RESUMO

Affording of qualified and specialized medical attendance in military hospitals is possible only in condition of presence of medical techniques and medical equipment in indispensable quantity. The units of medical supply have as a task to provide military hospitals by medical equipment. Nowadays the task of reoutfit of military hospitals by new medical equipment is one of the most significant for the medical service of Armed Forces. This task is prearrangely carrying into effect on the base of advanced program. The main purpose of this program is elevating of comprehensibility and quality of medical attendance. More, reoutfit of military hospitals permits effectuate a high-quality medical attendance, and permits medical service to participate in important national project "Health". The order fixed 215 norms and standards, including 1524 items of medical equipment. The price of medical equipment, included in the norms of supply of military-medical institutions of Armed Forces, is 18 mlrd of rubles.

The aim of the study was to compare the hypotensive effect and safety of carvedilol, a non-selective alpha-beta blocker, and monopril, an angiotensin-converting enzyme inhibitor (ACEi), in patients with arterial hypertension and a chronic renal disease. The main group (MG), 26 patients, was treated with carvedilol, which was administered as a hypotensive agent instead of the ACEi. The control group (CG) included 24 patients on a "standard" therapy with hypothiazide 12.5 mg a day, amlodipine 10 mg a day, and monopril 20 mg a day. Patients who received carvedilol displayed an earlier hypotensive effect (HE). Together with HE realization, the patients of both groups displayed a moderate increase in glomerular filtration (not more than by 10 ml/min), and an increase in creatinine blood level (not more than by 15%) during the first two weeks, which can be explained by a decrease in intraglomerular hypertension. A significant decrease in total cholesterol level and a more pronounced regress of left ventricular myocardial hypertrophy were noticed in the MG vs. the CG. Measurement of the serum levels of cortisol, aldosterone, and angiotensin I showed that the patients of the MG displayed a significant decrease in aldosterone level together with a clear tendency of cortisol level increase. The concentrations of these hormones in CG patients did not change significantly. The result demonstrate that prolonged administration of carvedilol leads to a noticeable decrease in the serum level of aldosterone, an increase in cortisol level, and electrolyte changes, such as an increase in sodium level and a decrease in potassium level in blood plasma.

The data of the examination and treatment of 1150 sick servicemen in different conditions of service in Republic of Chechnya in the military conflict in 1995-1996 are investigated. It was found out that the principal somatic pathology in the structure of sanitary losses of servicemen in the military conflict was respiratory organ diseases, mainly pneumonia, whose etiology as a rule does not depend on the character of the military-and-professional activities and places of troops' distribution. The chief role in the etiology of pneumonia was played by Streptococcus pneumoniae (43.4%). The peculiarities of the clinical picture of pneumoniaduring fighting are stipulated by chronic adaptation overstrain syndrome. Traditional schemes of treatment for such pneumonia do not ensure recovery of the sick within the usual period and do not prevent the development of various complications.

To evaluate the efficiency of iodcasein in prophylaxis of iodine-deficient states in servicemen the following parameters were studied: iodine sufficiency determined by ioduria median; thyroid volume measured on the base of US data; functional state of hypothalamo-pituitary-thyroid system evaluated by blood level of thyroid hormone and free thyroxin; physical performance assessed by veloergometric test, auto-and-verbal memory and attention change-over. Iodine deficiency was initially detected in 88.9% (main group) and in 94.4% (control group). Ioduria median corresponded to iodine deficit of middle severity in both groups. After the initial examination the persons from the main group were given iodized bread during 9 months. The monitoring results have confirmed the efficiency of iodine prophylaxis with the use iodcasein.

RESUMO

The authors analyze the incidence of infectious endocarditis (IE), the influence of diagnostic time on the lethality, the significance of surgical treatment. The main ways of further improvement of IE patient therapy are defined. They include the training of medical staff, centralization of the patients in the military medical institutions of the center by transfer from the districts after making the diagnosis in order to conduct the adequate medicamentous and appropriate surgical treatment.
