RESUMO

Tumor calcinosis is a rare condition. It is characterized by the presence of calcified masses in the juxta-articular regions without joint involvement. It particularly affects young adults and adolescents. Its exact pathogenesis remains poorly defined. The diagnosis is suspected clinically and radiologically but confirmed by histological examination. The treatment is mainly surgical, and the prognosis is often good. We report the original case of a chronic hemodialysis patient presenting with tumoral calcinosis by discussing our diagnostic and therapeutic approach according to data from the recent scientific literature.

RESUMO

Villonodular synovitis is a rare disease of the synovial tissue occurring most commonly in synovial joints such as the knee and ankle joints. We report the case of a patient presenting with villonodular synovitis of the hip revealed by a pathological fracture of the femoral neck, discussing our diagnostic and therapeutic approach with recent scientific data.

RESUMO

Osteoid osteoma is a benign bone tumor that frequently affects young adults. The clinical presentation is variable, and the course can lead to spontaneous regression or persistence, necessitating medical or surgical treatment. We report the case of a young patient with an exceptional localization of an osteoid osteoma at the lesser trochanter, exhibiting atypical clinical and radiological features. The tumor was resected via the Ludloff approach. Here, we discuss our diagnostic and therapeutic approach in light of the literature.

Isolated fractures of the distal end of the tibia are rare lesions; they can induce numerous complications and the diagnostic approach and management are not always simple. We report the case of a patient with an isolated fracture of the posterior margin of the tibia, exposing the different stages of its treatment compared to data from recent scientific literature.

Secretan's syndrome is a rare condition; the exact etiology remains unclear. It has no specific treatment and the care must be multidisciplinary and personalized. We report a case of a young female patient who presented with a unilateral and painful swelling of the dorsum of the right hand. The diagnosis and treatment of this patient were challenging.

Osteochondral damage to the talus is one of the most frequent causes of ankle pain. In contrast to other joints in the lower limb, osteochondral damage of the talus is often attributed to traumatic events. One option of treatment is mosaicplasty, which has proved to be a feasible choice for the treatment of osteochondral lesions of the talus; it has the potential to alleviate ankle pain and facilitate engagement in daily activities as well as sports. We present two different cases of osteochondral lesions of the talus, illustrating how this pathology can present clinically. Both cases involve males with no notable pathological antecedents. The first was the victim of a traffic accident, the second was the victim of a sports accident; they were admitted for the management of chronic ankle pain unimproved by analgesic treatment. Radiological findings revealed a talus osteochondral lesion in both patients, treated with an osteochondral autograft from the homolateral knee. Both patients progressed well, with the resumption of daily activities and sports. The notable result of current research is that mosaicplasty has been shown to have good results in those with large osteochondral lesions who want to return to normal activity.

Malignant tumors of the peripheral nerve sheaths are uncommon, constituting a small percentage, typically ranging from 2% to 5% of soft tissue sarcomas. Etiological diagnosis is often difficult but is guided by imaging and confirmed by histopathological and immunohistochemical examination. We report a case of a 46-year-old woman admitted for management of a mass in the medial parapatellar region of the right knee. Her medical history included a burn to the right leg five years ago and a previously undocumented resection of a medial parapatellar tissue mass in the right knee. Radiological examination showed a deep and superficial soft tissue mass in the medial soft tissue of the right knee opposite the patella, with no hemorrhagic components and heterogeneous enhancement after injection of gadolinium. Histopathology confirmed the diagnosis of a low-grade peripheral nerve sheath tumor, and a thoracic-abdominal-pelvic CT scan was performed, which was normal. Treatment consisted of a simple carcinological resection and local radiotherapy. No local recurrence was noted after one year of follow-up.

Tuberculosis of the pubic symphysis is rare; the diagnosis is often difficult but guided by CT and confirmed by histopathological examination. We report the case of a 25-year-old female with no particular past medical history, presenting with pain at the level of the pubic symphysis. Clinical examination showed a small renitent mass and pain on palpation without inflammatory signs. Radiological investigation showed demineralization, lysis, and diastasis of the pubic symphysis with a cystic image in favor of tuberculosis. A biopsy followed by resection was performed, and histopathology confirmed the diagnosis. The patient received medical care for nine months using the 2RHZE/7RH protocol (rifampicin, isoniazid, pyrazinamide, ethambutol), with good results at follow-up. Bone lesions of the pubic symphysis may exceptionally reveal tuberculosis, and the positive diagnosis is based on a histopathological test.
