RESUMO

It was performed a comparative research of remote results including local status and life quality of simultaneous surgical treatment in 64 patients with postoperative ventral hernias (W1-W4) and obesity (1-3 degrees). It was compared advanced hernioalloplasty with new synthetic materials including Prolene-Monocryl-composite mesh and absorbable adhesion barrier INTERCEED (TC7) in combination with abdominoplasty and hernioalloplasty with the same materials. It was concluded that simultaneous hernioalloplasty and abdominoplasty contributed statistically significant (p < 0.01) decreasing of number of recurrent hernias in 2 times, body mass index and body mass on 24% in comparison with single alloplasty.

RESUMO

The article analyzes the results of effect of combined and local cytokine- and ozone therapy on the indices of lipid peroxidation, endogenous intoxication and ferroproteins in 111 patients with diffuse peritonitis. It was shown, that combined sequential local and systemic cytokine and ozone therapy allows correcting the expression of endogenous intoxication and lipid peroxidation in diffuse peritonitis. This method suppresses an inflammation in the abdominal cavity. At the same time, it accelerates the elimination of intestine atony and thereby potentiates the possibilities of traditional methods of treatment.

RESUMO

The results of the ultrasound investigation and computed tomography of patients with gastric cancer, benign chronic and malignant gastric ulcers were thoroughly analyzed. The study included 55 patients aged 22 to 74 years. 20 patients had cancer; 22 had the malignant gastric ulcer and the rest 13 had benign chronic gastric ulcer. The study proved the high efficacy of modern hi-tech radiodiagnostic means for the on-time diagnosis of malignant gastric ulcers.

In 43 (39.1%) injured persons with gun-shot abdominal wounding postoperative complications have occurred, demanding reoperations. The most frequent of them were postoperative peritonitis, purulent complications, recurrent hemorrhage, early adhesive ileus. Inadequate abdominal cavity sanation in gun-shot wounding, perforation of deserosated places of intestinal wall, an acute ulcers of hollow organs, lost and not revealed foreign bodies in abdominal cavity were the causes of the progressing postoperative peritonitis occurrence. Absence of some signs, characteristic for postoperative peritonitis, made the diagnosis difficult, that is why in majority of observations the relaparotomy conduction was very delayed. Relaparotomy was conducted on the first-second day, as a rule, for insufficiently sanated abdominal cavity and on the 5-7th day--for perforation of the compound places in hollow organs. Antibacterial therapy was conducted for progressing peritonitis, duration of which depended on dynamics of the peritonitis course. The complications rate reduction was promoted by conduction of an active postoperative sanation of abdominal cavity and potent antibacterial therapy. One patient died because of the wounding severity.

RESUMO

Frequency of chronic gastroduodenal motility disorders and possibilities of ultrasound diagnostics of biliopancreatoduodenal zone in patients with gastroduodenal ulcers were studied. 95 patients, aged 20-78 years, were included in the study. Changes of echomorphology of the liver, gall bladder and pancreas reflected the severity of ulcer disease. Duodenal motility disorders defined the secondary nature of pathologic changes in involved biliopancreatic structures.

RESUMO

An analysis of results of surgical treatment of chronic duodenal obstruction in 95 patients with duodenal and gastric ulcer was carried on. The method of operation was selected for every patient according to the phase of disease. The Strong method of operation + the resection of a gastric stomach on Hofmeyster-Finsterer method were selected for compensated phase of chronic duodenal obstruction in patients with duodenal and gastric ulcer. The antiperistaltik duodenojejunostomy on Y.D. Vitebski + the resection of a gastric stomach on Hofmeyster-Finsterer method were selected. For sub- and decompensate phase of chronic duodenal obstruction. Long-term results were studied in 80 patients. In compensated phase excellent and good long-term results were noted in 56 patients and unsatisfactory results were noted in 2 patients. In sub- and decompensate phase excellent and good long-term results were noted in 21 patients and unsatisfactory result was noted in 1 patients.

We have investigated the role of lipid per oxidation in pathogenesis of hepatic enzymatic dysfunction during peritoneal intoxication. The investigations were carried out on 36 albino rats. I group were included intact animals, II group were included animals that peritoneal exudates reached with products of lipid per oxidation were injected to abdominal cavity and III group were included reached with animals that superoxiddismutase together with products of lipid per oxidation were injected to abdominal cavity. In all group of animals the concentration of AST, ALT, LDH, alkaline phosphates, GGT enzymes were studied and the results were compared. It was revealed that after injection peritoneal exudates to abdominal cavity, the concentration of AST is decreased and the other (ALT, LDH, alkaline phosphates, GGT) concentration are increased. In further days the blood concentration of all enzymes reached to maximal level. Additionally the injection of superoxiddismutase solution significantly decreases the concentration of these enzymes. It confirms the role of superoxiddismutase in prevention of poliorgan incipiency developing in peritonitis, as well ass in correction of hepatic dysfunction.

RESUMO

The treatment of diabetic patients with pyonecrotic lesions in the lower extremities requires prolonged reflexo-segmental balneophysiotherapy to normalize functions of the nerve centres. Curative volcanic mud solutions (15-24 g/l) enriched with organic and nonorganic biologically active compounds from volcanic deposits in Azerbaijan were used for the first time to treat such patients. Peloids were applied to the lumbar region (location of sympathetic nerve nodes) and the lower legs above and beneath the affected sites. The patients were subjected to 12-15 seances of peloidotherapy (at 40-41 degrees Celsius) each lasting 20-30 minutes. The treatment was preceded by wound sanation using the standard procedure and a course of antibiotic therapy based on individual antibiotocograms. A total of 86 daibetic patients with leg gangrene underwent rheovasographic thermovision examination that revealed enhanced blood supply to the affected extremities under the action of the applied peloids. Peloidotherapy resulted in the normalization of blood and urine glucose levels in 53 (63%) of the patients. Simultaneously, the doses of medicamentous therapy could be lowered. Wound and ulcer healing was completed in the majority of the patients (86%) by the end of balneophysiotherapy when fresh granulation tissue began to develop and signs of oedema to disappear. These patients no longer needed amputation.
