RESUMO

Isthmic aortic rupture or disruption should be systematically sought when there is high kinetic energy trauma to the thorax. This condition is extremely serious and life threatening. It needs to be diagnosed rapidly but diagnostic pitfalls must be avoided. CT angiography is the standard examination. The main CT signs of rupture or disruption of the thoracic aorta are periaortic hematoma, intimal flap, pseudo-aneurysm and contrast agent extravasation. There are three types of lesion: intimal, subadventitial or pseudo-aneurysmal, and complete rupture with lesion of the three tunicae, and it is important to grade them for better therapeutic management. The main diagnostic pitfalls of the CT scan are the presence of a ductus diverticulum and post-isthmic fusiform dilatation. Associated lesions must not be overlooked. The most common are ruptures of the aortic root and the thoracic aorta in the diaphragmatic hiatus.

RESUMO

Pelvic venous insufficiency is a frequent pathology in multiparous women. Diagnosis can be made by chance or suspected in the case of symptoms suggesting pelvic congestion syndrome or atypical lower limb varicosity fed by pelvic leaks. After ultrasound confirmation, dynamic venography is the reference pretherapeutic imaging technique, searching for pelvic varicosity and possible leaks to the lower limbs. MRI is less invasive and allows a three-dimensional study of the varicosity and, with dynamic angiography, it can assess ovarian reflux. It also helps to plan or even sometimes avoid diagnostic venography.

AIM: To evaluate the feasibility and efficacy of routine uterine artery embolization (UAE) immediately after planned caesareans performed in the cath lab for conservative treatment of placenta accreta. MATERIALS AND METHODS: A retrospective study included all patients who had a planned caesarean in the cath lab for conservative treatment of placenta accreta at Angers University Hospital, which is a tertiary care centre, from April 2001 to September 2010. Twelve patients underwent UAE immediately after caesarean with the placenta left partially or totally in situ. The success rate of embolization, blood loss, and complications were reported. RESULTS: Diagnosis of abnormal placentation was confirmed by caesarean findings in 14 cases. Four patients had a percreta form with bladder invasion. In seven cases blood loss was insignificant and UAE was prophylactic; no secondary haemorrhage was observed in this group. Postpartum haemorrhage occurred in five cases: control of immediate postpartum bleeding by embolization was successful in three and failed in two leading to hysterectomy. In one case uterine necrosis occurred 6 weeks after embolization, requiring a hysterectomy. Delayed complications resulted in hysterectomy and partial bladder resection 3 months after delivery for one of the patients with placenta percreta. CONCLUSION: UAE immediately after a caesarean performed in the cath lab is a feasible therapeutic option for conservative treatment of placenta accreta. Advantages include reducing stress and risks associated with transferring women with potentially unstable haemodynamics.

Dose delivery during CT coronary angiography with retrospective ECG gating is high especially due to the important slice overlapping. Optimization of the acquisition parameters is necessary to reduce patient exposure. First, the height of the scan field should be limited to the heart. Both kV and mA should be adjusted based on patient morphology. ECG gated exposure modulation with mA reduction during systole, a technique most applicable for patients with slow and regular heart rate, can result in a dose reduction up to 50%. The use of prospective ECG gating can also reduce patient dose. This technique also requires patients with slow and regular heart rate.

PURPOSE: To describe our practice day to day with a VARIAN linac "Clinac 2100" fully equipped with an On Board Imager (OBI) for patients with prostate cancer. MATERIALS AND METHODS: A volumetric and dosimetric study was performed in 2006 using ARTIVIEW software (AQUILAB) for nine patients and 76 Cone Beam CT kV (CBCT kV). We have contoured targets and organs at risk from CBCT kV slides acquisitions. Second, we achieved a dose-volume histogram (DVH) study for a patient treated in 2007 with IMRT technique in comparison with the 2006 study. RESULTS: 2006 analysis: The study showed a very important variability of organ measurements. Seminal vesicles were strongly influenced by adjacent organs; observed differences for prostate could be explained by contouring uncertainty on the apex. Inter-sessions motions could be observed for bladder, rectum and seminal vesicles (SV). Part of prostate volume not encompassed by PTV is about 2.5%; VS volume outside PTV is about 35%. Tumoral conformation index (TCI) is inferior to 97.5% in 22% of all cases. Anteroposterior displacements of the prostate barycentre is superior to 5mm. From this analysis, we recommended the strict respect of hygienodietetic rules, and we have adapted the system settings for better immobilization, which were applied for the 2007 study. For the 2007 analysis, since April 2007, most of patients are treated with IMRT for prostate cancer, at the second part of the radiation therapy to encompass only the prostate volume. Dose-volume histograms showed a great spreading out for 2006 patients, and not for the 2007 patient. CONCLUSIONS: IMRT and IGRT should permit a margin reduction for PTV. Strict respect of hygienodietetics rules is necessary to avoid rectal distension and local recurrence.

A 47 year-old man, who had a history of pelvic radiotherapy for the treatment of testicular tumour 30 years ago, was referred with minor tissue loss of the right lower extremity, grade III, category 5 of the Rutherford classification. His groin region presented with severe radiation damage. Arteriography demonstrated the occlusion of external iliac and femoral arteries. Revascula-risation was performed in the lateral decubitus position, with a ringed polytetrafluoroethylene (PTFE) graft bypass between normal common iliac and popliteal arteries through the greater sciatic foramen. Quick healing was observed. Patient is well 6 months postoperatively. Immediate and 6 month postoperative imaging demonstrated the good patency of the graft. A duplex ultrasound performed 6 month postoperatively showed no significant compression while the patient was in the sitting position.

PURPOSE: To compare iodixanol-enhanced multidetector-row computed tomography angiography (MDCTA) with digital subtraction angiography (DSA), perioperative angiography, or surgical findings in the evaluation of the abdominal aorta and its main branches. MATERIAL AND METHODS: 173 patients with known or suspected aortic aneurysms or stenosis/occlusion of the abdominal aorta or its major branches were enrolled. The iso-osmolar contrast medium iodixanol (320 mg Iota/ml) was used, and data were acquired using four-, eight-, or 16-active-detector-row scanners. Reference diagnoses were provided by surgical findings, interventional findings, or DSA. Diagnostic accuracy was estimated with reference to surgery or interventional arteriography or DSA. Image quality was assessed as excellent, good, sufficient, or insufficient, and correlations were made with attenuation values in the aortic lumen. Tolerability of iodixanol was monitored during the injection for discomfort and other adverse events, and for 72 hours after contrast injection. RESULTS: In 132 of 136 evaluable cases, MDCTA diagnosis matched the reference diagnosis, yielding an agreement rate of 97.1% (95% CI 92.6-99.2%). The quality of most MDCTA scans (147/173) was rated as excellent. Overall mean attenuation was 305.7 HU. MDCTA appeared more accurate than DSA for identification of lesion calcification, thrombus, irregularity, and ulceration. Tolerability of iodixanol was good, and no serious adverse events were reported. CONCLUSION: MDCTA using iodixanol is a promising, noninvasive alternative for evaluating patients with abdominal aortic disease.

RESUMO

Chronic ischemic gastritis is an unusual entity that is rarely distinguished from other forms of intestinal ischemia. On the basis of a case encountered and a subsequent review of the literature, the main features of this rare condition are described here. A better understanding and awareness of the disease should improve the diagnosis and ultimately also the prognosis.

PURPOSES: To retrospectively assess functional results of surgical treatment for renal artery dysplasia. PATIENTS AND METHOD: From January 1992 to December 2001, 11 symptomatic patients including 9 women (mean age 47 years, range 23-67), underwent surgery for renal artery dysplasia. Three patients presented with normalized hypertension, three others presented with isolated systolic hypertension, and the five remaining presented with grade II or III hypertension. Angiograms showed evidence of dysplasia of 16 renal arteries. Six arteries showed stenosis (37.5%), four aneurysms (25%), four both (25%), and two arteries showed wall irregularities (12.5%). Endovascular revascularizations were performed in five patients (45%) during the treatment of six renal arteries (37.5%). All endovascular procedures were angioplasties without any stent placement. Six patients (55%) underwent conventional procedures (in situ or ex-situ surgery) for the treatment of seven renal arteries (44%). RESULTS: In the short term, the treatment had a significant decrease on the systolic arterial pressure (p=0.019) and in the number of drugs used (p=0.009), but was not significant in the diastolic arterial pressure (p=0.084). The benefits persisted after two years of follow-up (PAS, p=0.023, PAD, p=0.047, drugs p=0.03). Benefits persisted in all of the six patients who had had hypertension for less than five years. There was 60% functional failure in the treatment of the five patients with a more than five years history of hypertension. CONCLUSION: This study confirms the short and long term's beneficial results of the surgical treatment of symptomatic renal artery dysplasia. Rapid care of those patients should optimize the functional results.

PURPOSE: To evaluate short and medium-term results of endovascular repair of the aortoiliac bifurcation for occlusive disease by the kissing stent technique. METHOD: Twenty patients were included in this retrospective study between May 1999 and November 2002 (13 men, mean age 58 years, range 42-86). Fifteen were treated for bilateral common iliac artery stenosis (75%), four for unilateral common iliac artery occlusion with or without controlateral stenosis (20%), and one for bilateral common iliac artery occlusion (5%). Each patient had a clinical examination and dupplex-scan prior to discharge, at three months, twelve months then yearly. RESULTS: All procedures were successful. Forty balloon expandable stents were deployed in the kissing position (mean length and diameter, 45 mm and 9 mm). Nine other stents were also deployed either in the abdominal aorta (one patient) either in the lower portion of the common iliac artery. Intra-operative complications were represented by one hematoma of the groin region treated surgically, one emboli of the femoral bifurcation treated by Forgarty balloon embolectomy, three dissections of the common iliac arteries treated by stenting, and one internal iliac artery occlusion not treated. Mean follow-up was 21 months (range 3-36). Three significant restenoses (>50%) were diagnosed during this period and one patient had an additional transluminal procedure. No intra-stent occlusion was found. Primary and secondary cumulative patencies at 12 and 36 months were respectively 94.7%, 84,4% and 100%, 89%. CONCLUSION: We confirm the feasibility, and the reliability of endovascular aorto-iliac kissing stent reconstruction in occlusive disease for selected patients.

RESUMO

Phase contrast magnetic resonance imaging (PC MRI) is a promising method for assessing coronary flow. MR angiography images in the presence of coronary stents display artifacts because of the metal present in the stent. Using a flow phantom, the goal of this in vitro study was to assess quantitatively the effects of flow dephasing caused by magnetic susceptibility in velocity measurements in a region where the artifact is not visualized in a magnitude image. The results showed that for high velocities, significant errors in measurements exist around the stent, outside the susceptibility artifact visible on a magnitude image. J. Magn. Reson. RESUMO

In a system consisting of synovial samples taken surgically and maintained in culture, the authors studied the regulation of enzyme synthesis by taking acid phosphatase as the test enzyme. By means of double labelling and the use of protein synthesis inhibitors, they were able to demonstrate that this synthesis was stimulated by the addition of rheumatoid factor to the culture medium and that it depended on the transcription activity of the genome and on the translation of the information molecules.

The authors report the results of 1258 chemical synovectomies carried out in subjects with rheumatoid polyarthritis on joints at Steinbrocker stage I and of 470 chemical synovectomies carried out during the first year of evolution of rheumatoid polyarthritis. Comparison with the results of chemical synovectomy carried out later demonstrate the superiority of early chemical synovectomy.

RESUMO

The authors report the results obtained from the treatment of 13 cases of myeloma by cyclic chemotherapy (melphalan) applied after cellular synchronization with vincristine. The clinical results (maximum 2 years after treatment) were good in 11 cases out of 13. The following laboratory values quickly returned to normal: sedimentation and calcaemia, but there was little change in the immunoglobulins.
