RESUMO

Temporomandibular joint ankylosis (TMJA) secondary to congenital infiltrating lipomatosis of the face (CILF) is an exceptionally uncommon condition which is characterized by the involvement of unilateral facial soft tissues and bones. In some cases, the extensive exophytic bony growth in the temporomandibular joint region often extends toward the skull base and lies near adjacent vital structures. Only very few cases of TMJA associated with CILF have been reported in the literature. The authors report a case of a 36-year-old female with right TMJA secondary to CILF. The bony overgrowth in the right temporomandibular joint region was arising from a deformed right mandibular condyle, extending towards the ipsilateral temporal bone, greater wing of the sphenoid, skull base, and approaching the lateral limit of foramen ovale, and foramen spinosum. To prevent any damage to the skull base and adjacent vital structures and to achieve adequate mouth opening, the authors have performed a unique technique of subankylotic osteotomy for the release of TMJA, instead of conventional gap arthroplasty.

BACKGROUND: Oral squamous cell carcinoma (OSCC) is known for its aggressive behavior and the high potential for locoregional recurrence (LRR), contributing to poor prognostic outcomes. The aim of this study was to investigate the role of histologic parameters in predicting LRR in patients with OSCC. MATERIALS AND METHODS: A retrospective analysis was performed on 58 OSCC patients treated between January 2018 and December 2022. Data were collected from medical records, focusing on demographics, clinicopathologic features, and treatment details. Different histopathologic factors such as depth of invasion, tumor stage (T), pathologic node stage (N), histologic grade of differentiation, perineural invasion, lymphovascular invasion, extranodal extension (ENE), and margin of resection were correlated with LRR. RESULTS: Out of 58 patients, 20 (34.4%) reported LRR within the first year of follow-up. In the recurrence group, 14 patients succumbed to death within 24 months. Among all the histopathologic parameters, our study found a statistically significant correlation between higher pathologic node stage, presence of ENE, and closest margin of resection (≤5 mm) with LRR. CONCLUSION: Higher pathologic node stage, presence of ENE, and closest margin of resection (≤5 mm) were the histopathologic factors associated with LRR, and can serve as deciding prognostic factors. Treatment intensification in early-stage disease with higher pathologic nodal stage, presence of ENE, and closest margin of resection (≤5 mm) may improve survival outcomes.

BACKGROUND: Of all body regions, lower extremity wounds have been and remain the greatest challenge. Free flaps have been accepted as a reasonable option to solve this dilemma, but they require the complexity of microsurgery. This study aimed to compare the postoperative and clinical outcomes of propeller flap and fasciocutaneous free flap in the reconstruction of complex lower limb defects. MATERIALS AND METHODS: This randomized controlled trial was conducted from July 2021 to June 2023. Selected patients were randomized into two groups: the propeller flap group and fasciocutaneous free flap group. Demographic data, preoperative parameters, and postoperative parameter, including esthetic analysis, scar assessment, neurosensory analysis, psychosocial analysis, and lower extremity function, were estimated and statistically significant compared between the two study groups. RESULTS: Road traffic accident (73.3%) was the most common etiology for lower limb defects in both groups. The foot and lower third of the leg were the most common site of defect, constituting 79.99%. The duration of surgery was significantly shorter in propeller flap as compared to fasciocutaneous free flaps. Flap size was smaller, with better neurosensory improvements in propeller flap group. Esthetic analysis, scar assessment, psychosocial analysis, and lower extremity functional analysis yielded similar results in both study groups. CONCLUSION: Propeller flap has better outcomes compared to free flap in terms of smaller size of flap, shorter duration of surgery, and reduced length of hospital stay. Comparable results were obtained with respect to esthetic score, scar assessment score, psychosocial analysis, and neurosensory recovery and lower extremity functional score.

BACKGROUND: Among the various local and regional flaps used for the reconstruction of intraoral defects after oral cavity cancer surgery, the pedicled buccal fat pad (BFP) flap is considered a reliable alternative. Reconstruction with a BFP flap is particularly feasible in oral cavity areas, such as the posterior maxilla, buccal mucosa, retromolar trigone, and gingivobuccal sulcus. The purpose of this study was to analyze the outcomes of pedicled BFP as a primary reconstruction flap for small to medium intraoral posterior oral cavity postresection defects operated in our institute. METHODS: This study was designed as a retrospective study. Thirty-seven patients with oral cavity cancer underwent wide local excision with adequate margins, followed by reconstruction of the defect with a pedicled BFP flap. Defect size, postoperative healing of the surgical site, mouth opening at 6 months, and any associated complications were assessed. RESULTS: The mean age of the patients was 47.38±9.95 years with a male-to-female ratio of 3.6:1. Mean defect size at the greatest dimension was 5.01±1.39 cm. The mean preoperative mouth opening of patients was 35±6.4 mm, whereas the mean postoperative mouth opening after 6 months of follow-up was 27.8±9.2 cm. CONCLUSION: The BFP flap is a convenient, reliable, and feasible reconstruction modality after oncological resection of posterior oral cavity cancers. Harvesting a BFP flap is minimally invasive with fewer complications, shorter recovery time, no donor site morbidity, and allows early initiation of adjuvant therapy postoperatively.

Cervical atresia is a rare congenital Müllerian duct anomaly that manifests as the absence or deformed nonfunctional presence of the cervix. Herein, a multi-layered biodegradable stent is fabricated using a homogeneous blend of silk fibroin with polycaprolactone using hexafluoroisopropanol as a common solution. Briefly, a concentric cylinder of 3D honeycomb layer is sandwiched within electrospun sheets for fixing at the cervico-uterine junction to pave the way of cervical reconstruction. An average length of 40 mm with 3 mm diameter is fabricated for the hybrid stent design. SEM evidences an evenly distributed pore architecture of the electrospun layer, and mechanical characterization of stent reveals a tensile strength of 1.7 ± 0.2 MPa, with a Young's modulus of 5.9 ± 0.1 MPa. Physico-chemical characterization confirms the presence of silk fibroin and poly caprolactone within the engineered stent. Following 14 days of pepsin enzymatic degradation, 18% degradation and a contact angle measurement of 97° are observed. In vitro cytocompatibility studies are performed using site-specific primary human cervical squamous, columnar epithelial cells, and human endometrial stromal cells. The study demonstrates non-cytotoxic cells' viability (no significant toxicity), improved cell anchoring, adherence among the stent layers, and proliferation in the 3D microenvironment. Furthermore, in vivo subcutaneous studies in the rodent model indicate that the implanted stent undergoes constructive remodeling, neo-tissue creation, neo-vasculature formation, and re-epithelialization while maintaining patency for 2 months.

Microplastic, which is of size less than 5 mm, is gaining a lot of attention as it has become a new arising contaminant because of its ecophysiology impact on the aquatic environment. These microplastics are found in freshwater or drinking water and are the major carriers of pollutants. Removal of this microplastic can be done through the primary treatment process, secondary treatment process, and tertiary treatment process. One approach for microplastic remediation is ultrafiltration technology, which involves passing water through a membrane with small pores to filter out the microplastics. However, the efficiency of this technology can be affected by the structure and type of microplastics present in the water. New strategies can be created to improve the technology and increase its efficacy in removing microplastics from water by knowing how various types and shapes of microplastics react during ultrafiltration. The filter-based technique, that is, ultrafiltration has achieved the best performance for the removal of microplastic. But with the ultrafiltration, too some microplastic that are of sizes less than of ultrafiltration membrane passes through the filter and enters the food chain. Accumulation of this microplastic on the membrane also leads to membrane fouling. Through this review article, we have assessed the impact of the structure, size, and type of MPs on ultrafiltration technology for microplastic remediation, with that how these factors affect the efficiency of the filtration process and challenges occur during filtration.

BACKGROUND: Airway management is the most essential skill in Anesthesiology and the inability to secure the airway is one of the most common reasons for anesthesia-related morbidity and mortality. This study aimed to evaluate and compare the insertion characteristics of laryngeal mask airway (LMA) ProSeal insertion using the standard introducer technique, 90-degree rotation technique, and 180-degree rotation technique in adult patients undergoing elective surgery. MATERIAL AND METHOD: This prospective, interventional, randomized, comparative study was carried out in the Department of Anesthesia and Intensive Care, Vardhman Mahavir Medical College & Safdarjung Hospital, New Delhi, after obtaining hospital ethical committee approval for 18 months of duration. Patients in the age group 18-65 years, of either gender, fulfilling American Society of Anesthesiologists grade I or II criteria, scheduled for elective surgery under general anesthesia with controlled ventilation using the LMA ProSeal were included in the study. The patients were randomized into three groups: Group I - Standard Introducer technique (n=40); Group NR - 90-degree rotation technique (n=40); Group RR - 180-degree rotation technique or back to front (airway) technique (n=40). RESULT: In this study, the majority (73.3%) of the patients were females with 31 patients in group I, 29 patients in group NR, and 28 patients in group RR. A total of 26.67% of male patients were included in the study. No significant difference in the gender distribution of the three groups was seen in the study. There was no incidence of failure in ProSeal laryngeal mask airway (PLMA) insertion in the NR group, while it was 2.50% in group I and 7.50% in group RR but the difference was not statistically significant. A statistically significant difference was seen in the incidence of LMA ProSeal blood staining (p=0.013). In the post-anesthesia care unit at 1 hour, the incidence of sore throat was 10% in patients with the NR group, 30% in the I group, and 35.44% in the RR group which was statistically significant. CONCLUSION: The study concluded that the 90-degree rotation technique was superior to both the 180-degree rotation and the introducer technique in adult patients in terms of insertion time, ease of insertion score, manipulation requirement, blood staining of PLMA, and post-op sore throat.

The identification of harmful metal ions in aquatic environments is a global concern since these contaminants can have serious consequences for plants, animals, humans, and ecosystems. A biosensor is a type of analytical equipment that combines a biological recognition element and a physical transducer to detect biological signals to produce a detectable indication proportionate to the concentration of the samples being analysed. The analyte spreads from the fluid to the biosensor's superficial. The analyte responds precisely and competently with the biosensor's biological component. The physicochemical properties of the transducer surface change as a result of this process. The visual or electric properties of the transducer surface alter as a result of this. The signal that is detected is an electrical signal. With the help of carbon-based nano-biosensors, metals from the aquatic environment can easily be detected, which is much simpler, less time-consuming, and less expensive as well.

An alga belongs to the multi-pertinent group which can add to a significant sector of environment. They show a prevailing gathering of microorganisms for bioremediation due to their significant capacity to inactivate toxic heavy metals. It can easily absorb or neutralize the toxicity of heavy metals from water and soil through phytoremediation. Biosorption is a promising innovation that focuses on novel, modest, and exceptionally successful materials to apply in phytoremediation technology. Furthermore, algal biomass can be used for biofuel generation after phytoremediation using thermochemical or biological transformation processes. The algal components get affected by heavy metals during phytoremediation, but with the help of different techniques, these are yield efficient. The extreme lipid and mineral substances of microalgae have been proven helpful for biofuel manufacturing and worth extra products. Biofuels produced are bio-oil, biodiesel, bioethanol, biogas, etc. The reuse capability of algae can be utilized toward ecological manageability and economic facility. In this review article, the reuse and recycling of algal biomass for biofuel production have been represented. This novel technique has numerous benefits and produces eco-friendly and economically beneficial products.


This article explains how algal biomass left over after phytoremediation can be reused and recycled to produce biofuel. It is a novel technique that is cost-effective, demands less time for biofuel production as it does not include cultivation and harvesting time, and produces products that are economically and environmentally beneficial for society.

Coronavirus disease 2019 (COVID-19) has turned out to be the most devastating viral disease that the world has encountered for the past century. The World Health Organization (WHO) declared it a pandemic on March 11, 2020. The disease mainly spreads through respiratory droplets which makes social distancing a primary tool of prevention. Many variant strains have emerged up since the pandemic started and the Delta variant is responsible for recent surge of cases in second wave of COVID-19 in India. Mass vaccination is the most efficacious precautionary measure that can be applied to stop the transmission and generate herd immunity. Vaccination does not give 100% prevention from infection, but it halts the severity of infection. Vaccine is the boon amidst the mayhem. Our study highlights that those vaccinated (particularly two doses) had clinically mild symptoms and mild computed tomography severity score (CTSS) with a speedy recovery. Those unvaccinated had moderate to severe symptoms with moderate to severe CTSS (>8) often requiring hospital admission and having poor prognosis. Thus, vaccine helps reduce the health burden of the already strained health care system. Immunization visit can also be used as an opportunity to disseminate message to encourage behavior, to reduce transmission risk of COVID-19 virus, to identify the signs and symptoms of disease, and to provide guidance on what to do.

Endometriosis is a debilitating gynecological disorder in women of reproductive age. Laparoscopy, a minimally invasive surgical procedure, provides a definitive diagnosis of the disease. Current treatments, including hormonal therapy and pain medication, are often associated with undesirable side effects limiting their long-term usage. This calls for exploring newer diagnostic and therapeutic options with minimal side effects. Curcumin is an established anti-endometriotic agent with inherent fluorescent properties; however, poor bioavailability limits its clinical utility. To address this shortcoming, various transition metals were conjugated with curcumin to improve its stability, specificity and pharmacological properties. The chemical stability, hemocompatibility and ability of the synthesized metallo-curcumin complexes (MCCs) to ameliorate endometriotic lesions were investigated. While all of the MCCs exhibited low hemolytic activity, their chemical and biological activities were largely dependent on the nature of the metal ion conjugated to the curcumin molecule. Copper-curcumin and nickel-curcumin complexes demonstrated superior therapeutic efficacy evidenced by enhanced antioxidant activity, selective cytotoxicity and increased accumulation in endometriotic cells mediated by an energy-dependent active transport process.

Dynamic graph embedding has gained great attention recently due to its capability of learning low-dimensional and meaningful graph representations for complex temporal graphs with high accuracy. However, recent advances mostly focus on learning node embeddings as deterministic "vectors" for static graphs, hence disregarding the key graph temporal dynamics and the evolving uncertainties associated with node embedding in the latent space. In this work, we propose an efficient stochastic dynamic graph embedding method (DynG2G) that applies an inductive feedforward encoder trained with node triplet energy-based ranking loss. Every node per timestamp is encoded as a time-dependent probabilistic multivariate Gaussian distribution in the latent space, and, hence, we are able to quantify the node embedding uncertainty on-the-fly. We have considered eight different benchmarks that represent diversity in size (from 96 nodes to 87 626 and from 13 398 edges to 4 870 863) as well as diversity in dynamics, from slowly changing temporal evolution to rapidly varying multirate dynamics. We demonstrate through extensive experiments based on these eight dynamic graph benchmarks that DynG2G achieves new state-of-the-art performance in capturing the underlying temporal node embeddings. We also demonstrate that DynG2G can simultaneously predict the evolving node embedding uncertainty, which plays a crucial role in quantifying the intrinsic dimensionality of the dynamical system over time. In particular, we obtain a "universal" relation of the optimal embedding dimension, Lo , versus the effective dimensionality of uncertainty, Du , and infer that Lo=Du for all cases. This, in turn, implies that the uncertainty quantification approach we employ in the DynG2G algorithm correctly captures the intrinsic dimensionality of the dynamics of such evolving graphs despite the diverse nature and composition of the graphs at each timestamp. In addition, this L0 - Du correlation provides a clear path to selecting adaptively the optimum embedding size at each timestamp by setting L ≥ Du .

The purpose of the study was to evaluate the longitudinal changes in electromyographic (EMG) activity of the masseter and temporalis muscles before and after temporomandibular joint (TMJ) total joint replacement (TJR) in ankylosis patients. A prospective longitudinal study was designed on patients undergoing unilateral or bilateral TMJ-TJR. EMG activity at rest and maximal voluntary contraction (MVC) was recorded for the muscles preoperatively (T0), and at one-week (T1) and six-month (T2) follow up. The study sample was composed of 10 (male: female 2.3:1) patients undergoing TMJ-TJR. The number of unilateral and bilateral cases was three and seven, respectively. In both unilateral and bilateral cases a statistically significant reduction in EMG activity of the masseter and anterior temporalis muscles was observed at T1. At T2, EMG activity of the muscles was found to be approaching the preoperative value. In unilateral cases, when the affected side at T0, T1, and T2 was compared with T0 for the unaffected side, statistically significant differences were seen for the masseter. In the case of the temporalis, similar results were seen except at T2 for the postural rest position. The results indicate that re-attachment of the masseter and temporalis muscles occurs progressively post TMJ-TJR.

Metabiotics have emerged as the safer alternatives to probiotics in last decade. Unlike probiotics that are live microbes, metabiotics are the low molecular weight bioactive metabolites produced by the gut microbiota. While offering a similar profile of health benefits as that of probiotics, metabiotics are free from the risks and uncertain responses associated with administration of live bacteria into the body. Metabiotics have demonstrated substantial effectiveness across the ethnicities, age, gender and nutritional habits in a number of metabolic disorders, including obesity. Obesity is attributed to the offsetting of the energy homeostasis of the body due to a number of genetic, endocrinological, and environmental factors leading to obesity. The obesogenic mechanisms are quite complicated as they result from a complex interplay among a number of factors. Owing to a variety of constituents exerting their action through different pathways, metabiotics offer a pragmatic option for treatment as well as prevention of obesity by addressing heterogeneous aspects of its aetiology. In this review, we categorize various components of metabiotics and discuss their cross-talk with host cells at the molecular level. We also discuss the challenges in understanding these interactions and their potential effects on obesity treatment and prevention strategies. Considering the alarming rise in obesity all over the world, metabiotics offer an attractive non-pharmacological approach to spearhead the strategies being designed to combat the challenges posed by the obesity epidemic.

Endometrial disorders collectively encompass a broad spectrum of pathologies, including but not limited to endometriosis, endometrial cancer and endometritis. The current therapeutic management of these diseases is associated with several limitations. This has prompted interest in the use of plant-based bioactive compounds as alternative strategies to achieve high therapeutic efficacy and avoid adverse effects. In this context, curcumin, a polyphenol abundantly present in turmeric, is gaining increasing attention for its therapeutic potential to restore homeostasis in endometrial dysfunctionality. We comprehensively review the multifaceted role of curcumin, discussing mechanistic insights in various endometrial pathologies. We also provide an in-depth analysis of the concerns and challenges associated with the role of curcumin in endometrial research and outline a road map for future investigations.

BACKGROUND: Dermatitis or eczema is a prevalent skin disorder worldwide and is also very common as a pediatric inflammatory skin disorder. Its succession gets worse with the multiple comorbidities which exhibit mechanisms that are poorly understood. Its management further becomes a challenge due to the limited effective treatment options available. However, the Novel Drug Delivery Systems (NDDS) along with new targeting strategies can easily bypass the issues associated with dermatitis management. If we compare the active constituents against phytoconstituents effective against dermatitis then phytoconstituents can be perceived to be more safe and gentle. OBJECTIVE: Administration of NDDS of plant extract or actives displays improved absorption behavior, which helps them to permeate through lipid-rich biological membrane leading to increased bioavailability. The newer efficient discoveries related to eczema can face various exploitations. This can be intervened by the subjection of patent rights, which not only safeguard the novel works of individual(s) but also give them the opportunity to share details of their inventions with people globally. CONCLUSION: The present review focuses on the available research about the use of nanoformulations in the topical delivery. It further elaborates the use of different animal models as the basis to characterize the different features of dermatitis. The review also highlights the recent nanoformulations which have the ability to amplify the delivery of active agents through their incorporation in transfersomes, ethosomes, niosomes or phytosomes, etc.
