INTRODUCTION: Eosinophilic fasciitis (EF) is a rare inflammatory and fibrosing disorder with clinical, biological and histological characteristics. It has sometimes been reported with hematologic malignancy, but has never been associated with malignant uveal melanoma. CASE REPORT: A 67-year-old woman with choroidal melanoma was found to have metastatic skeletal extension of her melanoma while she developed swelling and pain in the lower limbs. Histologic examination confirmed an EF and oral corticosteroid treatment was started. A treatment for the ophthalmologic and skeletal extension of her melanoma was initiated. The paraneoplastic character of this EF was entertained. CONCLUSION: The simultaneous occurrence of EF and metastatic skeletal melanoma is suggestive of the possibility of EF constituting a paraneoplastic syndrome.

INTRODUCTION: Temporal arteritis is the most common systemic vasculitis of the elderly. It is diagnosed with the combination of a clinico-biological syndrome and typical histologic features recognized on temporal artery biopsy (TAB). Cortisteroid therapy is quickly recommended, before the TAB is performed or before the histologic results confirm the diagnosis. It is recommended to perform TAB as soon as possible after the treatment has begun in order to avoid a presumed improvement or normalisation of the histological features. EXEGESIS: We report the cases of two patients, a 76-year-old woman and a 78-year-old man who had persistent clinical and histological features of temporal arteritis 5 years and one year respectively after corticosteroid therapy was initiated. CONCLUSION: Histological changes in the temporal artery biopsy may persist for as long as five years in a patient receiving a corticosteroid treatment for temporal arteritis. Even when largely delayed after the beginning of the treatment, temporal artery biopsy may prove to be important in diagnosing persistent temporal arteritis.

INTRODUCTION: The toxic myopathy caused by statins (HMG-CoA reductase inhibitors) is well established. Recent reports add to these effects systemic immune diseases including systemic lupus erythematosus, vasculitis, polymyositis or dermatomyositis. EXEGESIS: We report a case of dermatomyositis in a 69-year-old patient treated with pravastatin [Elisor]. She presented with typical features of dermatomyositis 2 years after she started a treatment with pravastatin. The treatment was discontinued and she slowly improved, with a transient dermocorticosteroid treatment. Eight other patients with dermatomyositis and chronic treatment with HMG-CoA reductase inhibitors are reported in the literature. All of them presented with classical features of dermatomyositis. The discontinuation of the treatment was followed by spontaneous clinical and biological improvement in 3/9 patients. The other patients received high doses of corticosteroids and improved, except one patient who died of respiratory failure (pulmonary fibrosis) despite the adjunction of oral cyclophosphamide [Endoxan]. In these patients, dermatomyositis can be considered as a severe adverse reaction to HMG-CoA reductase inhibitors although a distinct casual link cannot be definitely established. CONCLUSION: The increasing prescription of statins has led to the parallel increment of reported side-effects, where autoimmune diseases are now described. Among them, our case of dermatomyositis in a patient receiving pravastatin adds to the eight reported cases in the literature and highlights the potential role of statins as triggers of immune systemic diseases.

RESUMO

The purpose of this study is to report the case of a 34 year old patient, from Zaïre for whom a wrong diagnosis could have been fatal. Recent events in his affective and professional life, associated with typical symptoms, suggested a major depression. However after two months of being admitted a psychiatric ward, the resistance to treatment together with some biological data suggested further investigations. They revealed a subacuse meningo-encephalitis of parasitological etiology. The psychiatric aspects of trypanosomiasis commonly known by clinicians in Africa justifies, in patients from that continent, a systematic test for trypanosomiasis.

Right ventricular protection may be limited with current methods of cardioplegic delivery. Sensitive measurements of right and left ventricular function and metabolism were made in 30 patients undergoing elective coronary artery bypass surgery with cold cardioplegic arrest. Myocardial adenine nucleotide concentrations decreased with cardioplegia and reperfusion in both the right and left ventricles despite adequate levels of precursors, suggesting perioperative mitochondrial dysfunction. Postoperatively, right and left ventricular pressures were measured with micromanometer catheters and volumes were measured by nuclear ventriculography. Right and left ventricular systolic elastance was calculated by the isochronic method and by the end-systolic method. Both methods provided sensitive indexes of end-systolic elastance. This study demonstrated that right ventricular function and metabolism can be evaluated by methods analogous to methods used in the left ventricle. These results suggest that right ventricular functional and metabolic recovery are delayed despite apparently adequate myocardial protection. Sensitive measurements may permit improved assessment of alternative methods of right ventricular protection.

RESUMO

A nonintrusive method for the visualization of pulsatile flow velocity profiles is described. The method is based on the use of a photochromic dye that is added to the fluid being studied and a nitrogen laser which excites the dye producing a marker "line" whose movement can be photographed. A microcomputer is used as a system controller, to coordinate the system timing and to manage the data transfer. The method used for analysis of the photographs to determine the velocity profiles is described. Examples are presented of instantaneous velocity profiles obtained from velocity waveforms that are similar to those of the femoral artery. In addition, application of the system for studying the relationship between Doppler ultrasound spectral recordings and flow velocity profiles is discussed.

Quantitative analysis of continuous wave Doppler recordings is of clinical value in the noninvasive diagnosis of carotid arterial disease. Peak frequency measurements are useful and accurately detect severe stenoses but do not reliably diagnose minor or moderate stenoses because the measurement is dependent upon the probe to vessel angle, which cannot be measured accurately. Recent investigations have focused on efforts to overcome this limitation by quantifying the degree of spectral broadening that occurs as the result of flow disturbances downstream from a stenosis. In this study, an in vitro model was used to determine the optimum method for quantifying the instantaneous Doppler spectrum. The model generates blood flow velocity waveforms that are virtually identical to those found in the human internal carotid artery. Doppler recordings were made from normal tubes and distal to stenoses (39-87% cross-sectional area reduction). The spectra were quantified by the following angle-independent measurements: spectral broadening index and three standard statistical shape descriptors, namely the coefficients of variation, skewedness and kurtosis. Using this model, the results demonstrate an excellent relationship between the severity of the stenosis and each of spectral broadening index (r = 0.99), coefficient of variation (r = 0.96), and coefficient of skewedness (r = 0.99). The calculation of each of the measurements can be implemented quite easily, and a prospective trial is warranted to evaluate their clinical diagnostic accuracy.

The objective of this paper is to review the theoretical basis and clinical application of electrical impedance plethysmography in the noninvasive evaluation of peripheral arterial and venous disease. Theoretical, experimental and clinical studies have not demonstrated a direct relationship between electrical impedance changes and limb volume changes. Potential sources of error have also been identified. This has led to the development of clinical tests based on impedance plethysmography for the detection of peripheral arterial disease, venous insufficiency and venous outflow obstruction. Impedance plethysmography, using the method of venous occlusion, is presently the most commonly employed noninvasive method for the detection of deep venous thrombosis.

A new in vitro model has been developed for studying the changes in the ultrasound Doppler spectrum that occur in the region of a stenosis. Pulsatile flow in rigid acrylic tubes was produced by means of a modified hemodialysis pump. The Doppler spectral waveforms were measured using a continuous wave Doppler system, a probe of a known field pattern, a real-time high resolution frequency analyzer, and a video display and recording system. The flow velocity waveforms were found to be nearly identical to those seen in the human carotid. Measurements were made to determine the critical stenosis and the results are similar to those reported from in vivo studies. In a preliminary study, the extent of spectral broadening was found to be dependent on the recording site in relation to the stenosis, the severity of the stenosis, and the flow rate. Using qualitative methods it was not possible to determine either the influence of the shape of the stenosis or the phase of the cardiac cycle on spectral broadening.
