RESUMO

Venous stasis, disorders of oxygen metabolism and other disturbances in the post-thrombophlebitic syndrome result in reduction of the contractile function of the crus, i.e. muscle pumping capacity. Mutual influence of these factors result finally in the development of the so-called "vicious circle". Results of a study of 90 patients with different stages of the post-thrombophlebitic syndrome show a clear correlation between the stage of the disease and degree of reduction of the functional capacity of sural muscles. Measures for restoration of the contractile capacity of the crural muscles are advocated.

RESUMO

In the article, the expediency of the use of computerized tomography in the diagnosis of the different forms of acute pancreatitis in 49 patients is substantiated and results are analysed.

RESUMO

With the use of the methods of percutaneous contact polarography, roentgeno-contrast phlebography, Doppler ultrasound, 83 patients with different location and stage of the post-thrombophlebitic syndrome were examined. The significant alteration of the indices of polarographic curve versus the norm was defined. The most pronounced disorders were revealed in occlusion of the deep veins of the lower extremities.

The systemic, regional, and microcirculatory hemodynamics was studied complexly by radioisotope methods in 54 patients with acute thrombosis of the deep veins of the lower limbs and pelvis and in 58 patients with the postthrombophlebitic syndrome. It is shown that in acute venous thrombosis the regional stasis and hypertension are attended by considerable deposits of blood in the "affected" limb with a decrease in the venous return to the heart and disturbance of transcapillary exchange. In the postthrombotic syndrome the pathogenetic importance of venous stasis and hypertension is maintained both in the stage of occlusion and in the stage of recanalization. In this case, the hemodynamic changes in the edematous-algesic form differ from those in the edematous-ulcerative form. It is established that occlusion of the ilio-femoral-popliteal-tibial segment in acute venous thrombosis and postthrombophlebitic syndrome is characterized by the most pronounced shifts in regional hemodynamics.

The clinical, phlebographic, and hemodynamic signs of the postthrombophlebitic syndrome were studied. In 99.3% of patients the disease was found to be attended with edema of the extremities, in 100%--with the pain syndrome, in 64.2%--with pigmentation, in 59.7%--with tissue induration, and in 53.6% of patients--with trophic ulcers. A study of microcirculation revealed a significant increase of the time of tissue resorption. The venous pressure proved to be increased; the degree of its normalization is an objective test for the success of the operation. The volume blood flow in the affected extremity was 1.8 times that in the normal extremity. Contrast X-ray examination of the veins showed phlebographic signs of the disease. Reconstructive operations on the deep veins were performed in 109 patients; 85.4% had good remote results. Linton's operation was performed on 29 patients, Cockett's operation--on 17, and Felder's operation--on 8 patients; good remote results were noted in 75.7% of cases. For determining the tactics of the treatment and its prognostication, a classification of the postthrombophlebitic syndrome is suggested.
