RESUMO

BACKGROUND: Eosinophilic, ring-shaped structures, referred to as Liesegang rings (LRs), have been identified in aspirates from many organs, usually in relation to cystic or inflammatory lesions, but never before in relation to a dermoid cyst. CASE: LRs were intermingled with squamous cells in aspirates from a cyst of the floor of the mouth. The patient, a 23-year-old man, presented with a sublingual mass that had recently grown. Histopathologic study showed a ruptured dermoid cyst with mural foci of granulomatous foreign body reaction mixed with numerous ringlike structures, LRs. CONCLUSION: To the best of our knowledge, this is the first reported case of LRs in a dermoid cyst. Pathologists should be aware of this association when diagnosing cystic lesions.

RESUMO

The case of a neonate with a dermoid cyst originating within an occipital encephalocele is reported. The finding of the tumor was made during the routine repair of the meningoencephalocele. To our knowledge this is the first time that this association has been described.

The primary tumour tissues from 13 teratomas were investigated, 3 cases of immature teratoma (1 of pure type and 2 of mixed type), 5 cases of dermoid cyst, and 5 cases of mature solid teratoma, with special reference to N-myc gene amplification, productivity of neuron specific enolase (NSE), and the presence of double minutes (DMs). The possible relationship between these variables and malignancy of the tumours was also examined. N-myc gene amplification and NSE production were recognized in the primary tumour tissues and the first and the third passage cultured cells of all of the 3 cases of immature teratoma containing immature neural tissues. In 2 cases DMs were recognized. In dermoid cysts and mature teratoma, neither N-myc gene amplification nor NSE production were recognized in either the primary tumour tissues or cultured cells. The chromosomes were normal. Malignancy of teratoma is generally decided by the clinical stage and histological grade, but a more securely based decision is necessary where an immature teratoma contains immature neural tissues. The presence of N-myc gene amplification, NSE productivity and the presence of DMs may be valuable.

RESUMO

Multicystic or sieve-like areas are frequently present in the walls of mature cystic ovarian teratomas. This histological, immunohistochemical and ultrastructural study supports the contention that such cysts arise from oleous material which has been extravasated through the cyst wall and become lodged within fibrous tissue and small blood vessels. Some cysts may also arise from degenerate sebaceous glands.

RESUMO

This study in scanning electron microscopy (SEM) was carried out to observe the possible pathologic aspect of five teeth grossly arranged in normal sequence and of their surrounding tissues, found inside an ovarian cystic teratoma. Though the general morphology of the teeth was nearly normal, several anomalies affected the different mineralized dental tissues such as enamel hypoplasia, irregular growth of cementum, altered predentin layer, and immature osteofibrous bony outgrowths.

Among malignancies arising in dermoid cyst of the ovary, squamous cell carcinoma in situ is very unusual and set a problem of diagnosis and prognosis. In fact this diagnosis is very difficult and here we report the case of a 30-year-old woman with a squamous cell carcinoma in a dermoid cyst of the right ovary for which laminin immunohistochemical staining allowed us to prove beyond any doubt the in situ character. This demonstration is of great importance because the prognosis of a squamous cell carcinoma in dermoid cyst depends on the degree of penetration of the tumor; so in this young patient a conservative surgery could be carried out. Four years later the very good evolution confirms the accuracy of the diagnosis of squamous cell carcinoma in situ in dermoid cyst and emphasizes the benefit of laminin labelling in order to rule out the possibility of invasive carcinoma.

Previo diagnóstico histológico se caracterizaron los componentes estructurales e histoquímicos de 3 teratomas quísticos poco frecuentes. Los cortes histopatológicos seriados se procesaron con técnicas histoquímicas para mucosustancias (PAS, Alcian blue, azul de toluidina) y método tricrómico de Cason. Caso I: Presentaba un estruma ovárico con áreas de tejido tiroideo normal con sustancia coloide PAS positiva y variable reactividad con el azul de toluidina, y tejido adenomatoso macrofolicular con folículos quísticos con histiocitos en sus cavidades, microfolicular con coloide intensamente basófilo y PAS positivo, y sólido con folículos rudimentarios sin coloide. Caso II: Se trataba de un tumor de ligamento uterosacro, de rara localización, con componentes estructurales e histoquímicos compatibles con tejido traqueal. Se observaron células caliciformes y epiteliais con granulaciones apicales PAS positivas, alcianofílicas y metacromáticas. Había gran desarrollo de ácinos serosos, mucosos y mixtos. Caso III: Era un teratoma ovárico con elementos de tejido intestinal, traqueobronquial, nervioso, y piel, zonas de pulmón fetal y áreas calcificadas. Según las características analizadas, el primer tumor es un teratoma monoblástico altamente diferenciado con tejido tiroideo. El segundo se trata de un teratoma biblástico (endo y mesodermo) bien diferenciado e histoquímicamente funcional. El tercero es un teratoma triblástico con tejidos en diferentes grados de diferenciación y de aparente actividad funcional. Se concluye que con técnicas citoquímicas sencillas se puedem identificar los componentes blásticos de teratomas complejos y funcionales cuyo diagnóstico contribuye a la conducta terapéutica a seguir

En el presente trabajo se estudiaron un total de 35 casos de teratomas ováricos, los cuales se recibieron fijados en formalina al 10%, previamente diagnosticados por el patólogo, como quistes dermoides benignos del ovario. Un total de 7 casos presentaron estructuras mineralizadas especificamente hueso. En lo que respecta a tejido óseo en el inferior de estos tumores, se observa con relativa frecuencia, hueso haversiano, sin embargo se observó la presencia también de abundante hueso compacto, muchas veces de láminas paralelas, en algunos casos se aprecia hueso de aspecto grumoso y en otros hueso con zonas de hipomineralización o mal mineralizado. La presencia de tejido osteide fue escasa, así como también los osteoblastos; un posible osteoclasto se observó en uno de los casos. Se encontraron dos casos con presencia de hueso cercano a estructuras dentarias, lo cual indica la existencia de posible hueso alveolar y en consecuencia perteneciente a mandíbula. Con respecto al cartílago observado mostró un aspecto normal en algunos casos con grupos isógenos coronarios, pero en otros casos se observó la presencia de hipertrofía lacunar semejante a la encontrada en osificación de algunos huesos de la cara y huesesillos del oído

En el Servicio Maternidad Nor-Oeste del Hospital Central "Antonio Maria Pineda" de Barquisimeto, se presentó el caso de una joven de 15 años quien ingresó con el diagnóstico de embarazo de edad gestacional no precisada y feto muerto retenido, basado en ecosonograma que reportaba Obito Embrionario Anencéfalo. Siendo sometida a varias técnicas mecánicas y bioquímicas para expulsar el feto, sin éxito. Bajo anestesia general se evidencia útero normal y tumoración gigante de ovario

RESUMO

The ultrastructural findings of two cases of "blue nevus" in ovarian benign cystic teratomas are described. The bipolar or dendritic pigmented cells showed features of Schwann cells with melanin synthesis, but not of ordinary melanocytes. This study supports the neural origin of blue nevus.

A Phase 2 trial was conducted using intramuscular lymphoblastoid interferon (IFN, Wellcome Research Laboratories), 4 MU per day, in 10 patients with chemotherapy-resistant teratomas. There was stabilisation of disease in 2 patients both of whom were in retrospect considered to have had differentiated teratoma at the time of IFN administration. There was progression of presumed active anaplastic germ cell tumour in 8 patients. One of these patients, a 15-year-old boy with biopsy proven differentiated teratoma has received 2 courses of lymphoblastoid IFN and 1 course of recombinant leukocyte A IFN (Roche Products Ltd.) lasting 5 1/2, 8 and 8+ months respectively. He has had a mixed response in his differentiated tumour which on each occasion has been maintained for the duration that he received IFN. Rising HCG levels during his second course of interferon required additional cytotoxic chemotherapy. Lymphoblastoid IFN does not appear to be active against anaplastic germ cell tumours but both lymphoblastoid and recombinant leukocyte A IFN may be useful in the treatment of unresectable differentiated teratoma.

A case of peritoneal melanosis secondary to a ruptured benign cystic teratoma in a 28-year-old woman is reported. The patient had no history of cutaneous or ocular malignant melanotic lesions. The section of the left ovary contained benign teratoid elements. The ultrastructure of the omental and peritoneal pigmented nodules confirmed that the lesions consisted of histiocytes phagocytizing massive melanin pigments. Although no melanocytic neoplasm was found in the ovarian cystic teratoma, circumstance suggests that the ovarian, omental, and peritoneal pigmented lesions were secondary reactions to melanin released from the ruptured cystic teratoma.

A 1-month-old infant had a protuberant congenital corneal dermoid that extended into the anterior chamber. The dermoid was excised in two stages. A 12-mm lamellar keratectomy was followed three months later by a smaller (8-mm) penetrating keratoplasty. This technique minimized the complications associated with large corneal transplants and increased the chance of long-term success. The graft has remained transparent and the posterior segment appears to be normal. The infant can maintain constant fixation and recognize small objects and crawls without difficulty.

In this study the surface ultrastructure of 28 cystic ovarian teratomas was examined to determine their morphological appearance and the relation to biological behavior. The squamous epithelium of cystic tumors showed abnormal maturation and keratinization of surface cells with keratinous flakes occasionally in polypoid configurations. The respiratory epithelium showed an abundance of microvillous and ciliated cells, areas with disorganized surface structures were also seen. Piling up of squamous cells in heaps, piles, cauliflower- or onion-like arrangements were seen in lesions histologically regular. This study showed a distinct difference in surface ultrastructure as compared to ovarian cystadenomas emphasizing the difference in development. Surface ultrastructural studies also were useful in deciding the nature of histologically indeterminable lesions as well as in revealing areas of malignant potential.

RESUMO

Ultrastructural features of epidermoid cysts in the spinal leptomeninges were studied in two strains of mouse. Although the cysts were lined by stratified squamous epithelium, the normal sequence of epidermal differentiation in this epithelium was not observed in that the basal cell layer was absent in certain areas. The morphological features and genesis of this phenomenon are briefly discussed.
