RESUMO

Background: Fatigue Avoidance Scheduling Tool (FAST) is a computerized fatigue prevention tool, which was developed based on a Bio-mathematical model called SAPTE (Sleep, Activity, Performance and Task Effectiveness). Similarly, actigraphy technology is used as a tool for sleep evaluation. This study was an attempt to assess the employability of FAST (both independently and integrated with actigraphy) for detecting operational fatigue by determining the 'Measures for diagnostic accuracy'. Methods: 103 pre-flight medical checks from 51 aircrew were evaluated to detect fatigue both clinically and by using objective measures (FAST and actigraphy integrated FAST). The 'measures of diagnostic accuracy' namely sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), likelihood ratio positive (LR +ve), likelihood ratio negative (LR -ve) and Youden's Index were determined and compared. Results: The diagnostic measures for FAST were: sensitivity (68.4%), specificity (92.8%), PPV (68.4%), NPV (92.8%), LR+ve (9.5), LR-ve (0.34) and Youden's Index (0.61). For actigraphy integrated FAST these values were: sensitivity (84.2%), specificity (96.4%), PPV (94.2%), NPV (94.4%), LR+ve (23.5), LR-ve (0.16) and Youden's Index (0.80). Conclusion: The increase in sensitivity (from 68.4% to 84.2%), specificity (from 92.8% to 96.4%), PPV (from 68.4% to 94.2%), NPV (92.8% to 94.4%), LR+ve (9.5 to 23.5) and Youden's Index (0.61 to 0.80) indicated that actigraphy integrated FAST is a better screening tool in comparison to the independently employed FAST.

BACKGROUND: Studies from the Indian Navy have found junior sailors to have higher occupational stress than senior sailors. Positive social support can provide protection against detrimental effects of stress and facilitate in development of individual resilience. Therefore, the present study was conducted to explore the level of perceived social support among naval personnel. METHODS: A cross-sectional survey was conducted using "Interpersonal Support Evaluation List" (Cohen and Hoberman, 1983) among personnel posted on a capital ship of the Indian Navy. Five hundred sixty-eight sailors voluntarily participated in the study. RESULTS: Personnel living on board (in-living) were found to have significantly lower overall perceived social support score (76.14 ± 13.72) than personnel living in family accommodation (79.40 ± 14.14). In addition, in-living personnel were also found to have lower subscale scores viz "appraisal support", "self-esteem support", and "belonging support" (p < 0.05) but not "tangible support" score (p > 0.05). CONCLUSION: Our study brings out that young in-living sailors lack perceived social support. These young in-living sailors, being away from their families, often feel isolated and fall prey to social media, which in turn aggravates feeling of isolation and leads to psychological distress. Therefore, it is important for these individuals to have confidence in his mess mates and belief in commanders who form their primary sources of social support. To ensure this, it is the need of the hour to revitalize the age old divisional system by introduction of concepts of "Keep Your Mates Safe", "promotion of psychological resilience", and so on.

BACKGROUND: Fine needle aspiration cytology (FNAC) assisted with scrotal ultrasonography is the best preoperative diagnostic modality for palpable epididymal nodules. It also aids in their successive remedial approach as well as serves semi-therapeutically in cystic lesions. The objectives of this study are to recognize the spectrum of pathological conditions giving rise to epididymal nodules, then to compare them with corresponding ultrasound images, and to evaluate the histological features wherever practicable. METHODS: Total 62 patients underwent FNAC as well as sonographic evaluation for their epididymal nodules. Histopathology was performed in only 20 cases. RESULTS: Epididymitis either caused by tuberculosis (30.6%), or in its acute (11.3%) and chronic (8.1%) forms remained the commonest cytological diagnosis. Neoplastic lesions included mostly adenomatoid tumors (8.1%), and another case of seminomatous spread from ipsilateral testicular primary. Nineteen of the excised masses corroborated with their respective cytodiagnoses. The discrepant lesion was actually a papillary cystadenoma, which was cytologically misinterpreted as adenomatoid tumor. CONCLUSIONS: FNAC becomes the first-hand investigative measure for epididymal nodules, by virtue of its early, easy and highly accurate diagnostic implications. It segregates the patients into proper therapeutic protocol and thereby estranges those who really need operative management. When deployed together with ultrasound, the diagnostic accuracy of FNAC improves further.

RESUMO

Implantation of prosthetic vascular grafts is sometimes necessary to create vascular access as AV conduits for maintenance haemodialysis. Though more common in the western world, it is now also becoming an important option in our country. Here we summarise our experience with the procedure from 2001 to 2011. The technical configuration which seems to work better in India is different from what is commonly performed in the west. After a short review of the relevant basic sciences, our results are discussed and showed 95% one year graft patency. Conclusions drawn are objective and stresses on the need to increase awareness amongst all doctors about the importance of preserving forearm veins in patients with chronic kidney disease.
