RESUMO

AIMS: Continuous glucose monitoring (CGM) systems are standard of care for youth with type 1 diabetes with the goal of spending >70% time in range (TIR; 70-180 mg/dL, 3.9-10 mmol/L). We aimed to understand paediatric CGM user experiences with TIR metrics considering recent discussion of shifting to time in tight range (TITR; >50% time between 70 and 140 mg/dL, 3.9 and 7.8 mmol/L). METHODS: Semi-structured interviews and focus groups with adolescents with type 1 diabetes and parents of youth with type 1 diabetes focused on experiences with TIR goals and reactions to TITR. Groups and interviews were audio-recorded, transcribed and analysed using content analysis. RESULTS: Thirty participants (N = 19 parents: age 43.6 ± 5.3 years, 79% female, 47% non-Hispanic White, 20 ± 5 months since child's diagnosis; N = 11 adolescents: age 15.3 ± 2 years, 55% female, 55% non-Hispanic White, 16 ± 3 months since diagnosis) attended. Participants had varying levels of understanding of TIR. Some developed personally preferred glucose ranges. Parents often aimed to surpass 70% TIR. Many described feelings of stress and disappointment when they did not meet a TIR goal. Concerns about TITR included increased stress and burden; risk of hypoglycaemia; and family conflict. Some participants said TITR would not change their daily lives; others said it would improve their diabetes management. Families requested care team support and a clear scientific rationale for TITR. CONCLUSIONS: The wealth of CGM data creates frequent opportunities for assessing diabetes management and carries implications for management burden. Input from people with type 1 diabetes and their families will be critical in considering a shift in glycaemic goals and targets.

RESUMO

AIM: To explore reactions to and preferences for words/phrases used in communications about diabetes among adults with diabetes and parents of children with diabetes. METHODS: Eligible adults (aged 18+ years) living with diabetes, or parenting a child with diabetes, were recruited via social media to complete an online cross-sectional, mixed-methods survey. Study-specific items were used to examine 22 commonly used diabetes words/phrases in terms of participants' cognitive perceptions ('helpful', 'respectful', 'accurate', 'harmful', 'judgmental' and 'inaccurate') and emotional reactions ('optimistic', 'motivated', 'supported', 'understood', 'offended', 'blamed', 'distressed' and 'angry'). Open-ended questions invited further feedback on (non-)preferred language and its impact(s). Data were analysed using descriptive statistics and inductive thematic analysis. RESULTS: Participants (N = 865) included adults with diabetes (type 1: n = 519; type 2: n = 180, other types: n = 48) and parents of children with diabetes (n = 118). Words/phrases most commonly associated with negative perceptions/emotional responses were 'non-compliant' (60% judgmental; 47% felt blamed) and '…good/bad' (54% judgmental; 43% blamed). Positive perceptions were reported for 'managing diabetes' (73% helpful, 47% felt understood), 'person with diabetes' (72% respectful; 49% understood), '…within/outside target range' (60% helpful, 44% understood), and 'condition' (58% respectful; 43% understood). Participants' qualitative responses illuminated perceptions, experiences and impacts across five themes: (1) accuracy and simplicity; (2) identity; (3) blame, judgement and stigma; (4) respect and trust and; (5) support, hope and feeling understood. Themes were consistent across diabetes types. CONCLUSIONS: These findings provide novel evidence into (non-)preferred, and potential (negative and positive) impacts of, commonly used diabetes words/phrases, supporting the international #LanguageMatters movement.

RESUMO

INTRODUCTION: The popularity of vegetarian and vegan diets is linked to various motivations, such as health, ethics, ecology, and social and religious influence. India has the highest proportion of vegetarians and vegans. The practise of these diets is linked to moral and health reasons and environmental concerns. Vegetarianism may also be associated with eating disorders such as orthorexia (ON). AIM: The main aim of this study was to determine the psychosocial aspects of vegetarian diets. Understanding these aspects is crucial for identifying potential risks and developing effective interventions. This study investigated the reasons for following vegetarian diets, the duration of dietary adherence, the occurrence of feelings of restriction in selected situations, and the risk of orthorexia and other eating disorders. METHODS: A questionnaire survey was conducted among 186 individuals (82 vegetarians and 104 traditional dieters) between October 2023 and April 2024. The survey was administered via a Computer-Assisted Web Interview (CAWI) using Google Forms, distributed through social media, forums, and private messages. The inclusion criteria for the study group included consent, an age over 18, and a vegetarian diet, excluding those with eating disorders or diseases requiring strict diet therapy. The control group criteria were similar, excluding vegetarians and those requiring special diets. Four unreliable questionnaires were excluded from the analysis. The survey consisted of four sections: metric data, the ORTO-15 questionnaire, the EAT-26 questionnaire, and the TFEQ-13 questionnaire. RESULTS: The main motivations for following vegetarian diets were ethical and environmental (86.9%) and health (32.1%) reasons. Over half of the vegetarians had been following a plant-based diet for over five years. Vegetarians were more likely to feel restricted in restaurants and when grocery shopping. The ORTO-15 results indicate a higher risk of orthorexia among vegetarians (48.8% vs. 29.4% in the control group; p = 0.00673). The EAT-26 questionnaire showed a higher, but not statistically significant, risk of eating disorders among vegetarians (23.8% vs. 14.7%; p = 0.11391). The TFEQ-13 showed no significant differences between groups (Subscale 1: food restriction, p = 0.77279; Subscale 2: lack of control in overeating, p = 0.91935; Subscale 3: eating under the influence of emotions, p = 0.16612). CONCLUSIONS: This study concluded that ethical and environmental considerations and a belief in health benefits mainly drive vegetarians. An analysis of BMI revealed no significant differences between groups. The ORTO-15 results suggest a higher risk of orthorexia among vegetarians. The EAT-26 indicated a higher, but not statistically significant, risk of eating disorders among vegetarians and vegans. The TFEQ-13 showed no significant differences in restrictive eating, lack of control in overeating, and emotional eating. Vegetarians were likelier to encounter dietary difficulties in restaurants and shopping but less likely to feel socially excluded.

RESUMO

In this communication, we discuss the concept of psychological first aid, as relevant to diabetes management. Psychological first aid, in the diabetes care context, is defined as "the empathic support, counselling and education to improve coping skills, and optimize selfcare of persons living with diabetes, so that psychological well-being can be optimized." Various models can help provide structured psychological first aid. We feel that each and every health care provider should be able to provide effective first aid, including psychological first aid.

RESUMO

Oneirotherapy is a form of psychotherapy that is based on visual imagery and symbolism. In this communication. We discuss the role of oneirotherapy, or using vivid imagery, as adjuvant therapy in obesity care. Oneirotherapy can be used to promote acceptance of, and adherence to, prescribed therapy. It may also help improve quality of life and enhance self-esteem. However, oneirotherapy alone, without following a healthy lifestyle and or taking antiobesity medication, will help manage weight only in one's dreams.

RESUMO

AIM: This study aimed to assess the lived experiences of adolescent pregnant young women, with a specific focus on the psychosocial aspect of adolescent pregnancy, using a phenomenological approach. DESIGN: Employing the phenomenological research method, semi-structured in-depth interviews were conducted between 3 August and 18 August 2023 with 15 young women (mean age = 21.86 years; SD = 1.06) living in a province in eastern Turkey who had undergone adolescent pregnancy 3-5 years before. METHODS: The criterion sampling method, a form of purposive sampling, was used to form the sample group. Interviews were carried out until data saturation was reached. All interviews were recorded and transcribed, and thematic analysis was utilised to analyse the data. The study adhered to the COREQ checklist for reporting. RESULTS: Through data analysis, three main categories (reflections on adolescent pregnancy, challenges due to adolescent pregnancy, and effects of adolescent pregnancy) and nine sub-themes (emotions, roles and responsibilities, perception of social support, cognitive, emotional, behavioural, mental, physical, and social) emerged. CONCLUSION: Our study highlights the adverse psychosocial impact of adolescent pregnancy on young women. Young women who got married at an early age before attaining full physical, social, and psychological maturity grappled with the substantial burdens of adolescent pregnancy and adjusting to their new roles. The study's implications are significant, emphasising the necessity of addressing the psychosocial facets of adolescent marriage and pregnancy, and gaining valuable insights for the future.

RESUMO

This communication conceptualizes and characterizes the phenomenon of dialysis distress, commonly encountered in persons living with end stage kidney disease on dialysis. Dialysis distress can be defined as an emotional state, marked by extreme apprehension, anxiety, despair and/or dejection, due to a perceived inability to cope with the challenges and demands of living with dialysis. This concept can be extrapolated to persons who undergo renal replacement therapy such as renal transplant. Dialysis distress should be identified in a timely manner, and managed using appropriate support, counselling and education, delivered in an empathic manner.

RESUMO

OBJECTIVES: The Three Delays model is a well-established global public health framework for the utilization of obstetric services where each delay represents a series of factors affecting utilization: (1) Delay #1-Deciding to seek care, (2) Delay #2-Reaching an appropriate facility and (3) Delay #3-Receiving adequate care. The aim of this qualitative study was to explore the application of the Three Delays model to dental service utilization and describe factors attributed to delayed utilization within this framework. METHODS: This study utilized a framework analysis, underpinned by the Three Delays model, to examine delays in dental care utilization. A criterion purposive sample of English-speaking adults (18+ years) in Massachusetts and Florida, USA with limited dental care access was recruited. Data were collected via semi-structured interviews conducted in two phases: 17 individual interviews, followed by interviews with a subset of five participants over 3 months (a total of 18 interviews). The analysis involved inductive thematic coding and systematic organization within the framework. RESULTS: Major themes and subthemes were constructed from the participants' narratives, identified and categorized as factors in the Three Delays framework. Each of the delays was interrelated to the other two, and Delay #1 was the most common delay based on the participants' interviews. The themes and subthemes contributing to one or more delays included interpersonal communication, prior dental experience, financial considerations, childcare costs, social connection, technology literacy, time constraints, competing priorities, stressors such as eviction and immigration status and microaggressions including racism and stigma. CONCLUSION: The Three Delays model was applicable to the study of dental care utilization and factors that impact the decision to seek dental care, reaching an appropriate dental facility and receiving adequate dental care in this study context.

RESUMO

Metalloproteinases (MPs) are zinc-dependent enzymes with proteolytic activity and a variety of functions in the pathophysiology of human diseases. The main objectives of this review are to analyze a specific family of MPs, the matrix metalloproteinases (MMPs), in the most common chronic and complex diseases that affect patients' social lives and to better understand the nature of the associations between MMPs and the psychosocial environment. In accordance with the PRISMA extension for a scoping review, an examination was carried out. A collection of 24 studies was analyzed, focusing on the molecular mechanisms of MMP and their connection to the manifestation of social aspects in human disease. The complexity of the relationship between MMP and social problems is presented via an interdisciplinary approach based on complexity paradigm as a new approach for conceptualizing knowledge in health research. Finally, two implications emerge from the study: first, the psychosocial states of individuals have a profound impact on their overall health and disease conditions, which implies the importance of adopting a holistic perspective on human well-being, encompassing both physical and psychosocial aspects. Second, the use of MPs as biomarkers may provide physicians with valuable tools for a better understanding of disease when used in conjunction with "sociomarkers" to develop mathematical predictive models.

RESUMO

Many more cancers are treated with intent to cure now than in previous decades, but for most, this involves significant effects from which people need to recover psychologically and socially, as well as physically. This longitudinal photo-elicitation interview study uses grounded theory to explain how people discharged from specialist care made use of everyday social and material resources to manage this process at home. Recovery is presented as a curve in life's pathway requiring gradual reorientation, drawing on social worlds and domestic resources to calibrate this process. Findings are described in three stages: (1) responding to diagnosis and treatment, (2) using social resources for meaning-making, and (3) developing assets for recovery. During treatment, participants drew on past identities to reinforce their sense of self, and personalized health care communication supported this process. In the weeks after treatment, new frameworks of understanding were constructed from perspectives on cancer encountered in the family, workplace, and outpatient clinics. Recovery processes included the negotiation of personal change, the renegotiation of close relationships, and the use of everyday resources to regain three sensations: control, comfort, and continuity. Supportive care would benefit from an individualized exploration of the assets that can help people to negotiate this challenging phase as treatment comes to a close. Possibilities for self-care (the maintenance of health and well-being in the context of everyday life) can be explored and assessed through personalized discussion around the identities, social worlds, and everyday resources available to each individual.

RESUMO

OBJECTIVE: The purpose of this study was to conduct a scoping review to examine and summarize the characteristics of research related to mental health (MH) screenings and/or referrals to treatment in dental practices. METHODS: We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines for Scoping Reviews and searched multiple databases for terms connected with dental care, MH concerns, screening, and referral. Included articles: (1) described care provided in a dental practice, (2) described a situation where the patient is experiencing the potential MH problem, (3) did not involve dental anxiety exclusively, and (4) involved some form of MH screening and/or referral to treatment. Article analysis included a summary of key study characteristics, types of evidence, study design, and central concepts and definitions. RESULTS: The search generated 2050 records, with 26 ultimately included. Most studies involved only adults (22, 85%), but only three (12%) reported on rurality (two urban; one mixed) and only two each (8%) reported race or ethnicity. Fifteen (58%) articles were prospective and 11 (42%) were retrospective. The studies varied widely in study designs, from 11 (42%) cross-sectional methodologies to only one (4%) randomized controlled trial. Thirty-four screening tools were used to screen for symptoms of 43 MH conditions, with depression and anxiety screened for most frequently. Few articles discussed making referrals, practice workflows, or follow-up outcomes. CONCLUSIONS: Included studies provide evidence of viable options for dental practitioners regarding MH screening, referring, and conducting follow-up, but lack specificity regarding these processes. Overall, more research is needed to clarify what workflows are most efficient for dental practitioners and efficacious in identifying patients with MH concerns.

Abstract The present study aimed to verify the association between psychosocial aspects (social capital) and food patterns in adult women. A cross-sectional, population-based study was conducted with a representative sample of 1,128 women, aged 20 to 69 years, living in the urban area of the municipality of São Leopoldo, Rio Grande do Sul, Brazil, in 2015. The food patterns were identified based on the frequency of food intake and classified as: healthy (fruits, vegetables, and whole foods), at-risk (ultraprocessed foods), and Brazilian (rice and beans), while social capital was evaluated using a collective efficacy scale. It was observed that 18.9% of the sample was classified with high collective efficacy. After adjusting for potential confounding factors, a 44% higher probability was observed for adherence to the healthy pattern (PR [prevalence ratio] = 1.44; 95%CI [95% confidence interval]: 1.01-2.03; p = 0.040) and 71% higher for the Brazilian pattern (PR = 1.71; 95%CI: 1.18-2.47; p= 0.004) among women with a higher level of collective efficacy, when compared to those with a low level of collective efficacy. Thus, this study verified a significant relationship between psychosocial aspects and food intake in women.

RESUMO

Abstract This study aimed at assessing the association between psychosocial aspects and fruit and vegetable consumption in adolescents. Cross-sectional study developed with 327 adolescents from a public school in Brazil. A questionnaire developed for adolescents was applied to assess the influence of self-efficacy, family, peers, and body image on the consumption of fruits and vegetables. Specific questions assessed the intake frequency of fruits and vegetables, and the mean consumption was estimated using two 24-hour recalls. Scores related to psychosocial aspects were described as mean and median and the weight status was classified based on the body mass index. The average daily consumption of fruits and vegetables was 36.2 g and 45.4 g, respectively. Self-efficacy and the influence of peers were associated with an increase in the amount and frequency of fruits and vegetables consumption. Adolescents with higher scores of self-efficacy had greater average consumption of fruits and vegetables when compared to those with lower scores. The same was observed for the influence of peers. Self-efficacy and the influence of peers were the factors that most influenced the consumption of fruits and vegetables in adolescents.

Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder with multifaceted manifestations, affecting both physiological and psychosocial aspects of affected individuals. This abstract provides a succinct overview of the hormonal underpinnings in the pathogenesis of PCOS, focusing on altered luteinizing hormone (LH) action, insulin resistance, and hyperandrogenism. A prevailing theory suggests that insulin resistance exacerbates hyperandrogenism by influencing the synthesis of sex hormone-binding globulin and increasing androgen production from adrenal and ovarian sources. PCOS diagnosis relies on specific criteria related to hyperandrogenism, ovulatory dysfunction, and the presence of polycystic ovaries. Beyond its physical symptoms, PCOS profoundly impacts women's mental health and quality of life. The prevalence of PCOS underscores the urgency of understanding its hormonal intricacies. Insulin resistance and hyperandrogenism, particularly in the context of sex hormone-binding globulin suppression, play a central role in PCOS pathogenesis. Recognizing the key role of hormones, particularly insulin resistance and hyperandrogenism, provides a foundation for targeted interventions and treatment strategies. A comprehensive approach to PCOS must consider both its physiological and psychosocial dimensions to address the challenges faced by affected individuals.

Abstract Background Food choices can be influenced by psychosocial factors. Objective To investigate the relationships between social support and social capital with food consumption. Method This is a population-based cross-sectional study with a random sample of 1098 adults (≥18 years old) from a medium-sized city in southern Brazil. Social support was measured by Medical Outocomes Study (MOS) scale and social capital was assessed by social cohesion, neighbor support, informal social control, political effectiveness, and social action. For the assessment of food consumption was applied a scale based on the food guide for the Brazilian population. Results The mean age of the sample was 44 years (SD [Standard Deviation] = 15.8) and adequate food consumption was identified in 38.6% (95% CI: 35.7 to 41.5). After adjustment, there was a 40% higher probability of adopting adequate (healthy) food consumption among individuals with higher level of social support (OR = 1.47; 95% CI: 1.07 to 2.03), cohesion. social (OR = 1.40; 95% CI: 1.02 to 1.92) and support from neighbors (OR = 1.45; 95% CI: 1.04 to 2.02). Conclusion Both individual and contextual psychosocial aspects can play an important role in healthy food consumption in adults.

Abstract Progressive aging of the world population is a concern recognized by most public policy proposals in various areas, including health. The elderly belongs to a vulnerable social group, which requires consideration in public health interventions and policies. There is a considerable loss of sensory and motor capacities that decreases autonomy, which if associated with lesser income in older adults, hinders timely access to health benefits and its possibility to face disability appropriately. The consequences of hearing loss in the elderly are manifested in problems of speech comprehension, cognitive deterioration, and mental health disorders such as anxiety and depression. Unfortunately, interventions to increase user adherence to hearing aid use have not shown many positive results. A correct diagnosis allows to have an adequate orientation and rehabilitation through the generation of programs and strategies focused on social integration, allowing the quality of life to be maintained or improved not only in people with hearing loss. Implementing an approach that seeks to have a positive impact on adherence to hearing aid use should include and develop auditory rehabilitation programs that consider contextual aspects such as housing, support network and social integration, among others.

The Human Being is a bio-psycho-social system, whose structural and functional complexity is reflected in the manifestations of disease. Diabetes mellitus is a disease that has an impact on the psychic and psychosocial functionality of the patients. OBJECTIVE: To study aspects of a psychological and psychosocial nature in people with diabetes mellitus. METHOD: Quasi experimental research with a quantitative and comparative approach, consisting of 100 participants of both sexes: 50 subjects with diabetes mellitus in the age group 30 - 88 years and 50 subjects with identical socio-demographic characteristics, but who do not suffer from this disease. Methodological instruments used in data collection: Clinicalpsychological interview; Socio-demographic questionnaire; Clinical-dynamic disease questionnaire; Self-assessment questionnaire of psychic functionality and performance in family, social and work activities. RESULTS: In the group of participants with diabetes mellitus, compared to the control group, there are statistically significant differences of greater expressiveness of psychological aspects, such as nervousness, irritability, anxiety, and depression, and lower in psychosocial manifestations such as work capacity, mental availability to socialize with friends and to collaborate within the family. However, although the values are also higher in the categories of anxiety and pessimism and tiredness / fatigue, the differences do not reach the statistically significant level. CONCLUSION: Suffering of diabetes mellitus has a negative impact on the psychic and psychosocial functionality of patients and, therefore, negative repercussions on style and quality of life; this justifies the implementation of support and psychological monitoring measures to facilitate and promote the acceptance of the disease, adherence to treatments and adaptation to the new reality of life.
