RESUMO

Thyroid ectopy is the presence of thyroid tissue outside its normal cervical location. Clinical manifestations of thyroid ectopy are varied. The latter complications can be life-threatening. Emergency treatment is often surgical or endoscopic. We report a case of a 26-year-old man with tracheal thyroid ectopy, complicated by respiratory distress, in whom conventional treatments were not feasible. The patient was treated with radioiodine-131 administered in liquid form. The final control showed the complete resolution of the intra-tracheal mass. Intra-tracheal thyroid ectopy is a rare anomaly in which surgery is the traditional treatment. In certain cases where surgery is not feasible or refused, treatment with iodine-131 is a safe and effective alternative for the removal of ectopic thyroid tissue. The aim of our work is to show the significant efficiency of radioiodine therapy as an alternative for the treatment of complicated thyroid ectopia.

RESUMO

The radioiodine scan is a non-invasive imaging modality that allows for the visualization of functional thyroid tissue, as well as the detection of thyroid cancer remnants and metastases. However, it is important to note that radioiodine uptake is not exclusive to thyroid tissue and can lead to false-positive results if unexpected uptake occurs in non-thyroidal tissue. Herein, we present a case of a patient diagnosed with thyroid carcinoma, whose radioiodine scan demonstrated increased uptake in the thorax, corresponding to a lung carcinoma.

Bone xanthoma is a rare disease due to the presence of cholesterol deposits in the bone. We report a case of a 56-year-old patient who was hospitalized in orthopedic department for fracture on the left humerus. Histological examination of bone biopsy performed at this site has objectified cholesterol deposits. Laboratory tests showed hypercholesterolemia with hypertriglyceridemia. The diagnosis of bone xanthoma was selected. The fracture was treated surgically. An initial bone scan revealed bone lesions localized especially at the humerus and femur. The patient was put under fenofibrate and statins. The evolution was marked by gradual regression of lesions in bone scans of control.

PURPOSE: Lymph node metastasis is an important prognostic factor in prostate cancer (PC). The aim of this prospective study was to evaluate the accuracy of sentinel lymph node (SLN) biopsy by laparoscopy in staging locoregional patients with clinically localized PC. METHODS: A transrectal ultrasound-guided injection of 0.3 ml/100 MBq (99m)Tc-sulphur rhenium colloid in each prostatic lobe was performed the day before surgery. Detection was performed intraoperatively with a laparoscopic probe (Gamma Sup CLERAD) followed by extensive resection. SLN counts were performed in vivo and confirmed ex vivo. Histological analysis was performed by haematoxylin-phloxine-saffron staining, followed by immunohistochemistry (IHC) if the SLN was free of metastasis. RESULTS: The study included 93 patients with PC at intermediate or high risk of lymph node metastases. The intraoperative detection rate was 93.5% (87/93). Nineteen patients had lymph node metastases, nine only in SLN. The false-negative rate was 10.5% (2/19). The internal iliac region was the primary metastatic site (43.3%). Metastatic sentinel nodes in the common iliac region beyond the ureteral junction were present in 13.3%. Limited or standard lymph node resection would have ignored 73.2 and 56.6% of lymph node metastases, respectively. CONCLUSION: Laparoscopy is suitable for broad identification of SLN metastasis, and targeted resection of these lymph nodes significantly limits the risk of extended surgical resection whilst maintaining the accuracy of the information.
