Injuries of the foot are common among both elite and recreational runners. Overuse accounts for most of these injuries. Plantar fasciitis and tendinopathies of the midfoot and forefoot have a high incidence in running athletes. These injuries may present with significant pain but often resolve with rest and rehabilitation. Bone injuries caused by overuse also have a high prevalence among runners. The metatarsals, tarsal navicular, and sesamoids are most at risk for stress damage. Most running injuries are self-limited and pose little detriment if diagnosis is delayed. Navicular and sesamoid stress fractures may impart significant long-term consequences, and thus, a clinical suspicion of either fracture warrants definitive diagnosis and treatment. Barefoot running recently has garnered increased attention, but currently, there is a lack of prospective studies regarding its injury reduction.

Athletically caused sacral stress fractures represent an uncommon reason for low back pain. The clinician must consider this possible diagnosis when evaluating patients with nonradicular low back or buttock pain. Women distance runners are more commonly afflicted. Athletically caused sacral stress fractures are most often unilateral. This case report presents an illustrative case of bilateral sacral stress fracture occurring in a male runner.

This article reviews the literature regarding gastrointestinal disturbances specifically in runners. Upper gastrointestinal problems such as reflux, nausea, vomiting, and gastritis are reviewed. Exercise has been found to alter esophageal motility. These symptoms worsen with increased exertion as well. H(2) blockers have been found helpful for reflux and nausea. The lower intestinal problems of motility and blood loss are discussed. These problems are generally more common and more directly related to running. Also reviewed are endotoxin involvement and transient abdominal pain. Much of the literature deals with marathon and ultramarathon running, showing the extreme effects of running and exertional stress on the body; however, recreational runners may not be so afflicted as these study participants.
