RESUMO

Nursing students are confronted with the mysteries of the patient encounter during their internship in psychiatry. From this discovery, questions and enigmas remain to be solved. This ephemeral primary relationship, in a time span of a few weeks, is a source of frustration for them. In this context, the presence and professionalism of the team are precious assets that the student must seize. Discovery of the profession of psychiatric nurse illustrated by the testimonies of two students.

RESUMO

Nursing training is provided in nursing training institutes by a multidisciplinary teaching team. During this course, the nursing student will forge his or her professional identity. However, psychiatry is a field that is not very popular with students. Social representations, apprehension and lack of knowledge of the discipline, and the crisis that the sector is going through all work against this choice. Especially since young nurses are ill-equipped to develop a sufficiently good posture, in line with the expectations of the health care team. Nevertheless, some of those who have chosen to work in psychiatry emphasize the notion of encounters as a determining factor in their professional orientation. This encounter is based on three stages: an encounter with oneself, with specific knowledge and with the clinic.

RESUMO

The touch of oneself is essential in the construction of a conscious self. It contributes to the feeling of ownership of the body. Feeling touched and touching reveals the subject in a being-self inscribed in its environment. This sensory experience can be disturbing for the subject. A study conducted with adolescent girls with obesity has shown that this haptic experience, during a postural session, can generate a feeling of strangeness. This singular discovery of oneself reveals the opening towards new exploratory fields of the body self-consciousness, during our care.

Introduction : The COVID-19 pandemic has warranted the implementation of barrier actions and the wearing of personal protective equipment (PPE). These behavioral adaptations counteract spontaneous self-touching reflex gestures. I propose to explore the mind-body experience generated by these gesture changes in this health context.Method : I conducted a qualitative study with nine nurses who volunteered as reinforcements. The semistructured interviews I carried out were analyzed using the interpretative phenomenological analysis approach.Results : The perception of the risk of self-inoculation is not related to the level of virus exposure. Facial PPE is perceived both as reassuring and difficult to put up with. The experience of self-touches is influenced by the level of attention the subject pays to his or her face and by the medical background related to this. Three kinds of mind-body adaptations are used to control self-touching : control by avoidance, control by keeping one’s hands busy, and compensatory bodily strategies. The representations of the bodily self are modified in terms of gestures and of the feeling of humanness. This generates a form of bodily abandonment and a lived experience of soiled bodies.Conclusion : The results highlight the cognitive load involved in refraining from self-touching and with the wearing of facial PPE. They emphasize the importance of questioning the implications of these sensorimotor modifications. Supporting health professionals in assessing the mind-body repercussions in a pandemic situation appears key.

INTRODUCTION: The coiling of umbilical cord is the winding of the umbilical cord around a part of the fetal body once or several times. It is the most common abnormality of the cord, its prevalence varies according to the authors from 5.7% to 35.1%. In 2011, the rate of perinatal mortality due to the coiling of umbilical cord in Cameroun was 6.1%. However factors associated to it are little known in our context. Our purpose was to determine factors associated to the coiling of umbilical cord in three hospitals in Yaoundé. METHODS: We conducted an analytical case-control study in the Maternity Unit at the Central Hospital of Yaoundé, the University Teaching Hospital of Yaoundé and the Social and Health Nkoldongo Animation Center over a period of 4 months. The study-group composed of newborns with coiling of umbilical cord was compared with two control groups (newborns without coiling of umbilical cord). All the fetuses were cephalic at delivery (singleton pregnancies at term). Pre-established technical data sheets were filled with data collected and analyzed based on the Microsoft Excel 2017 and SPSS software Version 23. The parameters used for the analysis were the average age, standard deviation and frequency, the raw odds ratio (OR) and/or adjusted (aOR) with their 95% confidence interval. P was considered significant for any value less than 5. RESULTS: Out of a total of 3,300 deliveries, 500 newborns (15.15%) had coiling of umbilical cord. All the coils were around the neck. We retained and studied 136 newborns with coiling of umbilical cord (study group) vs 272 newborns without coiling of umbilical cord (control group). Factors independently associated with coiling were non editable: length of the cord = 70cm (ORa = 32 CI = 17.5-35 p = 0.02), male sex (ORa = 67.09 CI = 22.31 - 97.46 P = 0.001), APGAR score 5th minute <7 (ORa =76.98 CI = 2.19 - 27.05 P=0.017) and modifiable factors were gestational age = 42WA (ORa = 15.15 CI = 6.14-18.2 P = 0.001). CONCLUSION: The coiling of umbilical cord is a frequent cord abnormality. We suggest to the decision-makers to increase awareness among health workers and the population on the importance of ultrasound scan of the third trimester in order to detect coiling of umbilical cord and implement appropriate manage newborns. Clinicians should avoid as much as possible post-term pregnancy.

Care for patients with eating disorders is complex and plurimodal. Care plans need to be adapted in order to take into account the body in crisis. A series of hospital admissions combined with specific psychomotor approaches, can contribute to the patient being reappropriated with their own body.
