Background: Gender dysphoria is a distress caused by a mismatch between gender identity and the sex assigned at birth. About 0.5% of the population suffer from gender dysphoria, which represents 25 million people worldwide. Gender-affirming mastectomy is the most common procedure for female-to-male patients. Objectives: The aim of this single-center retrospective study is to present the outcomes after mastectomy and to evaluate patient satisfaction using the BODY-Q questionnaire. Methods: Several data regarding patient characteristics and surgery have been collected. A satisfaction survey has been sent to patients. Two groups, "NAC grafts" and "semicircular," have been compared for complications and satisfaction. Results: A total of 103 patients have had a transgender mastectomy performed by 3 surgeons, representing 206 mastectomies. There were 5 wound infections (4.8%), 8 seromas (6.8%), 10 hematomas (6.8%), and 23 partial/total nipple areolar complex (NAC) necrosis (20.4%). The complication rates in this study are similar to others in the literature. Few studies express interest in patient satisfaction after this type of surgery and even fewer use a suitable questionnaire. Conclusions: Transgender mastectomy is a safe and often necessary procedure to improve the quality of life of patients suffering from gender dysphoria. Nevertheless, there is currently no validated tool to assess postoperative satisfaction within this specific population group.

RESUMO

The use of skin substitutes in burn surgery and in the treatment of acute or chronic wounds is constantly evolving. For years, scientists have been researching skin substitutes that can be used in place of autologous skin. New products are regularly developed and approved for clinical use. In this article, we take a look at the skin substitutes most commonly used in Europe and briefly summarize the current clinical experience of our centre.


L'utilisation des substituts cutanés dans la chirurgie des grands brûlés et dans le traitement des plaies aiguës ou chroniques est en constante évolution. Depuis des années, les scientifiques recherchent des substituts cutanés qui peuvent être utilisés à la place de la peau autologue. De nouveaux produits sont régulièrement développés et approuvés pour l'utilisation clinique. Dans cet article, nous examinons les substituts cutanés les plus utilisés en Europe et résumons brièvement l'expérience pratique de notre centre.

Winslow's foramen hernia, or Blandin's hernia, is a rare internal hernia with a non-specific clinical presentation and its diagnosis may be difficult. The hernia occurs across the omental hiatus, bounded by the inferior vena cava posteriorly and the portal triad anteriorly. CT imaging provides several diagnostic clues in this condition. Prompt surgical management allows reduction before complications arise. We present a case of caecal internal herniation through Winslow's foramen in a patient who underwent gastrojejunal bypass about ten years ago. Laparoscopy with reduction of the caecal bascule and closure of the foramen is performed successfully.


La hernie du foramen de Winslow, ou hernie de Blandin, est une hernie interne rare, avec une présentation clinique non spécifique et son diagnostic peut être difficile. La hernie se produit au travers de l'hiatus omental, limité par la veine cave inférieure en arrière et la triade portale en avant. L'imagerie médicale par tomodensitométrie nous fournit plusieurs indices diagnostiques dans cette pathologie. La prise en charge chirurgicale rapide permet une réduction avant la survenue de complications. Nous présentons un cas de hernie interne caecale par le foramen de Winslow chez un patient ayant bénéficié d'un bypass gastro-jéjunal une dizaine d'années auparavant. Une laparoscopie avec réduction de la bascule caecale et fermeture du foramen est réalisée avec succès.

RESUMO

Abdominal aortic lacerations are very rare in closed trauma and are usually caused by high energy trauma. Aortic injuries should be assessed as a priority as they present an immediate life-threatening risk in the event of major haemorrhage. However, the clinical presentations are highly variable and often misleading. We -present a case of traumatic blunt abdominal aortic laceration following a fall down the stairs. The laceration involved the posterior part of the abdominal aorta secondary to a lumbar fracture with L1-L2 disc tear.


Les lacérations de l'aorte abdominale sont très rares dans les traumatismes fermés et sont généralement causées par des traumatismes à haute énergie. Les lésions aortiques doivent être évaluées en priorité car elles présentent un risque vital immédiat en cas d'hémorragie majeure. Cependant, les présentations cliniques sont très variables et souvent trompeuses. Nous présentons un cas de lacération aortique abdominale traumatique suite à une chute dans les escaliers. La lacération intéresse la partie postérieure de l'aorte abdominale sur une fracture lombaire avec déchirure du disque L1-L2.
