Background: Studies on the brain structures of shift workers are limited; thus, this cross-sectional study aimed to compare the brain structures and the brain structural correlates of subjective sleepiness and insomnia symptoms between shift workers and non-shift workers. Methods: Shift workers (n = 63) and non-shift workers (n = 58) completed questionnaires assessing subjective sleepiness and insomnia symptoms. Cortical thickness, cortical surface area, and subcortical volumes were measured by magnetic resonance imaging. The brain morphometric measures were compared between the groups, and interaction analyses using the brain morphometric measures as the dependent variable were performed to test the interactions between the study group and measures of sleep disturbance (i.e., subjective sleepiness and insomnia symptoms). Results: No differences in cortical thickness, cortical surface area, or subcortical volumes were detected between shift workers and non-shift workers. A single cluster in the left motor cortex showed a significant interaction between the study group and subjective sleepiness in the cortical surface area. The correlation between the left motor cortex surface area and the subjective sleepiness level was negative in shift workers and positive in non-shift workers. Significant interaction between the study group and insomnia symptoms was present for the left/right putamen volumes. The correlation between the left/right putamen volumes and insomnia symptom levels was positive in shift workers and negative in non-shift workers. Conclusion: Left motor cortex surface area and bilateral putamen volumes were unique structural correlates of subjective sleepiness and insomnia symptoms in shift workers, respectively.

Objectives: This study aimed to investigate whether occupational noise exposure is a risk factor for insomnia among male night-shift production workers. Methods: This study followed 623 male night-shift production workers at a tire manufacturing factory without insomnia for 4 years. Insomnia was evaluated based on the insomnia severity index at baseline and at 4-year follow-up. A score of ≥15 was defined as insomnia. The higher occupational noise exposure group was defined as those individuals exposed to 8-hour time-weighted-average noise above 80 dB (A). Results: Participants' mean age was 46.3 ± 5.6 years. Of the 623 participants, 362 (58.1%) were in the higher occupational noise exposure group. At 4-year follow-up, insomnia occurred in 3.2% (n = 20) of the participants. In a multiple logistic regression analysis, the odds ratio of insomnia was 3.36 (95% confidence interval 1.083-10.405, P = 0.036) in the higher occupational noise exposure group when compared with the lower noise exposure group after adjusting for confounders. Conclusion: Our findings suggested that occupational noise exposure affected insomnia in male night-shift production workers. To prevent insomnia, efforts are required to reduce workplace noise exposure levels. Alternatively, moving to a less noisy work environment should be considered for workers with severe insomnia.

BACKGROUND: Shift workers are at high risk of developing sleep disorders such as shift worker sleep disorder or chronic insomnia. Cognitive behavioral therapy (CBT) is the first-line treatment for insomnia, and emerging evidence shows that internet-based CBT is highly effective with additional features such as continuous tracking and personalization. However, there are limited studies on internet-based CBT for shift workers with sleep disorders. OBJECTIVE: This study aimed to evaluate the impact of a 4-week, physician-assisted, internet-delivered CBT program incorporating machine learning-based well-being prediction on the sleep duration of shift workers at high risk of sleep disorders. We evaluated these outcomes using an internet-delivered CBT app and fitness trackers in the intensive care unit. METHODS: A convenience sample of 61 shift workers (mean age 32.9, SD 8.3 years) from the intensive care unit or emergency department participated in the study. Eligible participants were on a 3-shift schedule and had a Pittsburgh Sleep Quality Index score ≥5. The study comprised a 1-week baseline period, followed by a 4-week intervention period. Before the study, the participants completed questionnaires regarding the subjective evaluation of sleep, burnout syndrome, and mental health. Participants were asked to wear a commercial fitness tracker to track their daily activities, heart rate, and sleep for 5 weeks. The internet-delivered CBT program included well-being prediction, activity and sleep chart, and sleep advice. A job-based multitask and multilabel convolutional neural network-based model was used for well-being prediction. Participant-specific sleep advice was provided by sleep physicians based on daily surveys and fitness tracker data. The primary end point of this study was sleep duration. For continuous measurements (sleep duration, steps, etc), the mean baseline and week-4 intervention data were compared. The 2-tailed paired t test or Wilcoxon signed rank test was performed depending on the distribution of the data. RESULTS: In the fourth week of intervention, the mean daily sleep duration for 7 days (6.06, SD 1.30 hours) showed a statistically significant increase compared with the baseline (5.54, SD 1.36 hours; P=.02). Subjective sleep quality, as measured by the Pittsburgh Sleep Quality Index, also showed statistically significant improvement from baseline (9.10) to after the intervention (7.84; P=.001). However, no significant improvement was found in the subjective well-being scores (all P>.05). Feature importance analysis for all 45 variables in the prediction model showed that sleep duration had the highest importance. CONCLUSIONS: The physician-assisted internet-delivered CBT program targeting shift workers with a high risk of sleep disorders showed a statistically significant increase in sleep duration as measured by wearable sensors along with subjective sleep quality. This study shows that sleep improvement programs using an app and wearable sensors are feasible and may play an important role in preventing shift work-related sleep disorders. Steroid hormone levels are closely related to the endogenous circadian rhythm induced by sleep-wake and dark-light cycles. Shift work that disrupts the circadian rhythm may influence the levels of steroid hormones. The association between shift work and alterations in female sex steroid hormone levels has been studied, but little is known about testosterone and its precursor pregnenolone levels in male shift workers. The present study investigated serum pregnenolone and testosterone levels in a group of shift and daytime male workers. All participants were sampled at the beginning of the morning shift. Lower levels of serum pregnenolone and total testosterone were found in the shift workers compared to the daytime workers. Variations in pregnenolone levels may have consequences for well-being, and they might produce consequences for the levels of hormones downstream of the steroid hormone cascade, such as testosterone. The low levels of testosterone found in shift workers demonstrate the perturbative effect of shift work on testosterone serum levels, which may be independent and/or related to pregnenolone synthesis.

Purpose: Actigraphy-based sleep detection algorithms were mostly validated using nighttime sleep, and their performance in detecting daytime sleep is unclear. We evaluated and compared the performance of Actiware and the Cole-Kripke algorithm (C-K) - two commonly used actigraphy-based algorithms - in detecting daytime and nighttime sleep. Participants and Methods: Twenty-five healthy young adults were monitored by polysomnography and actigraphy during two in-lab protocols with scheduled nighttime and/or daytime sleep (within-subject design). Mixed-effect models were conducted to compare the sensitivity, specificity, and F1 score (a less-biased measure of accuracy) of Actiware (with low/medium/high threshold setting, separately) and C-K in detecting sleep epochs from actigraphy recordings during nighttime/daytime. t-tests and intraclass correlation coefficients were used to assess the agreement between actigraphy-based algorithms and polysomnography in scoring total sleep time (TST). Results: Sensitivity was similar between nighttime (Actiware: 0.93-0.99 across threshold settings; C-K: 0.61) and daytime sleep (Actiware: 0.93-0.99; C-K: 0.66) for both the C-K and Actiware (daytime/nighttime×algorithm interaction: p > 0.1). Specificity for daytime sleep was lower (Actiware: 0.35-0.54; C-K: 0.91) than that for nighttime sleep (Actiware: 0.37-0.62; C-K: 0.93; p = 0.001). Specificity was also higher for C-K than Actiware (p < 0.001), with no daytime/nighttime×algorithm interaction (p > 0.1). C-K had lower F1 (nighttime = 0.74; daytime = 0.77) than Actiware (nighttime = 0.95-0.98; daytime = 0.90-0.91) for both nighttime and daytime sleep (all p < 0.05). The daytime-nighttime difference in F1 was opposite for Actiware (daytime: 0.90-0.91; nighttime: 0.95-0.98) and C-K (daytime: 0.77; nighttime: 0.74; interaction p = 0.003). Bias in TST was lowest in Actiware (with medium-threshold) for nighttime sleep (underestimation of 5.99 min/8h) and in Actiware (with low-threshold) for daytime sleep (overestimation of 17.75 min/8h). Conclusion: Daytime/nighttime sleep affected specificity and F1 but not sensitivity of actigraphy-based sleep scoring. Overall, Actiware performed better than the C-K algorithm. Actiware with medium-threshold was the least biased in estimating nighttime TST, and Actiware with low-threshold was the least biased in estimating daytime TST.

Mechanistic studies are needed to understand how rotating shift work perturbs metabolic processing. We collected plasma samples (n = 196) from 49 males, rotating car factory shift workers at the beginning and end of a night-shift (22:00-06:00 h) and day-shift (06:00 h-14:00 h). Samples underwent targeted LC-MS/MS metabolomics and concentrations of 130 metabolites were log2-transformed and pareto-scaled. An elastic net selected the most influential metabolites for linear mixed models examining within-person variation in metabolite levels at night-shift end (06:00 h) compared to day-shift start (06:00 h). Quantitative enrichment analysis explored differentially enriched biological pathways between sample time points. We included 20 metabolites (amino acids, biogenic amines, acylcarnitines, glycerophospholipids) in mixed models. Night-shift was associated with changes in concentrations of arginine (geometric mean ratio [GMR] 2.30, 95%CI 1.25, 4.23), glutamine (GMR 2.22, 95%CI 1.53, 3.24), kynurenine (GMR 3.22, 95%CI 1.05, 9.87), lysoPC18:2 (GMR 1.86, 95%CI 1.11, 3.11), lysoPC20:3 (GMR 2.48, 95%CI 1.05, 5.83), PCaa34:2 (GMR 2.27, 95%CI 1.16, 4.44), and PCae38:5 (GMR 1.66, 95%CI 1.02, 2.68). Tryptophan metabolism, glutathione metabolism, alanine metabolism, glycine and serine metabolism, and urea cycle were pathways differing between shifts. Night shift work was associated with changes in metabolites and the perturbation of metabolic and biochemical pathways related to a variety of health outcomes.

Objectives: Night Shift work is an increasingly common working order that affects human well-being and it is little known about its arrhythmic role in the cardiovascular system. We aimed to investigate the effect of working at night shifts on P-wave dispersion (Pd), QT and QTc dispersions (QTd, QTcd resp) on surface electrocardiography. Methods: We included 286 foundry workers who work at night shift and 100 foundry workers who work on day time only. The night shift workers were divided into three subgroups according to the length of time they worked at night shift. Surface electrocardiography and blood tests were applied for all participants. Results: Pd, QTd and QTcd values increased in the night shift workers compared to the day time workers (p < 0.05). In subgroup analysis; the night shift workers for more than 15 years had a significantly higher Pd, QTd and QTcd compared to others (p < 0.001). Correlation analysis revealed significant positive correlations with working duration and Pd (r = 0.578, p < 0.001) and QTcd (r = 0.417, p < 0.001). In the linear regression analysis, working duration at night shift was significantly associated with Pd and QTcd values, independent from other associated clinical risk parameters. Conclusions: This study makes one of the first attempts to assess changes in ECG parameters reflecting tendency to rhythm disturbances, in night shift workers. Our results further underline the importance of covering a comprehensive evaluation of ECG in periodical health check-ups in night shift workers to evaluate the risk of both atrial and ventricular arrhythmias.

Shift work disrupts an otherwise normal circadian rhythm, which may result in sleepiness among night-shift workers. Artificial light has been shown to alter the light-dark cycle of shift workers and reset or phase shift the biological clock, improving nighttime alertness in workers. However, the effect of light therapy on improving sleepiness in nighttime workers has not been effectively confirmed in nursing clinical studies, and it is worth using relevant studies to provide the best evidence in any clinical setting. Systematic review and meta-analysis were used. The study was performed using PRISMA. Academic Search Complete, Embase, MEDLINE, the Cochrane Library, and CINAHL were searched, from the inception of each database to 27 December 2021. The Cochrane risk of bias tool was used to assess the methodological quality of each study. Standardized mean differences (SMDs) with 95% confidence intervals (CIs) were synthesized using a random-effects model to assess the efficacy of lighting intervention to improve sleepiness in night-shift workers. Sensitivity analysis followed by subgroup analysis was employed to examine heterogeneity. The meta-analysis was performed using Review Manager 5.4.1 software. A total of 14 studies from 7 countries were included. The overall result shows that lighting interventions significantly improved sleepiness. Further, the blue-enriched white light with a color temperature greater than 5000 Kelvin was effective in improving sleepiness of night-shift workers. This study unveils the emergent knowledge that light interventions with blue-enriched white were effective in improving sleepiness for night-shift workers, including nurses. This finding can be applied to ensure patient safety, reduce accidents, and improve work efficiency and job satisfaction. Nurses constitute the largest health professional workforce. We suggest that hospitals can insert blue-enriched white light equipment for night-shift healthcare providers. Several evidence-based suggestions are made for further consideration.

Vitamin D deficiency is prevalent in many developed countries, and several studies suggest that vitamin D plays an essential role in brain function. A recent study showed that vitamin D deficiency was closely associated with daytime sleepiness and shorter sleep time. The relationshipbetween vitamin D levels and calcium levels is well established, and calcium level regulates slow-wave sleep generation. It is conceivable that the sleep disturbance in vitamin D deficiency may be due to an altered calcium level. Nonetheless, calcium levels, sleep disturbances, and activity rhythms have not been investigated directly. Therefore, we hypothesized that calcium and vitamin D levels might be important in regulating sleep and activity rhythm, and we analyzed the correlation with calcium levels by actigraphy analysis. Interestingly, a negative correlation was found between calcium level and sleep latency, total sleep time, use of sleep medicine, and daytime dysfunction among shift workers. In contrast, non-shift workers showed a negative correlation between the calcium level and the circadian phase. These findings suggest that low serum calcium levels may disrupt sleep-wake control and rest-activity rhythm, even if they are within the normal range.

Shift work does not synchronize with the 24-hour human circadian rhythm, so shift workers experience various problems related to this disruption. This study investigated the relationship between suicidal risk and emotional difficulties such as depression and anxiety in shift workers according to gender. We analyzed data collected from 79,009 health checkup recipients. We checked psychosocial factors for different genders using the Chi-square test and t-tests. To examine the relationship between shift work and suicidal ideation, we estimated an odds ratio after adjustment for age, depression, and insomnia. There was no difference in terms of the risk of suicidal ideation between shift workers and day workers among women (adjusted odds ratio [AOR], 0.957; 95% confidence interval [CI], 0.826-1.109), whereas the risk of suicidal ideation was significantly higher for shift workers than for day workers among men (AOR, 1.157; 95% CI, 1.021-1.310). This study confirmed that associated factors of shift work-related suicidal ideation could act differently by gender. Shift work for women is generally associated with emotional difficulties such as depression and anxiety, whereas maladaptation to shift work may be related to suicidal ideation among men. A gender-specific suicide prevention approaches will be needed for shift workers.

Volunteer and career firefighters are at risk of major depressive disorders, posttraumatic stress disorder (PTSD), alcohol use disorder, and other mental health disorders due to the demanding and unpredictable nature of their employment. The mental health risks are exacerbated by the need to work extended hours, night shifts, and/or rotating schedules, or the competing demands of other employment, especially in volunteer firefighters. The mental health disorders and risk factors interact with altered sleeping patterns. In the current study, we examined volunteer and career firefighters regarding the association between mental health and sleep, drawing from a national Canadian mental health survey of 1217 firefighters. Most (69%) of the firefighters reported less than ideal sleep quality and 21% screened positive for clinical insomnia, with no significant difference between volunteer and career subgroups. Firefighters with insomnia had higher odds ratios (OR) and frequencies for PTSD (OR = 4.98), generalized anxiety disorder (OR = 7.15), panic disorder (OR = 6.88), social phobia (OR = 4.98), and major depressive disorder (OR = 7.91), than firefighters without insomnia. The burden of sleep disorders and their association with mental health disorders suggests that sleep should be considered in health monitoring and self-management, environmental design, fire service work-organization policies, and health programming.

INTRODUCTION: Taiwan's firefighters use a shift rotation system with 2 days of work and 1 day of rest. Numerous papers have already explored the risks of shift work to the body. However, little data concern the impact of shift work on health as reflected in medical visits. This study used individuals' medical visit record in Taiwan's health insurance system. The locally called "health bank" contains individuals' medical visit record, health insurance payment points and the medicine used. METHODS: Consent was obtained from 150 firefighters who were serving under the shift rotation system to obtain their 2015 individual "My Health Bank" medical data. Comparisons were made between national health insurance data norm. RESULTS: Firefighters make significantly more visits for Western medicine than the annual average (firefighters 6.27 vs norm 5.24, P = 0.04142), more total number of medical visits (9.57 vs 7.75, P = 0.0102), more annual average payment points for Western medicine (4079 vs 2741, P = 0.003151), and a greater average number of total annual medical visit points (7003 vs 4940, p = 0.0003157). Firefighters had significantly higher incidents of respiratory diseases, urogenital diseases, skin and subcutaneous tissue diseases, musculoskeletal system and connective tissue diseases, injuries, and illness from poisoning than did the norm (P<0.05). CONCLUSION: A persuasive health-survey-based method for workers in high occupational hazard industries was proposed in this study, and the result was highly correlated with risk factors of fireworkers. The proposed study method is potential to investigate risk factors of other working.

BACKGROUND: Night-shift workers experience circadian rhythm disruption, changes in sleep time duration, and effects on their eating habits. All these factors may be related to the release of inflammatory mediators and may affect oral inflammation and periodontal health status. The objective of this study was to analyze the effects of sleep time duration on oral inflammation and periodontal health status in night-shift workers and non-night-shift workers. METHODS: This study involved two groups with 27 participants each: one group of night-shift workers and one group of non-night-shift workers. Examination of depth of pocket and bleeding on probing (BOP) was conducted with a periodontal probe. Non-stimulating saliva samples were collected to analyze the levels of melatonin, malondialdehyde (MDA), and tumor necrosis factor α (TNF-α) using ELISA. Comparisons for each parameter were performed using independent t-tests, and the relationships between duration of sleep and depth of pocket, BOP, salivary melatonin, MDA, and TNF-α were calculated using linear regression. RESULTS: The night-shift worker group had a short sleep time duration (p = 0.000). The salivary melatonin level of the night-shift workers was lower than that of the non-night-shift workers (p = 0.000). MDA, depth of pocket, and BOP were higher in the night-shift workers (p = 0.000). Only salivary melatonin showed a correlation with sleep time duration in the night-shift worker group (p < 0.05). Neither subject group showed an effect of sleep time duration on depth of pocket, BOP, salivary melatonin, MDA, or TNF-α (p > 0.05). CONCLUSION: Night-shift workers showed higher rates of oral inflammation and periodontal health status, but there was no relationship between these factors and sleep time duration.

BACKGROUND: This study aimed to identify the validity of breast cancer symptom questionnaire of worker's special health examination and its relationship with breast ultrasonography findings in young female night workers. METHODS: The breast cancer symptom questionnaire data of worker's special health examination and breast ultrasonography results in young female shift workers who worked in one electronic manufacture company were collected from 2014 to 2018. RESULTS: Of the 857 workers, 18 had a Breast Imaging Reporting and Database System category 4 or higher. Among other variables, shift work tenure alone was associated with the risk of having a Breast Imaging Reporting and Database System category higher than 4. The sensitivity, specificity, positive predictive value, and negative predictive value of the symptom questionnaire were 16.7%, 87.7%, 2.8%, and 98.0%, respectively. CONCLUSION: The current breast cancer symptom questionnaire of the worker's special health examination is inappropriate due to its low sensitivity and positive predictive value. In the future, female night workers will need alternative measures for more accurate screening for breast cancer.

STUDY OBJECTIVES: Social jetlag, a mismatch between biological and social timing, has been reported to be associated with depressive symptoms among general population. However, evidence on this association is lacking among non-shift workers, who are under pressure to adapt themselves to a work schedule. We investigated the cross-sectional association of social jetlag with depressive symptoms among Japanese non-shift workers. METHODS: This study included 1,404 employees, aged 18-78 years, who completed a study questionnaire at a periodic health checkup. Social jetlag was calculated as the absolute value of the difference in the midpoint of sleep times between weekdays and weekends. Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale. Multivariable logistic regression was used to estimate the odds ratio (OR) with adjustments for potential confounders including diet and chronotype. RESULTS: Of the study participants, 63.5%, 28.4%, and 8.1% had less than 1 hour, 1 to less than 2 hours, and at least 2 hours of social jetlag, respectively. Greater social jetlag was significantly associated with an increased likelihood of having depressive symptoms. The multivariable-adjusted OR (95% confidence interval) were 1.30 (0.95 to 1.78) and 2.14 (1.26-3.62) for 1 to less than 2 hour and at least 2 hours compared to less than 1 hour of social jetlag. The association between social jetlag and depressive symptoms appeared to be linear, according to restricted cubic spline regression. CONCLUSION: Results suggest that greater social jetlag is associated with an increased likelihood of having depressive symptoms among non-shift workers.

INTRODUCTION: Rotational shift work has a considerable effect on immune function and cause inflammation. In addition, it may lead to unhealthy dietary intake. No earlier study has examined the association between dietary patterns and inflammation in rotational shift workers. Therefore, this study was conducted to investigate the association between dietary patterns and circulating proinflammatory cytokines among shift workers. METHODS: This cross-sectional study was conducted among 257 male shift workers. Dietary intake of participants was examined using the semiquantitative Food Frequency Questionnaire. Serum concentrations of interleukin-6 (IL-6), tumour necrosis factor-α (TNF-α) and high sensitivity C-reactive protein (hs-CRP) were measured using suitable commercial kits. RESULTS: Three main dietary patterns included green vegetables, yellow vegetables and cruciferous vegetables (VEG), liquid oils and mayonnaise, fast food and eggs (LFE), as well as tea and coffee, refined grains and spice (TRS). Subjects with the highest adherence to VEG dietary pattern had a significantly lower concentration of IL-6 (p<0.01) and TNF-α (p<0.001) as compared with those with the lowest adherence. On the other hand, a significant negative association was found between LFE dietary pattern and serum concentrations of IL-6 (p=0.01) and TNF-α (p=0.02). However, no significant association was found between adherence to VEG (p=0.34) or LFE (p=0.99) dietary patterns and levels of hs-CRP and between adherence to TRS dietary pattern and any of the inflammatory cytokines. CONCLUSION: Adherence to VEG and LFE dietary patterns was inversely and directly associated to serum IL-6 and TNF-α concentrations in shift workers, respectively. However, no significant association was found between adherence to these two dietary patterns and serum hs-CRP concentrations and between TRS dietary pattern and any of the inflammatory cytokines.

BACKGROUND: Once a person is diagnosed with diabetes, aggressive management is imperative to minimize poor glycemic control devastating outcomes. However, for some patients reaching optimum blood glucose levels is challenging due to the complexity of diabetes care. To achieve good blood glucose control, patients affected by diabetes must engage in self-care activities that include routine blood glucose check, dietary control, physical activity, medication regimen, and routine medical provider visits. Diabetes-associated self-care activities aimed to reach good blood glucose control can be hindered by multiple factors including shift work. OBJECTIVE: To evaluate self-management activities of individuals affected by diabetes who are employed as shift workers. This study also informs primary care nurse practitioners of the challenges shift workers face in managing their disease. METHODS: This was a cross-sectional descriptive study. Participants were individuals affected by type II diabetes from a single primary care practice (N = 86); 45 were individuals working on the dayshift and 41 worked on the evening/night shift. Each participant completed the diabetes self-management questionnaire and author-developed demographic/supplemental questionnaire. CONCLUSIONS: There were no differences in self-reported diabetes management activities (i.e., physical activity, glucose management, and healthcare use) between the two groups. Thirty-nine percent of participants working shifts reported worse sleep patterns compared to their dayshift counterparts (X 2[1, N = 85] = 8.73, p = .003). Evening/night shift workers also reported more symptoms such as leg pain, fungal infection, numbness of the feet and legs, dizziness, and vision changes (X 2[1, N = 79] = 43.037, p < .001). IMPLICATIONS FOR PRACTICE: A better understanding of the impact that shift work has on diabetes care may help healthcare providers formulate meaningful treatment plans to meet the needs of evening/night shift diabetic workers. The use of a patient-centered medical home is one strategy.

PURPOSE: This study aimed to investigate whether insomnia among newly-employed shift-working nurses is associated with menstrual cycle irregularity. METHODS: We followed 287 nurses employed between 2015 and 2016 for 12 months. An Insomnia Severity Index (ISI) was completed to determine levels of insomnia 6 months after beginning work. At baseline and at 12-month follow-up, menstrual cycle information was obtained using clinical interviews. We analyzed baseline data separately by prevalence (participants with menstrual irregularity, n = 287) and incidence (participants with no reported menstrual irregularity, n = 238). RESULTS: In a multivariate logistic regression analysis, including age, body mass index, and physical activity, insomnia was associated with a 2.05-fold increase in the odds of newly developing menstrual cycle irregularity compared with not having insomnia [odds ratio (OR) 2.05, 95% confidence interval (CI) 1.12-3.77]. Insomnia was associated with a 3.05-fold increase in the prevalence of menstrual cycle irregularity compared with not having insomnia (OR 3.05, 95% CI 1.81-5.13). As the ISI score increased, both the incidence and prevalence odds of menstrual cycle irregularity tended to increase. CONCLUSIONS: Insomnia can induce menstrual cycle irregularity among nurses working shifts, and insomnia may have an important role in the pathway from shift work to menstrual dysfunction, aside from the possible effects of circadian rhythm disruption.

OBJECTIVE: The aim of this research was to determine the relationship among protein (PRO) intake, body composition, and muscle strength in overweight and obese firefighters. A secondary objective was to evaluate differences in body composition and muscle strength among overweight and obese firefighters with low (L; < 0.8 g·kg-1), moderate (M; 0.8-1.0 g·kg-1), and high (H; > 1.0 g·kg-1) PRO intake. METHODS: Relative PRO intake [r_PRO] was evaluated from 3-day dietary logs, self-reported by 43 overweight and obese male career firefighters (mean ± standard deviation; age = 37.3 ± 7.2 years; body mass index = 33.2 ± 5.0 kg·m-2; percent body fat [%BF] = 28.9 ± 4.0%). Body composition (fat mass [FM], %BF, lean mass [LM], percent LM [%LM]) and muscle strength (peak torque [PT], relative peak torque [r_PT] of the leg extensors) were measured using dual-energy x-ray absorptiometry and isokinetic dynamometry, respectively. RESULTS: Greater r_PRO was associated with less FM, %BF, LM (r = -0.498 to -0.363) and greater %LM (r = 0.363), but not muscle strength (p > 0.05). Fat mass (r = -0.373) and %BF (r = -0.369) were associated with lower r_PT; %LM was associated with greater r_PT (r = 0.373). Individuals with L r_PRO had greater FM (mean difference ± standard error: L-H = 10.08 ± 3.18 kg), %BF (L-H = 3.8% ± 1.4%) and lower %LM (L-H = -3.7% ± 1.3%) than those with H r_PRO (p < 0.05) but no significant differences in muscle strength (p > 0.05). CONCLUSIONS: Protein intake > 0.8 g·kg-1 was associated with more favorable body composition in male career firefighters.

OBJECTIVES: This study aimed to elucidate the effects of two naps taken at night on morning waking state and performance. METHODS: The participants were 12 women. The experiment was performed in a laboratory over 2 days (16:00-09:00). In this crossover comparative study, three experimental nap conditions were used (naps from 22:30 to 00:00 and from 02:30 to 03:00 (22:30-NAP), 00:30 to 02:00 and 04:30 to 05:00 (00:30-NAP), and no naps (NO-NAP), respectively). Measurement items were a Visual Analog Scale for sleepiness and fatigue, the Psychomotor Vigilance Test (PVT), and single-digit addition calculations (10 min) every hour for 18 h from 16:00 to 09:00, excluding nap times. RESULTS: Sleep inertia and sleepiness were noted directly after napping. Less sleepiness and fatigue were noted in the nap groups between 06:00 and 09:00 in the morning than in the NO-NAP condition and PVT response times were faster. Since participants in the nap groups were able to conduct more single-digit addition calculations, the performance of these groups appeared to be superior to that of the NO-NAP condition. Furthermore, the performance of calculations was significantly better in the 00:30-NAP than in the 22:30-NAP. CONCLUSIONS: Taking two naps during a simulated night shift helps improve sleepiness and fatigue and maintain performance. Taking a nap in the early morning appears to be promising for improving the waking state.
