RESUMO

Coccidioidomycosis is a systemic fungal infection first described in 1892. More than 95% of annual cases occur in Arizona and California. It is an opportunistic infection (OI) transmitted via inhalation of airborne spores (arthroconidia) and rarely via percutaneous inoculation into a tissue or solid organ transplantation in patients who are immunocompromised and with HIV. With the advent of antiretroviral therapy (ART), the incidence of OIs has markedly reduced; however, OIs continue to occur, particularly in patients who present late for medical care or delay ART initiation. In rare cases, immunodeficient individuals may experience a paradoxical worsening or unmasking of OI symptoms, known as the immune reconstitution inflammatory syndrome (IRIS). We present a case of a 31-year-old man with disseminated coccidioidomycosis affecting the spleen, lymph nodes, lungs, bone marrow, and adrenals who developed IRIS after the initiation of ART.

RESUMO

The current rampant coronavirus infection in humans, commonly known as COVID-19, a pandemic that may cause mortality in humans, has been declared a global emergency by the World Health Organization (WHO). The morbidity and mortality rates due to the pandemic are increasing rapidly worldwide, with the USA most affected by the disease. The source COVID-19 is not absolutely clear; however, the disease may be transmitted by either by COVID-19-positive individuals or from a contaminated environment. In this review, we focused on how the COVID-19 virus is transmitted in the community. An extensive literature search was conducted using specific keywords and criteria. Based on the published report, it is concluded that COVID-19 is primarily transmitted human-to-human via oral and respiratory aerosols and droplets with the virus-contaminated environment play a lesser role in the propagation of disease. Healthcare providers and the elderly with comorbidities are especially susceptible to the infection.

Abstract: Thyroid nodules are cell growths in the thyroid which might be for in one of two categories benign or malignant. Nodular thyroid disease is common and because of the associated risk of malignancy and hyper-function; these nodules have to be examined thoroughly. Hence diagnosing thyroid nodule malignancy in the early stage can mitigate the possibility of death. This paper presents an intelligent thyroid nodules malignancy diagnosis using texture information in run-length matrix derived from 2- level 2D wavelet transform bands (approximation and details). In this work, ANOVA test has been used to for feature selection to reduce for feature selection about 45 run-length features with and without wavelet generated, before feeding those features which clinical importance to the Support Vector Machine(SVM) and Decision Tree (DT) classifier to perform the automated diagnosis. The validation of this work is activated using 100-thyroid nodule images spliced equally between the two categories (50 Benign and 50 Malignant). The proposed system can detect thyroid nodules malignancy with an average accuracy of about 97% using SVM classifier for the run- length matrix, features derived from spatial domain while the average accuracy is increased to 98% in case of hybrid feature derived from spatial domain and 2-level wavelet decomposition. For the other proposed classifier (DT), the average accuracy in case of spatial domain based features is 93% whereas the average accuracy of the hybrid features system is 97%. Hence the proposed system can be used for the screening of thyroid nodules.

The production of cellulase by Trichoderma reesei RUT C-30 under solid-state fermentation (SSF) on wheat bran and cellulose was optimized employing a two stage statistical design of experiments. Optimization of process parameters resulted in a 3.2-fold increase in CMCase production to 959.53IU/gDS. The process was evaluated at pilot scale in tray fermenters and yielded 457IU/gDS using the lab conditions and indicating possibility for further improvement. The cellulase could effectively hydrolyze alkali pretreated sorghum stover and addition of Aspergillus niger ß-glucosidase improved the hydrolytic efficiency 174%, indicating the potential to use this blend for effective saccharification of sorghum stover biomass. The enzymatic hydrolysate of sorghum stover was fermented to ethanol with ∼80% efficiency.
