RESUMO

Since the occurrence of the chloroquino-resistance, chemoprophylaxis for all is not anymore the sound principle to malaria prophylaxis for travellers and expatriates. Protection against malaria has now to be based on comprehensive actions (chemoprophylaxis, control of infecting bites, treatment of malaria cases as soon as first symptom occur), they have to be combined, as a whole or not, according to the area, the duration and the type of tropical stay, and even sometimes according to some parameters peculiar to an individual. The development of concepts concerning the epidemiology of human malaria and the use of antimalarial drugs, either as protective or curative, lead more and more to the necessity for any traveller or expatriate to take medical advice from a specialized physician.

RESUMO

A new case of primary biliary cirrhosis associated with sarcoidosis is reported in a 38 year-old woman. The diagnosis of sarcoidosis was based on the occurrence of cutaneous nodules, mediastinal lymph node enlargement, pulmonary reticular infiltrate, parotitis and diffuse arthritic pain, all disappearing after steroid therapy. Hepatic biopsy showed epithelioid-cell granuloma without necrosis. The diagnosis of primary biliary cirrhosis was made 6 years later as based on intrahepatic cholestasis associated with high titer of antimitochondrial antibodies and chronic destructive cholangitis. This association (5 other cases have already been reported) raises the problem of a possible link between the two diseases which could represent different patterns of a single immunological disorder.

RESUMO

Thirty-two patients have been hospitalised in Marseilles since 1979 for recurrent oedema following their return from a tropical region. In spite of strong suspicion, the diagnosis of Loa loa was confirmed only five times. One patient was diagnosed with filariasis due to Mansonella perstans. The diagnosis of loiasis was usually presumptive and there were often wide variations in the laboratory findings. Two patients were thought to have an allergic reaction to diethylcarbamazine. Another patient was felt to have an allergy to streptococcal antigen. One patient who had lived in the Extreme Orient presented with an inflammatory fasciitis. Two patients presented with neurological manifestations: one with eosinophilic meningo-encephalitis, one with a neuropathy of the median nerve. In one case, the oedema was attributed to congenital lymphoedema.

RESUMO

Three different types of education and teaching are offered by the Education and Research Center in Tropical Health and Medicine: To 7th and 8th year students in medicine, given morning and afternoon over a period of 13 weeks, this type of education in tropical health and medicine requests a high degree of participation from the student; it is very well structured in its objectives and program with more than 50% of directed studies and guided training. Evaluation is carried out both in and at the end of sylabuses. To male and female nurses, teaching is given morning and afternoon over a period of 5 weeks. From year to year, the increased number of participants attests the success of such a teaching. To students at the end of their University programmes, whatever they may be. This multidisciplinary education is given over a period of two years, twice a month, late in the afternoon.

RESUMO

A case report of trypanosomiasis (T. rhodesiense), observed in a French tourist is described. The clinical picture associated a cytolytic hepatic jaundice, a thrombopenia with hemorrhagic manifestations, a renal insufficiency and secondarily a neutropenia with anemia. Moderate and transitory anomalies in the cerebrospinal fluid were noted. The cure was effectuated by administration of pentamidine.

RESUMO

The authors present 6 cases of müllerian papillary adeno-fibromata (or mixed mesodermal müllerian benign tumours) of which 4 are anatomo-clinical presentations and 2 are histological presentations, as well as 3 cases of müllerian adenosarcoma (mixed müllerian mesodermal tumour with stroma only sarcomatous). The study of these cases added on to those found in the literature leads to the following conclusions: The first group can be accepted. The second group will be of mild malignancy. All the same, 4 of which the sarcomatous nature is not in doubt at all (malignant from the start) recurred rapidly. Those in which the sarcomatous nature was attenuated or doubtful (15 cases) seemed to have a much longer evolution with some local recurrences. Only 3 patients have been followed up for more than 5 years. It is absolutely essential to carry out further observations before confirming for certain the prognosis of this new type of tumour.

RESUMO

The study of nine cases of "eosinophilic lung" using specific immunological techniques indicates that the latter completes the geographical, clinical, radiological and hematological criteria of this syndrome. The search of circulating antibodies confirms the filarial etiology and individualises a remarkable immunological profile thus, suggesting a diagnosis in the case of occult filariases. The intensity of the immunological reactions brings a supplementary argument in favor of the hypothesis of a poor host-parasite relationship in the pathogeny of secondary manifestations to an infestation by W. bancrofti, B. malayi and other animal filariae such as B. pahangi.
