BACKGROUND: In sub-Saharan Africa, the origins of asthma and high prevalence of abnormal lung function remain unclear. In high-income countries (HICs), associations between birth measurements and childhood asthma and lung function highlight the importance of antenatal and early life factors in the aetiology of asthma and abnormal lung function in children. We present here the first study in sub-Saharan Africa to relate birth characteristics to both childhood respiratory symptoms and lung function. METHODS: Children attending schools in two socioeconomically contrasting but geographically close areas of Nairobi, Kenya, were recruited to a cross-sectional study of childhood asthma and lung function. Questionnaires quantified respiratory symptoms and preterm birth; lung function was measured by spirometry; and parents were invited to bring the child's immunisation booklet containing records of birth weight and serial weights in the first year. RESULTS: 2373 children participated, 52% girls, median age (IQR), 10 years (8-13). Spirometry data were available for 1622. Child immunisation booklets were available for 500 and birth weight and infant weight gain data were available for 323 and 494 children, respectively. In multivariable analyses, preterm birth was associated with the childhood symptoms 'wheeze in the last 12 months'; OR 1.64, (95% CI 1.03 to 2.62), p=0.038; and 'trouble breathing' 3.18 (95% CI 2.27 to 4.45), p<0.001. Birth weight (kg) was associated with forced expiratory volume in 1 s z-score, regression coefficient (ß) 0.30 (0.08, 0.52), p=0.008, FVC z-score 0.29 (95% CI 0.08 to 0.51); p=0.008 and restricted spirometry, OR 0.11 (95% CI 0.02 to 0.78), p=0.027. CONCLUSION: These associations are in keeping with those in HICs and highlight antenatal factors in the aetiology of asthma and lung function abnormalities in sub-Saharan Africa.

BACKGROUND: Although 1 billion people live in informal (slum) settlements, the consequences for respiratory health of living in these settlements remain largely unknown. This study investigated whether children living in an informal settlement in Nairobi, Kenya are at increased risk of asthma symptoms. METHODS: Children attending schools in Mukuru (an informal settlement in Nairobi) and a more affluent area (Buruburu) were compared. Questionnaires quantified respiratory symptoms and environmental exposures; spirometry was performed; personal exposure to particulate matter (PM2.5) was estimated. RESULTS: 2373 children participated, 1277 in Mukuru (median age, IQR 11, 9-13 years, 53% girls), and 1096 in Buruburu (10, 8-12 years, 52% girls). Mukuru schoolchildren were from less affluent homes, had greater exposure to pollution sources and PM2.5. When compared with Buruburu schoolchildren, Mukuru schoolchildren had a greater prevalence of symptoms, 'current wheeze' (9.5% vs 6.4%, p=0.007) and 'trouble breathing' (16.3% vs 12.6%, p=0.01), and these symptoms were more severe and problematic. Diagnosed asthma was more common in Buruburu (2.8% vs 1.2%, p=0.004). Spirometry did not differ between Mukuru and Buruburu. Regardless of community, significant adverse associations were observed with self-reported exposure to 'vapours, dusts, gases, fumes', mosquito coil burning, adult smoker(s) in the home, refuse burning near homes and residential proximity to roads. CONCLUSION: Children living in informal settlements are more likely to develop wheezing symptoms consistent with asthma that are more severe but less likely to be diagnosed as asthma. Self-reported but not objectively measured air pollution exposure was associated with increased risk of asthma symptoms.

Transdisciplinary research (TDR) approaches have been cited as essential for overcoming the intractable sustainability challenges that the world is currently facing, including air pollution, water management and climate change. However, such approaches can be difficult to undertake in practice and can consequently fail to add value. Therefore, examples of what works in practice (and what does not) are helpful to guide future research. In this study, we used a conceptual TDR framework as the basis to examine and evaluate the strengths and weaknesses of our approach in a project exploring air pollution in an informal settlement in Nairobi, Kenya. Reflection diaries exploring experiences of participation in the project were undertaken by the project team (comprising academic and community partners) at multiple time points throughout the project. These reflection diaries played an important role in evaluation and for providing space for team learning. Diaries were thematically coded according to the TDR framework to explore aspects of the project that worked well, and areas which presented challenges. We draw upon our reflections, and the extant literature, to make practical recommendations for researchers undertaking TDR projects in future. Recommendations focus on three key project stages (pre-funding, funded period, post-funding) and include; building the team in a way that includes all key stakeholders in relevant and appropriate roles, giving everyone sufficient time to work on the project, and ensuring regular and open communication. Building these recommendations into the design and delivery of transdisciplinary sustainability science projects will support progress towards achieving the Sustainable Development Goals (SDGs). BACKGROUND: The vast array of citizen science projects which have blossomed over the last decade span a spectrum of objectives from research to outreach. While some focus primarily on the collection of rigorous scientific data and others are positioned towards the public engagement end of the gradient, the majority of initiatives attempt to balance the two. Although meeting multiple aims can be seen as a 'win-win' situation, it can also yield significant challenges as allocating resources to one element means that they may be diverted away from the other. Here we analyse one such programme which set out to find an effective equilibrium between these arguably polarised goals. Through the lens of the Open Air Laboratories (OPAL) programme we explore the inherent trade-offs encountered under four indicators derived from an independent citizen science evaluation framework. Assimilating experience from the OPAL network we investigate practical approaches taken to tackle arising tensions. RESULTS: Working backwards from project delivery to design, we found the following elements to be important: ensuring outputs are fit for purpose, developing strong internal and external collaborations, building a sufficiently diverse partnership and considering target audiences. We combine these 'operational indicators' with four pre-existing 'outcome indicators' to create a model which can be used to shape the planning and delivery of a citizen science project. CONCLUSIONS: Our findings suggest that whether the proverb in the title rings true will largely depend on the identification of challenges along the way and the ability to address these conflicts throughout the citizen science project.

The objective of the present narrative review was to provide a conceptual framework to address common misconceptions in the field of traumatic brain injury (TBI) and enhance clinical and research practices. This framework is based on review of the literature on TBI knowledge and beliefs. The comprehensive search of the literature included seminal and current texts as well as relevant articles on TBI knowledge and education, misconceptions, and misattributions. Reviewed materials ranged from 1970 to 2013 and were obtained from PubMed and PubMed Central online research databases. Research findings from the reviewed literature were integrated with existing social and cognitive psychological concepts to develop a framework that includes: (1) the identification antecedents of TBI-related misconceptions and misattribution; (2) understanding of how inaccurate beliefs form and persist as the result of pre- and postinjury cognitive operations such as informational cascades and attribution biases; and (3) a discussion of ways in which these beliefs can result in consequences in all domains of a survivor's life, including physical and mental health, stigma, and discrimination. This framework is intended to serve as a first stage of development of a model that will improve treatment endeavors and service delivery to individuals with TBI and their families.

There is increasing recognition of the importance for local biodiversity of post-mining sites, many of which lie near communities that have suffered significant social and economic deprivation as the result of mine closures. However, no studies to date have actively used the knowledge of local communities to relate the history and treatment of post-mining sites to their current ecological status. We report a study of two post-mining sites in the Yorkshire coalfield of the UK in which the local community were involved in developing site histories and assessing plant and invertebrate species composition. Site histories developed using participatory GIS revealed that the sites had a mixture of areas of spontaneous succession and technical reclamation, and identified that both planned management interventions and informal activities influenced habitat heterogeneity and ecological diversity. Two groups of informal activity were identified as being of particular importance. Firstly, there has been active protection by the community of flower-rich habitats of conservation value (e.g. calcareous grassland) and distinctive plant species (e.g. orchids) which has also provided important foraging resources for butterfly and bumblebee species. Secondly, disturbance by activities such as use of motorbikes, informal camping, and cutting of trees and shrubs for fuel, as well as planned management interventions such as spreading of brick rubble, has provided habitat for plant species of open waste ground and locally uncommon invertebrate species which require patches of bare ground. This study demonstrates the importance of informal, and often unrecorded, activities by the local community in providing diverse habitats and increased biodiversity within a post-mining site, and shows that active engagement with the local community and use of local knowledge can enhance ecological interpretation of such sites and provide a stronger basis for successful future management.

Citizen science is having increasing influence on environmental monitoring as its advantages are becoming recognised. However methodologies are often simplified to make them accessible to citizen scientists. We tested whether a recent citizen science survey (the OPAL Air Survey) could detect trends in lichen community composition over transects away from roads. We hypothesised that the abundance of nitrophilic lichens would decrease with distance from the road, while that of nitrophobic lichens would increase. The hypothesised changes were detected along strong pollution gradients, but not where the road source was relatively weak, or background pollution relatively high. We conclude that the simplified OPAL methodology can detect large contrasts in nitrogenous pollution, but it may not be able to detect more subtle changes in pollution exposure. Similar studies are needed in conjunction with the ever-growing body of citizen science work to ensure that the limitations of these methods are fully understood.

UNLABELLED: Abstract Primary objective: To provide an overview of useful clinical information for healthcare providers involved in traumatic brain injury (TBI) rehabilitation, including current methods used with survivors of TBI, therapeutic considerations in light of this population's cognitive, emotional and social difficulties and issues regarding the therapeutic working alliance from both survivor and provider perspectives. RESEARCH DESIGN: Non-systematic clinical review. METHOD: The literature was intended to be comprehensive to reflect both past and present contributions to the field. To that end, citations were included from seminal and current texts as well as relevant original and review articles from 1985-2012 in PubMed and PubMedCentral online research databases. MAIN OUTCOMES AND RESULTS: This article highlights the usefulness of psychotherapy for treatment of psychiatric symptoms in the TBI population, reviews available modalities and offers considerations and suggestions to facilitate and improve treatment. CONCLUSIONS: Although challenging and perhaps frustrating at times, psychotherapy with this population can be validly attempted and ultimately very rewarding for both the survivor and therapist. Future research should seek to perform controlled studies to examine therapeutic efficacy and compare gains by injury severity in the hopes of creating best practice guidelines for practitioners.
