RESUMO

This report describes a patient with Vertical Maxillary Excess without open bite in whom surgical orthodontic treatment to reduce lower facial height remarkably improved function and facial esthetics. The patient was a 22-year-old male whose main concern was crowding and temporomandibular joint clicking sounds. The clinical and radiological findings led to the diagnosis of Vertical Maxillary Excess with a mild skeletal class II malocclusion. The proposed treatment plan comprised a bimaxillary surgery without premolar extractions. LeFort I osteotomy was planned to reposition the maxilla superiorly by 7-8 mm. This surgery was combined with a bilateral sagittal split osteotomy for mandibular anterior derotation to adjust the mandible to the occlusal and anteroposterior change. Postoperatively, the mandibular plane angle (GoGn-SN) was decreased by 4° and skeletal class I was achieved (ANB, 4°). In addition, lip incompetence was corrected and the excessive gingiva exposure upon smiling was significantly improved. The patient was satisfied with the treatment result and reported the temporomandibular joint clicking sounds disappearing after surgery.

RESUMO

Nasopharyngeal teratomas are rare and represent only 2% of all teratomas. They can lead to an embryopathogenic mechanical obstacle responsible for a cleft palate with few reported cases of this association in the literature. We report the case of a 14-month-old girl with this atypical association.

Central odontogenic fibroma is an uncommon, benign, slow-growing intraosseous mesenchymal odontogenic tumour. It presents a diagnostic dilemma to the clinician and the pathologist because its clinical and radiological features resemble other odontogenic and/or non-odontogenic tumours, and the differential diagnosis is based on histological examination. In this report, we describe our experience with a case of a 23-year-old female patient with central odontogenic fibroma of the mandible that was diagnosed as 'simple type'. Highlighting a subtype that was dropped from the last World Health Organization classification of head and neck tumours is important to accumulate more information about this lesion and to show its different features. Despite its rarity, central odontogenic fibroma should be included in the differential diagnosis of intrabony tumours of the jaws. These findings can better educate oral and maxillofacial surgeons about the unusual nature of this lesion, help establish a correct diagnosis and give the appropriate therapeutic management.

RESUMO

Septic arthritis of the temporo-mandibular joint (TMJ) is a rare disease that has been reported infrequently. To the best of the authors' knowledge, only one case of bilateral TMJ septic arthritis has been reported. The contamination may result from direct extension of adjacent infection (dental or ENT), from hematogenous spread of blood-borne organisms or from direct inoculation. The most common presenting are trismus and pain, although swelling, tenderness and erythema have also been described. In addition, patients may develop fever, regional lymphadenopathy and malocclusion. Through a successively bilateral case of TMJ arthritis, without obvious portal of entry of the bacteria, we will analyze characteristics and treatment of this disease.
