Based on the comprehensive development of the emission inventory of air pollution sources, the emission inventory of self-owned mobile sources of Tianjin coastal ports was researched and formulated. In this study, a gridded emission inventory with a resolution of 3 km×3 km was established for six types of air pollutants from road and non-road mobile sources. The spatial and temporal distribution characteristics of pollutant emissions were analyzed, and the uncertainty of the inventory was analyzed using the Monte Carlo method. The results showed that in 2020, the self-owned mobile sources of coastal ports emitted 148.22 t PM10, 135.34 t PM2.5, 1061.04 t SO2, 4027.16 t NOx, 756.60 t CO, and 237.07 t VOCs, of which the total emissions of road and non-road mobile sources accounted for 6.66% and 93.34% of the mobile source emissions, respectively. The main contributors to motor vehicle pollutant emissions from road mobile sources in the whole port area were small, medium, and large passenger vehicles (gasoline) and heavy trucks (diesel). The main contributors to the pollutants emitted by non-road mobile sources were ships and construction machinery. Uncertainty analysis results showed that the overall uncertainty of mobile sources ranged from -13.3% to 16.53%.

Abstract Voriconazole increases tacrolimus blood concentration significantly when coadministrated. The recommendation of reducing tacrolimus to 1/3 in voriconazole package insert seems not to be satisfactory in clinical practice. In vitro studies demonstrated that the magnitude of inhibition depends on the concentration of voriconazole, while voriconazole exposure is determined by the genotype status of CYP2C19. CYP2C19 gene polymorphism challenges the management of drug-drug interactions(DDIs) between voriconazole and tacrolimus. This work aimed to predict the impact of CYP2C19 polymorphism on the DDIs by using physiologically based pharmacokinetics (PBPK) models. The precision of the developed voriconazole and tacrolimus models was reasonable by evaluating the pharmacokinetic parameters fold error, such as AUC0-24, Cmax and tmax. Voriconazole increased tacrolimus concentration immediately in all population. The simulated duration of DDIs disappearance after voriconazole withdrawal were 146h, 90h and 66h in poor metabolizers (PMs), intermediate metabolizers (IMs) and extensive metabolizers(EMs), respectively. The developed and optimized PBPK models in this study can be applied to assit the dose adjustment for tacrolimus with and without voriconazole.

To investigate the effects of drought stress on the growth of sweet potato, photosynthesis, yield, and water consumption characteristics were investigated in field-grown sweet potato plants (Jishu 21, a drought-tolerant cultivar) at different growth stages in 2014-2015. There were five drought treatments, including the whole growth period (DS), the early stage of growth (rooting and branching period, DS1), the middle stage of growth (storage root initiation period, DS2), and the late stage of growth (storage root bulking period, DS3), and a normal irrigation treatment (WW) as control. The results showed that sweet potato plants in DS significantly decreased in dry mass, biomass, and harvest index during two years. The biomass of sweet potato under DS, DS1, DS2 and DS3 was decreased by 31.3%, 21.2%, 19.6% and 7.7%; the harvest index was decreased by 19.9%, 14.5%, 14.1% and 6.5%, and the dry mass was decreased by 45.3%, 33.1%, 31.3% and 14.2%, respectively. The leaf area index in DS, DS1, DS2 and DS3 was decreased by 77.1%, 60.1%, 39.2% and 17.1% at 100 days after transplanting (DAT), and the net photosynthesis was decreased by 56.7%, 26.6%, 18.7% and 9.5% at 90 DAT, respectively. Plants under drought stress reduced diurnal evaporation, transpiration rate, water consumption, daily water consumption, and soil water use efficiency, but increased irrigation water use efficiency. Drought stress decreased leaf area index, Pn, biomass and the allocation of biomass to storage root, resulting in a significant reduction of yield. The early drought and the long stress duration had significant impacts on leaf area index, Pn, biomass and harvest index, leading to a greater reduction in yield and water use efficiency. Therefore, drought stress at the early growth stages of sweet potato should be avoided during cultivation.

Personal exposure to PM2.5 associated with heavy metals were investigated at and around the same road by cycling, walking, taxi and bus in Tianjin, China. One trip on each mode was undertaken during 4 h of both morning and evening peak hours. Results of one-way analysis of variance (ANOVA) to compare mean concentrations of PM2.5 and each metal measured by four modes, the enrichment level of heavy metals in four modes and the carcinogenic, non-carcinogenic risk and probabilistic estimation of health risks of metals (Cr, Ni, Cu, Zn and Pb). Arithmetic means of PM2.5 personal exposure were 323.66, 313.37, 214.84 and 160.71 µg/m3 for cycling, walking, bus and taxi, which resulted from the difference of source (vehicle exhaust and road dust) of exposure to PM2.5. Na has the highest concentration, followed by Al, Ca, K, Fe, Mg, Zn, Ni, Pb, Cu and Cr. The higher Na concentrations were observed in Tianjin in light of its major sea salt influence. The concentrations of Ca, Mg, Fe and Zn in four modes followed different orders, while other metals have no significant difference between four modes. Enrichment factors of metals in PM2.5 showed that some metals are enriched, ranging from contaminated to extremely contaminated, for example, Ni, Cu, Zn, Pb, Na and Cr. Others are barely enriched such as Ca, K, Mg and Fe. It illustrated the former is mainly effected by anthropogenic activates and the source of latter comes from crust. From the results of non-carcinogenic and carcinogenic risks of metals, the intake of metals with inhalation for 4 h by four modes did not pose a significant potential chronic-toxic risk and was an acceptable or tolerable risk at present. But uncertainty analysis of health risks showed there were 4.05 and 6.87% probability that make carcinogenic risk values to exceed 10-4 when male choose walking/cycling to work. Commuters' rush hour exposures were significantly influenced by mode of transport. We suggest that future work should focus on further research between heavy metals in PM2.5 exposure and its specific epidemiology effects.

A campaign was conducted to assess and compare the personal exposure in L3 of Tianjin subway, focusing on PM2.5 levels, chemical compositions, morphology analysis, as well as the health risk of heavy metal in PM2.5. The results indicated that the average concentration of the PM2.5 was 151.43 µg/m3 inside the train of the subway during rush hours. PM2.5 concentrations inside car under the ground are higher than those on the ground, and PM2.5 concentrations on the platform are higher than those inside car. Regarding metal concentrations, the highest element in PM2.5 samples was Fe; the level of which is 17.55 µg/m3. OC is a major component of PM2.5 in Tianjin subway. Secondary organic carbon is the formation of gaseous organic pollutants in subway. SEM-EDX and TEM-EDX exhibit the presence of individual particle with a large metal content in the subway samples. For small Fe metal particles, iron oxide can be formed easily. With regard to their sources, Fe-containing particles are generated mainly from mechanical wear and friction processes at the rail-wheel-brake interfaces. The non-carcinogenic risk to metals Cr, Ni, Cu, Zn and Pb, and carcinogenic hazard of Cr and Ni were all below the acceptable level in L3 of Tianjin subway.

The research aimed to investigate the therapeutic effects and mechanisms of Opuntia dillenii Haw polysaccharide (OPS) on atherosclerosis of rats. First atherosclerotic rat models were established by high-fat and high-calcium diet. Thirty days later, the rats were treated with low dosage of OPS (0.2 g x kg(-1) x d(-1)) or high dosage of OPS (0.4 g x kg(-1) x d(-1)) by intraperitoneal injection for 60 days continuously. At the end of treatment, thoracic aorta rings were prepared and vasorelaxation of rat thoracic aorta in different experiment groups were determined by using 620M multi wire myograph system in vitro. Blood and livers of rats were collected. Then plasma levels of total cholesterol (TC), triglyceride (TG) and low density lipoprotein (LDL) of rats were separately determined using whole automatic biochemical analyzer; protein level of hepatic apolipoprotein B (ApoB) and that of hepatic diglyceride acyltransferase (Dgat1) were measured by Western Blot technique. Results showed that the ability of rat thoracic aorta to relax decreased markedly in the model group compared with that in the normal group, and significant differences existed in vasorelaxation ratios induced by different concentrations of carbamylcholine chloride (Carb) between these two groups (P < 0.01). After OPS treatment, the ability of rat thoracic aorta to relax improved markedly, the vasorelaxation ratios induced by Carb at 5 and 10 µmol x L(-1) were respectively 0.34 ± 0.08 and 0.62 ± 0.15 in the group treated with low dosage of OPS, while the ratios induced by Carb at 1 and 5 µmol x L(-1) were respectively 0.54 ± 0.08 and 0.98 ± 0.02 in the group treated with high dosage of OPS, which were all significantly different with those in the model group (P < 0.01). Plasma contents of TC, TG and LDL reduced significantly by the treatments both with low and high dosages of OPS compared with those in the model group (P < 0.01). Protein level of hepatic ApoB and that of hepatic Dgat1 decreased significantly after the treatment with high dosage of OPS compared with those in the model group (P < 0.01). These results indicate that OPS can markedly improve the vasorelaxation of thoracic aorta of atherosclerotic rats and has significant anti-atherosclerotic effect; inhibiting the expression of ApoB and Dgat1 and thus decreasing the amounts of TC, LDL and TG serving as one of the molecular mechanisms of its antiatherosclerosis effect.

Previous studies have demonstrated that pluripotency-associated transcription factors, such as Oct3/4, Nanog and Sox2, play a crucial role in the development and malignant progression of various types of tumors. Breast cancer is the most frequent cancer among females, being a heterogeneous disease, with distinct morphologies, metastatic behavior and therapeutic responses. The expression of Oct3/4, Nanog and Sox2 in 3 human breast cancer cell lines, MCF7, T-47D and MDA-MB-231, was determined. The expression of Oct3/4, Nanog and Sox2 mRNA was determined by reverse transcription polymerase chain reaction (RT-PCR) and protein expression was detected by immunocytohistochemistry. RT-PCR revealed that all three human breast cancer cell lines tested expressed evident Oct3/4, Nanog and Sox-2 mRNA at various levels. Higher levels of Oct3/4 were identified in MCF7 and MDA-MB-231 cells compared with T-47D cells. Higher levels of Nanog were observed in MCF7 and T-47D cells compared with MDA-MB-231 cells and the highest expression of Sox-2 was detected in MCF7 cells. The nuclear localization of Oct3/4, Nanog and Sox-2 was confirmed by immunostaining. Oct3/4, Nanog and Sox2 were expressed in human breast cancer cell lines. Further studies are required to characterize the role of Oct3/4, Nanog and Sox2 in human breast cancer.

BACKGROUND: Appropriate antimicrobial therapy of community-acquired pneumonia (CAP) is mainly based on the distribution of etiology and antimicrobial resistance of major pathogens. We performed a prospective observational study of adult with CAP in 36 hospitals in China. METHODS: Etiological pathogens were isolated in each of the centers, and all of the isolated pathogens were sent to Zhongshan Hospital for antimicrobial susceptibility tests using agar dilution. RESULTS: A total of 593 patients were enrolled in this study, and 242 strains of bacteria were isolated from 225 patients. Streptococcus pneumoniae (79/242, 32.6%) was the most frequently isolated pathogen, followed by Haemophilus influenzae (55/242, 22.7%) and Klebsiella pneumoniae (25/242, 10.3%). Totally 527 patients underwent serological tests for atypical pathogens; Mycoplasma pneumoniae and Chlamydia pneumoniae infections were identified in 205 (38.9%) and 60 (11.4%) patients respectively. Legionella pneumophila infections were identified in 4.0% (13/324) of patients. The non-susceptibility rate of isolated Streptococcus pneumoniae to erythromycin and penicillin was 63.2% and 19.1% respectively. Six patients died from the disease, the 30-day mortality rate was 1.1% (6/533). CONCLUSIONS: The top three bacteria responsible for CAP in Chinese adults were Streptococcus pneumonia, Haemophilus influenza and Klebsiella pneumonia. There was also a high prevalence of atypical pathogens and mixed pathogens. The resistance rates of the major isolated pathogens were relatively low except for the high prevalence of macrolide resistance in Streptococcus pneumoniae.

OBJECTIVE: To investigate the clinical effect of Compound Xuanju Capsule combined with apomorphine hydrochloride on penile erectile dysfunction (ED). METHODS: We treated 115 ED patients with Compound Xuanju Capsule plus apomorphine hydrochlorid (trial group), and another 111 with apomorphine hydrochloride alone (control group), both for two months. Then we compared the IIEF-5 scores between the two groups. RESULTS: After treatment, the IIEF-5 scores were 17.85 +/- 2.68 and 13.96 +/- 3.25 in the trial and control group, respectively, significantly higher than 11.42 +/- 2.68 and 13.96 +/- 3.25 before treatment (P < 0.01). There were statistically significant differences between the two groups either in post-treatment IIEF-5 scores (P < 0.01) or in the rates of obvious effectiveness, effectiveness and total effectiveness. CONCLUSION: Compound Xuanju Capsule combined with apomorphine hydrochloride has a good curative effect on ED, and deserves general clinical application.

OBJECTIVE: To further elucidate the CT characteristics and diagnostic approaches to non-acquired immune deficiency syndrome patients with pulmonary cryptococcosis. METHODS: The histories of forty-two pulmonary cryptococcosis (PC) patients diagnosed in Zhongshan Hospital from 2003-2008 were collected and analyzed for demography data, underlying conditions, clinical symptoms, chest CT and diagnostic studies. RESULTS: None of the 42 PC patients had avian or its feces contacting history, and 71.4% (30/42) of them were immunocompetent. The most frequent CT lesions were multiple nodules (67.9%) with peripheral predominance (67.9%), and cavitations (50%) often presented within them. Masses/consolidation (31.4%) and patching lesions (2.9%) could exist occasionally. Positive detection rates of non-aggressive examinations including sputum, bronchoalveolar lavage fluid and bronchofibroscopy aspiration were 4.3%, 8.3% and 6.3% respectively, while those of aggressive approaches including transbronchial lung biopsy (TBLB), thin needle aspiration biopsy (TNAB) and pneumonectomy by surgery were 64.7%, 64.3% and 100% respectively. Non-aggressive serum cryptococcus antigen test was performed in 14 patients who had been diagnosed by histopathology or pathogen culture, and all of them were positive. CONCLUSION: Our study suggests that PC is common in immunocompetent population. Avian or its feces contacting is not so important as used opinion to PC differential diagnosis. CT characteristics of PC are diversiform and always change very slowly. Besides the most frequent multiple nodules with subpleural predominance, pulmonary lesions can present as masses, consolidation or patching. Aggressive techniques such as TBLB and TNAB are benefit to clinical diagnosis of PC, and non-aggressive serum cryptococcus antigen test may be promising for its early diagnosis as well as clinical course follow-up and therapeutic effect evaluation.

OBJECTIVE: To study the risk factors of ventilator-associated pneumonia (VAP) in intensive care unit (ICU), in order to offer basic epidemiological data for the prevention of VAP in ICU. METHODS: A prospective cohort study on VAP was carried out in intubated or tracheotomied patients in ICU of Fudan University Zhongshan Hospital from Dec.1999 to Feb. 2001. Single factor analysis and muti-variable logistic regression analysis were adopted to identify the possible risk factors of VAP. RESULTS: (1) Ninety-eight patients were enrolled in this study, of which 52 were diagnosed as having VAP. The incidence of VAP was 53.1%. The incidence of VAP was 32.4 cases per 1000 intubation days. (2) Single factor analysis showed that history of chronic obstructive pulmonary emphysema, use of H2-receptor blocker, the days of antibiotic use, the types of antibiotics, enteral feeding, APACHEII scores, the duration of mechanic ventilation, pH of gastric juice, hypoalbuminemia, tracheotomy, reintubation, colonization of gram negative bacilli in oropharynx, and conscious disturbance were related to the occurrence of VAP. (3) Multi-variable logistic analysis showed statistical significance in combination of over two types of antibiotics (OR = 7.59, 95% CI 4.31 - 38.29), reintubation (OR = 4.73, 95% CI 2.33 - 11.67), APACHE II scores over 15 (OR = 2.02, 95% CI 1.59 - 26.74), pH of gastric juice over 4 (OR = 1.23, 95% CI 1.02 - 1.54) and prolongation of mechanic ventilation (OR = 1.15, 95% CI 1.01 - 3.75). CONCLUSIONS: Various factors contributed to VAP in ICU. Further clinical trials are needed for evidence of the above-mentioned possible risk factors.

OBJECTIVE: To investigate the risk factors for gastric bacterial colonization and its role in the endogenous pathogenesis of ventilator-associated pneumonia (VAP). METHODS: The type and concentration of gastric colonized bacteria and its relationship with the time when samples were collected, and with the type and occurrence order of the pathogens detected in samples from lower respiratory tract after the onset of VAP were analyzed dynamically in the patients with tracheal-intubation or tracheotomy in intensive care unit (ICU). RESULTS: (1) The isolation rate of colonized bacteria in gastric cavity was associated with the pH of gastric juice. When the pH of gastric juice was > 4, the isolation rate of Gram-negative bacilli (GNB) in gastric cavity markedly increased, achieving 52.5% in VAP group, and the incidence of VAP was higher (P = 0.017). The pH value of gastric juice was positively correlated with the logarithmic concentration of GNB in gastric cavity (P = 0.001). (2) As the duration of intubation prolonged, the isolation rate of enterobacteriaceae in VAP group increased, which was 45.2% on the fifth day of intubation. In contrast, the isolation rate in non-VAP group was 11.1% (P < 0.01). (3) The colonization of enterobacteriaceae in gastric cavity was 1 - 2 days earlier than that in oropharynx. The order was statistically significant (P = 0.015). (4) The reverse order of stomach-pharynx-lower respiratory tract colonization was found in 12 cases of the total 52 VAP patients and the order of stomach to lower respiratory tract colonization was found in 3 cases. CONCLUSIONS: (1) The pH value of gastric juice proved to be the major factor which influenced the colonization of bacteria especially GNB in gastric cavity. (2) The gastric cavity was probably a colonization place of GNB especially enterobacteriaceae. (3) The enterobateriaceae in gastric cavity tended to colonize reversely to oropharynx. (4) The phenotypic analysis of the pathogens showed that the reverse stomach-pharynx-lower respiratory tract infection route existed in VAP patients.
