RESUMO

Flexor tendon rupture in the wrist of patients with rheumatoid arthritis is a rare complication, and there is no standard treatment for the wrist joint. Here, we present the case of a rupture of the flexor digitorum profundus of the left index finger owing to a rheumatoid wrist. Plain radiography and computed tomography showed carpal collapse, especially lunate, and arthrosis between the capitate and lunate. For stability and mobility of the wrist and index finger, resection of the lunate and radiotriquetral (RT) arthrodesis using the distal ulna as a bone graft and arthrodesis of the distal interphalangeal joint of the index finger were performed. At 2 years postoperatively, her wrist was painless and stable on radiography without recurrence of tendon rupture, and the arc of motion of the dorsal-palmar flexion of the wrist joint was 125°. RT arthrodesis could be a surgical choice of "mobile" partial wrist arthrodesis.

RESUMO

This is the first report of adalimumab-associated pulmonary cryptococcosis. A 56-year-old female with rheumatoid arthritis without a history of pulmonary disease was simultaneously administered adalimumab (40 mg/2 wks), methotrexate (4 mg/wk), and isoniazid (200 mg/day). Five months later, chest radiography revealed a small spiculated pulmonary nodule, and the laboratory test results, including levels of tumor markers and plasma ß-D-glucan, were within normal ranges. Since the lesion continued to grow, even after discontinuing adalimumab, it was surgically resected. Grocott staining of the tissue sample revealed black-brown fungi, identified as Cryptococcus neoformans in culture. The patient now remains well, without adalimumab therapy.

The acetabular component orientation of total hip arthroplasty is of critical importance to the clinical results. Although navigation systems have recently been introduced, acetabular component alignment guides are still used in most of ordinary hospitals. However, the accuracy of alignment guides themselves has not been evaluated. Fifteen types of alignment guide were examined. In all the alignment guides, the angles actually indicated and those stated by manufacturers were different. Our results showed that usage of modern alignment guides inherently misleads anteversion into decrease by a mean of 6 degrees (maximum, 12 degrees ) and inclination into increase by a mean of 2 degrees (maximum: 4 degrees ). Such setting of alignment guides could be one of the factors of error in acetabular component orientation.

The use of highly cross-linked polyethylene in total knee prostheses is still controversial. The aim of the present study was to compare radiographic and clinical results of using conventional and highly cross-linked polyethylene in cruciate retaining total knee prostheses of completely the same design. Two hundred and two consecutive total knee arthroplasties (NexGen CR, Zimmer) were performed using the same procedure. The first consecutive 113 knees had conventional polyethylene insert and following consecutive 89 knees had highly cross-linked polyethylene insert (Prolong, Zimmer). Differences in the age, gender, and diagnosis between two groups were not statistically significant. Preoperative range of motion (ROM) of the knee, and Knee Society Score (KSS) was better in highly cross-linked polyethylene group. Clinical and radiographic results were evaluated at two years after operation. The difference of ROM and KSS between groups was not statistically significant. There was no revision surgery. No knee exhibited osteolysis, aseptic loosening, or polyethylene failure. There was no early catastrophic clinical failure due to use of the new material.

We retrospectively reviewed the clinical and radiographic results of 30 revision total elbow arthroplasties in patients with rheumatoid arthritis using original surface prostheses developed in our institution. We asked whether a surface prosthesis is a suitable option for reconstructing a failed surface total elbow arthroplasty in terms of reproducing clinical performance compared with other devices such as semiconstrained prostheses. All 30 failed total elbow arthroplasties were performed with various surface prostheses and revised with Dogo Onsen Hospital surface prostheses. The mean age of the patients at the time of surgery was 64.9 years (range, 44-83 years), and the minimum followup was 3 years (mean 6.7 years; range, 3-11 years). The Mayo elbow performance index improved from 42.3 preoperatively to 76.3 at last followup. Pain, stability, and function scores improved after revision surgery, but range of motion scores were unchanged. Our data indicate a surface prosthesis is a reasonable option for reconstructing failed surface total elbow arthroplasties in patients with rheumatoid arthritis.

Nuclear factor of activated T-cells (NFAT) and calcineurin are essential regulators of immune cell and mesenchymal cell differentiation. Here we show that elevated intracellular calcium induces chondrogenesis through a calcineurin/NFAT signaling axis that activates bone morphogenetic protein (BMP) expression. The calcium ionophore, ionomycin, induced chondrogenesis through activation of calcineurin. The calcineurin substrate, NFAT4, also induced chondrogenesis and chondrocyte gene expression. Significantly, the BMP antagonist, noggin, or dominant negative BMP receptors blocked the effects of elevated intracellular calcium on chondrogenesis. This suggested that calcineurin/NFAT4 activates BMP expression. Consistent with this, BMP2 gene expression was increased by ionomycin and suppressed by the calcineurin inhibitor, cyclosporine A. Furthermore, activated NFAT4 induced BMP2 gene expression. These results have important implications for the effects of NFATs during development and adaptive responses.
