[Results of Palliative Resection in an Elderly Colorectal Cancer Patient].

With the aging of society, surgery for elderly colorectal cancer(CRC)patients is also increasing. We examined 11 elderly CRC patients who underwent palliative resection in our institute. The reasons other than age for which palliative resection was chosen, included dementia, basic disease, and social backgrounds such as living alone, etc. Although surgery was possible according to the ECOG PS and other examinations before surgery, 3 patients(27.3%)who developed respiratory or circulatory complications after surgery died in the hospital. From the viewpoint of retrospective P-POSSUM evaluation, unreasonable surgical decisions were not made. However, recovery was difficult once complications occurred in the subject group. The postoperative hospital stay, excluding inpatient deaths, was over 1 month due to rehabilitation, discharge adjustment, etc. Therefore, palliative treatment other than surgery should be considered for elderly CRC patients.

[A Case of P0CY1 Gastric Cancer without Recurrence for 13 Years Postoperatively].

We report a case of P0CY1 gastric cancer that is successfullytreated with gastrectomyand S-1/Lentinan combined chemotherapyand has not recurred for long time postoperatively. Case: A 70-year-old woman. The patient was diagnosed with advanced gastric cancer at the upper stomach. We performed total gastrectomywith D2 lymph node dissection. The patho- logical stage was pT4aN2M1P0CY1H0, pStage Ⅳ. Postoperative chemotherapywith S-1/CDDP(CDDP 80mg/bodyweight, S-1 80mg/bodyweight)was administered. Considering that the patient did not well-tolerate the treatment, particularlydue to myelosuppression, we shifted to S-1 monotherapy. Myelosuppression was prolonged even after stopping the chemothera- py; hence, we administered S-1/Lentinan(S-1 80mg/bodyweight , Lentinan 2mg/time)combined chemotherapy. At present, no signs of recurrence were observed for 13 years postoperatively.
