RESUMO

12 population groups of Hungary, 1514 individuals altogether, have been studied for polymorphisms of alpha 1antitrypsin, serum cholinesterase, paraoxonase and delta-aminolevulinic acid dehydrase, N-acetyltransferase variation and aldehyde dehydrogenase deficiency. A possible relationship between their allele frequencies and environmental factors in the context of ecogenetic and pharmacogenetic phenomena in Hungary is discussed.

The pregnancy outcomes of 109 women who were intoxicated during pregnancy due to excessive amounts of drugs and other agents were studied. Seventy per cent of the women attempted suicide, and the intoxication in 78% of all cases was caused by drugs. The stillbirth rate (4%) was higher than that in previous and subsequent pregnancies of these index females. The rate of spontaneous abortions, major congenital abnormalities, minor anomalies, infant mortality, and specific childhood diseases did not increase significantly. Seven major congenital abnormalities occurred in 96 live births; five of these definitely had no causal relation with the intoxication. The mean birthweight of the liveborn infants was not significantly lower than that of their sibs but it was significantly lower than that of sibs born previously and that of the Hungarian population. The percentage of mentally retarded offspring (6.5%) exceeded the national figure (about 3%) and the rate of sibs (2.7%); however, the difference was not significant. Three of 93 children had serious behavioral problems.

A cytogenetic investigation was conducted in 18 self-poisoned pregnant and 16 self-poisoned non-pregnant women and in 31 pregnant and non-pregnant controls. Blood samples for analysis of chromosomal aberrations and SCEs were collected from women who were at different early stages of pregnancy. The difference between self-poisoned women and controls was very highly significant in the case of chromatid-type and unstable chromosome-type aberrations and highly significant in the case of SCEs. Further, the frequency of chromatid aberrations in pregnant women relative to non-pregnant ones was significantly lower suggesting a possible protective effect of pregnancy.

Reproductive outcome as assessed by personal interview was studied after severe self-poisoning in 1399 index patients below the age of 30 years and 881 control cases. Rates of delayed conception, infertility, spontaneous abortion, ectopic pregnancy and stillbirth were no higher in the study sample after self-poisoning than in the control sample. Among livebirths of index and control cases, there were no significant differences with respect to the sex ratio, mean birth weight, distribution of birth weight groups, percentage with low birth weight, twin occurrence, birth prevalence and distribution of congenital anomalies verified medically, rates of early death and some kinds of postnatal diseases. Consequently, there is no evidence that semilethal drug intoxication produces epidemiologically detectable germinal mutations in a sample involving 2503 pregnancies.

RESUMO

Individuals who poison themselves in a suicide attempt with extremely large doses of chemicals may comprise a population that is useful for the study of the possible mutagenicity of drugs and other compounds. After the demonstration of the demographic data of completed and attempted suicide cases, the methodology of short-term somatic and long-term germinal mutagenesis systems are described. Finally the advantages and disadvantages of this human experimental epidemiological model are discussed, pointing out the practical and theoretical importance of the approach.

Pregnancy outcome of 142 self-poisoned pregnant females is analysed. Thirty three pregnancies were terminated, 13 ended with fetal death. Health conditions of 96 livebirths are demonstrated in detail.
