RESUMO

In the current qualitative research study, we focused on understanding the ecological systems, contexts, behaviors, and strategies of parents ( N = 435) advocating for their children with an intellectual and developmental disability diagnosis, specifically Down syndrome (DS). Based on the data analysis, parents of children with DS advocate for their children frequently, in a variety of settings, with different actions, attitudes, motivations, and outcomes. The most common settings where advocacy occurred were primarily school and healthcare systems. The goals of parents often included inclusiveness, equality, and acceptance, whereas a few parents reported advocating due to discrimination and judgment. Implications for further research and professional practice also are described.

RESUMO

The present study explored the heterogeneity of military service members' psychological and relational functioning using a sample of 7,866 soldiers in committed romantic relationships from the Army Study to Assess Risk and Resilience in Service members (Army STARRS). A latent profile analysis was conducted to identify and predict unique clusters of soldiers' relative psychological (i.e., posttraumatic stress disorder, depression, and anxiety symptom severity) and relational (i.e., relationship happiness, how often their relationship was going well, how often they confided in their partner, how often they considered or discussed ending their relationship, relational insecurity, and relational turbulence) functioning. Results revealed 4 distinct classes of comparative psychological and relational health: hardy soldiers and healthy relaters (70.2%), stressed soldiers but healthy relaters (13.1%), hardy soldiers but strained relaters (12.7%), and stressed soldiers and strained relaters (4.0%). Class descriptions, as well as implications for theory and clinical practice are reviewed. Notably, 7 out of 10 soldiers in this nationally representative sample reported little to no psychological or relational distress. These findings invite more adaptive narratives of service member resilience and a more nuanced conceptualization of gradations in psychological and relational functioning. Parenting a child with Down syndrome may pose unique challenges for parents' relationship quality. This study used structural equation modeling with a sample of 351 mothers of children with Down syndrome to test if hope mediated the association between mothers' various coping behaviors and mothers' relationship quality. Hope was defined as a generalized positive state that comes from a personal sense of agency. Results indicated a greater degree of religious coping and internal coping were each significantly associated with more hope, whereas support seeking was not related with more hope. Higher hope was significantly associated with greater relationship quality. Bootstrapped indirect effects from both religious coping and internal coping to hope, and then hope to relationship quality, were identified. Implications for therapists and future research are described.

Suicide among United States active-duty Army soldiers rapidly increased over the past two decades. Using a sample of 322 soldiers from the Army STARRS study, the researchers examined if romantic relationship factors (i.e., hostile disagreements and relationship distress) were linked with suicidal thoughts in Army soldiers, and if these associations were moderated by a recent separation or divorce. Hostile disagreements and relational distress were both significantly associated with higher rates of suicidal ideation. These associations were significantly amplified in magnitude when in the context of a recent separation or divorce. Implications include novel assessment, prevention, and treatment efforts focused on romantic relationships that may reduce the likelihood of soldiers experiencing thoughts of suicide.

We assessed a brief, systemic retreat-style intervention that was developed to address concerns about the utilization of services for veterans coping with traumatic stress. A total of 76 dyads (N = 152) were assessed before and after a 4-day retreat, which included psychoeducation, group and conjoint therapeutic sessions, and recreational relaxation components. Overall, participants reported a reduction in trauma symptoms, but only support persons experienced a significant increase in posttraumatic growth from pretest to posttest. Both veterans and their romantic partners reported an increase in relationship adjustment after the retreat. Opportunities to address the needs of this population by removing barriers to treatment and reducing feelings of isolation, as well as implications for similar treatments are discussed.

This study explored the preliminary experiences of parents upon learning of their child's diagnosis of Down syndrome. Qualitative data from a web-based, national survey were analyzed based on two groups: prenatal (n  =  46) or postnatal (n  =  115) diagnosis. Three primary categories emerged from the data analysis: prenatal screening/testing decisions by parents, the adjustment process for parents, and postdiagnosis resources and support for parents. Participants' rationale behind pursuing testing ranged from wanting to be better prepared to not pursuing testing because it was not a factor in continuing the pregnancy. Participant reactions to the diagnosis involved a range of intense preliminary emotions; participants described their extreme grief and loss experience at the initial news of the diagnosis, which also was ambiguous in nature and required differing timelines of adjustment. Finally, participants described experiences with medical professionals, information/education, and faith/religion as resources and areas of support, although not all were described as positive in nature. Participants in both groups identified having negative experiences with medical professionals during the diagnosis process. The results indicated the importance of these early experiences for parents of children with Down syndrome and emphasize providing effective education, resources, and practical information from reliable sources.

RESUMO

This study investigated the comprehensiveness of disaster mental health state plans and their adherence to published best practices in three states that experienced post-9/11 federally-declared disasters. There were 59 disaster mental health best practices used in this study to assess each state disaster mental plan's compliance with best practices; the states demonstrated a range of adherence to the best practices. This research may serve as a guide for those developing disaster mental health plans and encourage further considerations in disaster mental health response.

RECENT EMPHASIS ON THE ETHICAL CONDUCT of researchers has resulted in a growing body of literature exploring the impact of trauma-focused research on participants. To date, pregnant women have not been widely included in trauma-focused research, possibly because they are considered a vulnerable population in research. The current research investigated how 41 expectant mothers responded to participation in a trauma-focused study. Overall, the results of this study suggest that trauma-focused research is well tolerated by pregnant women. Specific findings indicated that pregnant women considered more traumatized perceived greater benefits from participation. Best practices for conducting ethical trauma-focused research with pregnant women are provided. As well, recommendations for future research are outlined in an effort to further extend the ethical understanding of the benefits and costs of trauma-focused research with potentially vulnerable populations.
