETDRS chart, the mean reading time to near vision was 23.6±1.4 seconds, as measured by Jaeger chart. In the statistical analysis, the comparison of visual acuity measured by Snellen chart, with the reading chart to near vision (Jaeger) measuring reading time, was statistically significant (p=0.04). Conclusion: Diabetic patients with legal blindness as per Snellen chart showed their real underestimated residual visual capacity. It is important to consider reading time and use of the ETDRS chart for better accuracy.

Resumo Objetivo: Determinar a prevalência de fatores predisponentes de baixa acuidade visual entre os alunos do Colégio Universitário Geraldo Reis em Niterói-RJ. Métodos: Trata-se de um estudo observacional transversal realizados em duas etapas. A primeira realizou-se a medida da acuidade visual dos alunos voluntários que aderiram ao termo de assentimento. Na segunda etapa foram selecionados aqueles alunos cuja acuidade visual não ultrapassaram 0,8 em pelo menos um dos olhos ou que apresentaram diferença de acuidade visual entre os olhos de duas linhas ou mais na tabela de Snellen, sendo encaminhados para avaliação oftalmológica completa no Serviço de Oftalmologia do Hospital Universitário Antônio Pedro / Universidade Federal Fluminense. Resultados: Do total de 325 alunos matriculados, 134 (41,2%) participaram da primeira etapa do estudo e, destes, apenas 39 (29%) apresentaram baixa visão. Dos 39 alunos selecionados para a segunda etapa do estudo, apenas 14 (36%) se voluntariaram a prosseguir para avaliação oftalmológica completa, tendo as ametropias (57,14%), a ambliopia (21,42%) e o estrabismo (14,28%) como as principais causas de baixa visual identificadas. Conclusão: A prevalência de baixa visão(low vision=baixa visão tem definição, não se chama baixa visual) para essa comunidade foi de 29% e as principais causas identificadas foram as ametropias, as ambliopias e o estrabismo. Campanhas de conscientização e os problemas de adesão aos programas de triagem devem ser considerados em novos estudos.
Abstract Purpose: To determine the prevalence of predisposing factors of low visual acuity among the students of the Geraldo Reis University College in Niterói-RJ. Methods: This was a cross-sectional observational study during which the visual acuity of the volunteer students who adhered to the assent term was measured. Those students whose visual acuity did not exceed 0.8 in at least one eye or who presented a difference between the eyes of two lines or more in the Snellen table were selected for the next stage of the study and were referred for complete ophthalmologic evaluation in the Service of Ophthalmology of the Antônio Pedro University Hospital / Fluminense Federal University. Results: Of the total of 325 students enrolled, 134 (41.2%) participated in the first stage of the study and of these, only 39 (29%) presented visual impairment. Of the 39 students selected for the second phase of the study, only 14 (36%) volunteered to proceed for a complete ophthalmologic evaluation, with ametropias (57.14%), amblyopia (21.42%) and strabismus (14, 28%) as the main causes of visual impairment identified. Conclusion: The prevalence of visual impairment for this community was 29% and the main causes identified were ametropias, amblyopia and strabismus. Awareness campaigns and problems of adherence to screening programs should be considered in new studies.

OBJECTIVE: To better understand the clinical spectrum and course of congenital Zika syndrome (CZS) during the first 18 months of life of children whose mothers had rash during pregnancy. METHODS: This longitudinal observational study evaluated the clinical progress from birth until 18 months of life of children of mothers who developed rash during or up to 3 months before gestation. Maternal rash occurred from November 2015 to May 2017. The study subjects were divided into three groups: children whose mothers tested positive by RT-qPCR for Zika virus (ZIKV) (Group 1), children whose mothers tested negative by RT-qPCR for ZIKV (Group 2), and children whose mothers did not undergo any testing for ZIKV (Group 3) but tested negative for other congenital infections. RESULTS: Between April 2016 and July 2018, we studied 108 children: 43 in Group 1, 26 in Group 2 and 39 in Group 3. The majority of children were admitted into the study within 6 months of life. CZS was diagnosed in 26 children, equally distributed in Groups 1 and 3. Of 18 children with microcephaly, 6 were in Group 1 (1 postnatal) and 12 were in Group 3 (5 postnatal). Maternal rash frequency was 10 times higher during the first trimester than in the other trimesters (OR: 10.35; CI 95%: 3.52-30.41). CZS was diagnosed during the follow-up period in 14 (54%) cases. Developmental delays and motor abnormalities occurred in all children and persisted up to 18 months. Epilepsy occurred in 18 (69%) of the cases. CONCLUSIONS: Infants born of mothers exposed to ZIKV during pregnancy showed progression of developmental, motor and neurologic abnormalities even if they were born asymptomatic. Continued postnatal monitoring of such newborns is necessary to preclude disability-associated complications.
