Our objective was to determine the effects of isoacids (ISO) on the lactation performance, digestibility, and milk fatty acid (FA) profile of Holstein cows fed 2 forage NDF levels (FL). The study was 10-wk long (including 2-wk for covariate) utilizing a randomized complete block design. Sixty-four mid-lactating Holstein cows [662 ± 71 kg BW, 119 ± 51 DIM, 2 ± 0.9 parity] were blocked by parity, DIM, and prior milk yield (MY) for multiparous cows or genetic merit for primiparous cows, and randomly assigned to 1 of the 4 diets (n = 16). Diets were arranged as a 2 × 2 factorial, with 2 FL containing 21 (HF) and 17% forage NDF (LF) without (WIA) or with ISO supplementation (IA, 7.85 mmol/kg DM and 3.44 mmol/kg DM for isobutyrate and 2-methylbutyrate, respectively). Diets were balanced for similar NEL (1.58 Mcal/kg DM), CP (16.0%) and total NDF (27.2%). Feed intake and MY were recorded daily. Nutrient digestibility for each cow was determined using indigestible NDF as a marker, and fecal samples were collected at 8-time points (4 h intervals between samples). Individual cow milk samples composited over a 10-wk period were analyzed using gas chromatography for FA profile. The statistical model included FL, ISO, and FL × ISO as fixed effects and block as a random effect (lme4 in R). The ISO did not affect DMI (P = 0.13), while LF had greater DMI than HF diets (27.8 vs. 26.0 kg/d; P < 0.01). However, ISO increased MY (34.7 vs. 37.2 kg/d; P < 0.01) and ECM (41.9 vs. 39.0 kg/d; P < 0.01) by 7% in HF but not in the LF diet, suggesting FL × ISO interaction (P = 0.04). Interestingly, ISO increased ADG (0.4 kg/d) but decreased MUN by 9% only in LF diet as indicated by FL × ISO interaction (P < 0.01). Additionally, ISO increased DM, OM, NDF, and CP digestibility by 10-24% in HF (P < 0.01), but not in LF (FL × ISO; P > 0.05). As expected, ISO increased milk odd chain FA profiles in the IA groups irrespective of FL, e.g., the IA had greater C15:0 (1.87 vs. 1.54 g/100g FA; P = 0.03) and a tendency to be greater C17:0 levels (0.86 vs. 0.76 g/100g FA; P = 0.05) compared with WIA groups. Overall, ISO improved MY and nutrient digestibility in the HF whereas it increased ADG and decreased MUN in LF diet. Additionally, ISO increased milk odd chain FA (C15:0 and C17:0) regardless of FL.

INTRODUCTION: We describe the programmatic scale-up of stool testing using Xpert® MTB/RIF Ultra (Ultra), as recommended by the WHO to improve childhood pulmonary TB (PTB) diagnosis. METHODS: USAID's Alliance for Combating TB in Bangladesh Activity, in collaboration with the National TB Control Programme, is implementing a stool-based diagnostic approach at 51 healthcare facilities in Bangladesh to improve PTB detection. Specimens from children (<15 years) with presumptive TB were tested using 'stool Ultra' with routine TB diagnostics. Physicians confirmed TB diagnosis and provided treatment as per national guidelines. RESULTS: Between March 2022 and December 2023, 16,429 specimens were tested, 871 (5.3%) were positive, and 642 (73.7%) showed 'trace detected' results. Positivity was significantly higher among females, and children presented with 'only cough ≥2 weeks', 'cough ≥2 weeks + weight loss', or 'fever ≥2 weeks, weight loss, fatigue + contact history'. Positivity was higher among '10-14 years old' children; however, 'trace detected' was highest among '5-9 years', followed by children aged '>2-<5 years' and '0-2 years'. CONCLUSIONS: Testing stools using Ultra provides a more effective way of diagnosing bacteriologically positive PTB in children. However, positivity varies with presenting symptoms/criteria, highlighting the need for careful diagnostic evaluation to ensure optimum use of limited diagnostic resources.


INTRODUCTION: Nous décrivons l'extension programmatique du dépistage des selles à l'aide de Xpert® MTB/RIF Ultra (Ultra), comme recommandé par l'OMS pour améliorer le diagnostic de la TB pulmonaire infantile (TBP). MÉTHODES: L'Alliance de l'USAID pour la lutte contre la TB au Bangladesh L'activité, en collaboration avec le Programme national de lutte contre la TB, met en œuvre une approche de diagnostic basée sur les selles dans 51 établissements de santé au Bangladesh afin d'améliorer la détection du TBP. Des échantillons prélevés chez des enfants (<15 ans) atteints de TB présumée ont été analysés à l'aide de « selles Ultra ¼ avec un diagnostic de routine de la tuberculose. Les médecins ont confirmé le diagnostic de TB et fourni un traitement conformément aux directives nationales. RÉSULTATS: Entre mars 2022 et décembre 2023, 16 429 échantillons ont été testés, 871 (5,3%) étaient positifs et 642 (73,7%) présentaient des résultats de « traces détectées ¼. La positivité était significativement plus élevée chez les femmes et les enfants présentaient « seulement de la toux ≥2 semaines ¼, « de la toux ≥2 semaines + perte de poids ¼ ou « de la fièvre ≥2 semaines, perte de poids, fatigue + antécédents de contact ¼. La positivité était plus élevée chez les enfants de 10 à 14 ans ; Cependant, les « traces détectées ¼ étaient les plus élevées chez les enfants de 5 à 9 ans, suivis des enfants âgés de « >2 à <5 ans ¼ et de « 0 à 2 ans ¼. CONCLUSIONS: L'analyse des selles à l'aide d'Ultra offre un moyen plus efficace de diagnostiquer la TBP bactériologiquement positive chez les enfants. Cependant, la positivité varie selon les symptômes/critères présentés, ce qui souligne la nécessité d'une évaluation diagnostique minutieuse pour assurer une utilisation optimale des ressources diagnostiques limitées.

Past studies have shown that isoacids (ISO) improve dairy cow performance, with effects varying based on dietary forage levels, leading us to speculate that ISO supplementation may also differentially affect enteric methane (CH4) emissions depending on dietary forage levels. Therefore, our primary objective was to examine the effects of ISO supplementation on enteric CH4 emissions in lactating dairy cows fed 2 forage NDF levels (FL), along with monitoring feed particle sorting and chewing behaviors to assess any potential interactions. Sixty-four (64) mid-lactation Holstein cows were utilized in a 10-wk long randomized complete block design trial. Parity, DIM, and prior milk yield (MY) for multiparous cows or genetic merit for primiparous cows were used as blocking factors. Cows were randomly assigned to 1 of 4 diets (n = 16 per diet) with a 2 × 2 factorial arrangement of treatment combinations, including 2 FL, 17 (LF) and 21% forage NDF (HF), without or with ISO supplementation (7.85 mmol/kg DM for isobutyrate and 3.44 mmol/kg DM for 2-methylbutyrate, respectively). Enteric CH4 and chewing activity (rumination and eating time) were measured using the GreenFeed system and sensor-based ear tag system, respectively. The particle size of each diet and ort from individual cows was measured using the Penn State Particle Separator, and a sorting index was calculated. A sorting index of 100 indicates no sorting, while values above or below 100 indicate sorting for or against, respectively. Data were analyzed using a mixed model including FL, ISO, and FL × ISO as fixed effects and block as a random effect (lme4 in R). Our result shows that ISO increased sorting index for long particle in LF (96.1 vs. 109; P < 0.01) but decreased it in HF (100.8 vs. 92.5; P = 0.04). In contrast, ISO did not affect the physically effective particle sorting index (P = 0.51) or intake (P = 0.27) regardless of FL. In alignment with the long particle sorting index, ISO decreased eating and chewing time in the HF but increased them in the LF diet (P < 0.01). In contrast, rumination time was comparable between FL (P = 0.70) and ISO levels (P = 0.19). In the LF diet, ISO supplementation reduced daily CH4 production (g/d) by 9% and intensity (g/kg of MY) by 18% (P < 0.01). In the HF diet, ISO supplementation led to a 10% increase in daily CH4 (P < 0.01) but did not change CH4 intensity (P = 0.17; g/kg of MY) due to improved milk production. Overall, ISO altered feed sorting, feeding behaviors and enteric CH4 emissions depending on FL.

Inflammatory bowel disease (IBD) is a chronic idiopathic inflammatory disorder of the gastrointestinal tract with relapsing and remitting course. Recurrent abdominal pain and discomfort in association with abnormal defecation in the absence of structural abnormality of the gut is the key feature of irritable bowel syndrome (IBS). Faecal biomarker may be used a precise tool in the differentiation of IBD and IBS. The aim of this study was to measure faecal calprotectin (FC) in patients with IBD and IBS and compare between them. This was a cross-sectional study done in the department of Gastroenterology, BSMMU, Bangladesh from May 2017 to August 2018. IBD patients were diagnosed on the basis of compatible history, clinical examination, laboratory, radiological and endoscopic findings. IBS patients were selected by using the Rome IV criteria. Quantitative faecal calprotectin ELISA (BUHLMANN Quantum Blue) test was done and compared between IBD and IBS patients. In this study, ninety (90) patients were enrolled, 45 patients with IBD and 45 patients with IBS. Mean age of the IBD patients was 32.24±9.76 years and IBS patients was 33.80±9.70 years. There were 28(62.2%) male and 17(37.8%) female patients with IBD and 30(66.7%) male and 15(33.3%) female patients with IBS. We found faecal calprotectin (FC) level was 445.68±237.35µg/gm in IBD patients and 39.16±17.31µg/gm in IBS patients. There was a significant difference of faecal calprotectin level between IBD and IBS patients (p-value <0.001). The sensitivity and specificity of faecal calprotectin to differentiate IBD from IBS was 91.1% and 86.7% respectively. The test accuracy was 88.9%. Area under ROC was 0.959 (95% CI, 0.909 to 1.0). This study showed that faecal calprotectin appears to be clinically useful, non-invasive, rapid and reliable marker to differentiate IBD from IBS.

Umbilical granuloma may be defined as a small mass of granulation tissue which develops at the base of the umbilicus after separation of the cord. It consists of true granulation tissue with fibroblast and abundant capillaries. Treatment options are chemical cauterization, electro cauterization and sometimes need surgical excision. Aims of this study were to develop a simple and feasible method for the treatment of umbilical granuloma with table salt (NaCl). This was a multi-center prospective type of observational study and conducted from January 2023 to March 2024 in the department of Pediatrics Surgery, Mymensingh Medical College Hospital, Mymensingh and other Private Hospitals of Mymensingh, Bangladesh. Total 66 patients were included among them 59(89.09%) cured and 7(10.6%) patients need other options. No complications and recurrence was seen in 3 months follow up. Treatment of umbilical granuloma with table salt (NaCl) is a simple, effective, low cost and feasible method.

The co-doping of vanadium pentoxide (V2O5) with rare-earth (RE) elements, namely 1.5 % holmium (Ho) and 1.5 % ytterbium (Yb) has been conducted using an eco-friendly, straightforward hydrothermal approach to assess the combined effects on structural, optical, and photocatalytic properties. The application of the density functional theory (DFT) approach effectively examined the impact of RE ions on the photocatalytic efficiency of co-doped V2O5. The stable orthorhombic crystal structure of co-doped V2O5 has been confirmed using DFT and X-ray diffraction without a secondary phase. It appears that homogeneous nucleation occurs while heterogeneous nucleation slows down in co-doped samples, as evidenced by the larger crystallite sizes in co-doped samples compared to doped ones. It means a result, the co-doped samples exhibit photodegrades more quickly and have a higher rate constant than the doped samples. This is because they have less dislocation density (4.26 × 10-3 nm-2) and internal micro-strain (4.93 × 10-3). The bandgap and degradation efficiency are determined by the UV-vis spectroscopy and found to be 2.33 eV and 95 %, respectively, at the optimal pH of 7 in the visible range. The co-doped sample has a rate constant of 24 × 10-3 min-1, which is the highest in the RE-doped V2O5 system. This is a good reason to think of co-doped V2O5 as a possible catalyst.

The study investigated the feasibility of enzymatic extraction for guava juice and evaluated the effects of various preservatives on its shelf life. The crushed guava puree was undergone different pectinase enzyme concentrations over three incubation periods. The findings revealed that pectinase concentrations of 0.1 % and 0.2 %, when incubated for 1 and 2 h, were the most effective. Juice yields ranged from 65.24 % to 78.64 %, with Total Soluble Solids (TSS) varying from 9.12°Brix to 11.56°Brix. The physicochemical properties of the guava juice resulted 84.2 % moisture, 2.16 % protein, 0.77 % fat, 3.27 % fiber, 0.65 % acidity, 2.25 % reducing sugars, 8.27 % non-reducing sugars, 79.53 % antioxidant activity, 173.2 mg/100g of ascorbic acid, 10.52 TSS, 109.7 mg/100g of phenolic content, and a pH of 3.2. Eight juice samples were prepared as per formulation with sodium benzoate and potassium metabisulfite (KMS) at concentrations of 150 ppm, 200 ppm, and 250 ppm, in addition to one refrigerated sample and one control. The stability of these guava juice samples was monitored every 15 days over a 90-day period. Results showed that acidity, TSS, pH, reducing sugars, and non-reducing sugars changed over time. Samples with preservatives exhibited slower changes compared to the control. Phenolic compounds diminished more quickly at ambient temperature than in refrigerated or preservative-treated samples. Initially, phenolic content and antioxidant capacity were 44 mg GAE/100g and 44 %, respectively, but declined to 10-15 mg and 15-17 % by the end of the storage period. Color changes were more noticeable in samples stored at room temperature, whereas preservatives effectively reduced color degradation caused by enzymatic browning. Moreover, ascorbic acid retention was better in samples with preservatives and those stored under refrigeration. The ascorbic acid degradation rate was highest at room temperature (0.023 day^-1) and lowest with 250 ppm KMS (0.016 day^-1). Microbiological tests indicated that the juice remained safe for 40 days at room temperature, 90 days under refrigeration, and approximately 85 days with preservatives.

The blood-based detection of Alzheimer's disease (AD) is becoming challenging since the blood-brain barrier (BBB) restricts the direct circulation of AD molecules in the blood, thereby precluding the detection of AD at an early-stage. Herein, we report the development of a novel CNT-metal-porous graphene hybrid (CNT-MGH) nano-interdigitated array (n-IDA) electrochemical 8-well biosensor for the successful early-stage diagnosis of AD from blood. Laser-induced graphene (LIG) technology has been used to fabricate the proposed CNT-MGH n-IDA 8-well sensor. Firstly, the electrochemical characterization (i.e., electrode gap, material composition, etc.) of the proposed sensor was demonstrated by measuring p-aminophenol (PAP) with a limit of detection (LOD) of 0.1 picomole. Subsequently, the CNT-MGH n-IDA 8-well sensor was then used to diagnose AD via novel blood biomarkers p-Tau 217 and p-Tau 181 using an electrochemical enzyme-linked immunosorbent assay (e-ELISA) enzyme by-product PAP. During e-ELISA, the alkaline phosphatase enzyme (IgG-AP) tagged to the detection antibody produced an electroactive ELISA by-product PAP by reacting with the enzyme-substrate 4-aminophenyl phosphate (PAPP). Finally, the CNT-MGH n-IDA 8-well sensor was then used to measure the current generated by the redox reaction via the e-ELISA by-product PAP. While quantified, the proposed CNT-MGH n-IDA 8-well sensor successfully detected p-Tau 217 and p-Tau 181 proteins in blood with LODs of 0.16 pg ml-1 and 0.08 pg ml-1, respectively.

PURPOSE: To develop a reliable method to generate a mouse model of branch retinal artery occlusion (BRAO) using laser-induced thrombosis of a major artery in the mouse retina. Also, to develop a reliable method to detect retinal hypoxia as predictive biomarker for the risk of neuronal cell damage in BRAO. METHODS: A reliable and reproducible model of laser-induced BRAO was developed in mouse retina using Rose Bengal. To characterize retinal hypoxia in BRAO, pimonidazole immunostaining and HYPOX-4 molecular imaging methods were used. Terminal deoxynucleotidyl transferase dUTP nick end labelling (TUNEL) was used to characterize neuronal cell damage in the BRAO retina. Expression of mRNA in retinal tissues from BRAO and age-matched control retinas were analyzed using qRT-PCR. RESULTS: Occlusion of a branch retinal artery near the optic nerve head (ONH) caused a pattern of retinal tissue hypoxia covering about 12.5% of the entire retina. TUNEL-positive cells were localized in all layers in BRAO retinal tissue cross sections. In addition, qRT-PCR data analysis suggests that BRAO is associated with both inflammation and hypoxia. CONCLUSIONS: This study provides a reliable method for BRAO in mouse retina and demonstrates the utility of molecular imaging method to detect retinal hypoxia as predictive biomarker for the risk of neuronal cell damage in BRAO. In addition, our data suggest that BRAO retinas are associated with inflammation and also associated with hypoxia-related neuronal cell damage. PERSPECTIVES: Imaging areas of retinal hypoxia may provide accurate diagnosis, evaluating retinal tissue injury from BRAO.

Multiple myeloma (MM), mature B-cell lineage neoplasm, is characterized by abnormal clonal proliferation of plasma cells and presence of monoclonal protein (M protein). The study was conducted to reveal presenting features, laboratory findings, Eastern Cooperative Oncology Group (ECOG) performance status and skeletal survey on patients with multiple myeloma. This descriptive, cross-sectional study was carried out in the Department of Haematology, Dhaka Medical College Hospital, Dhaka, Bangladesh from January 2019 to July 2020 with a sample size of 81. Data were collected in a case record form after obtaining informed verbal consent from patients and /or their legal guardians. Relevant ethical issues and data quality assurance were taken into consideration. Data were analyzed with SPSS, Version 25.0 with presentation in figures and tables with frequency, percentage, mean and standard deviation based on data nature. Statistical tests were carried out as appropriate with 5.0% level of significance for assessing statistical association. Mean age of the patients was 58.9±12.0 years. Male female ratio was 2:1. 35(43.2%) patients were smokers with only 2(2.5%) had family history of haematological malignancies. Bone pain (72.8%) was the most common presenting feature, while hypertension (59.1%), diabetes mellitus (29.5%), respiratory illness (11.3%) and cardiac disease (11.4%) were the common co-morbidities. Most common ECOG performance status was ECOG-1(48.1%). Mean haemoglobin (Hb) was 9.4±2.3gm/dl and mean erythrocyte sedimentation rate (ESR) was 89.5±42.1 mm in 1st hour. Mean serum creatinine level was 2.0±1.85 mg/dl and ≥2.0mg/dl in 42(34.2%). Among 50 documentation serum lactate dehydrogenase (LDH) was raised in 18(36.0%). Mean serum calcium level was 9.6±1.8mg/dl >11.0mg/dL in 10(14.5%) cases. Serum albumin <3.5gm/dl in 37(49.3%), ß2-microglobulin >5.5mg/dl in 37(57.8%) cases, International staging system (ISS) stage III was in 59.4% and Bence Jones Protein (BJP) was present in 46.7% cases. Lytic lesions were present in 75.0%, In 38(74.5%) patients vertebrae were involved, while in 18(35.2%) ribs were involved, in 14(27.5%) patients skull was involved and in 3(5.9%) patients involved bones were femur, humerus, sternum and scapula. Mean plasma cells percentage was 62.1±24.9%. Immuno-Fixation Electrophoresis (IFE) revealed IgG (72.7%), IgA (18.2%), Free light chain (FLC) (9.1%). FLC ratio was ≥100 in 29.0% cases. Significant statistical association was observed between serum creatinine with Hb concentration (p<0.05), serum creatinine level with ISS staging (p<0.05) and serum calcium level (p<0/05), while insignificant association was revealed between BJP present status and serum creatinine level (p>0.05). Bone pain, fatigue, fever and neurological impairment were the common presenting features. Anaemia, renal impairment and skeletal lytic events were the prominent physical findings. ISS staging was statistically associated with serum creatinine level, while serum calcium level was associated with serum creatinine and lytic lesions.

BACKGROUND: Thyroid cancer is the most common endocrine malignancy. Current therapies are successful, however some patients progress to therapeutically refractive disease. The immunotherapeutic potential of the CXCL8-chemokine/CXCR2-chemokine-receptor system is currently being explored in numerous human cancers. This study aimed to evaluate if the targeting of CXCR2 by its selective antagonist, AZD5069, could modulate CXCL8-mediated pro-tumorigenic effects in thyroid-cancer (TC) cells in vitro. METHODS: Normal human primary thyroid cells (NHT) and TC cell lines TPC-1 (RET/PTC), BCPAP, 8505C and 8305C (BRAFV600e) were treated with AZD5069 (100 pM-10 µM) over a time-course. Viability and proliferation were assessed by WST-1 and crystal violet assays. CXCL8 and CXCR2 mRNA were evaluated by RT-PCR. CXCL8-protein concentrations were measured in cell culture supernatants by ELISA. CXCR2 on cell surface was evaluated by flow-cytometry. Cell-migration was assessed by trans-well-migration chamber-system. RESULTS: AZD5069 exerted negligible effects on cell proliferation or viability. AZD5069 significantly reduced CXCR2, (but not CXCL8) mRNAs in all cell types. CXCR2 was reduced on the membrane of some TC cell lines. A significant reduction of the CXCL8 secretion was found in TPC-1 cells (basal-secretion) and NHT (TNFα-induced secretion). AZD5069 significantly reduced basal and CXCL8-induced migration in NHT and different TC cells. CONCLUSIONS: Our findings confirm the involvement of the CXCL8/CXCR2-axis in promoting pro-tumorigenic effects in TC cells, further demonstrating its immunotherapeutic significance in human cancer.

In recent years, the proliferation of Massive Open Online Courses (MOOC) platforms on a global scale has been remarkable. Learners can now meet their learning demands with the help of MOOC. However, learners might not understand the course material well if they have access to a lot of information due to their inadequate expertise and cognitive ability. Personalized Recommender Systems (RSs), a cutting-edge technology, can assist in addressing this issue. It greatly increases resource acquisition through personalized availability for various people of all ages. Intelligent learning methods, such as machine learning and Reinforcement Learning (RL) can be used in RS challenges. However, machine learning needs supervised data and classical RL is not suitable for multi-task recommendations in online learning platforms. To address these challenges, the proposed framework integrates a Deep Reinforcement Learning (DRL) and multi-agent approach. This adaptive system personalizes the learning experience by considering key factors such as learner sentiments, learning style, preferences, competency, and adaptive difficulty levels. We formulate the interactive RS problem using a DRL-based Actor-Critic model named DRR, treating recommendations as a sequential decision-making process. The DRR enables the system to provide top-N course recommendations and personalized learning paths, enriching the student's experience. Extensive experiments on a MOOC dataset such as the 100 K Coursera course review validate the proposed DRR model, demonstrating its superiority over baseline models in major evaluation metrics for long-term recommendations. The outcomes of this research contribute to the field of e-learning technology, guiding the design and implementation of course RSs, to facilitate personalized and relevant recommendations for online learning students.

Freshwater pearl farming is an emerging sector of aquaculture in Bangladesh which plays a growing role at major jewelry markets. With some improved techniques, high quality image or designer pearls are now produced from freshwater mussels Lamellidens marginalis. Yet it is difficult to reach in conclusion as the quantities produced, culture techniques used, and the upgrading of the existing culture technique are not well documented. Furthermore, many obstacles such as proper dissemination of culture technologies among the interested peoples, optimization of the culture environment and culture methods, standardization of breeding protocol and so on need to be addressed by the scientific community. This review article reports for the first time about the status of freshwater pearl culture in Bangladesh highlighting the fundamentals of pearl production, culture techniques used in farms, challenges, and prospects for upgradation of current culture principles in Bangladesh.

Stroke is the common cause of death and disability worldwide, as well as in Bangladesh. Serum electrolytes abnormalities or dyselectrolytaemia is one of the major acute complications of stroke. Dyselectrolytaemia or serum electrolytes (sodium and potassium) abnormalities are more common in patients with acute stroke that can be easily measured. The study was planned to find out the serum electrolytes (sodium and potassium) abnormalities in acute stroke patients. This cross-sectional study was conducted in the Department of Neurology and Medicine, Mymensingh Medical College and Hospital from January 2019 to June 2020. Total 84 purposively selected patients with acute strokes were evaluated following informed written consent. Diagnosis was confirmed by neuroimaging of brain. Moreover, serum electrolytes level was measured for each patient. Data were collected by interviews, clinical examinations & laboratory investigations of the patients using a case record form and analysis was carried out by the help of SPSS 25.0. Mean age of the patients with acute strokes were 57.65±15.79 years. About two thirds (60.7%) of the patients were male and the remaining (39.3%) were female. Sodium imbalances were observed in 32.2% and potassium imbalances in 25.0% cases. About 66.7% haemorrhagic strokes patients and 42.2% ischaemic strokes patients had dyselectrolytaemia (p<0.05). More than twenty eight percent (28.6%) of all stroke patients had hyponatraemia, which was more common (35.9%) among haemorrhagic strokes patients (p<0.05). Of all stroke patients 21.4% had hypokalaemia, which was more common (28.2%) in haemorrhagic strokes patients (p<0.05). This study reveals that, serum electrolytes (sodium and potassium) abnormalities are more common in haemorrhagic than ischaemic strokes, which is mainly hyponatraemia and hypokalaemia.

Coronary artery disease (CAD) is considered as a major cause of morbidity and mortality worldwide. Inflammatory cytokines play an important role in the pathogenesis and progression of atherosclerosis. The aim of the study was to find out the association of C-reactive protein (CRP) and triglyceride (TG) level on the severity of CAD in patients with ischemic heart disease (IHD). This cross-sectional study was performed in the Department of Cardiology, National Institute of Cardiovascular Diseases, Dhaka, Bangladesh during the period of March 2018 to February 2021. Total 431 patients with ischemic heart disease were enrolled after taking informed written consent. CRP values were categorized into normal (<6 mg/L), borderline (6-10 mg/L) and high (>10mg/L) and TG level were categorized into normal (<150 mg/dl), borderline (150-199mg/dl) and high (≥200 mg/dl). Patients with ischemic heart disease (IHD) were stratified according to CRP value and TG level. Severity of CAD was assessed by the Gensini score. Most of the patients (33.4%) belonged to age 51-60 years. The mean age was 51.31±10.30 years. The majority (74.5%) of patients were male. Among risk factors, the highest 205(47.6%) patients were smokers followed by hypertension 190(44.1%) and diabetes mellitus 175(40.5%). The association of TG and CRP with the whole spectrum of IHD was found statistically significant (p<0.05). Severe CAD was found higher in high TG and high CRP level group compared with the other groups and was statistically significant. Inflammation assessed by high CRP and hypertriglyceridemia associated with the risk and severity of CAD.

BACKGROUND: The aim of this study is to estimate the factors at both the individual and community levels related to the adequacy of iodized salt in households in Bangladesh. METHODS: For this study we utilized the 2019 Multiple Indicator Cluster Survey data. A total of 61 242 households were chosen as samples from Bangladesh. In our study of socio-economic disparities, we applied a concentration indexing method. To identify the factors associated with the adequacy of iodine in salt at both the individual and community levels, we employed multilevel logistic regression. Aside from the multilevel regression used in the study, we also applied spatial analysis. RESULTS: The results indicated that the prevalence of iodine adequacy in household salt was found to be 57.8% (95% confidence interval 57.4 to 58.2). Rural areas have a higher concentration of iodine than urban areas. According to the multilevel model, younger women (adjusted odds ratio [aOR] 0.70), Muslim women (aOR 0.89), illiterate women (aOR 0.80) and those from poor households (aOR 0.33) were found to be less likely to consume iodine in concentrated salt compared with their counterparts. Disabled women and those with low media exposure have a lower likelihood of iodine adequacy in salt compared to their reference group. Furthermore, households in urban areas exhibited higher odds of having iodine adequacy in salt compared with households in rural areas. Barisal, Chattogram, Dhaka, Khulna, Mymensingh, Rajshahi and Rangpur Divisions have lower iodine adequacy in salt compared with Sylhet Division. CONCLUSIONS: The findings reveal that religion, physical disability and exposure to media exert an equal influence on the presence of iodized salt intake. Moreover, women's age, wealth status, education level and the educational background of the household head positively contribute to the adequacy of iodine in household salt. In light of these results, policymakers are advised to prioritize efforts aimed at enhancing iodine concentration, with a particular focus on mass media advertising, especially in rural areas (excluding Sylhet Division).

Till the day the rectal cancer deaths in the world is in eighth position. For rectal cancer surgery, short-term benefits are expected to be similar for laparoscopic resection of rectum (LRR) and open resection of rectum (ORR). In Bangladesh though the rectal cancer is the sixteenth most common cause of cancer deaths, there is lack of data regarding outcomes of laparoscopic and open surgical approaches for carcinoma rectum. Purpose of study was to compare oncopathological outcomes by quality of surgical resection between LRR and ORR groups. The quasi experimental study was conducted among 46 subjects who attended in the Department of Colorectal Surgery, Bangabandhu Sheikh Mujib Medical University, Bangladesh from July 2020 to June 2021 with rectal adenocarcinoma within 15cm from the anal verge. Equal number of subjects were allocated for LRR and ORR group. Oncopathological end points such as circumferential resection margin (CRM), distal resection margin (DRM), lymph nodes and quality of mesorectal excision were assessed and compared. Here every patients have given their written consent for this study. Mean age of the subjects for LRR and ORR were 45.47±12.66 and 44.47±12.48 years respectively. Majority of the subjects were in age above 40 years (67.0%). The proportion of male (56.5%) were higher than those of female (43.5%). The complete resection was better in LRR (91.3%) than ORR (87.0%) group though statistically not significant. CRM was lower in LRR (0.0%) than ORR (13.0%) group in respect of frequency distribution. DRM was negative for both LRR and ORR group (95.7% each) and it was not statistically significant. ORR met the National Comprehensive Cancer Network (NCCN) criteria of harvesting 12 lymph nodes were as in LRR group 10 lymph nodes were harvested. The mean harvested lymph nodes were 12.2±5.55 and 10.1±5.55 in LRR and ORR group respectively. The study demonstrated that LRR is better (though statistically not significant) in respect of complete resection and CRM while in harvesting lymph nodes, ORR met the NCCN criteria but LRR does not. There is no difference observed regarding DRM in both groups. On oncopathological point of view both the group showed almost equally effective results. Laparoscopic surgery can be opted as the standard operative technique for surgical management of rectal cancer.

A wide variety of CreERT2 driver lines are available for genetic manipulation of adult-born neurons in the mouse brain. These tools have been instrumental in studying fate potential, migration, circuit integration, and morphology of the stem cells supporting lifelong neurogenesis. Despite a wealth of tools, genetic manipulation of adult-born neurons for circuit and behavioral studies has been limited by poor specificity of many driver lines targeting early progenitor cells and by the inaccessibility of lines selective for later stages of neuronal maturation. We sought to address these limitations by creating a new CreERT2 driver line targeted to the endogenous mouse doublecortin locus as a marker of fate-specified neuroblasts and immature neurons. Our new model places a T2A-CreERT2 cassette immediately downstream of the Dcx coding sequence on the X chromosome, allowing expression of both Dcx and CreERT2 proteins in the endogenous spatiotemporal pattern for this gene. We demonstrate that the new mouse line drives expression of a Cre-dependent reporter throughout the brain in neonatal mice and in known neurogenic niches of adult animals. The line has been deposited with the Jackson Laboratory and should provide an accessible tool for studies targeting fate-restricted neuronal precursors.

To achieve the health benefit from the natural of Basella. Albafruit. This study intended to figure out the bioactive compounds in the two varieties of B. alba (native and hybrid) fruit extract and measurement its biological activities like antioxidant, anti-inflammatory, cytotoxic activities and a molecular docking were performed to observed the pharmaceutical impact on the anti-inflammatory Cyclooxygenase-2 (COX-2) enzyme. The cold extractions along with GC-MS were used for the extraction of and analysis of phytoconstituents from B. alba fruit. The hemolytic inhibitory and BSA (Bovine serum albumin)-denaturation assay, DPPH(2,2-diphenyl-1-picrylhydrazyl) and H2O2-free radical scavenging analysis, and brine shrimp lethalness test were performed to measure the biological activities of the extracted The biological activities assay results showed that the ethanol extract of native malabar spinach exhibited dose-dependent antioxidant activity. The IC50 value 21.55 ± 1.51 µg/mL was for DPPH scavenging assay and 23.36 ± 0.36 µg/mL was for H2O2 scavenging analysis. In anti-inflammatory activity assessment study, the IC50 values of the ethanol extracts were 20.52 ± 0.91 µg/mL for BSA inhibition and 20.43 ± 1.30 µg/mL for RBC hemolytic inhibitory study. In this study, cytotoxicity test results reveal that aqueous extract exhibited no cytotoxicity as compared to ethanol and ethyl acetate extract (LD50 = 875.27 µg/mL). Conversely, the current study insist the in silico analysis, to find out the anti-inflammatory activity of the investigated two fruit varieties due to pharmacokinetics analysis, toxicity properties analysis, ADMETand molecular docking. The result of this study signified that both (native and hybrid) malabar spinach fruit varieties contain phytoconstituents with potent antioxidant, anti-inflammatory, and cytotoxic action.Moreover, the in vitro and in silico results suggest that the native and hybrid fruit varieties of the extracts could be a superior striver for future appraisal as a prospective therapeutically active ingredient.

The space-time fractional nonlinear Klein-Gordon and modified regularized long-wave equations explain the dynamics of spinless ions and relativistic electrons in atom theory, long-wave dynamics in the ocean, like tsunamis and tidal waves, shallow water waves in coastal sea areas, and also modeling several nonlinear optical phenomena. In this study, the improved Bernoulli sub-equation function method has been used to generate some new and more universal closed-form traveling wave solutions of those equations in the sense of beta-derivative. Using the fractional complex wave transformation, the equations are converted into nonlinear differential equations. The achieved outcomes are further inclusive of successfully dealing with the aforementioned models. Some projecting solitons waveforms, including, kink, singular soliton, bell shape, anti-bell shape, and other types of solutions are displayed through a three-dimensional plotline, a plot of contour, and a 2D plot for definite parametric values. It is significant to note that all obtained solutions are verified as accurate by substituting the original equation in each case using the computational software, Maple. Additionally, the results have been compared with other existing results in the literature to show their uniqueness. The proposed technique is effective, computationally attractive, and trustworthy to establish more generalized wave solutions.
