RESUMO

We present successful treatment of dissected thoracoabdominal aorta using combined thoracic endovascular aortic repair and transfemoral replacement of Amplatzer™ vascular plug in a 38-year-old patient. Computed tomography angiography revealed a false lumen from the left subclavian artery to the left common iliac trunk, with re-entries connecting it to the true lumen of the aorta. We replaced the prosthetic endovascular graft just below the left subclavian artery to the top of the superior mesenteric artery. Amplatzer™ vascular plug was replaced below the diaphragm in a closed transfemoral procedure to prevent re-entry. The patient was discharged in good condition and 2 years postoperation imaging showed complete pseudoaneurysm closure.

RESUMO

Statin medications are considered as important lipid lowering agents to prevent subsequent cardiovascular events. However, there was limited evidence regarding the bibliometric analysis on preclinical and clinical studies. In January, 2021, the data was retrieved from Scopus and Dimensions database. For detail analysis, we focused on Scopus. Thirty-three thousand two hundred forty-seven research documents were found in the database which contained the word "Statin" either in titles, abstracts and/or keywords of the research documents. They mostly comprised of research articles (n = 22586/67.93%), reviews (n = 6366/19.15%). Scopus classified these documents in various subject categorized like medicine, biochemistry, genetics and molecular biology, pharmacology, toxicology and pharmaceutics, nursing, neuroscience, biological sciences, immunology, and microbiology to name a few. The highest documents are published in 2016 (n = 540/7.16%), followed by 2015 (n = 534/7.08%), 2017 (n = 527/6.99%), 2014 (n = 514/6.81%), and 2020 (n = 504/6.68%). The top sources as well as total number of authors, institutes, and countries involved in publications are described. For detail analysis, we designed the publication and citation clubs. Based on Vosviewer analysis we also provided details about co-authorship network for authors, institutes, and countries. In order to understanding the research focus of the publications, we performed the co-words analysis. The present study may provide details and research trends about statin publications.

We studied the effects of administering exogenous surfactant for the treatment of respiratory distress in premature neonates (born before 37 weeks of gestational age [GA]) and compared the role of different risk factors on the outcome as well as survival rate. All the neonates (242) suffered from moderate to severe respiratory distress, identified by clinical signs, chest X-ray, respiratory distress syndrome (RDS) score >6, and blood gas measurements. All the neonates included were treated by administering surfactant (Beractant or Poractant alfa, dosage 100 mg/kg). The INSURE method was "successful" in 74% of patients, meaning there was no need for a second dose of surfactant or mechanical ventilation repetition. The factors that determined the "success" (Table II) were as follows: type of delivery, weight, GA, and number of fetuses. The factors affecting survival were: number of fetuses, mechanical ventilation dependency, pregnancy complications, and type of surfactant. The INSURE method reduced mortality (91.3% survived).

BACKGROUND: Internal nasal valve collapse is a preventable complication of rhinoplasty, for which the spreader graft is the gold standard. More recently, the spreader flap technique has been espoused as an alternative to spreader grafting. Here, we evaluated the efficacy of this technique in a randomized trial setting. METHODS: The autospreader was used in 32 patients who were candidates for primary rhinoplasty; 34 who did not undergo a spreader flap procedure were recruited as a control group. Acoustic rhinometry was used in every patient preoperatively and 1 year postoperatively. Average volume and minimal cross-sectional area (MCA) of the nose and findings were collected. Also, the pre- and postoperative patients' subjective assessment about their satisfaction from their appearance and nasal obstruction were evaluated according to visual analog scale (VAS). RESULTS: Among the study group patients, 46 (69.7%) were female and 20 (30.3%) were male subjects. Their mean age was 50/5 ± 6/24 years. The difference in MCA in the control group was -0.1 and +0.6 (on right and left sides, respectively), and in the spreader flap group it was -0.03 and +0.05, which showed an increase in MCA in the left side and a decrease in the opposite side of both groups. However, their difference was not statistically significant (p = 0.50). Also, the differences between preoperative and postoperative VAS were not significant in either nasal obstruction (p = 0.68) and cosmetic satisfaction (p = 0.38). CONCLUSION: Spreader flap is an attractive technique in preserving the middle vault in nasal plastic surgery. However, its effect needs more evaluation.

RATIONALE: Today, there is relatively no debate on the notion that NMDA receptor antagonist agents in the hippocampus induce anxiolytic-like effects through distinct mechanisms. There is also a bulk of studies showing the involvement of the dopaminergic system in NMDA induced behaviors. Thus, on the basis of the involvement of dopaminergic system in anxiety-related behaviors, the present study aimed to investigate the involvement of the dorsal hippocampal (CA1) dopaminergic system in anxiolytic-like responses induced by MK801 (NMDA receptor antagonist) in male Wistar rats. METHODS: We used the elevated plus maze to test anxiety. This apparatus has widely been employed to test parameters of anxiety-related behaviors including the open arm time percentage (%OAT), open arm entries percentage (%OAE), locomotor activity, grooming (the rat rubs its face), rearing (the rat maintains an erect posture) and defecation (the number of boli defection). RESULTS: The data showed that, intra-CA1 injection of MK801 (2 µg/rat) increases %OAT and %OAE but not other exploratory behaviors, indicating an anxiolytic-like effect. Moreover, sole intra-CA1 injection of SCH23390, dopamine D1 receptor antagonist, (0.25, 0.5 and 1 µg/rat) and sulpiride, dopamine D2 receptor antagonist, (0.25,0.5 and 0.75 µg/rat) did not alter anxiety-like behaviors. Co-administration of subthreshold doses of SCH23390 (0.5 µg/rat) and MK801 (0.5 g/rat), induced anxiolytic-like behaviors. Furthermore, intra-CA1 administration of different doses of sulpiride (0.12, 0.5 and 0.75 µg/rat), 5 min before the injection of an effective dose of MK801 (2 µg/rat), decreased %OAT and %OAE, however did not alter other exploratory behaviors induced by MK801. CONCLUSION: Our results suggested a modulatory effect of the CA1 dopaminergic system on the anxiolytic-like effects induced by MK801.
