RESUMO

Rhabdomyosarcoma (RMS) is the most common mesenchymal tumor in children and adolescents, with 10% of cases occurring in the orbits. RMS should be suspected whenever children present with rapidly progressing unilateral exophthalmos. Its symptoms depend on the lesion's origin and location. We report the clinical case of a 19-year-old male patient admitted to the hospital because of blurred vision and bulging eyes that gradually increased over several months. Magnetic resonance imaging showed a mass located mainly in the left orbit, pushing and deforming but not invading the eyeball. The lesion had grown into the left ethmoid sinus wall. The histopathological incisional biopsy results were with alveolar RMS.

Background: In Vietnam, after the enactment of 37/2018/QD-TTg, a distinct shift could be observed in the international publications by radiological societies. Objctive: This study examined trends in the international publications achieved by Vietnamese radiological societies following the enactment of 37/2018/QD-TTg. Methods: This retrospective study evaluated data that are freely accessible and available online and, therefore, did not require institutional review board approval. We assessed the publication characteristics of the members of four Vietnamese radiological societies: Vietnamese Society of Radiology and Nuclear Medicine (VSRNM, n = 67); Radiological Society of Ho Chi Minh City (RSHCM, n = 25); Vietnamese Society of Ultrasound in Medicine (VSUM, n = 29); and Vietnamese Society of Interventional Radiology (VSIR, n = 18), following the enactment of 37/2018/QD-TTg. Results: Following the enactment of 37/2018/QD-TTg, we observed that total publications, number of first authors, number of last authors, number of articles indexed in Web of Science, and number of articles indexed in Scopus were significantly higher for the VSRNM and VSIR than for the RSHCM and VSUM. All publication parameters were significantly higher men, individuals who secured professorship, PhD and Master's degree holders, public hospital employees, those in the radiological field, and those in the north and south regions, compared with the values for women, individuals without professorships, Specialist II and I degree holders, private hospital employees, those in the nuclear medicine field, and those in the middle region, respectively. Conclusion: The enactment of 37/2018/QD-TTg triggered a new trend in international publication among the members of four Vietnamese radiological societies. However, strong discrepancies remain in total publications, number of first authors, number of last authors, number of articles indexed in Web of Science, and number of articles indexed in Scopus among the members of the four main Vietnamese radiological societies.

RESUMO

Ischiopagus conjoined twinning is a rare congenital defect. The surgical separation of conjoined twins is difficult because of the complex anatomy and physiology. Careful preoperative assessment, planning, and effective surgical teams are critically important for achieving a good outcome. We describe the successful separation of ischiopagus tetrapus conjoined twins as a representative case demonstrating the growth of pediatric surgery in southern Vietnam.

RESUMO

Pericallosal artery aneurysm (PAA) is a relatively uncommon type of intracranial aneurysm that tends to rupture more frequently and cause higher mortality rates than other types of cerebral aneurysms. Surgery to address PAA is difficult due to its deep-seated location, the size of the aneurysmal sac, and the limited surgical field. In recent years, with the development of percutaneous interventions, endovascular treatment has become the preferred, minimally invasive intervention method for the treatment of pericallosal aneurysms. In this article, we present a case of PAA that was successfully treated with flow diversion therapy in a 51-year-old male.

Lymphoma with cardiac involvement is a high-risk lesion, especially in children. We report a rare clinical case of multifocal precursor B-cell lymphoblastic lymphoma in a child with cardiac involvement. A 4-year-old boy presented to the Vietnam National Children's Hospital with a vague headache, but magnetic resonance imaging of the head was normal. After 1 week, the patient showed symptoms of chest pain, fatigue, dyspnea, and abdominal pain. On transthoracic echocardiography and multislice computed tomography of the thorax, a mass was detected in the right atrial wall. Abdominal ultrasound showed a small bowel intussusception, multiple nodules in the intestinal wall, and mesenteric lymph nodes. Histopathology of the bowel confirmed the diagnosis of multifocal precursor B-cell lymphoblastic lymphoma. The patient responded to 3 cycles of chemotherapy for lymphoma. Therefore, combining multiple imaging methods allowed for early diagnosis and improved treatment.

Foreign body (FB) ingestion is a common condition encountered in clinical practice, especially among the pediatric age group; however, this occurrence is rare among adults. Some FBs can induce the perforation of the gastrointestinal tract, including fish bones, chicken bones and toothpicks. The ingestion of FBs is rarely associated with bowel perforation, and most FBs are passed spontaneously. The ingestion of sharp and pointed objects typically produces adverse events related to the upper gastrointestinal system, and FBs are rarely retained in the colon. Bowel perforation caused by the ingestion of FBs should be diagnosed and treated in a timely manner. Here, we present the unusual case of a 51-year-old male who presented to the emergency room with complaints of acute abdominal pain secondary to fish bone ingestion, which triggered cecum perforation.

Inflammatory myofibroblastic tumor (IMT) is an uncommon neoplasm that rarely arises in the genitourinary system. IMTs in the vulva in infants are extremely rare in the literature. The tumor consists of myofibroblastic spindle cells accompanied by inflammatory cell infiltration. In this article, we aimed to describe the case of IMT in the vulva. A newborn girl presented with a mass in the vulva detected in the prenatal period. The patient was treated with surgery and chemotherapy. Follow-up 8 months after surgery showed no signs of recurrence. In conclusion, IMT has a variable clinical presentation, surgery is the optimal approach, but in cases without complete resection, chemotherapy is essential.

Lipomatous pseudohypertrophy of the pancreas is a rare disease with unknown etiology, and the pancreas parenchyma is replaced by pancreatic parenchyma by fat tissue. In this article, we aimed to report the case of a 26-year-old male patient admitted to hospital with loss of appetite for 6 months. Abdominal computed tomography (CT) and magnetic resonance imaging (MRI) scans showed diffuse enlargement and fatty replacement over the whole pancreas, with scattered remnants of pancreatic parenchyma. Histologic results defined lipomatous pseudohypertrophy of the pancreas. To summarize, this case report is to put forward this extremely rare presentation and to sensitize clinicians that this entity can be a cause of exocrine pancreatic insufficiency, which requires patient follow-up for the appropriate treatment.

Adrenocortical carcinoma (ACC) is a rare malignancy that arises from the adrenal cortex and can be classified as either non-functioning or functioning. A patient with non-functioning ACC may present no specific symptoms. Imaging analysis can provide some information to a clinician who suspects ACC, such as tumor size, density, washout, necrosis, hemorrhage, and calcification. Histopathology is used to confirm and determine the origin of the malignancy and can provide relevant prognostic information. Microscopic findings can be used to obtain information such as the Weiss score, resection surface features, Ki-67 proliferative index, and the degree of capsular and vascular invasion. Surgery can be curative for localized tumors, and adjuvant therapy using mitotane and cytotoxic chemotherapy is often employed for advanced-stage tumors. We describe a case report of a 32-year-old man with a non-functioning ACC that highlights the importance of radiological and pathological features in the diagnosis of ACC and their use as prognostic factors.

Hypovascular pancreatic neuroendocrine tumors are uncommon pancreatic tumors and commonly misdiagnosed as pancreatic ductal adenocarcinoma or chronic mass-forming pancreatitis. The liver is the organ most commonly affected by neuroendocrine tumor metastases but hepatic neuroendocrine tumor metastases are quite difficult to discriminate from other hepatic metastases and primary hepatic tumors. We describe a case of a 47-year-old man with incidentally detected multiple hepatic lesions on ultrasound. On further imaging technique including computed tomography and magnetic resonance imaging, the patient had an abnormal hypoenhancing lesion at the pancreatic tail and multiple hyperenhancing hepatic metastases that were diagnosed as hypovascular pancreatic well-differentiated neuroendocrine tumor Grade 2 with multiple hypervascular hepatic metastases after liver biopsy and surgery. Neuroendocrine tumor is a rare etiology among hypoenhancing pancreatic tumors, and must be considered to discriminate from pancreatic adenocarcinomas in cases there are multiple hyperenhancing hepatic metastases on the arterial phase without typical washout on the portal venous phase.

Malignant tumors that originate from the mesenchymal tissue of the mammary gland, known as breast sarcomas, are very rare and can be divided into 2 types: primary and secondary (therapy-related development). Breast sarcomas are aggressive tumors associated with a poor prognosis. Treatment options include the coordination of surgery, chemotherapy, and radiotherapy. We present a case of a 51-year-old female who presented to the hospital after noticing a palpable mass in the left breast and bloody nipple discharge. These symptoms lasted for more than 4 months. Postoperative histopathology revealed an undifferentiated pleomorphic breast sarcoma. After 8 months of treatment, the patient experienced metastasis to the brain and lungs.

A respiratory fungal infection is a severe clinical problem caused by endemic or opportunistic fungi. Although pulmonary fungal infections may be associated with no symptoms, the patient may also present with cough, fever, chest pain, and hemoptysis. Trichophyton spp., a dermatophyte fungus, is a dermatologic pathogen and can cause a deep, sometimes fatal infection. Here, we report the first case of a deep Trichophyton spp. Infection in Vietnam and possibly the first case of Trichophyton spp. pneumonia in the literature, which responded completely to medical therapy. This article highlights the epidemiology of fungal lung infections and describes the clinical approach for when to suspect and how to diagnose this disease.

Ependymoblastoma is an uncommon, exceedingly malignant brain neoplasm that adversely influences children's quality of life. Ependymoblastoma represents a subtype of primitive neuroectodermal tumors, categorized as grade IV, according to the 2007 World Health Organization (WHO) classification of central nervous system tumors. Ependymoblastomas are often located in the supratentorial zone and often associated with the ventricular system. Histopathological sections of the tumor revealed uniform, primitive, small blue cells, with multi-layered rosettes, accompanied by abundant mitoses. The clinical and imaging features of ependymoblastomas are not specific, which can result in misdiagnosis as other brain neoplasms. In this paper, we described the identification of a fourth-ventricular ependymoblastoma that was misdiagnosed as pilocytic astrocytoma, despite the utilization of advanced magnetic resonance imaging (MRI) protocols.

INTRODUCTION: Pseudo journals, hijacked journals, fraudulent journals, fake journals, and predatory journals waste valuable research when authors publish their studies in them. AIM: This article described novel suggested features for the identification of fraudulent journals and aimed to explain this issue to help inexperienced scientists avoid publishing in predatory journals. METHODS: The articles related to this topic in were retrieved from PubMed and trustable Internet sources. RESULTS: Unfortunately, some fake journals have made their way into reputable databases, such as PubMed, PubMed Central, MEDLINE, SCOPUS, and Web of Science; thus, the serious question has been raised regarding how we should address this problematic phenomenon. We recommended 28 suggested characteristics of predatory journals for readers to take into consideration. CONCLUSION: Unaware of the detrimental effects associated with publishing in disreputable journals, inexperienced researchers can fall victim to them. Together, as both readers and writers, we should completely boycott predatory journals.

Nitrous oxide (N2O) is a weak anesthetic gas that was first used in 1844 in the field of dental anesthesia. However, currently, N2O is being abused for entertainment purposes in the form of N2O-filled balloons, called funky balls, which can cause many adverse effects, especially nervous system injury. This study aimed to investigate the detailed clinical and subclinical features associated with N2O intoxication. We retrospectively reviewed 47 patients diagnosed with N2O intoxication, from May 2018 to July 2019, and collected demographic data, clinical and laboratory tests, and spinal cord magnetic resonance imaging (MRI) findings. The mean time of funky ball use was 8.8 months, with a mean use of 36.3 balls per day. All patients presented with superficial sensory disorders. Reductions in muscle strength, decreased vibration sensation, and decreased or lost tendon reflexes were the most common clinical signs of N2O intoxication. Romberg sign and Lhermitte sign were observed in 39 patients (83%) and 21 patients (44.7%), respectively. Spinal cord lesions on MRI were observed in 32 patients (68.1%), which mostly presented with an inverted V sign. The total duration of N2O use, the number of days of using N2O per week, and the presence of Lhermitte sign (P<0.05) were significantly different between patients with and without spinal cord lesions on MRI. Serum levels of homocysteine and vitamin B12 were significantly different between the time of admission and discharge (P<0.05). Our study indicated that the days of using N2O per week was significantly associated with Spinal Cord Injuries (SCI) on MRI. According to the Receiver Operating Characteristic (ROC) curve analysis, a cutoff days of using per week value of 2.5 days could predict SCI with a sensitivity of 81.3%, a specificity of 73.3%, and an area under the ROC curve (AUC) of 0.813. Changes in the serum levels of homocysteine and vitamin B12 were effective markers for the evaluation of treatment response.

INTRODUCTION: The Vietnamese Society of Interventional Radiology (VSIR) is one of the world's youngest interventional radiology (IR) societies, with more than 150 members. During the first decade of the development phase, in addition to a number of achievements, the VSIR has continued to address problematic circumstances. AIM: This article aims to describe the current status of VSIR, to introduce VSIR to other IR societies. METHODS: We investigated VSIR, using internet sources and related academic and clinical papers. RESULTS: VSIR was founded in 2010 and has succeeded in advancing education and scientific research during the last 10 years. CONCLUSION: VSIR is widely involved and accepted in Vietnamese medicine. We recommend the launch of an IR training program and support a unified global IR platform, for the benefit of both clinicians and patients in the near future.

INTRODUCTION: In Vietnam, the successful publication of research in indexed journals is mandatory to obtain academic appointments and promotions in medical colleges and institutions, according to the current guidelines established by the State Council for Professorship and Ministry of Education and Training. AIM: This study aimed to investigate the impacts of novel Vietnamese government regulations on radiological PhD and professorship candidates. METHODS: This study evaluated freely accessible data, available online, and, therefore, did not require institutional review board approval. We assessed the numbers of radiological PhD candidates at Hanoi Medical University and the numbers of published Vietnamese radiological papers, from 2012 to 2019, indexed in the SCImago database. In addition, we evaluated the numbers of qualified radiological professors and associate professors employed at universities during the same period. We did not include nuclear medicine PhD and professorship candidates, in this study. The data are presented as bar and line charts. RESULTS: Following the enactment of 08/2017/TT-BGDDT and 37/2018/QD-TTg, we observed that the numbers of radiological PhD and professorship candidates were significantly reduced. From 2012 to 2019, only one candidate qualified for appointment as a radiological professor. However, the number of radiological papers rose dramatically during the same time period. CONCLUSION: The enactment of 08/2017/TT-BGDDT and 37/2018/QD-TTg had strong impacts on the numbers of PhD and professorship candidates. Owing to these new regulations, the number of published, international, peer-reviewed radiological papers has increased; however, some undesired consequences may have occurred, such as papers being published in predatory or suspected predatory journals, double or triple submissions, and plagiarism.

PURPOSE: The purpose of this study was to evaluate the safety and efficacy of mechanical thrombectomy with a direct aspiration first-pass technique (ADAPT) using large-bore catheters in patients with acute ischemic stroke due to large vessel occlusion (LVO) in a hospital in Vietnam. METHODS: This was a retrospective review of patients with acute ischemic stroke due to LVO who were diagnosed and underwent mechanical thrombectomy using ADAPT with large-bore catheters at Bach Mai Hospital from January 2017 to June 2018. RESULTS: Seventy-three patients (47.9% female; age: 61.29 ± 14.49 years) met study criteria. The average procedure duration was 45.09 ± 38.26 min. Successful recanalization post-ADAPT (thrombolysis in cerebral infarction 2b-3) was achieved in 72.6% (53/73) of patients. Good functional outcome (Modified Rankin Scale 0-2) at 3 months was achieved in 50.7% (37/73), with poor functional outcome in 24.7% (18/73). The 90-day mortality rate was 24.7% (18/73). The hemorrhagic transformation rate was 31.6%, in which 19.2% were symptomatic. Vessel perforation occurred in 5.5% (4/73) of patients but in all cases was associated with the guidewire and not the reperfusion catheter. Vessel dissection occurred in 1.4% (1/73) and vasospasm in 5.5% (4/73) of patients. CONCLUSION: Mechanical thrombectomy using ADAPT with large-bore catheters for acute ischemic stroke due to LVO is a method that yielded good results in recanalization and clinical recovery in a Vietnamese patient population.

INTRODUCTION: Chronic venous insufficiency (CVI) is a chronic condition, triggered by reflux through the saphenous vein network. AIM: To determine the efficacy of endovenous laser ablation (LA) and radiofrequency ablation (RFA) for CVI treatment in the lower extremities, at the Bach Mai Radiology Center. METHODS: This retrospective study was approved by the institutional review board of Bach Mai Hospital. The study recruited 49 people, from August 2016 to April 2018, with recurrent venous insufficiency in the lower extremities and measured 56 ablated veins. RESULTS: In this study, 8 patients (10 veins, with a mean diameter of 5.83 ± 0.96 mm) were treated with RFA, and 41 patients (46 veins, with a mean diameter of 7.96 ± 3.47 mm) were treated with LA. The occlusion rates for LA- and RFA-treated veins were very effective, at 95.7% and 90%, respectively. No significant differences in occlusion rates or clinical improvements were observed between the two ablation methods. On the first day post-treatment, the visual analog score (VAS) value for the LA group was significantly higher than that for the RFA group. Furthermore, ecchymosis, 1 day after treatment, and hyperpigmentation, paresthesia, and numbness, 1 month after treatment, were only observed in the LA group. CONCLUSION: Both LA and RFA were minimally-invasive and safe therapies. No serious complications requiring further interventions were reported and the treatment effectively improved the clinical symptoms of patients. Based on our study, we recommend that RFA should be considered for moderate dilated saphenous vein cases, whereas LA should be indicated for large dilated saphenous vein cases, with or without aneurysm.

INTRODUCTION: Each country has its system of the training program, but to be concordant with the world in the radiology field, the process needs to have common points or common criteria. On maintaining the integrity of intersociety collaboration in the field of radiology, it is necessary to understand each country's training program for each specialty. AIM: This retrospective study aims to compare the postgraduate thesis characteristics from various sources in the field of radiology. METHODS: This was a retrospective study evaluating data that is publicly available online and at libraries and institutional review board approval, as such, was not demanded. We selected 40 published theses from the library of Pham Ngoc Thach University of Medicine and University of Medicine and Pharmacy at Ho Chi Minh City and Hanoi Medical University in Vietnam which graduated from 2008 to 2018. Of these, there were 10 PhD, 10 specialists II, 10 master's, and 10 residency theses selected. RESULTS: A total of 40 theses were analyzed from participants with a median age of 36.5. The male/female ratio was 23/17. Most of the theses were subspecialty in diagnostic radiology (87.5%) and focused on pathological radiology (95%). Adult patients were the major objectives of the theses accounted for 87.5% with predominant materials of magnetic resonance imaging counted for 47.5%. Theses in PhD group were the largest items regarding the total number of pages as well as the number of figures, and the number of references. Nonetheless, both domestic and international publications related to all theses were truly low. CONCLUSION: The postgraduate thesis of radiology in Vietnam has many different forms but mainly focuses on diagnostic and pathological radiology with materials of magnetic resonance imaging in adults. The number of international publications regarding the thesis was very small.
