OBJECTIVE: Radiotherapy is an important treatment for a wide variety of malignancies, although many cancer patients who receive radiotherapy suffer from serious side effects during and after their treatment. Thymoquinone (TQ), the main active ingredient of Nigella sativa, has been reported to have various pharmacological properties, such as antioxidant, hepatoprotective, neuroprotective, antidiabetic, anti-inflammatory, nephroprotective, anticarcinogenic in many pharmacological and toxicological studies. In this study, we aimed to investigate whether there is a radioprotective effect of TQ on the lung tissue of rats exposed to ionizing radiation. MATERIALS AND METHODS: This study was designed as a prospective, placebo-controlled study. A total of 40 Sprague-Dawley rats were divided into four groups to test the radiation-protective effectiveness of TQ administered by intraperitoneal injection. Biochemical parameters were studied to assess the radiation-protective effects of TQ. RESULTS: Oxidative stress parameters, such as oxidative stress index (OSI), lipid hydroperoxide (LOOH) and total oxidant status (TOS), in lung tissue of the rats treated with TQ, were found to be lower than in received irradiation alone. Anti-oxidative parameters, such as total antioxidant status (TAS) level and paraoxonase (PON) activity, were statistically higher in the TR (IR plus TQ group) group compared with other groups. CONCLUSIONS: Findings show that TQ clearly protects lung tissue from radiation-induced oxidative stress and can be used as a radioprotective agent.

BACKGROUND: To assess the shear bond strength (SBS) of four calcium silicate-based cements (CSCs) to composite resin using a universal adhesive in self-etch (SE) and etch-and-rinse (ER) modes and to evaluate surface microstructure and chemical composition of CSCs after acid-etching. METHODS: In total, 30 specimens of each cement, Biodentine (BD), mineral trioxide aggregate (MTA) Angelus (MTA-A), ProRoot MTA (MTA-P) and MTA Repair HP (MTA-HP), were prepared and assigned into 2 groups (n = 15) according to universal adhesive (Clearfil Universal Bond Quick) applied in SE and ER mode. After composite resin (Clearfil Majesty Esthetic) was applied, the SBS was measured. Scanning electron microscope-energy dispersive spectroscopy (SEM-EDS) analysis was used to evaluate the changes in surface structure and elemental composition. RESULTS: MTA-P demonstrated significantly higher SBS than all CSCs, except for MTA-HP in the SE mode. The ER mode exhibited significantly higher SBS compared to the SE mode for MTA-P and MTA-A. Acid-etching caused morphological differences and affected elemental distribution. CONCLUSIONS: MTA-P may be a more suitable pulp capping material due to its superior SBS compared to BD and MTA-A. The application of a universal adhesive in ER mode may be suggested to increase the SBS of MTA-P and MTA-A to composite resin.

Previous literature has reported changes in the length or thickness of the patellar tendon (PT) following primary total knee arthroplasty (TKA). This study aims to determine the structural changes in both the length and thickness of the PT after primary TKA using ultrasound (US) and to investigate the association between these changes and clinical outcomes after a minimum follow-up of 48 months. This prospective study used the US on 60 knees of 32 patients (aged 54-80, mean 64.8±7 years) before and after primary TKA to evaluate changes in both the length and thickness of the patellar tendon. Clinical outcomes were assessed with HSS and Kujala scores. At the latest follow-up evaluation, there was a significant overall shortening of PT by 9.1% (p<0.001), in addition to significant global thickening by 20% (p<0.001). Besides, there was significant thickening by 30% in the proximal 1/3 (p<0.01) and 27% in the middle 1/3 (p<0.01) segments of PT. There was a significant negative correlation between the thickening identified in all three parts of the tendon and both clinical outcome measures (p<0.05). The results show the presence significant changes in PT in terms of length and thickness after primary TKA; in addition, increased thickness in PT was more strongly and significantly associated with inferior clinical outcomes, including functionality and anterior knee pain, than shortness in PT. This study also suggests that the US is a viable, non-invasive method for documenting PT changes in both length and thickness after TKA with serial scans.

OBJECTIVE: Unlike adults, there is no valid and reliable scoring system for upper gastrointestinal system bleeding (UGB) in children. The Sheffield scoring system, which is awaiting confirmation, is the single scoring system which can be predictive for children who require high-risk, endoscopic therapeutic intervention. The aim of this study was to evaluate the efficacy of the Sheffield scoring system, the clinical characteristics of patients, and the treatments applied. PATIENTS AND METHODS: Evaluation was made of a total of 86 children with UGB who underwent esophagogastroduodenoscopy and for whom the Sheffield score was calculated. The decision for therapeutic intervention was made according to the clinical status independently of the score. The demographic data of the patients, clinical symptoms and findings, risk factors, and treatments were examined retrospectively. RESULTS: The Sheffield score was calculated as ≤8 in 67.4% of the patients and >8 in 32.6%. Endoscopic hemostatic intervention was applied to 15.1% of the patients. The rate of therapeutic endoscopy was significantly high in the high-score group. In 11 patients with Sheffield score >8, the bleeding was brought under control with octreotide treatment administered before endoscopy and no invasive intervention was applied. The sensitivity and specificity of the Sheffield score were determined to be at a good level in the prediction of the requirement for therapeutic endoscopy and octreotide treatment. CONCLUSIONS: The Sheffield score can reliably predict the need for endoscopic treatment with high sensitivity and specificity. In children with a high score, the need for an invasive intervention can be reduced with the administration of vasoactive treatment before esophagogastroduodenoscopy. The Sheffield score can thus be of guidance in the determination of the need for vasoactive treatment.

OBJECTIVE: There are no reports of tracheomediastinal fistula development after tracheostomy. CASE REPORT: A 72-year-old female patient with post acute COVID-19 was transferred to our intensive care unit. After two unsuccessful weaning attempts, a tracheostomy was performed at hospitalization on day 32. The patient's body mass index was 35 kg/m2 and she had a narrow neck anatomy. A percutaneous tracheostomy was performed using the Griggs method without any problems. Pneumothorax, pneumomediastinum, subcutaneous emphysema, and hemorrhage were not observed. Twenty-two days after the tracheostomy, the patient developed subcutaneous emphysema and experienced a sudden decrease in oxygen saturation. Bedside anterior-posterior chest X-ray did not detect pneumothorax and a tracheoesophageal fistula was found via esophageal endoscopy. A tracheomediastinal fistula was observed just below the cannula distal end via computed tomography. CONCLUSIONS: There are multiple reasons why a tracheomediastinal fistula could develop after tracheostomy. Therefore, this condition should be considered in cases of sudden subcutaneous emphysema and oxygen deterioration following tracheostomy.

BACKGROUND: Pseudotumor cerebri (PTC) occurs when the pressure inside the skull increases for no obvious reason. The aim of this study was to investigate three different methods: the optic nerve sheath diameter (ONSD) method, ONSD/eyeball transverse diameter (ETD) index, and ONSD/orbital transverse diameter (OTD) index for the initial detection of elevated intracranial pressure in patients with PTC. MATERIALS AND METHODS: A retrospective study of magnetic resonance data from adult PTC patients (n = 42) and control group (n = 40) was performed. ONSD and OTD measurements were made 3 mm and 10 mm posterior to the globe, after intracranial pressure was measured with lumbar puncture. The sensitivity, specificity, and overall accuracy of the findings on magnetic resonance imaging were calculated. RESULTS: The optic nerve sheath was enlarged in the PTC group compared with the control group. It showed 97% sensitivity and 100% specificity and 79% sensitivity and 87.5% specificity for 3 mm and 10 mm, respectively. The ONSD/ETD and ONSD/OTD indices were increased in the PTC group compared with the control group. For 3 mm posterior to the globe, the ONSD/ETD index had 90.5% sensitivity and 92% specificity, and the ONSD/OTD index had 86% sensitivity and 95% specificity. For 10 mm posterior to the globe, the sensitivity and specificity of the ONSD/ETD and ONSD/OTD indices were 86% and 80% and 74% and 82.5%, respectively. CONCLUSIONS: According to our study, the ONSD method and the ONSD/ETD and ONSD/OTD indices are reliable diagnostic markers for PTC. These noninvasive techniques may be useful in monitoring the invasive intracranial catheter and have wide potential clinical applications in district hospitals, emergency departments and intensive care units.

In this study, we have synthesized B doped Ca5(PO4)3OH (HAP) by a sonication chemical method. The thermoluminescence (TL) properties of the family of synthesized samples (B doped Ca5(PO4)3OH (HAP) were investigated using an IRSL-TL 565 nm filter. This gave the highest TL intensity of each phosphor after 2 Gy ß-irradiation. Three TL glow peaks of B doped Ca5(PO4)3OH (HAP) are centered at around 84, 208 and 324 °C (with a heating rate of 2 °Cs-1). The trapping parameters such as activation energy (E), order of kinetics (b), frequency factor (s) were calculated by using initial rise (IR), various heating rates (VHR) and computerized glow curve deconvolution (CGCD) method. The response of TL glow curves remained constant within ±5% deviation from the initial value after 9 cycles of reuse; but only at tenth cycle the deviation goes up to 6%.

Ser/Thr protein phosphatases (PPs) regulate a substantial range of cellular processes with protein phosphatases 1 (PP1) and 2 A (PP2A) accounting for over 90 % of the activity within cells. Nevertheless, tools to study PPs are limited as PPs inhibitors, particularly those selective for PP1 inhibition, are relatively scarce. Two examples of PP1-selective inhibitors, which share structural similarities, are tautomycin (TTM) and tautomycetin (TTN). This work describes the development of PP1/PP2A inhibitors that incorporate key structural features of TTM and TTN and are designed to conserve regions known to bind the active site of PP1/PP2A but vary regions that differentially contact the hydrophobic groove of PP1/PP2A. In all 28 TTN analogues were synthetically generated that inhibit PP1/PP2A activity at <250 mM; seven possessed inhibition activity at 100 nM. The IC50 values were determined for the seven most active analogues, which ranged from 34 to 1500 nM (PP1) and 70 to 6800 nM (PP2A). Four of the seven analogues possessed PP1 selectivity, and one demonstrated eightfold selectivity in the nanomolar range (PP1 IC50 =34 nM, PP2A IC50 =270 nM). A rationale is given for the observed differences in selectivity.

Angiogenesis is the formation of new blood vessels. Angiogenesis affects cancer growth and is a useful target for cancer therapeutics. The effects of geldanamycin on angiogenesis in cases of gastric cancer are poorly understood. We investigated the effects of different doses of 17-allylamino-17-demethoxygeldanamycin (17-AGG), a semi-synthetic derivative of geldanamycin, on the interactions between cellular matrix proteins and angiogenesis factors in a gastric cancer cell line. We examined cancer cells on laminin and collagen I coated surfaces to determine their response to the angiogenic effect of these matrix molecules. We also evaluated the expression levels of VEGF, MMP-9, ES and TSP-1 using ELISA. We found that application of 17-AAG to the gastric cancer cell line on culture dish plastic decreased VEGF, TSP-1, ES and MMP-9 expression, whereas of all of these proteins were increased by laminin and collagen coating. 17-AAG currently is in clinical trial phase 2 and may be a promising drug for treatment of gastric cancer.

BACKGROUND: This paper determined the morphometric measurements of the brainstem including mesencephalon, pons and medulla using magnetic resonance imaging (MRI) in Turkish healthy population. MATERIALS AND METHODS: Two hundred sixty-three (263; 158 females and 105 males) subjects aged from 18 to 65 years were included in this study. The measurements were taken from subjects having brain MRI in the Radiology Department. Statistical analysis was done by SPSS 21.00 package programme. ANOVA test and χ2 test were used to determine the relation between measurements and age and sex groups. The p < 0.05 value was considered as significant. RESULTS: The overall means and standard deviations of the measurements were: pons anteroposterior diameter, 15.41 ± 1.27 mm; pons vertical diameter, 22.02 ± 2.07 mm; mesencephalon anteroposterior diameter 9.39 ± 1.00 mm; mesencephalon vertical diameter, 15.20 ± 1.53 mm; distance between the interpeduncular fissure and aqueduct, 11.72 ± 1.58 mm; distance from cerebral peduncles to aqueduct, 13.64 ± 1.66 mm; anterior surface of the pons midway between the mesencephalon and medulla to the fourth ventricular floor, 21.62 ± 1.64 mm; the shortest anteroposterior diameter of the medulla at the pontomedullary junction, 13.46 ± 1.28 mm, and the shortest anteroposterior diameter of the medulla at the medullospinal junction, 10.24 ± 1.43 mm in females, respectively, whereas the corresponding values were 15.58 ± 1.53 mm; 22.64 ± 2.35 mm; 9.37 ± 1.66 mm; 15.64 ± 1.52 mm; 11.14 ± 1.31 mm; 13.01 ± 1.30 mm; 21.97 ± 1.65 mm;13.47 ± 1.19 mm; 9.91 ± 1.35 mm in males, respectively. There were significant differences in some parameters such as pons vertical diameter, mesencephalon vertical diameter, distance between the interpeduncular fissure and aqueduct, and distance between cerebral peduncles to aqueduct between sexes. CONCLUSIONS: The brainstem dimensions of healthy population provide important and useful knowledge in terms of comparison of abnormalities clinically. These data may be valuable for the representatives of clinical disciplines.

BACKGROUND: This paper was undertaken to determine the morphometry of pituitary gland diameter, pituitary gland height, intercavernous distance, optic chiasm diameter and optic chiasm height in skulls of Turkish population aged between 18 and 60 years. MATERIALS AND METHODS: It was a retrospective study in which 292 subjects were included 187 females and 105 males, ranging from 18 up to 60 years. Subjects underwent brain magnetic resonance imaging in the Radiology Department. Statistical analysis was performed with SPSS 21.00 programme. ANOVA test, χ2 test, and Pearson correlation analysis were used to determine the relation and significance between measurements and age group. The p < 0.05 value was considered as significant. RESULTS: The groups were divided into five groups according to age. The overall means and standard deviations of the measurements were: pituitary gland width, 13.09 ± 1.99 mm; pituitary gland height, 4.91 ± 1.10 mm; intercavernous distance, 15.93 ± 3.05 mm; optic chiasm width, 12.82 ± 1.27 mm; and optic chiasm height, 2.80 ± 0.49 mm in females, respectively whereas, the same measurements were 12.96 ± 1.74 mm; 4.79 ± 0.95 mm; 16.08 ± 3.11 mm; 13.13 ± 1.37 mm; 2.86 ± 0.70 mm in males, respectively. Height of the pituitary gland reached a maximum in the age group of 18 to 20 years in both females and males and there was a decrease in the pituitary gland height in the subsequent age groups. CONCLUSIONS: Knowledge of the variation in the size of pituitary gland, intercavernous distance and optic chiasm is important to evaluate the dimensions of these structures for clinical and pathological processes.

OBJECTIVE: Ischemia-reperfusion (IR) injury is associated with various clinical conditions, such as myocardial infarction, shock, and surgery under vascular occlusion. We aimed to investigate the protective and therapeutic effects of apocynin (AP) on liver injury induced by IR in an in vivo rat model. METHODS: Thirty-two rats were randomly divided into 4 experimental groups with n = 8 in each group: sham, IR, AP, and IR + AP. AP (20 mg/kg) was intraperitoneally administered to rats in the AP and IR + AP groups for 30 minutes before 60 minutes of ischemia and followed by 60 minutes of reperfusion. All rats were killed on the same day to evaluate tissue levels of oxidants and antioxidants (catalase, malondialdehyde, myeloperoxidase, superoxidedismutase (SOD), and total glutathione). RESULTS: IR decreased SOD levels in IR group when compared with the sham group. AP supplementation to IR group significantly ameliorated SOD levels (P < .05). Also, IR caused elevation of myeloperoxidase production when compared with the sham group, whereas AP treatment prevented these hazardous effects (P < .05). However, plasma total glutathione, catalase, and malondialdehyde levels did not differ between the AP + IR and the IR rats. CONCLUSION: The main finding of the present study was that AP may be protective against liver IR injury. Our results suggested that AP pretreatment suppressed oxidative stress and increased antioxidant levels in an rat model of liver IR.

RESUMO

Strontium was used for the first time as an activator agent for magnesium tetraborate (MBO) and photoluminescence (PL) and thermoluminescence (TL) properties were determined using a spectrophotometer and a TL reader, respectively. The results proved that 0.25wt% Sr ratio played an important role in prolonging the afterglow and the phosphor gave the main TL peak with the highest intensity at 200°C like an ideal case. Therefore, it is worthwhile to carry out continuous and systematic research on it.

In this paper, the thermoluminescence (TL) dosimetric characteristics of MgO doped natural amethyst samples (Mg-NA) are presented. The morphologies and chemical structures of the powder form samples were identified using XRD, FTIR, SEM, SEM mapping and EDX. Comparison of the TL intensities showed that 10wt% Mg-NA was nearly 150 times more sensitive than undoped amethyst and the main dosimetric properties proved that 10wt% Mg-NA may be a promising phosphor for clinical and radiotherapy purposes.

OBJECTIVE: Pre-eclampsia (PE) is among the most commonly researched topics in perinatology. Fetuin-A (FA), a multifunctional protein, has roles in the inhibition of ectopic calcification and the regulation of serum glucose levels. Reduced FA is an indicator of inflammation. This study was performed to investigate the potential role of inflammation in the pathophysiology of early- and late-onset PE by measuring FA levels. STUDY DESIGN: In total, 110 patients were included in this study. Early- and late-onset PE were defined as a diagnosis at <34 weeks or ≥34 weeks of gestation, respectively. Plasma FA levels were determined by immunoassay, which was performed in duplicate using a sandwich enzyme-based technique. Parametric data were appraised using an independent two-sample t-test, and non-parametric data were compared using the Mann-Whitney U-test. Multivariate logistic regression was performed to investigate the impact of certain parameters on PE. Receiver operating characteristic analysis was used to evaluate the diagnostic performance of FA. RESULTS: There were 24 patients with early-onset PE and 19 gestational-age-matched controls. Plasma FA levels were significantly lower in the early-onset PE group compared with the controls (17.3±3.3ng/ml vs 21.4±3.5ng/ml, p<0.05). There were 36 patients with late-onset PE and 31 gestational age-matched controls. Plasma FA levels were significantly higher in the late-onset PE group compared with the controls [26.1ng/ml (range 13.4-52.0) vs 18.4ng/ml (range 14.9-24.9), p<0.05]. Besides the parameters used in the diagnosis of PE, the single most important variable related to PE was FA. The optimal cut-off level for FA in the prediction of early-onset PE was 19.6ng/ml [sensitivity 79%, specificity 83.3%, area under the curve (AUC) 0.796, 95% confidence interval (CI) 0.650-0.943, p=0.001]. FA did not show a statistically discriminative value in differentiating late-onset PE from control subjects (AUC 0.196, 95% CI 0.085-0.306). CONCLUSION: Early- and late-onset PE were associated with lower and higher levels of FA, respectively. A relationship was found between inflammation and early-onset PE but not late-onset PE.

Flexor tendon injury is the most commonly seen hand injury. Tendons are the structures which enable the muscles to adhere to the bones and transmit the movement starting from the muscle to the bones. They have significant role in hand movement. As treatment method, surgical repair: (1) Primary repair, (2) Secondary repair; techniques are used. With the scientific studies conducted, flexor tendon morphology, kinematics, biomechanics, biological properties and tendon improvement became quite understandable, good suturing materials were developed and consequently successful studies regarding primary repair results were published. Flexor tendon surgery has reached its current level with the accumulation of knowledge obtained from the increased clinical and experimental studies. This study addresses flexor tendon injuries and surgical treatment methods. We achieved 76.12% full functioning fingers in the results; we obtained following the physical treatment application that started on the 10th day after primary repair that we performed in 67 patients who applied with acute flexor tendon Zone 1-2 and 3 incision. Flexion restriction and contracture developed in 16 patients (23.88%) at various levels. Scar softening and revision surgery for contracture excision was performed in 8 patients (11.94%) with Zone 2 injury due to contracture. Superficial skin infection developed in 21 patients (31.34%) and was treated. Revision surgery was performed in 6 patients with Zone 1 injury due to rupture occurred during rehabilitation.

BACKGROUND: Cognitive impairment, anxiety and depression are important problems for patients with chronic kidney failure. Cognitive impairment, anxiety, and depression may be related to various factors, such as complications of hemo/peritoneal dialysis, uremic encephalopathy, psychosocial burden of the disease, and various comorbidities in patients with chronic kidney failure. Successful kidney transplantation (KT) improves kidney, endocrine, metabolic, and vascular systems, mental functions, and the quality of life of the patients. METHODS: A total of 181 patients with chronic kidney failure were studied: 54 currently on hemodialysis, 58 on peritoneal dialysis, and 69 with KT. All participants were given a detailed sociodemographic form, including data about the reason of kidney failure, duration of treatment (hemodialysis, peritoneal dialysis, and KT), and comorbid illnesses. Participants were evaluated with the use of the Hospital Anxiety and Depression Scale (HADS) for evaluating depressive and anxiety symptoms and the Brief Cognitive State Examination (BCSE) for detecting possible cognitive impairment. RESULTS: Patients with KT had lower levels of anxiety and depression symptoms than patients with hemodialysis and peritoneal dialysis. The KT group scored better than the hemodialysis and peritoneal dialysis groups on the BCSE. The peritoneal dialysis group scored higher on the BCSE than the hemodialysis group. The hemodialysis group scored higher on the HADS than the peritoneal dialysis group. CONCLUSIONS: In this study it was found that KT patients have better cognitive and mood regulation outcomes than hemodialysis and peritoneal dialysis patients with chronic kidney failure. With this knowledge we suggest that patients with kidney failure should have KT for having better cognitive functions and mood state as soon as possible.

BACKGROUND: We compared the caregivers of hemodialysis (HD) patients and caregivers of patients with renal transplantation (Tx) in terms of anxiety, depression, sleep quality, and caregiver burden. We believe that caregivers of HD have more difficult conditions than caregivers of the patients with Tx. METHODS: This cross-sectional study analyzed the psychological status of caregivers of Tx patients compared with those of HD patients with using the Hospital Anxiety and Depression Scale, Zarit Burden Interview, and Pittsburg Sleep Quality Indexes. We recruited 133 caregivers-65 caregivers in the Tx group and 68 in the HD group. RESULTS: Mean age was 43.1 ± 8.5 years. The age, sex, income level, and education level were similar between the 2 groups. Caregivers in the HD group had significantly higher rates of anxiety and depression compared with the Tx group (P = .007 and P < .001, respectively). Good sleep quality rates for caregivers in the Tx group and caregivers in the HD group were 92% (n = 60) and 63% (n = 43), respectively. Poor sleep quality was significantly higher in caregivers in the HD group compared with caregivers in the Tx group (P < .001). Caregiver burden scores were significantly higher for caregivers in the HD group compared with caregivers in the Tx group (P < .001). CONCLUSIONS: We suggest that Tx is the more appropriate renal replacement therapy for caregivers who undertake the care of patients with end-stage renal disease and chronic kidney disease.

OBJECTIVES: Ankylosing spondylitis is a chronic inflammatory disease which physically, psychologically, and socially affects the patient's life. Previous studies have reported a correlation between ankylosing spondylitis and depression. In this study we investigated the effect of infliximab on depression in ankylosing spondylitis patients. METHODS: A total of 29 patients with ankylosing spondylitis were enrolled in this prospective study. Infliximab was administered intravenously at a dose of 5 mg/kg at baseline, weeks 2 and 6. The measurements of morning stiffness, modified Schober's test, chest expansion, erythrocyte sedimentation rate, C-reactive protein, Bath ankylosing spondylitis disease activity index, Bath ankylosing spondylitis functional index and Beck depression inventory scores were compared with baseline and 12th week. RESULTS: The modified Schober's test and chest expansion increased, the morning stiffness duration, erythrocyte sedimentation rate and C-reactive protein levels decreased after infliximab treatment (p < 0.001, respectively). There was statistically significant decrease in Bath ankylosing spondylitis disease activity index, Bath ankylosing spondylitis functional index and Beck depression invantory scores of patients after 12 weeks (p < 0.001, respectively). CONCLUSION: Infliximab can improve depression and its symptoms in patients with ankylosing spondylitis.
