OBJECTIVE: To study the associations of nine genetic variants with the risk and dynamics of recovery (outcome) of ischemic stroke (IS) using the developed protocol for the search for genomic markers based on a bioinformatic approach to the study of single nucleotide polymorphisms (SNPs) in human orthologues of rat genes differentially expressed under conditions of induced cerebral ischemia. MATERIAL AND METHODS: We identified and analyzed nine SNPs in 553 Russians (331 IS patients and 222 controls). The National Institutes of Health Stroke Scale (NIHSS) was used to assess stroke severity. Functional recovery after stroke was assessed using the modified Rankin scale (mRS). The principles of selection of polymorphic markers analyzed in the study were determined according to the protocol developed by us earlier. Selected SNP tags were genotyped using real-time polymerase chain reaction (PCR) TaqMan. RESULTS: The relationship of SNP with both the risk of IS and the dynamics of its recovery was investigated. SNP rs66782529 (LGALS3) was associated with negative IS outcomes (p=0.048). SNPs rs62278647 and rs2316710 (PTX3) were significantly associated with IS risk (p=0.000029 and p=0.0025, respectively). The associations for rs62278647 and rs2316710 were found only in females, suggesting a gender-related PTX3 polymorphism. CONCLUSION: This study not only reveals some new genetic links to IS and its consequences, but also shows how the study of gene variations in a rat model of cerebral ischemia can be useful in the search for genetic markers of this disease in humans.

RESUMO

Stroke is a heterogeneous syndrome, and the definition of risk factors, treatment and prevention tactics depends on the specific pathogenesis of the disease. Risk factors for ischemic and hemorrhagic stroke are similar, but at the same time there are significant differences. The concept of stroke triggers is discussed separately. In addition, a deep understanding of the pathogenetic mechanisms and the development of new strategies for therapy and prevention require an understanding of the genetic mechanisms of stroke risk. Genetic factors may be more modifiable than previously thought. To reduce the burden of stroke in the population, timely identification and management of modifiable risk factors is essential.

OBJECTIVE: The aim of the observational program was to study the effect of the use of the drug recognan (citicoline) on the state of higher mental functions (memory, attention, visual-motor coordination, dynamic praxis,verbal thinking and imagination) in patients with mild cognitive impairment. MATERIAL AND METHODS: A survey of 54 subjects (16 of them male and 38 female) aged 18-50 years (average age 28.5±10.5 years) was conducted with a diagnosis of «Mild cognitive impairment¼ («F06.7¼). The group was randomized into 2 subgroups: the main subgroup (26 people) received oral therapy with the drug Recognan, for 30 days, with the daily dosage of the drug being 500 mg. In the control group (28 people), nootropic drug therapy was not performed. Standard psychometric techniques were used to study higher mental functions. All subjects were examined three times (initially, in the middle of the study - on day 15, at the end of the study - on day 30). RESULTS AND CONCLUSION: After 2 weeks of treatment with recognan, there was an improvement in concentration in 81.9%, memory in 50% (p=0.008), verbal imagination productivity in 68.2% (p=0.015), counting functions in 60% (p=0.015), visual-motor coordination and dynamic praxis - in 86.4% (p=0.003), increased speed and efficiency of mental work (p=0.001). After a 30-day course of treatment with recognan, there was an improvement in memory in 58.3% of patients (p=0.007), an increase in concentration in 64%, an improvement in counting functions in 64.3% (p=0.011), verbal imagination productivity in 63.3%, visual-motor coordination and dynamic praxis in 86.4% (p=0.007), speed and efficiency of mental work (p=0.006), which indicates a complex positive effect of recognan on higher mental functions in patients with mild cognitive impairment cognitive impairment.

The article presents the results of psychological study of characteristics of food behavior in young people in the conditions of forced self-isolation and COVID-19 coronavirus infection pandemic. The common research methods were applied to establish actual food disorders and intensification of their symptoms during isolation. The food behavior of each individual can be considered as network of interactions between various natural factors (physiological, psychological, economic, sociocultural ones). It is conditioned by stress level due to pandemic. The comparative analysis of study results concerning specifics of food behavior in conditions of self-isolation in young and middle-aged people demonstrated that there is significant relationship between external, emotionogenic food behavior, restrictive food behavior and age of subjects. The correlation analysis established statistical significance of a number of scales. In sample of middle-aged objects, relationships between external and emotionogenic food behavior were established. The emotionogenic condition was associated with dissatisfaction with one's own body, and body image - with restrictive food behavior. The direct statistical significance of dependence of restrictive behavior from age was revealed. In all cases, more intensive manifestation of food disorders was detected in sampling of young people that confirms hypothesis that middle-aged people are inclined to other types of food disorders as compared with young people. The severity of disorders is also less pronounced. The actuality of the analyzed problem is determined by both trends in development of scientific knowledge and existing needs of social practice.

AIM: To study the efficacy of recognan (citicoline) in the treatment of cognitive, emotional, autonomic, asthenic disorders in patients with asthenic syndrome. MATERIAL AND METHODS: Thirty-eight subjects (17 men and 41 women), mean age 27.75±12.05 years, with asthenic syndrome (ICD-10 F48.0 - Neurasthenia) were studied. The sample was randomized into 2 subgroups: the main group (n=20) received oral therapy with recognan (100 mg in 1 ml) for 30 days, while the daily dosage was 500 mg (5 ml). In the control group (n=18), no drug therapy was performed. Patient's state was assessed with a large battery of psychological tests and psychometric scales. The follow-up period was 30 days. All participants were examined three times (initially, in the middle of the study (day 15), in the end of the study (day 30)). RESULTS AND CONCLUSION: Asthenic syndrome was detected in 100% of the patients (total asthenia was noted in 70%, decreased activity in 70%, decreased motivation in 40%, physical asthenia in 45%, mental asthenia in 50%). Recognan (citicoline), used for 2 weeks or one month, has a positive effect on the compensation of autonomic, asthenic cognitive and emotional disorders and increases stress resilience of the patients.

AIM: To study an influence of polymorphic variants of hemostasis system genes on the risk of ischemic stroke (IS) in patients of the Slavic population under the age of 50 years. MATERIAL AND METHODS: Ninety-two patients (19 women and 73 men), aged 18-50 years, were examined. The diagnosis of stroke was confirmed by neuroimaging (CT or MRI) in all patients. Polymorphic alleles of GP1BA, F2, F5 were studied by a real-time polymerase chain reaction using the TaqMan technology. RESULTS AND CONCLUSION: An analysis of the GP1BA -5T/C polymorphism showed that it was associated with IS in young men, lacunar stroke and stroke due to thrombosis of the brachiocephalic arteries. This association was not found in young women. The F5 G1691A polymorphism was associated with lacunar stroke. The F2 G20210A polymorphism was associated with stroke due to thrombosis of the brachiocephalic arteries.

The sample was randomized into 2 groups (29 each): the main sub-group received oral therapy with recognan for 30 days, while the daily dosage was 500 mg (5 ml). In the control group, no drug therapy was performed. Patient's state was assessed with a large battery of psychological tests and psychometric scales. The follow-up period was 30 days. All participants were examined three times (initially, in the middle of the study (day 15), in the end of the study (day 30)). RESULTS AND CONCLUSION: Asthenic state was detected in 69%, autonomic dysfunction in 82.7%, sleep disorders in 75.8% of subjects. A high level of state anxiety was revealed in 22.2%, trait anxiety in 29.8%, a high level of stress was determined in 6,9%. It is shown that the use of recognan (citicoline) has a positive effect on compensation of autonomic and asthenic disorders, improves cognitive status, corrects psycho-emotional disorders, when used as a short (2 week) and month course.

RESUMO

The main treatment option for rectal cancer is surgery, which "gold standard" is the total mesorectumectomy. There are presented literature review and the results of own research devoted to comparative analysis of outcomes of laparoscopic and open total mesorectumectomy. Current data and own experience show the oncological adequacy and safety of laparoscopic approach however the controversy of some results reveal the necessity of further investigation.

A comparative sequence analysis of the LEA (Late Embryogenesis Abundant (LEA)-like gene) intron fragment was performed in Pinus sibirica and P. pumila differing in geographic origin. It was demonstrated that in P. sibirica this fragment was represented by two types of PCR products, 224 and 202 bp in size. Similarly, in accessions of P. pumila, two PCR products of 224 and 159 bp in size were identified. Comparison of 224 bp fragments in P. sibirica and P. pumila showed that they differed in single nucleotide substitutions. Analysis of the intron fragment in a plant, which was characterized as an interspecific hybrid based on morphological characters, showed that it had fragments of 224 and 159 bp in size. The sequence of 224 bp fragment was similar to that of the corresponding fragment in P. sibirica. The structure of the short fragment was the same as the structure of the corresponding fragment in P. pumila. The data obtained are discussed in terms of the use of the sequences examined for species taxonomic classification and of an analysis of species hybridization.

The Siberian stone pine (Pinus sibirica Du Tour) is one of the main forest-forming coniferous species of the boreal ecosystems of Western Siberia. We used the isozyme method to analyze 11 ecotypes representing the latitudinal and longitudinal profiles within the species range, including samples from the geographic boundaries of the distribution. The genetic structure of the ecotypes is described on the basis of the variability for 26 isozyme loci encoding for 16 enzyme systems. The greatest genetic diversity was observed in the taiga ecotypes in the central part of the studied area, while the ecotypes along the species range boundaries were shown to be genetically depauperized. Approximately 8.1% of the observed genetic diversity is attributed to differences between the studied ecotypes. We detected high levels of genetic diversity for the Fest_2, Pgm_1, Sod_4, and a few otherloci, as well as a correlation between allele frequencies and geographical locations of the populations. The results of multivariate analysis of allelic frequencies as well as cluster analysis allowed us to discriminate three major groups of ecotypes: north-eastern, central and south-western. In view of our results, we compare two hypotheses: one which attributes the spatial distribution of genetic variations to the selectivity for some of the polymorphic allozyme loci, and the other based in the history of the formation of the range of the Siberian stone pine.

Authors studied 198 patients with first-ever cerebral stroke. Affective disorders, including depression, generalized anxiety disorder (GAD) and phobias, were identified in 71.2% of patients. The study demonstrated that the anxiety syndrome developed in the post-stroke period may be regarded as relatively independent. Some post-stroke anxiety disorders had different risk and pathogenetic factors. GAD is associated with young age and phobias are associated with female sex and inherited predisposition. In contrast to post stroke depression, anxiety-phobic disorders develop more frequently in patients with mild stroke.

An aim of the study was to evaluate clinical features and risk factors of post stroke apathy. The study included 209 patients with first-time stroke. Assessments of neurological and mental status were carried out on the 1st, 7th day, after 2 weeks, 1 month, 3, 6 and 12 months after stroke. The frequency of apathy was 28.2%. Three types of apathy were singled out: classical, depressive, and combined. Apathy was significantly associated with the older age of patients, right-sided brain lesion and worst recovery of neurological deficit. Patients with depressive apathy were significantly older than patients without apathy, classic apathy was more common in men, and depressive apathy - in women. Depressive apathy was significantly associated with small lesions, while classic or combined apathy was significantly associated with the average volume of lesions.

RESUMO

We have studied 198 patients with first-ever stroke. The spectrum of affective disorders included depressions, generalized anxiety disorder, phobias. The study has demonstrated that mentioned post stroke affective disorders have different risk and pathogenetic factors. In stroke patients, affective disorders were caused by the damage of different brain areas. Depression was associated with the damage of left frontal/temporal cortical areas and left subcortical structures. On the contrary, anxiety disorders were associated with the damage of these structures in the right hemisphere.

Platelet serotonin content in patients in the acute period of stroke is an important index of clinical changes during the post stroke period as well as a predictor of development of mental disorders. We studied the association between two polymorphisms (5-HTTLPR and Val66Met BDNF) and the platelet serotonin content in 47 patients with stroke. We also investigated the moderating effect of genetic variants on the association between platelet serotonin content and development of affective and anxiety disorders in stroke patients in the acute period of stroke. The interaction effect of two polymorphisms on levels of platelet serotonin was found. The lowest level was observed in patients with the diplotype LL*ValVal, the highest level--in the group of patients with the LL genotype and genotypes containing at least one copy of a Met allele. No moderating effect of genetic variants on the relationship between serotonin content and affective or anxiety disorder was found.

RESUMO

Fifty patients with affective disorders developed after first cerebral stroke were studied. A spectrum of affective disorders included post-stroke depressions, generalized anxiety disorders, anxiety-depressive disorders and pseudo post-stroke depressions. Platelet serotonin (PS) levels were measured in these patients as well. The levels of PS at the first three days after stroke and their changes towards the 28th day are associated with the development of post-stroke affective disorders. The development of anxiety-depressive disorders in the acute period of stroke is related to the localization of brain lesion focus. The low levels of PS in patients at the first three days after stroke may be the risk factor of delayed affective disorders development.

RESUMO

One hundred and forty-nine patients with ischemic stroke who had different affective disorders developed one year after stroke (depression, anxiety, depression-anxiety disorders) were included in the study. Based on the computer data analysis of EEG, we singled out the characteristics of bioelectrical brain activity in patients with different types of poststroke affective disorders in acute, early and late restorative periods.
