Identification of the most appropriate chemically extractable pool for evaluating Cd and Pb availability remains elusive, hindering accurate assessment on environmental risks and effectiveness of remediation strategies. This study evaluated the feasibility of European Community Bureau of Reference (BCR) sequential extraction, Ca(NO3)2 extraction, and water extraction on assessing Cd and Pb availability in agricultural soil amended with slaked lime, magnesium hydroxide, corn stover biochar, and calcium dihydrogen phosphate. Moreover, the enriched isotope tracing technique (112Cd and 206Pb) was employed to evaluate the aging process of newly introduced Cd and Pb within 56 days' incubation. Results demonstrated that extractable pools by BCR and Ca(NO3)2 extraction were little impacted by amendments and showed little correlation with soil pH. This is notable because soil pH is closely linked to metal availability, indicating these extraction methods may not adequately reflect metal availability. Conversely, water-soluble concentrations of Cd and Pb were markedly influenced by amendments and exhibited strong correlations with pH (Pearson's r: -0.908 to -0.825, P < 0.001), suggesting water extraction as a more sensitive approach. Furthermore, newly introduced metals underwent a more evident aging process as demonstrated by acid-soluble and water-soluble pools. Additionally, water-soluble concentrations of essential metals were impacted by soil amendments, raising caution on their potential effects on plant growth. These findings suggest water extraction as a promising and attractive method to evaluate Cd and Pb availability, which will help provide assessment guidance for environmental risks caused by heavy metals and develop efficient remediation strategies.

Background: The pericapsular nerve group (PENG) block offers effective postoperative pain relief following hip fracture surgery. This research investigated three doses of bupivacaine, all administered in the same total volume, for performing ultrasound-guided PENG blocks during hip fracture procedures. Methods: This randomized, double-blinded clinical trial was conducted on 135 patients aged between 18 and 70 years of both sexes who underwent hip fracture surgeries. Participants were randomized into three groups (n = 45). Ultrasound-guided PENG block was applied, the groups received 20 mL of local anesthetics. The first group received 0.5% bupivacaine, the second group received 0.375% bupivacaine, and the third group received 0.25% bupivacaine. The following parameters were recorded: onset of sensory block, resting NRS after passively raising the limb by 15° half an hour post-procedure, quality of recovery score (QoR-15) at 24 h postoperative. Results: The 0.25% bupivacaine group exhibited a longer sensory block onset than the other groups (p ≤ .05). Significant differences were demonstrated between the groups regarding the time to 1st analgesia (p = .033) and total morphine consumption (p = .025). NRS at baseline and T30 post-block did not show significant differences between the studied groups. No significant differences were detected postoperatively in rest and dynamic NRS (p ≤ .05). Patient satisfaction, QoR-15 score, and ease of spinal positioning did not differ between the groups. Conclusion: Compared to 0.25% bupivacaine, PENG block with 0.5% and 0.375% bupivacaine provided a rapid onset sensory block, delayed first analgesic requirements, and reduced total morphine consumption after hip surgeries. Caffeine (CFF) is efficiently absorbed after ingestion, and approximately 80% of ingested CFF is metabolized to paraxanthine (PXT). Although PXT has approximately twice the adenosine receptor antagonist activity of CFF, there are few reports measuring the metabolite concentrations during CFF intoxication. Furthermore, no studies have examined the efficacy of hemodialysis (HD) on PXT or the indicators that contribute to treatment strategies for patients with acute CFF intoxication. This study analyzed the association between CFF and PXT blood levels, the blood biochemical data, and the vital signs of 27 cases with information on CFF intake and elapsed time data. It was found that HD was not as effective as CFF against PXT in CFF intoxication; however, HD was effective in cases with relatively high PXT concentrations (>10 µg/mL). Simultaneous analysis of CFF and PXT would make it possible to estimate the time elapsed from CFF intake and the risk of hyperCKemia, which may develop in cases left untreated for a prolonged period after ingestion. Therefore, the measurement of PXT, in addition to CFF, is expected to provide useful information for understanding the pathogenesis of CFF intoxication and the development of treatment strategies of acute CFF intoxication.

Copper (Cu) is vital for macroalgae's functions, but high concentrations can be toxic. Rising CO2 levels affect algal growth and Cu bioavailability. In this study, the results reveal that at 5 °C, low Cu increased Ulva linza growth, while high Cu and elevated CO2 decreased growth. At 10 °C, low Cu and elevated CO2 enhanced growth, but high Cu did not have a significant impact. At 15 °C, high Cu reduced growth, but elevated CO2 offset this effect. Furthermore, under elevated CO2 conditions, the chloroplast structure of the algae appeared to be denser, accompanied by a large amount of starch granules, compared to low CO2 conditions. These results emphasize that lower temperatures, in conjunction with elevated CO2 concentration, could intensify the toxic effects of high Cu concentrations on thalli. However, at higher temperatures, elevated CO2 concentration appeared to be capable of mitigating the detrimental effects of heavy metals on algae.

This paper analyzes the dynamic impact of economic, social, and governance factors on PM2.5 concentrations in 89 countries from 2006 to 2019. Using the GMM-PVAR approach and Impulse-Response Functions, we examine how shocks to specific variables affect PM2.5 concentrations over a 10-year period. Our findings reveal that the influence of these factors on PM2.5 levels varies over time. For example, a shock in urbanization has no effect on PM2.5 concentrations in the first year, but in the second year, pollution increases significantly. In the third period, PM2.5 levels decrease, but they rise again in the fourth period, albeit not significantly. By the fifth period, pollution decreases until a new equilibrium is reached in the sixth period. Additionally, a shock in financial development, government effectiveness, industrialization, trade openness, or GDP has no effect on PM2.5 concentrations in the initial period. However, during the second period, air pollution decreases, followed by an increase in the third period and a decrease again in the fourth period. These dynamic patterns highlight the need for environmental policies that consider the evaluation time horizon. Our analysis is supplemented by the Granger causality test, guiding specific policy recommendations based on our findings.

BACKGROUND: Pneumococcus is a common cause of pneumonia, meningitis, and other serious infections in children. The previous study has proved that the 13-valent pneumococcal conjugate vaccine (PCV13) has sufficient immunogenicity in children. The data on long-term persistence of immunity will help the follow-up development work of pneumococcal vaccines. METHODS: Children who received the full vaccination course of the tested PCV13 in the previous clinical trial were enrolled again, and these who received other pneumococcal vaccines, or were infected with one or more serotypes of S. pneumoniae corresponding to PCV13 before enrollment were excluded. Participants were divided into four groups by age which is same as that of previous trial. The study lasted for 5 years, during which we measured pneumococcal antibodies of 13 serotypes included in PCV13 at particular points in time. Geometric mean concentrations (GMCs) and seropositive rates (the rate of IgG concentration ≥0.35 µg/mL) of antibodies against 13 serotypes were calculated. RESULTS: For the participants aged 2 months, five years after primary vaccination, except for serotypes 3 and 4, seropositive rates were 100%. GMCs of IgG antibodies against 13 serotypes ranged from 0.733 to 15.160 µg/mL. All of the participants aged 7-11 months had the serotype-specific IgG concentration ≥0.35 µg/mL four years after primary vaccination with the exception of serotypes 3, 4, 6 A and 9 V. IgG GMCs were 0.753-11.031 µg/mL. All participants aged 12-23 months and 2-5 years old had the serotype-specific IgG concentration ≥0.35 µg/mL three or two years after primary vaccination respectively, except for serotype 3. IgG GMCs ranged from 0.815 to 13.111 µg/mL, and 0.684 to 12.282 µg/mL respectively. CONCLUSION: PCV13 was applied to the population aged 2 months and 7 months - 5 years old with a good immune persistence, providing more extensive evidence of long-term efficacy for that vaccine. Cadmium (Cd) is a common additive in polyvinyl chloride (PVC) and polypropylene (PP) plastics. Aquatic organisms were inevitably co-exposed to PVC/PP microplastics (MPs) and Cd, but their combined toxicity is still unknown. In this study, adult zebrafish were exposed to 200 µg/L MPs (PVC or PP) and 10 µg/L Cd alone or in combination for 28 days to investigate their toxicity and mechanisms. Results showed that combined exposure with PVC/PP enhanced the Cd accumulation in the zebrafish intestine. Subsequently, toxicology analyses showed that both PVC and PP possessed synergistic toxicity with Cd, manifested by the exfoliation and necrosis of intestinal epithelial cells, and increased levels of interleukin-1ß (IL-1ß), superoxide dismutase (SOD) and malondialdehyde (MDA). PP exhibited a stronger synergistic effect than PVC. Integration of non-targeted metabolomics and 16S rRNA gene sequencing revealed that combined exposure to PVC and Cd induced intestine toxicity mainly through bile acid (BA) biosynthesis, fructose (Fru) and mannose (Man) metabolism, and pentose phosphate pathway (PPP). The combined exposure of PP and Cd induced toxicity through the arginine (Arg) and glutathione (GSH) metabolisms. Meanwhile, combined exposure of PVC/PP and Cd increased the abundance of intestinal Proteobacteria and pathogen Vibrio, and decreased the abundance of Gemmobacter. These changes indrectly promoted the synergistic toxicity of PVC/PP and Cd through metabolites, such as indole-3-pyruvate (IPyA), chenodeoxycholic acid (CDCA), and cholic acid (CA). These findings highlighted that more attention should be paid to the toxicity of chemicals at environmentally relevant concentrations, particularly those co-existing with MPs.

Objective: Asylum seekers often experience ethnic discrimination on the flight or in the host country, which may be associated with chronic stress and impaired mental health. Dysregulation of the hypothalamic-pituitary-adrenal axis, a known physiological correlate of chronic stress, can be assessed using hair cortisol concentrations (HCC). The present study aimed to investigate how different forms of perceived ethnic discrimination are associated with mental health outcomes, HCC, and protective factors in asylum seekers living in Germany.Methods: Somatic symptoms (PHQ-15), symptoms of posttraumatic stress (PDS), depressive symptoms (PHQ-9), different forms of ethnic discrimination (active harm, passive harm, institutional discrimination), and protective factors (in-group identification, social support) were assessed cross-sectionally in 144 asylum seekers (average age 32 years, average duration of stay in Germany nine months; 67% men). HCC were obtained from 68 participants. Multiple regression analyses were conducted and social support and in-group identification were tested as potential moderators.Results: Active ethnic discrimination was positively associated with all assessed mental health outcomes, and all forms of ethnic discrimination positively correlated with depressive symptoms. Ethnic discrimination was not associated with HCC. When controlling for other possible influences (e.g. age, gender, traumatic events), passive harm was negatively associated with depressive symptoms (ß = -0.17, p = .033) and active harm was positively associated (ß = 0.28, p = .022) with somatic symptoms. After the inclusion of the protective factors, the associations were no longer significant. Lower social support was associated with higher depressive symptoms (ß = -0.35, p < .001), posttraumatic stress (ß = -0.77, p < .001), and somatic symptoms (ß = -0.32, p < .001), but did not moderate the associations between ethnic discrimination and the mental health outcomes.Conclusions: Perceived ethnic discrimination may negatively influence asylum seekers' mental health but does not seem to be associated with HCC. Social support was associated with psychological symptom severity, but did not buffer the effects of ethnic discrimination on mental health.


The study examined the relationship between ethnic and institutional discrimination, protective factors (social support and in-group identification), hair cortisol concentrations (HCC) and mental health in asylum seekers.Ethnic discrimination was associated with mental health outcomes and social support, but not with HCC.Lower social support was associated with higher depressive and somatic symptoms, but did not moderate the relationship between ethnic discrimination and mental health.

Deterioration of air quality in Kuala Lumpur caused by mobile sources and traffic-related activities with interaction with climatic conditions and dispersion in the atmosphere. This study was focused on predicting the averaged 1-h concentration of particulate matter (PM10) that was emitted from private cars in Kuala Lumpur by applying the air dispersion model American Meteorological Society (AMS)/United States Environmental Protection Agency (EPA) Regulatory Model (AERMOD) in 2014. The AERMOD model indicates that private cars in Kuala Lumpur recorded 1-h concentration of PM10 below the Malaysian Ambient Air Quality Guidelines (RMAQG) in 2014. The highest concentration was 59.2 µg/m3 in 2014 and mainly concentrated around the city center and highway networks. The highest concentrations are recorded in early morning and late at night, with the highest concentrations at 4 am and 2 am and 48.68 µg/m3 and 38.42 µg/m3, respectively. Late night to early morning is identified as the dominant time interval for pollutant dispersion. The findings demonstrated that air quality had a significant impact on the 1-h concentration of PM10 emitted from private cars in Kuala Lumpur.

The ever-increasing plastic waste accumulation in the marine environment necessitates a deeper understanding of microalgae interactions with micro- and nanoplastics (MNP), and the role of extracellular polymeric substances (EPS). EPS, known for its adhesive properties and produced as an algal stress response, may facilitate aggregation of both algae and MNPs, thereby impacting ecological and hydrodynamic processes such as the trophic transfer or vertical transport of MNPs. Moreover, gaining a deeper understanding of the impact of weathering processes on the potential toxicological effects of plastic particles, and the comparative significance of plastic-specific effects relative to those of naturally occurring particles such as kaolin clay, is imperative. Therefore, this study investigated the impact of fragmented, polydisperse virgin polyethylene terephthalate (PET, Daverage = 910 nm) and weathered PET (Daverage = 1700 nm) on the growth and the production of EPS of Rhodomonas salina. Algae were exposed to a range of low MNP concentrations (10, 100 and 1000 and 10,000 MNPs ml-1) for 11 days. A natural particle control (kaolin, Daverage = 1600 nm) was deployed to differentiate particle effects from plastic effects. It was observed that exposure to both weathered PET and virgin PET resulted in initially increased growth rates (7.80 % and 7.28 % respectively), followed by significant decreases in algae cell density (-30.1 % and -11.2 % respectively). Furthermore, exposure to weathered PET caused a simultaneous elevation in cellular EPS production (76.51 %). The effects of plastics were significantly larger than the effect of kaolin. Also, the observed effects were amplified by the weathering of the plastics. These observations underscore the interactions between particle type, age and concentration, and their distinct impacts on algae density and growth inhibition. The observations indicate a role for EPS as an algal protection mechanism, potentially affecting the environmental fate of MNP - microalgae aggregates.

The wide range of applications of nanomaterials (NM) in different fields has led to both uncontrolled production and release into environmental compartments, such as aquatic systems, where final disposal occurs. Some efforts have been made to estimate their concentrations in environmental matrices; however, little is known about the actual effects of environmental NM concentrations on biota. The aims of the present review are to (i) expose the state of the art of the most applied NM and their actual concentrations regarding how much is being released to the aquatic environment and which are the predicted ones; (ii) analyze the current literature to elucidate if the aforementioned conditions were proven to cause deleterious effects on the associated organisms; and (iii) identify gaps in the knowledge regarding whether the actual NM concentrations are harmful to aquatic biota. These novel materials are expected to being released into the environment in the range of hundreds to thousands of tons per year, with Si- and Ti-based NM being the two most important. The estimated environmental NM concentrations are in the low range of ng to µg/L, except for Ti-based ones, which concentrations reach values on the order of mg/L. Empirical information regarding the ecotoxicity of environmental NM concentrations mainly focused on metal-based NM, however, it resulted poor and unbalanced in terms of materials and test species. Given its high predicted environmental concentration in comparison with the others, the ecotoxicity of Ti-based NM has been well assessed in algae and fish, while little is known regarding other NM types. While only a few marine species were addressed, the freshwater species Daphnia magna and Danio rerio accounted for the majority of studies on invertebrate and fish groups, respectively. Most of the reported responses are related to oxidative stress. Overall, we consider that invertebrate groups are the most vulnerable, with emphasis on microcrustaceans, as environmentally realistic metal-based NM concentration even caused mortality in some species. In the case of fish, we assumed that environmental concentrations of Ti-based NM represent a growing concern and threat; however, further studies should be carried out by employing other kinds of NM. Furthermore, more ecotoxicological information is needed in the case of carbon-based NM, as they are expected to considerably increase in terms of released amounts and applications in the near future.

BACKGROUND: Since 2016, diquat has replaced paraquat in China, resulting in increased diquat poisoning cases. However, understanding of diquat poisoning is still limited. This study aimed to investigate the relationship between initial diquat plasma concentration, severity index, and in-hospital mortality in acute diquat poisoning cases. METHODS: This retrospective cohort study, conducted from January 2016 to July 2023 in a tertiary care hospital, used univariate logistic regression to examine the link between the initial diquat plasma concentration, severity index, and in-hospital mortality in acute diquat poisoned patients. A receiver operating characteristic curve assessed the predictive value of these parameters for prognosis. RESULTS: Among the 87 participants, the median age was 32 years, 35 (40.2%) were female. The overall mortality rate was 37.9%. Logistic regression analysis revealed that the initial diquat plasma concentration and severity index were associated with increased in-hospital mortality. These factors also effectively predicted the prognosis of acute diquat poisoning, with an area under the receiver operating characteristic curve of 0.851 and an optimal diquat concentration threshold of 2.25 mg/L (sensitivity 90.9%, specificity 74.1%, P < 0.05) and an area under the receiver operating characteristic curve of 0.845 with an optimal cut-off value for the sevity index of 9.1 mg/L*min (sensitivity 97%, specificity 74.1%, P < 0.05). DISCUSSION: Our results are limited by the retrospective design of this study. However, if validated, these results could impact management strategies, especially in East Asia. Further research is needed due to potential confounding factors. CONCLUSIONS: The findings suggest that a higher initial plasma concentration and severity index in patients with acute diquat poisoning were correlated with higher in-hospital mortality. Prospective validation will confirm the predicative value of these findings.

Purpose: This study aimed to characterize blood oxygen level-dependent (BOLD) effects in proton magnetic resonance (1H-MR) spectra obtained during optogenetic activation of the rat forelimb cortex to correct and estimate the accurate changes in metabolite concentration. Methods: For a more comprehensive understanding of BOLD effects detected with functional magnetic resonance spectroscopy (fMRS) and to optimize the correction method, a 1 Hz line-narrowing effect was simulated. Then, proton functional magnetic resonance spectroscopy (1H-fMRS) data acquired using stimulated echo acquisition mode (STEAM) at 9.4T in rats (n = 8) upon optogenetic stimulation of the primary somatosensory cortex were utilized. The data were analyzed using MATLAB routines and LCModel. Uncorrected and corrected 1H-MR spectra from the simulated and in vivo data were quantified and compared. BOLD-corrected difference spectra were also calculated and analyzed. Additionally, the effects of stimulated and non-stimulated water on the quantification of metabolite concentration swere investigated. Results: Significant mean increases in water and N-acetylaspartate (NAA) peak heights (+1.1% and +4.5%, respectively) were found to be accompanied by decreased linewidths (-0.5 Hz and -2.8%) upon optogenetic stimulation. These estimates were used for further defining an accurate line-broadening (lb) factor. The usage of a non-data-driven lb introduced false-positive errors in the metabolite concentration change estimates, thereby altering the specificity of the findings. The water and metabolite BOLD contributions were separated using different water scalings within LCModel. Conclusion: The linewidth-matching procedure using a precise lb factor remains the most effective approach for accurately quantifying small (±0.3 µmol/g) metabolic changes in 1H-fMRS studies. A simple and preliminary compartmentation of BOLD effects was proposed, but it will require validation.

Antimicrobial dosing can be a complex challenge. Although a solid rationale exists for a link between antibiotic exposure and outcome, conflicting data suggest a poor correlation between pharmacokinetic/pharmacodynamic targets and infection control. Different reasons may lead to this discrepancy: poor tissue penetration by ß-lactams due to inflammation and inadequate tissue perfusion; different bacterial response to antibiotics and biofilms; heterogeneity of the host's immune response and drug metabolism; bacterial tolerance and acquisition of resistance during therapy. Consequently, either a fixed dose of antibiotics or a fixed target concentration may be doomed to fail. The role of biomarkers in understanding and monitoring host response to infection is also incompletely defined. Nowadays, with the ever-growing stream of data collected in hospitals, utilizing the most efficient analytical tools may lead to better personalization of therapy. The rise of artificial intelligence and machine learning has allowed large amounts of data to be rapidly accessed and analyzed. These unsupervised learning models can apprehend the data structure and identify homogeneous subgroups, facilitating the individualization of medical interventions. This review aims to discuss the challenges of ß-lactam dosing, focusing on its pharmacodynamics and the new challenges and opportunities arising from integrating machine learning algorithms to personalize patient treatment.

OBJECTIVE: To determine the feasibility of catheterizing the portal vein to obtain serial portal vein blood samples in steers. We hypothesized that the portal vein catheterization would be a successful continuous sampling technique with minimal adverse effects in steers. METHODS: 2 groups of steers were used: a pilot group (n = 2) and experimental group (n = 6). In both groups, steers were sedated with xylazine. The right rib spaces were clipped and aseptically prepped. The portal vein was visualized via ultrasound, and a 14-gauge catheter was placed percutaneously and advanced into the portal vein. A guide wire was passed through the catheter, followed by a tissue dilator and then a vascular balloon catheter. In the pilot group, blood chemistries were performed prior to portal vein catheterization and then again once the catheter was placed (with samples from both the jugular vein and portal catheter). The liver was also examined at necropsy for any gross lesions in both groups. RESULTS: All steers tolerated the portal vein catheters well, with the catheters lasting for the full length of the study period (7 days). The only observed adverse reaction was a superficial abscess at the catheter site (n = 3). On necropsy, 1 liver had gross discoloration, but no other abnormalities were noted. There were no significant changes in biochemistry profiles before or after portal vein catheterization. CONCLUSIONS: Portal vein catheterization is a novel and feasible serial sampling technique of the portal vein. CLINICAL RELEVANCE: This technique can be used in future pharmacokinetic, nutrition, metabolism, or toxicity studies.

The primary objective was to evaluate the toxicity of triflumezopyrim (TFP) on P. fuscipes larvae and adults at lethal and sublethal levels through topical application. Sublethal effects were assessed by examining developmental period, fecundity, life-table parameters, and fitness parameters. Enzymatic and transcriptional analyses were conducted to determine the impact of TFP on P. fuscipes physiology and gene expression. The LC50, LC30, and LC10 of TFP against P. fuscipes larvae and adults were lower than the field-recommended dose (48.75 mg a.i. L-1), indicating direct toxicity and sublethal effects during immature stages. Exposure to LC30 of TFP extended developmental periods for 2nd-instar larvae and pupae, reduced oviposition, larval predation efficiency, and body weight in both sexes. Sublethal concentrations affected antioxidant, detoxification, and energy reservoir enzymes significantly. Transcriptional analysis revealed impacts on insecticide detoxification, resistance, and stress-related genes. KEGG analysis showed glycerolipid metabolism is the most regulated pathway, and UGT2B10 regulated several detoxification-related pathways under TFP stress. These findings prompt reconsideration of the role of TFP in paddy field IPM due to its adverse effects on P. fuscipes, emphasizing the importance of assessing its ecological impacts before widespread application in agricultural practices.

BACKGROUND: A protocol was established for ventilator-associated tracheitis or pneumonia using inhaled tobramycin 300 mg every 12 hours in mechanically ventilated children via a vibrating mesh nebulizer, 30 cm from the endotracheal tube in the inspiratory loop of the mechanical ventilator. OBJECTIVES: The primary objective was to determine the incidence of detectable tobramycin trough concentrations >0.5 µg/mL. Secondary objectives included a comparison of clinical characteristics between those with and without detectable concentrations and identification of patients with acute kidney injury (AKI) as defined by the Kidney Diseases Improving Global Outcomes (KDIGO) criteria. METHODS: This was a single-center retrospective study of critically ill children <18 years without cystic fibrosis receiving inhaled tobramycin between July 1, 2016, and August 31, 2021. Data collection included demographics, tobramycin regimen, and renal function. Analysis was performed using SAS 9.4, with a P-value <0.05, and a multivariable regression model was performed to identify factors for detectable concentrations and AKI. RESULTS: Forty-four patients (66 courses) were included, with an overall age of 0.83 years. Thirty (68%) patients had detectable concentrations and 9 (20.5%) developed AKI. No significant differences in demographics, diagnosis, mechanical ventilation settings, and number of nephrotoxins were noted between those with and without detectable concentrations or AKI. Multivariable regressions did not identify factors associated with detectable concentrations or AKI. CONCLUSION AND RELEVANCE: Detectable concentrations occurred with the majority of courses, with AKI associated with approximately one-fourth of courses. Clinicians should consider utilizing trough monitoring for all mechanically ventilated critically ill children receiving inhaled tobramycin.

Ammonia nitrogen (NH4+-N) and total phosphorus (TP) were the major control pollutants in the Yangtze River Basin. Based on measured data from 2003 to 2020, the temporal and spatial variations in concentrations and fluxes of NH4+-N and TP in the Jianli to Hankou (JL-HK) reach of the Middle Yangtze River were studied, and the impacts of flow-sediment factors, tributary inflows, and others on variations in NH4+-N and TP fluxes were discussed. The results showed that: ① In recent years, NH4+-N and TP concentrations in the mainstream have declined significantly, with annual NH4+-N and TP concentrations at each monitoring station in 2020 averagely decreasing by 41% and 34% compared to those in 2003, respectively. Spatially, NH4+-N and TP concentrations decreased and then increased along the mainstream. NH4+-N and TP concentrations of tributary inflows, which include the Dongting Lake and Han River, were generally lower than that of the mainstream. The multi-year average values of NH4+-N and TP concentrations were both averaged at 0.12 mg·L-1 in the mainstream and were averaged at 0.11 mg·L-1 and 0.09 mg·L-1 in the tributary inflows. ② The flux differences between the upper and lower sections net of tributary confluences showed that NH4+-N and TP fluxes were lost in the Jianli to Luoshan (JL-LS) sub-reach and increased in the Luoshan to Hankou (LS-HK) sub-reach in most years. NH4+-N and TP fluxes decreased in the JL-LS sub-reach, which was related to the lower NH4+-N and TP concentrations in lateral inflows, such as Dongting Lake, and thus lowered the NH4+-N and TP concentrations in the mainstream. The LS-HK sub-reach showed the opposite trends, and the water and sediment loads increased in this sub-reach. Across the whole JL-HK reach, TP flux as well as water and sediment loads were recharged along the reach, whereas NH4+-N flux was reduced greatly, which could be attributed to the pollution abatement conducted in the Yangtze River Basin, which mainly focused on NH4+-N. ③ The correlation analysis results showed that NH4+-N fluxes had the strongest correlation with NH4+-N concentrations but not significantly correlated with discharges and sediment transport rates, indicating that NH4+-N was mainly controlled by point source pollution in the study reach. TP fluxes had higher correlations with discharges and sediment transport rates in high flow level periods, and the correlations between TP fluxes and TP concentrations were better in low flow level periods, reflecting that point source pollution contributed more to TP in dry seasons compared to flood seasons.

OBJECTIVES: To assess the impact of nutritional and multiple-micronutrient supplementation to lactating mothers on the micronutrient status of mother-infant dyad at 6 months of age postnatally. DESIGN: This study was a trial that aimed to investigate the impact of maternal nutritional supplementation on infant growth. A secondary objective was to assess the effect on the micronutrient status of mother-infant pairs. The intervention group mothers received snacks with 600 kcal energy, 20 g protein and daily micronutrient tablets. SETTING: Blood samples were collected from both mothers and infants at 6 months. PARTICIPANTS: The participants in this study were mother-infant pairs. The micronutrient status of these pairs was assessed through blood samples, focusing on vitamins A, D, B12, ferritin, Zn and folate. RESULTS: Micronutrient analysis of serum samples from 600 mother-infant pairs showed that mothers in the intervention group had higher levels of serum ferritin (mean difference (MD) 14·7 ng/ml), retinol (MD 0·6 µmol/l), folate (MD 3·3 ng/ml) and vitamin D (1·03 ng/ml) at 6 months postpartum. Additionally, the supplementation was associated with a higher mean ± sd of serum ferritin (MD 8·9 ng/ml) and vitamin A (MD 0·2 µmol/l) levels in infants at 6 months. CONCLUSIONS: The study found that supplementing maternal nutrition with additional dietary and micronutrient intakes during lactation improved maternal micronutrient status and slightly increased ferritin and vitamin A levels in infants at 6 months. The findings highlight the importance of nutritional interventions for improving the micronutrient health of mother-infant pairs, with significant public health implications.Trial registered at www.clinicaltrials.gov (CTRI/2018/04/013095).

Globally, the concentration of heavy metals and sediment toxicity analysis are significant liabilities to aquatic environments. This scrutiny outlines the sediment textures, heavy metals and toxicity status associated with environmental pollution indices in the core sediment of the Cauvery and Vettar estuaries, East coast of India. The impact of rapid industrialization, urbanization, harbour activities and agricultural activities influences on the twain estuary is a significant concern to designate the environment. The contamination status of the sediments affects the potential biodiversity, ecological risks and human health. A total of two core sediments were recovered from the Cauvery and Vettar estuaries in March 2023 to decipher the environmental pollution status. Meticulous observation of the textural studies underscores the prevalence of sand content in Cauvery, and Vettar sediments consist of predominate clay content and minor silt contents. Furthermore, the organic matter is augmented in the Vettar River due to the higher input of waste disposal, seaweeds and algae due to the surrounding landmass. Twain core sediments argue that heavy metal concentration is decreasing in order as Fe > Zn > Ni > Pb > Cu > Cr by using portable X-ray fluorescence (pXRF) spectrometry. Remarkable results of environmental pollution indices such as Igeo, Ef, Cf, Cd and mCd state very highly polluted, extreme enrichments, high contamination and very high degree of contamination. Furthermore, the potential ecological risk indices such as PLI, SQGs, and PERI argue polluted, medium to high toxicity and moderate adverse ecological risk to the estuarine regions. Statistical analysis of the heavy metal affirms the enrichment of Fe metals may derive from lithogenic and/or anthropogenic influences, and the other studied metals such as Cu, Ni, Zn, Pb and Cr may be influenced by the anthropogenic activities in the aspect of point and non-point pollution sources. This could result from both estuaries undergoing higher pollution, in which the Vettar estuary is a considerable environmental risk zone compared to the Cauvery river due to the impact of industrial effluents and rapid urbanization activities. This finding underscores the urgent need for enhanced estuarine sediment quality study and comprehensive assessment of sediment toxicity, regulating the beneficial acumen for the government to follow the suitable remediation on the embellish policy of river and marine environments.
