Dopplerographic and manometric examination was conducted in 429 patients with obliterating lesions of the lower limb arteries in order to find optimal diagnostic combinations. The data of noninvasive diagnosis were verified in 119 cases. The data obtained bore evidence that coincidence of the findings of ultrasonic dopplerography and multilayer manometry is indicative, with a high degree of probability, of the presence or absence of lesions of the lower limbs. The findings of ultrasonic dopplerography are the main criteria in appraising affections of the aortoiliac segment and those of multilayer manometry--in appraising affections of the femoropopliteal segment. The findings of both methods are of equal significance in appraising the tibial segment. Multilayer manometry allows indirect appraisal of the condition of arteries in the channel of the deep femoral artery.

The work has analyzed results of 91 transaortal endarterectomies and 66 prostheses of renal arteries with the autovein. A preliminarily arterialized autovein is proposed as an autotransplant. The arterialization is achieved by inclusion of the autovein in the peripheral arterial system where the vein is gradually transformed and within 10-12 weeks its structure and biomechanical properties become closer to those of the artery of the musculo-elastic type. Preliminary arterialization excludes dilatatory changes of the autovenous transport.

The article discusses the choice of operative methods and the stages in the treatment of patients with atherosclerosis of the abdominal aorta and lower limb arteries with consideration for the involvement of the coronary and cerebral arteries in the process. A total of 749 reconstructive operations were conducted on the abdominal aorta, its unpaired branches, arteries of the lower limbs, and coronary and extracranial arteries in 676 patients. The principle of extraperitoneal operations on the abdominal aorta was regarded as the basis in improving the results of treatment of patients with concurrent lesions. The authors substantiate the choice of a one-stage or many-stage type of operations on the vessels of different regions with due account for the localization of the affection, the degree of vascular insufficiency in each channel, and the life prognosis.

The mechanisms of positive effects of plasmapheresis were studied in 52 patients with angiological pathology who were given 135 sessions of plasmapheresis. Most of the patients had obliterating atherosclerosis (28) and endarteritis (16). The clinical effect obtained in 80.7% of the patients with a severe regional ischemia was associated with the improved hemorheology (lower blood viscosity, lower fibrinogen level, higher deformability of erythrocytes), with hypocoagulation and normalization of the correlation of high and low density lipoproteins in atherosclerotic patients. These effects are of relatively long durations, the repeated courses of plasmapheresis within 6-8 months being, as a rule, successful.

Based upon an analysis of results of 1210 operations and 160 laparoscopies in children for "acute appendicitis" the authors make a conclusion that about 20% of children were subjected to unnecessary appendectomies. Laparoscopy allows to make the correct diagnosis in the absolute majority of children with obscure clinical picture of acute appendicitis and to choose a rational medical treatment.

Different kinds of anesthesia used in 2644 patients were analysed with special reference to the fact that patients with diseases of goiter had pronounced endocrine shifts. It was shown that there were no reasons to oppose different methods of anesthesia to one another. Each of them has its indications and contraindications corresponding to specific features of the disease.

Based on an analysis of immediate and long-term results of treatment of 178 patients with acute iliofemoral venous thrombosis the authors recommend thrombectomy as an early operative treatment. The operative treatment has prevented the development of the postthrombophlebitic syndrome in most patients.

An analysis of clinico-roentgenological data of 74 patients has been made. The patients were operated upon for acute appendicitis (43) and cholecystitis (31) and their postoperative period was complicated by local peritonitis. The importance of indirect and roentgenological symptoms for diagnosis of abscesses in the abdominal cavity is stressed.

Results of treatment of 500 patients with blood vessel trauma are analyzed depending on trauma nature, shock severity, regional ischemia, technique of restorative surgery. Biochemical and ultrastructural changes in muscles are described in experimental conditions in ischemia of different degrees of severity to prognosis the reversibility of disorders. Restoration of blood circulation has been achieved in 82% of patients (5.1% mortality). Beside open and closed vessel trauma, it is expedient to distinguish between penetrating and non-penetrating injuries. Results of treatment of blood vessel trauma should be differentiated depending on the degree of regional ischemia, severity of traumatic shock and blood loss, combination of trauma and injury localization. Mechanical suture improves anastomosis quality, simplifies technique and reduces time of intervention. Introduction of microsurgery principles widens possibilities of vessel restorative surgery. The authors suggest classification of iatrogenic injuries of vessels, which promotes accurate diagnosis and choice of optimal surgical technique.

An analysis of 185 vagotomies for chronic duodenal ulcers has been made. It has been established that the surgery was an adequate method in 92,1% of the patients. Lethal outcomes and complications were given a detailed consideration. A conclusion was made that for the improvement of results of organ-preserving operations of special significance was analysis of the course of the ulcer disease and the anatomical alterations. It facilitates solution of the question of expediency of vagotomy and correct selection of its variant.
