RESUMO

The study was made of the action of electromagnetic decimetric waves (DMW) of mild and strong heat power and placebo on intraorganic circulation using functional polyrheography and morphological examination of the kidneys with infectious-inflammatory process. DMW of the above power have a marked hemodynamic action via stimulation of the circulation of the intermediary zone with an effect of redistribution of the intrarenal blood flow. Weak heat DMW in experimental pyelonephritis exhibit a distinct antiinflammatory effect while strong DMW impact aggravates vascular reaction associated with inflammation. This deteriorates the inflammation and gives rise to hyperimmune reactions. The findings provide pathogenetic grounds for DMW therapy. It is recommended for treatment of patients with chronic pyelonephritis with dynamic obstruction of the upper urinary tracts allowing for the power of the impact and activity of the inflammation.

RESUMO

The experimental study investigated action of electromagnetic waves (84 mW/cm2 and 395 mW/cm2) on functional-morphological features of healthy kidney of female rats. A marked hemodynamic effect of decimetric waves is shown which operates via stimulation of intermediary zone with redistribution of intrarenal circulation. Disturbance of circulation occurs in dysregulation of the intrarenal blood flow under overstimulation of the organ circulation with 395 mW/cm2 decimetric waves. This entails disorders of hemodynamics, flebostasis, stromal edema, dystrophy of tubular epithelium, periglomerular sclerosis causing aseptic interstitial inflammation. This is also proved by levels of prostaglandin E2 in tissue of renal papillae. These findings should be considered in administration of decimetric waves in patients with renal inflammation.

It is in the gastrobiopsies that morphological features of chronic gastritis--inflammation and activity according to the International Classification of Gastritis (1990)--are more pronounced in the atypical hyperplasia and dysplasia than in normal epithelium. This means that inflammatory cells may play a role in the development of atypical hyperplasia and moderate dysplasia. Nuclear-cytoplasmic ratio in cases of the foveolar epithelium dysplasia was 0.89 and in the dysplasia of the metaplastic epithelium of intestinal type it was 0.99, this being higher than in normal epithelium. Complete intestinal metaplasia was large-focal or diffuse, while incomplete was predominantly small-focal.

Helicobacter pylori (HP) in 41 patients (10 with stomach ulcer and 31 with duodenum ulcer) was found in gastro-biopsies from the stomach antrum before and after the treatment. The following methods to reveal HP were used: Giemsa staining, immunoperoxidase reaction with polyclonal monospecific antibodies against HP, urease rapid test. HP was also revealed in the blood serum by antibodies against HP by means of enzyme immunoassay. Electron microscopy was performed in 5 patients and bacteriologic study in 10 patients. Immunohistochemical method allows one to judge about real HP colonization of the gastric mucosa. Bacterioscopy with Giemsa staining gives good results but they are not true markers of HP presence this being explained by other methods. Serologic enzyme immunoassay gives an idea about the patient contamination, but not about HP eliminations as a result of treatment. Urease-test is adjuvant.

The work compares the results of fine-needle aspiration biopsy (FAB) of the thyroid gland in nodular and diffuse-nodular euthyroid goiter (63 patients) with the findings of pathomorphological study of the operative material (51 patients). The authors' original device for puncture of the thyroid node is described. The suggested method makes it possible to raise the efficacy of obtaining an adequate amount of aspired material to 92%. On the basis of the results of FAB and ultrasonic examination (USE) unfounded surgical intervention was avoided in 12 of the 63 cases. The results of the study bear evidence of the high diagnostic efficacy of FAB as to obtaining the cytologic material and revealing malignant degeneration (96% specificity). The authors believe that FAB in combination with USE should be an obligatory component of complex clinico-instrumental examination of patients with various diseases of the thyroid, nodular forms of affection in particular.

RESUMO

Clinico-morphological study of the gastroduodenal system and also morphometric investigations of the degree of gastroduodenitis activity and the condition of the local immune system were made in 30 children aged from 4 to 15 years with juvenile rheumatoid arthritis (JRA). Microbiological, histological and electron microscopic methods for revealing Campylobacter pylori (CP) were used. Interrelationship between various clinical manifestations of JRA with the nature of gastroduodenal system affection and with the incidence of microbial colonization of SP was determined. All the patients with JRA had lesions in the gastric and duodenal mucosa, and some morphological features were revealed in them. SP was diagnosed in 86.7 per cent. There was a relation between the microbial clonization of SP and the degree of the rheumatoid process activity. It is suggested that SP does not play an etiological role in the development of gastroduodenitis in JRA but may serve as a pathogenetic factor in the development of an erosive-ulcerous lesion.

Distribution of lectins (PNA, SBA, WGA, and Con A) has been studied in 53 stomach biopsies. Positive lectin reaction in the foveolar epithelium is observed, including dysplastic foci in the apical plasmalemma and in the cytoplasm around the nucleus this corresponding to the Golgi complex. It was mainly the apical plasmalemma of the columnar epithelium in the foci of intestinal metaplasia. Differences in the receptor localization were established in non-complete intestinal metaplasia, hyperplastic polyps and adenomas this depending on the cell type. Carcinoma cells not infrequently had all the plasmalemma stained with granules of the reaction product being distributed through all the cytoplasm. Lectin reactions are more pronounced in carcinomas accumulating mucus. Lectin binding by tumorous cells containing mucus was enhanced after the elimination of sialic acid as a result of the incubation with neuraminidase. Intensity of lectin reactions somewhat differs in various histological forms of stomach cancer.

The paper deals with reparative potential of gastric and duodenal mucosa in ulcer in connection with the presence and quantity of Campylobacter pylori (CP) which is found practically in all patients not only with duodenal localization of ulcer but with mediogastric and pyloroantral localization as well. No correlation between the CP presence and CP mucosa colonization on the one hand and the type of reparative processes in the mucosa is found. The presence or the absence of CP as well as the degree of CP mucosal dissemination have no influence on the clinical course of the ulcer. Antiulcer treatment aiming at aggression factors (histamine H2-receptors blockers) and protective factors (enprostil) and lacking antibacterial activity improves the reparative processes but does not influence CP. Thus, CP is not etiological and probably is not a main pathogenetic factor in ulcer development. Its participation in the genesis of ulcer is possibly brought about under conditions of suppression of local protective mechanisms.

Gastric biopsies from 90 children (from 5- to 15-year-old) with chronic gastroduodenitis were examined for Campylobacter pylori (CP) by bacteriological, histological (Giemsa stain, argentation by Warthin-Starry, acridine orange stain) and electron microscopical methods. CP is detected in 81.2% children. The degree of colonization was low or moderate. The increase of occurrence was noted with age and duration of disease. Persistence of CP went down with the increase of atrophic change in the gastric mucosa and duodenum, but there was no clear-cut correlation with the morphological variant of gastritis and the stage of disease. Colonization with CP is regarded as an important part in the pathogenesis of chronic gastroduodenitis in children.

RESUMO

Ameloblastoma (also known as adamantinoma, adamantine epithelioma, adamantinoblastoma, and epithelial odontoma) is a benign tumor of ectodermal origin arising from odontogenic epithelium, most commonly in the lower molar region in individuals aged between 20 and 50 years. Several histological variants are recognized, the most common being the follicular type. The present communication demonstrates the desirability of conducting parallel cytological and histological studies in cases of jaw tumors. Results of one study are reported; the cytologic profile of the tumor enabled the authors to identify cell type and establish the odontogenic origin and benign nature of the tumor. Such studies are stated to be essential in developing criteria for cytologic diagnosis of odontogenic tumors.
