RESUMO

A school-based oral health promotion program was established on a Bahamian island that lacks access to dental care. An interprofessional team of dental hygiene (DH) and occupational therapy (OT) students and faculty delivered an annual service program from 2016 to 2018 based on the Ecological Model of School-Based Health Promotion. The interprofessional team provided oral preventative services and class-based lessons to children to reinforce positive oral health behaviors. Efforts to sustain the program involved partnering with the Bahamian community stakeholders to expand networks of influence, developing effective communication systems, and integrating oral health into the school curriculum. The interprofessional approach enabled a sustained and comprehensive program. This report describes the unique DH and OT interprofessional collaboration and program sustainability efforts.

OBJECTIVE: The purpose of this study was to understand the age-related changes, adaptations, and contexts that support older workers in the manufacturing sector. METHOD: In a multiple-case study, 10 older workers from two manufacturing companies completed the Job Content Questionnaire, Health and Productivity Questionnaire, and semistructured interviews and underwent task observations for 8 hr over 4 mo. RESULTS: Age-related changes affecting job tasks included near vision, upper extremity strength, grip strength, joint stiffness, and endurance. Older workers adapted job tasks by changing their work style, using equipment available, and relying on close coworkers for assistance. They adapted their personal lifestyle to maintain the energy necessary to meet job demands. Organizational contexts that supported older workers highlighted job control, social support, and mutual respect. CONCLUSION: This study suggests that occupational therapy practitioners can provide individualized adaptations, environmental modifications, and health promotion strategies to help older workers meet job demands while balancing work and home occupations.

OBJECTIVE: The purpose of the study was to examine the efficacy of a multidisciplinary train-the-trainer model for improving fitness and food label literacy in third-grade students. DESIGN: University student trainers taught ABC for Fitness and Nutrition Detectives, established programs to promote physical activity and nutrition knowledge, to 239 third-grade students in 2 communities over a 6-month period. A total of 110 children were in the intervention group and 129 children in the control group (2 schools each). Outcomes included the Food Label Literacy and Nutrition Knowledge test and the fitness measures of curl-ups, push-ups, 0.5-mile run, and sit and reach. Focus groups were conducted as process feedback. SETTING: Four public schools in 2 different communities. PARTICIPANTS: A total of 200 third-grade students. INTERVENTION: ABC for Fitness and Nutrition Detectives. MAIN OUTCOME MEASURES: Food Label Literacy and Nutrition Knowledge test and the fitness measures of curl-ups, push-ups, 0.5-mile run, and sit and reach. RESULTS: Nutrition knowledge increased in the intervention group by 25.2% (P < .01). Fitness measures in the intervention schools showed greater improvement than those in the controls for curl-ups (P < .01), push-ups (P < .01), sit and reach left (P = .07), and 0.5-mile run (P = .06). Process feedback from 3 teachers and 60 students indicated satisfaction with the program. CONCLUSION: Adaptation of the train-the-trainer approach for Nutrition Detectives and ABC for Fitness was effective for delivering these health-related programs.

PURPOSE: To understand the medication routines used by older adults taking four or more medications daily. METHOD: One hundred forty-nine community-dwelling older adults were interviewed about the individual routines, storage locations, equipment, and assistance that enabled their adherence to a medication regimen. A subsample of 84 older adults was observed completing one medication routine in their home environments. FINDINGS: Medication habits were embedded in mealtime, wake-up, and sleep routines for 91% of the sample. Participants developed unique, individualized behaviors for taking medications that were choreographed within broader daily routines. The primary locations for storing medications were the kitchen and bathroom. Equipment used to promote adherence was primarily pillboxes or self-made adaptations. More than 50% of the entire sample required some type of assistance related to medication adherence. IMPLICATIONS: Findings support the role of occupational therapists in collaborating with clients to develop individualized medication routines to promote medication adherence.

Older workers are the fastest growing segment of the labor force, yet little is known about designing jobs for older workers that optimize their experiences relative to aging successfully. This study examined the contribution of workplace job design (opportunities for decision-making, skill variety, coworker support, supervisor support) to dimensions of successful aging (social network, emotional support, personal control, generativity) in a sample of 109 older workers in the retail homebuilding industry. Older workers completed four standardized tests reflecting the job design and successful aging variables. Results indicated that job design contributed to 23% of the variance in generativity and 15.5% of the variance in personal sense of control scores. Skill variety and coworker support were the most important job design variables for successful aging outcomes. The potential for workplaces to become social institutions that contribute to dimensions of successful aging is discussed.

Occupational stress represents a cause of attrition from the field of dental hygiene. However, few studies have examined occupational stress in dental hygienists with a focus on the dynamics of the organization and the concept of emotional labor. The purpose of the study was to investigate the nature of occupational stress (organizational stress and emotional labor) in experienced dental hygienists and to examine the relationship between occupational stressors and manifestations of stress (or personal strain). Two hundred invitations were sent to randomly selected dental hygienists in the Northeast. Only 40 dental hygienists met the inclusion criteria for the study and 30 individuals participated in the study. Participants completed a demographics questionnaire, an Emotional Labor Survey, the Occupational Stress Inventory, and a Musculoskeletal Discomfort Body Map. Overall, levels of occupational stressors, personal strain, and coping resources were within norms. The sample indicated high frequency of musculoskeletal discomfort in the neck (87%), upper back (63.3%), lower back (63.3%), shoulders (53%), and wrists (36.7%). Moderate correlations existed among occupational stressors related to organizational ambiguity, emotional labor, personal strain, and musculoskeletal discomfort. Emotional labor emerged as an occupational stressor that may warrant future investigation.

RESUMO

The increasing complexity of the dental office environment influences productivity and workflow for dental clinicians. Advances in technology, and with it the range of products needed to provide services, have led to sprawl in operatory setups and the potential for awkward postures for dental clinicians during the delivery of oral health services. Although ergonomics often addresses the prevention of musculoskeletal disorders for specific populations of workers, concepts of workflow and productivity are integral to improved practice in work environments. This article provides suggestions for improving workflow and productivity for dental clinicians. The article applies ergonomic principles to dental practice issues such as equipment and supply management, office design, and workflow management. Implications for improved ergonomic processes and future research are explored.

Increasing numbers of older blue collar workers are remaining in the workforce. However, the effect of age-related changes on older workers' job performance is not well documented. This study examined how older blue collar workers continue to perform physically demanding jobs as they age. The research question was: How do older maintenance workers adapt to their job tasks given age-related changes in functioning? Qualitative case studies were conducted on two older workers: a Custodian and a Mechanic. Workers completed a Musculoskeletal Body Map and participated in multiple semi-structured interviews and work observations over a two month period. Triangulated data revealed that older workers self-initiated job adaptations for age-related and medical conditions that impacted their jobs. The most common job adaptations were means to reduce the physical demands of their jobs. Three other contextual themes emerged relative to the context of work and older adulthood: job content ("what's important in a job"), generational differences, and the broader role of work in older adulthood ("you can't separate home and work"). Findings are discussed relative to theories of successful aging and adaptation. Recommendations for managing and promoting successful aging for older workers are provided based on study results.

BACKGROUND: Caring for one's children is among the most ubiquitous of occupations. However, few studies have examined the ergonomic risks involved in parents caring for children at home. PURPOSE: The purpose of this study was to identify the frequency, type, and severity of musculoskeletal symptoms in parents of children less than 4 years old. The study further examined the factors that contribute to musculoskeletal pain in this sample. METHODS: A convenience sample of 130 parents with children younger than 4 years old completed a seven-page survey that included questions related to the parents' demographics, their musculoskeletal discomfort, their performance of child-care tasks with high biomechanical risks (such as carrying a child in a car seat), and parents' perceived psychological strain related to caring for their children. RESULTS: Ninety-two percent (92%) of the providers were mothers. Sixty-six percent (66%) of the sample reported the presence of musculoskeletal pain. The parts of the body most affected were the low back (48%), neck (17%), upper back (16%), and shoulders (11.5%). Factors associated with musculoskeletal pain were performing child-care tasks defined as having high biomechanical risks (p = .001), the perception that caring for children is highly demanding (p = .003), and performing hobbies less than 1 hour per week (p = .04). Parents' working status, age, and participation in other daily activities were not significantly related to musculoskeletal discomfort. CONCLUSION: This study demonstrates the high prevalence of musculoskeletal pain in parents of children under the age of 4 years. It underscores the association between physical and psychological factors in the development of musculoskeletal discomfort. It suggests the need for occupational therapy wellness programs that focus on preventing musculoskeletal discomfort and providing support for the parenting role.
