RESUMO

PURPOSE: As part of the Avahan India AIDS Initiative, a behaviour change communication programme sought to increase consistent condom use (CCU) among male clients of female sex workers (FSWs) in four Indian states through the use of outdoor static promotional materials, interpersonal communication and mid-media activities. This paper presents key findings related to programme coverage levels, trends over time in self-reported condom use, and correlations between levels of exposure to programme activities and self-reported condom use. METHODS: Five stratified two-stage cluster sample surveys were conducted between April 2006 and November 2008 (sample sizes ranged from 1741 to 2041). The independent samples were composed of clients of FSWs in selected clusters. Multivariate logistic regression was used to model whether behavioural outcomes varied between baseline and endline, and whether they varied with levels of exposure to the intervention. RESULTS: Over two-thirds of men in each survey round recalled one or two of the main intervention channels. An increase in CCU with FSWs was found between baseline and endline (63.6% vs 86.5; p<0.01). Men exposed to two intervention channels reported higher CCU than men exposed to none or only static outdoor media (89.4% vs 82.0%, p<0.05). This trend was sustained for condom use at last sex (96.2% vs 92.6%, p<0.05). CONCLUSION: Evaluation suggests that a multi-channel behavioural intervention delivered at sites where FSWs are solicited can increase CCU among male clients of FSWs. Further research is needed on the dynamics of exposure to multiple communication channels and the contributions of complementary interventions.

PURPOSE: The primary focus of this study is to investigate which characteristics of health service quality are most likely to determine client satisfaction with health services among adolescents in Mongolia. METHODS: Data were gathered from 1301 male and female clients. Exit interviews were used to measure client satisfaction; 82 clinics were visited. All clients between the ages of 10 and 19 years were asked to participate in the client exit interview; those who agreed to participate completed the questionnaire. Bivariate and multivariate analyses were conducted to determine significant associations between service satisfaction and the independent variables. All variables showing a significant bivariate association with service satisfaction (p < or = .05) were retained for logistic regression analyses. RESULTS: The strongest determinant to client satisfaction related to acceptability: adequate facility physical environment, receiving adequate information about the facility, and if the facility was private (i.e., other people didn't know the services the client received). Additionally, clients who said they received some interruptions, either by other health workers or clients, were significantly less likely to be satisfied with the services. CONCLUSIONS: This study demonstrates the importance of understanding and measuring different aspects of health service quality in defining client satisfaction. Although both accessibility and acceptability of services have been shown to be important in other studies, characteristics relating to acceptability emerged as critical in determining client satisfaction among adolescents in Mongolia. Efforts to improve health service delivery to adolescents need to understand and address the "adolescent friendly" characteristics that are most salient, and least fulfilled, in each particular context.

Recent data indicate that nearly one in three Bolivian adolescent females becomes pregnant prior to reaching age twenty. This article presents the results of a study undertaken to address the question of why some female adolescents in La Paz, Bolivia, become pregnant while others in similar circumstances avoid early pregnancy. The study utilized mixed qualitative-quantitative methods based on a case-control design. Among the potential explanatory factors considered were family structure, parental relationships, partner relationships, knowledge of pregnancy risks, self-esteem, and locus of control. Significant differences between girls experiencing a pregnancy and those who had not were observed on two of the six factors considered-relationships with parents and self-esteem. Girls who had experienced a pregnancy were less likely to have reported affectionate and supportive parents, more likely to have reported fighting in their home, and exhibited lower levels of self-esteem than those who had never been pregnant. Focus-group discussions suggested that adolescent females in La Paz lack trustworthy support networks that would empower them to seek information regarding sex and contraception and to act upon such information.
