RESUMO

Pica is a psychological disorder of intentional and craving consumption of non-nutritive substances over a period of time. This is seen at an age when such a behavior is developmentally inappropriate. Substances such as wall paint, soil, hair, and feces have been reported as being consumed. Complications of this condition may range from being self-limiting to life-threatening. Radiological examinations play a crucial role in the diagnosis and management of this condition. We present a case of a 9-year-old boy who presented with chronic abdominal pain with history of persistent consumption of pencil erasers. Abdominal radiographs showed radio-opaque foreign bodies, and etiological diagnosis was made when the radiologist obtained a detailed history from the patient's mother. We also discuss the radiographic evaluation of the pencil eraser and the reason why it is densely radio-opaque.

RESUMO

The incidence of extrathoracic tuberculosis (ETB) continues to increase slowly, especially in immunocompromised and multidrug-resistant tuberculosis (TB) patients. ETB manifests with nonspecific clinical symptoms, and being less frequent, is less familiar to most physicians. Imaging modalities of choice are computed tomography (lymphadenopathy and abdominal TB) and MR imaging (central nervous system and musculoskeletal system TB). ETB commonly involves multiple organ systems with characteristic imaging findings that permit accurate diagnosis and timely management.

BACKGROUND: India had a population of 1,259,271,000 in 2012, with 31% of its population composed of children younger than 15 years. In comparison, children younger than 15 compose 20% of the population in the United States and 16% in Canada. Despite the differences in this demographic, little emphasis is placed on pediatric radiology in India. OBJECTIVE: To conduct a needs assessment evaluating the adequacy of pediatric radiology in India and to establish a pediatric radiology education program in India. MATERIALS AND METHODS: We developed a questionnaire to assess radiologists' perspective on the quality of training in pediatric radiology. Responses were obtained from attendees at a pediatric radiology education program in Mumbai. These data were used to obtain funding and implement a program to increase awareness and stimulate pediatric radiology training at select institutions across India. RESULTS: Out of 86 respondents, 82% indicated that their academic institutions did not place emphasis on dedicated pediatric radiology training, and 63% indicated they received less than 2 weeks of dedicated training. Of the respondents, 77% said their institutions practiced pediatric radiology with inadequate standard of care and 75% of respondents said pediatric radiology would gain increasing importance in the future. Outcomes of the implemented program included increased awareness of pediatric radiology and establishment of a pediatric radiology fellowship program in India. CONCLUSION: Education and training in pediatric radiology in India is inadequate. Focused initiatives have the potential to improve the standards set for pediatric radiology in India. Similar initiatives could help develop pediatric radiology in other developing countries.

Complications related to the use of new diagnostic and therapeutic techniques are inherent to innovation in medicine. Appropriate consent should be obtained before subjecting patients to these techniques. In spite of doing this, when a complication does occur, one can easily relate to its devastating impact on the patient and his/her relatives. The toll that such events take on the treating physician also needs to be considered. The burden of conscience when a patient consents to such a procedure with implicit faith in the physician is immense. A case of irreversible paraplegia due to non-target embolisation of the anterior spinal artery in a young lady undergoing bronchial artery embolisation for hemoptysis is discussed. A feeling of "sadness and guilt" and a scientific analysis of the cause for the adverse event result in changing the protocol of the procedure leading to increased safety for future patients. Wide consultation with colleagues and help from institutional review boards are useful in assuring the treating physician about the justification of performing such procedures and help in coping should complications occur. The manner in which these events are managed, especially by medical teachers in teaching hospitals, is an important learning point for medical students and doctors in training. The need for appropriate open forums in institutions is emphasised, where such events are shared by physicians, resulting in unburdening themselves and potentially in education for all present.

There is a growing need for introducing objective structured clinical examination (OSCE) as a part of radiology practical examinations in India. OSCE is an established, reliable, and effective multistation test for the assessment of practical professional skills in an objective and a transparent manner. In India, it has been successfully initiated and implemented in specialties like pediatrics, ophthalmology, and otolaryngology. Each OSCE station needs to have a pre-agreed "key-list" that contains a list of objective steps prepared for uniformly assessing the tasks given to students. Broadly, OSCE stations are classified as "manned" or "unmanned" stations. These stations may include procedure or pictorial or theory stations with clinical oriented contents. This article is one of a series of measures to initiate OSCE in radiology; it analyzes the attributes of OSCE stations and outlines the steps for implementing OSCE. Furthermore, important issues like the advantages of OSCE, its limitations, a strengths, weaknesses, opportunities, and threats (SWOT) analysis, and the timing of introduction of OSCE in radiology are also covered. The OSCE format in radiology and its stations needs to be validated, certified, and finalized before its use in examinations. This will need active participation and contribution from the academic radiology fraternity and inputs from faculty members of leading teaching institutions. Many workshops/meetings need to be conducted. Indeed, these collaborative measures will effectively sensitize universities, examiners, organizers, faculty, and students across India to OSCE and help successfully usher in this new format in radiology practical examinations.
