RESUMO

STATEMENT OF PROBLEM: Three-dimensional (3D) printing of cobalt chromium (Co-Cr) removable partial dentures (RPDs)by selective laser melting (SLM) has been claimed to be less challenging than by conventional casting and provides significant improvements. However, the adaptation and fit of the SLM framework and the optimum build orientation are still unclear. PURPOSE: The purpose of this in vitro and clinical study was to evaluate the effect of different build orientations on the adaptation of removable partial denture frameworks fabricated by SLM technology in vitro and to compare the adaptation of the SLM and conventional RPD frameworks clinically. MATERIAL AND METHODS: A master model simulating a maxillary arch of Kennedy class III modification 1 was scanned and duplicated to create a virtual 3D cast and reference cast. Four groups (n=40) of Co-Cr RPD frameworks were fabricated. For the SLM groups, the Co-Cr framework was virtually designed and exported for SLM printing. The SLM printing was done in 3 different build orientations: 0-degree (n=10), 45-degree (n=10), and 90-degree (n=10) groups. Other Co-Cr frameworks were conventionally cast (n=10). All Co-Cr frameworks were scanned and virtually superimposed with the master model using a surface-matching software program. The gap under 9 selected points in the palatal major connectors was analyzed and calculated. A smaller gap indicates more surface adaptation and close contact between the palatal major connector and the master model. The data were analyzed using the Kruskal-Wallis and Dunnett T3 tests (α=.05). Three patients with a partially dentate maxillary arch were enrolled in the clinical part based on inclusion criteria. Two RPD frameworks were provided for each patient (conventional casting and SLM printing). The adaptation of each framework was assessed by measuring the gap between the palatal major connector of the framework and the palate with light-body silicone. The differences in adaptation between the conventional and SLM frameworks were compared by using independent t tests (α=.05). RESULTS: The in vitro study identified significant differences in the adaptation of the palatal major connector among the 4 groups (P<.001), except for conventional and 0-degree SLM printing (P=.999). The conventional and 0-degree SLM frameworks exhibited the best adaptation, with the lowest gap underneath the palatal major connector of the RPD framework (0.01 ±0.02 mm and 0.01 ±0.01 mm, respectively). In the clinical part of the study, no significant difference was found between the adaptation of SLM and the conventional framework (P=.430) CONCLUSIONS: The adaptation of SLM printing can be maximized with less gap under the palatal major connector of the RPD framework when using the 0-degree build orientation. Co-Cr frameworks produced with SLM printing technology were comparable with conventionally produced frameworks; hence, SLM printed frameworks can be an alternative for clinical applications when optimum SLM parameters are applied.

BACKGROUND: The present systematic review and meta-analysis investigated the available evidence about the adherence of Candida Albicans to the digitally-fabricated acrylic resins (both milled and 3D-printed) compared to the conventional heat-polymerized acrylic resins. METHODS: This study followed the guidelines of the Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA). A comprehensive search of online databases/search tools (Web of Science, Scopus, PubMed, Ovid, and Google Scholar) was conducted for all relevant studies published up until May 29, 2023. Only in-vitro studies comparing the adherence of Candida albicans to the digital and conventional acrylic resins were included. The quantitative analyses were performed using RevMan v5.3 software. RESULTS: Fourteen studies were included, 11 of which were meta-analyzed based on Colony Forming Unit (CFU) and Optical Density (OD) outcome measures. The pooled data revealed significantly lower candida colonization on the milled digitally-fabricated compared to the heat-polymerized conventionally-fabricated acrylic resin materials (MD = - 0.36; 95%CI = - 0.69, - 0.03; P = 0.03 and MD = - 0.04; 95%CI = - 0.06, - 0.01; P = 0.0008; as measured by CFU and OD respectively). However, no differences were found in the adhesion of Candida albicans between the 3D-printed digitally-fabricated compared to the heat-polymerized conventionally-fabricated acrylic resin materials (CFU: P = 0.11, and OD: P = 0.20). CONCLUSION: The available evidence suggests that candida is less likely to adhere to the milled digitally-fabricated acrylic resins compared to the conventional ones.

Accuracy is a necessity in implant impressions to fabricate accurately fitting implant-supported prostheses. This in vitro study aimed to explore the impact of the number of scan bodies on scanning quality by comparing scans of 2 vs 4 implants, and to determine if their accuracy and precision meets acceptable clinical threshold. Two mandibular edentulous models were used: one with 4-parallel implants (4-IM) and the other with 2-parallel implants (2-IM). Each model was scanned 10 times with an intraoral scanner, while reference scans were obtained with a high-precision laboratory scanner. The accuracy of test scans was evaluated by superimposing them onto reference scans and measuring 3D and angular deviations of the scan bodies. To assess the precision, the repeatability of the scans was analyzed by measuring the 3D SDs. Independent t test was used to compare angular deviations, the Mann-Whitney U test was used for 3D deviations and 3D SDs, and 1 sample t test was used for comparing means to the clinical threshold. Angular and 3D deviations were statistically not significant between the 2 groups (P = .054 and 0.143). 3D deviation values were higher than the 150-µm threshold for 2-IM (201 µm) and 4-IM (290 µm); angular deviation in 2-IM was 0.600 degrees and 0.885 degrees for 4-IM. There was no statistically significant difference in the precision of scans between the 2 groups. (P = .161). Although scanning quality improved when 2 scan bodies were used, the difference was not statistically significant. Moreover, full-arch implant scanning did not meet acceptable levels of accuracy and precision.

BACKGROUND: Photodynamic therapy (PDT) has been recently proposed as a promising alternative therapy for Denture Stomatitis (DS). The present systematic review and meta-analysis investigated the current available evidence regarding the efficacy of PDT in the management of DS. MATERIALS AND METHODS: PubMed, Scopus, Web of Science, Google Scholar, and ProQuest were searched up to June 7, 2023. All relevant clinical trials were included. RevMan software was used for the statistical analyses. RESULTS: Elven randomized clinical trials (460 DS patients) were included. Eight studies assessed the efficacy of PDT vs. topical antifungal therapy, while three studies assessed the adjunctive use of PDT (PDT + antifungal therapy) vs. topical antifungal therapy alone. The results revealed comparable efficacy of PDT and conventional antifungal therapy on candida colonization at 15 days (MD: 0.95, 95% CI: -0.28, 2.19, p = 0.13) and at the end of follow-up (MD: -0.17, 95% CI: -1.33, 0.98, p = 0.77). The pooled two studies revealed relatively better efficacy of adjunctive use of PDT with antifungal therapy on candida colonization compared to antifungal therapy alone at 15 days (MD: -6.67, 95% CI: -15.15, 1.82, p = 0.12), and at the end of follow-up (MD: -7.14, 95% CI: -19.78, 5.50, p = 0.27). Additionally, the results revealed comparable efficacy of PDT and topical antifungal therapy on the clinical outcomes. CONCLUSIONS: PDT might be considered a viable option for DS either as an adjunct or as an alternative to the topical antifungal medications. Further studies with adequate sample sizes and standardized PDT parameters are warranted.

STATEMENT OF PROBLEM: Intraoral scanning of implants supporting complete arch prostheses is limited because of the lack of geometric heterogeneity and unique reference points, creating inherent errors in the image stitching process by the scanner software program. PURPOSE: The purpose of this in vitro study was to evaluate the significance of geometric heterogeneity on complete arch implant scanning by using a novel auxiliary geometric device. Three different clinical simulations were tested to assess its significance. The study also assessed whether scans produced using the auxiliary device would meet a clinically acceptable threshold. MATERIAL AND METHODS: A total of 60 scans (n=20) were performed using an intraoral scanner in 3 different clinical simulations: 2 parallel implants, 4 parallel implants, and 4 implants with a 30-degree posterior angulation of the distal implants. Scanning alternated between using the auxiliary geometric scanning device (test groups; 4IP+, 4IA+, 2IP+) and not using the device (control groups; 4IP-, 4IA-, 2IP-). A reference scan for each model was prepared from a high precision laboratory scanner. The scans were analyzed for accuracy in 3-dimensional deviation, interimplant distance deviation, and angular deviation by using an inspection software program. The effect of the auxiliary device was statistically analyzed by comparing scans of the same group using the paired t test for normally distributed data and the Wilcoxon Signed Rank test when data were not normally distributed (α=.05). RESULTS: Significant effects of the auxiliary geometric device were found in 3-dimensional, distance and angular deviations (P<.05). Scans performed using the device were significantly more accurate in most implant positions (P<.05). Linear and angular deviations were clinically acceptable for all test groups. However, the deviations were above the clinically acceptable threshold for the control groups. CONCLUSIONS: Using an auxiliary geometric device significantly improved scanning accuracy and produced scans with clinically acceptable deviations, while standard digital scans exceeded the accepted clinical threshold.

BACKGROUND: A possible relationship between periodontitis (PD) and COVID-19 and its adverse outcomes has been suggested. Hence, the present systematic review and meta-analysis aimed to investigate the available evidence regarding the potential association between periodontitis (PD) and COVID-19 and its adverse outcomes. MATERIALS AND METHODS: PubMed, Scopus, Web of Science, and Google Scholar were searched for relevant studies published up to April 15th, 2023. Studies that evaluated the association between PD and COVID-19 were included. Risk of bias was evaluated by two reviewers, and meta-analyses were performed using RevMan 5.3 software. RESULTS: A total of 22 studies involving 92,535 patients from USA, Europe, Asia, the Middle East and South America were included; of these, 12 were pooled into the meta-analysis. Most of the studies (19 studies) reported a significant association between PD and COVID-19. The pooled data found a significant association between PD and COVID-19 outcomes: more severe symptoms (OR = 6.95, P = 0.0008), ICU admissions (OR = 3.15, P = 0.0001), and mortality (OR = 1.92, P = 0.21). Additionally, compared to mild PD, severe PD was significantly associated with higher risks of severe COVID-19 outcomes: severe symptoms (P = 0.02); ICU admission (P = 0.0001); and higher mortality rates (P = 0.0001). The results also revealed 58% higher risk for COVID-19 infection in patients with PD (P = 0.00001). CONCLUSIONS: The present findings suggest a possible association between poor periodontal health and the risk of poor COVID-19 outcomes. However, owing to the observed methodological heterogeneity across the included studies, further prospective cohort studies with standardized methodologies are warranted to further unravel the potential association between periodontal disease and COVID-19 and its adverse outcomes.

STATEMENT OF PROBLEM: Attaining a passive fit in implant restorations is desirable but clinically difficult to achieve, especially in screw-retained prostheses. At a certain magnitude, this misfit will not cause mechanical and biological complications, but the exact level has yet to be determined. PURPOSE: The purpose of this systematic review was to gather, compare, and appraise studies that attempted to determine the biological and mechanical tolerance of misfits. MATERIAL AND METHODS: The review protocol was published in the Prospective Register for Systematic Reviews (PROSPERO; registration no. CRD42021268399) and follows the Preferred Reporting for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. An electronic search was conducted through PubMed, Ebscohost, and Web of Science followed by a manual search up to December 2021. RESULTS: A total of 413 manuscripts were identified by electronic and manual search. After removing duplicates, nonrelevant titles, and abstract screening, 62 manuscripts were eligible for full-text assessment. Finally, a total of 13 articles (1 cross-sectional study, 1 retrospective and prospective, 7 in vitro studies, and 4 animal studies) met the eligibility criteria and were included in this review. A wide range of tolerable misfits were reported. Vertical misfit up to 1 mm and horizontal misfit up to 345 µm were associated with no adverse outcomes. CONCLUSIONS: The current literature provides inadequate data to determine a clinical threshold of an acceptable misfit. However, this review demonstrated that the mechanical response to misfit is more critical than the biological response.

STATEMENT OF PROBLEM: Additive manufacturing by selective laser melting (SLM) has been claimed to be less challenging than conventional casting of cobalt-chromium (Co-Cr) removable partial dentures (RPDs), providing significant improvements. However, how the physicomechanical properties of Co-Cr RPDs fabricated by SLM compare with those fabricated by conventional casting is unclear. PURPOSE: The purpose of this in vitro study was to evaluate the physicomechanical properties of Co-Cr RPD palatal major connectors fabricated by SLM compared with those fabricated by conventional casting. MATERIAL AND METHODS: A master die simulating a maxillary arch of Kennedy class III modification 1 was scanned to create a virtual 3-dimensional (3D) cast. Two groups of 5 Co-Cr RPD major connectors were fabricated. In the 3D printing group, the Co-Cr major connector was virtually designed and exported for direct SLM 3D printing. For the conventional group, Co-Cr major connectors were constructed conventionally. The Co-Cr major connectors were virtually superimposed with the master die for surface adaptation analysis. Additional comparative analyses of surface roughness, relative density, microhardness, and microstructure of the 2 groups were performed. Data were analyzed by using independent t tests (α=.05). RESULTS: The overall volumetric and linear discrepancies were significantly higher (P<.05) in the 3D printing group. Significant differences in the surface roughness (P<.05) and microhardness (P<.05) were observed, with the 3D printing group having higher surface roughness and microhardness than the conventional group. Unlike conventional connectors, the microstructure of 3D-printed connectors showed fine homogeneous granules. CONCLUSIONS: Compared with the conventional casting technique, SLM 3D printing enabled the fabrication of Co-Cr RPD major connectors with higher microhardness and fine homogenous microstructure. However, the surface adaptation and surface roughness of SLM 3D printing Co-Cr connectors were worse than those produced conventionally. Both techniques showed similar relative densities.

PURPOSE: To develop a new digital method for color-mixing analysis and to evaluate the validity of this method for quantifying masticatory performance. MATERIALS AND METHODS: Specimens of red-green (RG) chewing gum were prepared as a bicolor test food. A total of 300 specimens were masticated by 20 healthy volunteers for different numbers of mastication cycles (from 1 to 25). The boluses were flattened and scanned, and the digital images were analyzed using ImageJ software. Two parameters (spatial and value) of color mixing were measured, and multiple regression analysis was performed to estimate the number of mastication cycles. The estimated number of mastication cycles that the healthy reference cohort needed to achieve a certain degree of color mixing was proposed as the mastication index (MI). The validity of this method was assessed using Pearson correlation between the MI and concurrent measurements with ViewGum software (variance of hue) within a group of 10 healthy subjects and 10 complete denture wearers. RESULTS: Independent samples t test showed a significant difference in MI between healthy subjects and denture wearers (P < .001). A significant correlation was observed between the MI and ViewGum outcomes (r = -0.95, P < .001). CONCLUSION: The new proposed method proved to be valid and has the potential for evaluating masticatory performance in both research and clinical settings.

Application of ozone is recommended for sterilisation in dental procedures. This study explored the antimicrobial effect of 0.1 ppm ozonated-water on selected common oral commensals. Based on deviation of their growth curves pattern upon ozone treatment, the inhibitory effect of ozone was determined. SEM examination of the ozone-treated microbes recorded its possible morphological effect. Findings suggested a bacteriostatic action of ozone when microbes were treated at the early phase, while, it was bactericidal when treated during the active phase of the growth cycle. Hence, suggesting rinsing the oral cavity with ozonated-water at 0.1 ppm immediately after tooth brushing may suppress microbial growth and slow biofilm formation. While, rinsing on already developed biofilm may result in microbial cell lysis that halted microbial growth and reduce microbial population in the biofilm. Both justify the great potential of ozone (0.1 ppm) for use as antimicrobial agent for the control of biofilm development in the oral cavity.

PURPOSE: The purpose of this study was to evaluate and compare Oral Health-Related Quality of Life (OHRQoL), denture satisfaction, and masticatory performance in edentulous patients provided with mandibular implant-supported overdentures (ISODs) retained with telescopic attachments and those of conventional complete dentures (CCDs). Peri-implant soft tissue changes were also evaluated at various intervals during a 1-year observation period. MATERIALS AND METHODS: Participating patients received new CCDs and later received two mandibular interforaminal implants and had their mandibular CCDs converted into ISODs with telescopic attachments. Questionnaires were used to assess OHRQoL (Shortened Oral Health Impact Profile-14, Malaysian version) and denture satisfaction at different stages of treatment with CCDs and ISODs. Objective masticatory performance with the CCDs and ISODs was recorded with a mixing ability test. Evaluations were carried out at 3 months with the new CCDs, 3 months after mandibular ISOD provision, and 1 year after receiving the ISOD. Peri-implant parameters were additionally assessed at specific intervals during the treatment period. The data obtained were statistically analyzed and compared. RESULTS: In the 17 patients who completed the protocol, significant improvements were observed in OHRQoL and patient satisfaction when CCDs were modified to ISODs, after 3 months, and at 1 year. Significantly better mixing ability with the ISOD was noted, with the highest values observed at 1 year. Statistically insignificant differences were observed for all the peri-implant parameters, except for gingival recession, for which significant changes were observed 6 months after ISOD delivery (values had stabilized by 1 year). CONCLUSION: Telescopic crown attachment-retained mandibular ISODs improved OHRQoL, dental prosthesis satisfaction, and masticatory performance compared to CCDs. Peri-implant soft tissue response and implant stability were found to be favorable after 1 year.
