RESUMO

The aim of the study is to determine the psychological well-being of patients who underwent stem cell transplantation. This cross-sectional study was conducted with 100 patients. Data were collected face-to-face using an introductory information form and the Brief Symptom Inventory.When the results of the patients were examined, the interpersonal sensitivity of the sub-dimensions of the scale was found to be 5.0 ± 4.06, depression 7.60 ± 5.37, and anxiety disorder 7.90 ± 5.34. There was a significant difference between the diagnosistime of the patients and all sub-factors of the scale, except phobic anxiety. It was found that the psychological state of the patients was directly related to the time of first diagnosis. As a result, the importance of following the psychological processof the patients during the treatment process was revealed when planning nursing care.

OBJECTIVE: To understand the practice of nurses in the context of the care model at the bedside of the patient in an Intensive Care Unit, in the light of the ethics of virtue. METHOD: Qualitative research, carried out in an Intensive Care Unit of a University Hospital MG/Brazil. Data were collected in February 2016 through a semi-structured interview applied to the 12 nurses who worked at the unit. The data were submitted to Thematic Content Analysis. RESULTS: The "bedside" care model encourages innovative practice, (re)directing nurses in the search for their telos, patient care, overcoming the fragmentation of nursing care and reconfiguring professional identity. However, the organization of the model in the scenario raised challenges related to the recognition of nurses by the multi-professional team, generating identity ruptures. CONCLUSION: The adopted bedside model interferes in the nurses' relationship with care, bringing together professional and patient, enhancing innovative and excellent practice.

RESUMO

The Covid-19 pandemic has dominated people's lives since late 2019, for more than nine months now. Healthcare resources and medicine have been completely consumed by the Covid 19 illness globally. This is a particularly difficult time for health systems because of the onerous responsibility to care for large numbers of sick people, protecting populations from contracting the infection by effective quarantine, isolation, and containment measures. In addition to this burden of work, healthcare providers are also overcome by fear of contracting the infection and transmitting it to their loved ones. It is during such difficult times that the integrity of healthcare providers is challenged. In this paper I will describe some challenges that a healthcare provider in a typical low resource setting faces during this pandemic time, and will propose the idea of "flexible adamancy" to address these challenges to the health system's integrity.

Resumo Objetivo Compreender as particularidades de comunidades quilombolas para a construção da prática do enfermeiro na Estratégia Saúde da Família, sob o prisma da Ética Feminista. Método Estudo de caso único integrado qualitativo, sustentado no referencial epistemológico da ética feminista. A coleta ocorreu ente fevereiro-junho/2018 por meio de entrevistas individuais, coletivas e observação, com a participação de 59 quilombolas e 07 enfermeiros da equipe de Estratégia Saúde da Família, na Região Metropolitana de Belo Horizonte. Dados submetidos à análise de conteúdo com auxílio do software ATLAS.ti 8.0. Resultados As particularidades dos quilombolas são identificadas nos aspectos da afrodescendência, como os vínculos de parentesco e manifestações culturais e religiosas. Viver na comunidade significa compartilhamento e preservação cultural. A prática do enfermeiro, nessas comunidades, é construída por meio do encontro do indivíduo/família/coletividade, do fortalecimento do vínculo e do respeito e valorização da cultura e modos de vida das pessoas. Reitera-se uma prática de compartilhamento de responsabilidade, na dualidade construída entre quilombolas e profissionais. Conclusões e implicações para a prática A prática do enfermeiro em comunidades quilombolas é pautada no reconhecimento de especificidades sociais e culturais de grupos com ancestrais do continente africano e contribui para o fortalecimento da integração do cuidado no encontro indivíduo-profissional, com responsabilidades compartilhadas.

La colección se realizó entre febrero y junio de 2018 por medio de entrevistas individuales y colectivas y observación con la participación de 59 quilombolas y 07 enfermeras del equipo de Estrategia de Salud Familiar, en la Región Metropolitana de Belo Horizonte. Datos enviados al análisis de contenido con la ayuda del software ATLAS.ti 8.0. Resultados Las particularidades de los quilombolas son identificados en los aspectos del afrodescendiente, como los lazos de parentesco y las manifestaciones culturales y religiosas. Vivir en la comunidad significa compartir y la preservación cultural. La práctica de las enfermeras, en estas comunidades, se construye a través del encuentro del individuo/familia/comunidad, el fortalecimiento del vínculo y el respeto y la apreciación de la cultura y las formas de vida de las personas. Se reitera una práctica de compartir la responsabilidad, en la dualidad construida entre quilombolas y profesionales. Conclusiones e implicaciones para la práctica La práctica de las enfermeras en las comunidades de quilombolas se basa en el reconocimiento de las especificidades sociales y culturales de los grupos con antepasados ​​del continente africano y contribuye a fortalecer la integración de la atención en el encuentro individuo-profesional con responsabilidades compartidas.


Abstract Objective To understand the particularities of quilombola communities for the construction of the nurses´ practice in the Family Health Strategy, from the perspective of Feminist Ethics. Method Qualitative single integrated case study, based on the epistemological framework of feminist ethics. The collection took place between 2018 February-June by means of individual, collective interviews and observation with the participation of 59 quilombolas and 07 nurses from the Family Health Strategy team, in the Metropolitan Region of Belo Horizonte. Data submitted to content analysis with the aid of ATLAS.ti 8.0 software. Results The particularities of quilombolas are identified in the aspects of afrodescendence such as kinship ties, and cultural and religious manifestations. Living in the community means sharing, and cultural preservation. The nurses´ practice is built, in these communities, by means of the individual/family/community, by strengthening of interpersonal relationships and of the respect and appreciation of people's culture and ways of life. A practice of sharing responsibility is reiterated, in the duality built between quilombolas and professionals. Conclusions and implications for practice The practice of nurses in quilombola communities is based on the recognition of social and cultural specificities of groups with ancestors from the African continent and contributes to strengthening the integration of care in the individual-professional meeting with shared responsibilities.

Resumo Objetivo: Identificar tipos penais no exercício da enfermagem e a existência de fatores relacionados a conduta profissional nas ocorrências éticas envolvendo tipos penais a partir da análise de depoimentos contidos nos processos éticos julgados no Conselho Regional de Enfermagem de São Paulo (Coren/SP). Método: Estudo exploratório-descritivo de abordagem quanti-qualitativa, nos processos éticos contendo ilícitos éticos considerados tipos penais. Os dados foram coletados nos processos ético-profissionais de enfermagem julgados pelo Coren/SP em 2012 e 2013. Resultados: Os tipos penais evidenciados na análise foram: abandono de incapaz; apropriação indébita; importunação sexual; estelionato; exercício ilegal de profissão; exercício ilegal da medicina; falsidade ideológica; extravio, sonegação ou inutilização de livro ou documento; falsificação de documento; falsificação, corrupção, adulteração ou alteração de produto destinado a fins terapêuticos ou medicinais; furto; homicídio; lesão corporal; maus tratos; peculato; tráfico de entorpecentes; uso de documento falso e vilipêndio a cadáver. Dos depoimentos emergiram sete categorias temáticas: Arrependimento Posterior; Percepção do ato ilícito; Fatores relacionados às condições de trabalho; Fatores Psicológicos e/ou sociais; Fatores Financeiros; Inobservância da responsabilidade e Dualidade. Conclusão: Os resultados do estudo foram de suma importância para identificar as ocorrências envolvendo tipos penais e com isso, a necessidade de se aprofundar a discussão sobre os problemas éticos na prática cotidiana do trabalho em enfermagem. Nessa perspectiva, o estudo denota a necessidade de se investir em melhores condições de trabalho e na formação qualificada dos profissionais de enfermagem no seu cotidiano, valorizando-os e incentivando-os à prática ética e humanizada do outro e de si.


Resumen Objetivo: Identificar tipos penales en el ejercicio de la enfermería y la existencia de factores relacionados con la conducta profesional en los casos éticos que incluyen tipos penales a partir del análisis de declaraciones en procesos éticos juzgados en el Consejo Regional de Enfermería de São Paulo (Coren/SP). Métodos: Estudio exploratorio y descriptivo, de enfoque cuantitativo y cualitativo en procesos éticos que contienen ilícitos éticos considerados tipos penales. Los datos se recolectaron de los procesos ético-profesionales de enfermería juzgados por el Coren/SP en 2012 y 2013. Resultados: Los tipos penales encontrados en el análisis fueron: abandono de incapaz; apropiación indebida; hostigamiento sexual; estafa; ejercicio ilegal de la profesión; ejercicio ilegal de la medicina; falsedad ideológica; pérdida, apropiación o inutilización de libro o documento; falsificación de documento; falsificación, corrupción, adulteración o modificación de producto destinado a fines terapéuticos o medicinales; hurto; homicidio; lesión corporal; malos tratos; malversación; tráfico de estupefacientes; uso de documento falso y profanación de cadáver. De las declaraciones surgieron siete categorías temáticas: Arrepentimiento posterior; Percepción del acto ilícito; Factores relacionados con las condiciones de trabajo; Factores psicológicos o sociales; Factores financieros; Incumplimiento de responsabilidad y Dualidad. Conclusión: Los resultados del estudio fueron de suma importancia para identificar los casos que incluyen tipos penales y, por lo tanto, también lo es la necesidad de profundizar la discusión sobre problemas éticos en la práctica cotidiana del trabajo de enfermería. Bajo esta perspectiva, el estudio denota la necesidad de invertir en mejores condiciones de trabajo y en la formación calificada de los profesionales de enfermería en su cotidianidad, además de valorizarlos e incentivarlos a una práctica ética y humanizada del otro y de sí mismo.


Abstract Objective: To identify criminal acts in nursing practice and the existence of factors related to professional behavior in ethical issues involving crime, based on the analysis of testimonies contained in ethical lawsuits judged by the Regional Nursing Council of São Paulo (Coren/SP). Method: Exploratory descriptive study with a quantitative and qualitative approach, analyzing ethical lawsuits containing ethical illicit acts considered criminal acts. Data were collected in the ethical-professional nursing lawsuits judged by Coren/SP in 2012 and 2013. Results: The criminal acts evidenced in the analysis were: abandonment of disabled person; embezzlement; sexual abuse; swindling; criminal impersonation; illegal practice of medicine; identity fraud; loss, fraud or destruction of a book or document; forgery of document; counterfeiting, contamination, adulteration or alteration of product intended for therapeutic or medicinal purposes; theft; bodily injury; maltreatment; peculation; drug trafficking; use of false document and abuse of corpse. The testimonies gave rise to seven thematic categories: Repentance for the past; Perception of the illegal act; Factors related to working conditions; Psychological and/or social factors; Financial factors; Non-admission of liability; with responsibility and Duality between perception and fact. Conclusion: The results of the study were extremely important to identify the occurrences involving criminal acts and demonstrate the need to broaden the discussion on ethical issues in daily nursing practice. In this perspective, the study demonstrates the need to invest in better working conditions and quality training for nursing professionals in their daily lives, valuing them and encouraging them to have an ethical and humane practice towards the other and themselves.

RESUMO

Registered nurses are an essential workforce group across the globe. They use their expertise and skill sets every day in clinical practice to protect, promote, and advocate on behalf of patients and families under their care. In this article we discuss the physical, emotional, and moral stresses that nurses are experiencing in their day-to-day practice settings created by the novel coronavirus. We consider the demands placed on nurses by unexpected patient surges within hospital environments and inadequate personal protective equipment and other critical resources, challenging nurses' ability to meet their professional and ethical obligations. We also share our thoughts on supporting nurses and others now, and ideas for needed healing for both individuals and organizations as we move forward. Finally, we argue for the need for substantive reform of institutional processes and systems that can deliver quality care in the future when faced with another devastating humanitarian and public health crises.

RESUMO

This article aims to reveal the ethical framework surrounding hospitalized school students, showing that, in the context of disease, traditional ethics do not work. From a philosophical perspective, the target audience are teachers and volunteers who teach at hospitals, but also nurses and other professionals who work with sick children. The development of an ethical framework based on the ethics of care (EoC) will enable teachers to guide their activity in hospitals, highlighting the need for another ethical framework in order to achieve a teaching practice that is fully responsible and compassionate. In an ethical framework centered on the sick child, concepts such as "care" and "well-being" are mobilized by understanding how they relate to the psychological well-being of hospitalized students. I propose that an educational attitude rooted in admiration, respect and love can be a good guide for teaching practices in hospitals, offering an alternative to the ethical limitations of codes based on a universal conception of justice.

RESUMO

OBJECTIVES: to reflect on the principle of equity from the perspective of social justice and its applicability in the dimensions of the nursing work process. METHODS: theoretical essay on the challenges for the practice with equity in the dimensions of the Nursing work process: teaching, care, research, political participation and administration. RESULTS: the principle of equity should: in education be transversal in the training of future professionals; in assistance to occur without privilege or discrimination, treating the unequal unequally and the equals equally; in research be the target in the creation of new knowledge; in political participation to count on the contribution of social movements; and in the administration to count on the creation of specific executable policies. FINAL CONSIDERATIONS: there are still inequalities in access to services by part of the Brazilian population. Nursing should promote in its work process the implementation of equity promotion policies with a view to social justice.

This essay speaks to the legacy value of nurses' caring for all people, no matter how they feel about the person's values or lifestyle, including the current issues around gender identity and sexual orientation. This legacy is deeply imbedded in the moral ethics of nursing and supports the proposition that if there isn't caring, it isn't nursing.

OBJECTIVE: To understand the professional practice of nurses and their influence on the development of moral sensibility. METHODS: A qualitative and descriptive study, conducted between November 2015 and February 2016 in the hospitalization units of the medical clinic of two large hospitals located in Belo Horizonte, Minas Gerais. 14 nurses participated. Data collection was done through interviews, guided by semi-structured scripts. The data were submitted to content analysis. RESULTS: Two categories emerged: Professional practice of the nurse: internal and external goods and Moral sensitivity and the interface with the professional practice of nurses. The development of the moral sensitivity of nurses is influenced by factors related to professional practice, such as interpersonal relations, ethical education and management activities. Final considerations: In the professional practice, moral sensibility is an integral part of the ethical decision-making process in services, being essential for quality care.

BACKGROUND: Ethical care provided by nurses to earthquake victims is one of the main subjects in nursing profession. OBJECTIVES: Given the information gap in this field, the present study is an attempt to explore the nurses' experience of ethical care provided to victims of an earthquake. RESEARCH DESIGN AND METHOD: A hermeneutic phenomenological study was performed. The participants were 16 nurses involved in providing care to the injured in Kermanshah earthquake, Iran. They were selected using purposeful sampling, and in-depth and semi-structured interviews were carried out. The transcribed interviews were analyzed based on the hermeneutic approach using the analysis method proposed by Diekelmann et al. ETHICAL CONSIDERATIONS: The study was approved by the Research Council and Ethics Committee of Urmia University of Medical Sciences, Iran. FINDINGS: Data analyses revealed four themes and 10 sub-themes that illustrated nurses' experience of ethical care during earthquake. The themes were (1) Respecting humanistic values (sacrifice, stepping beyond task description, and voluntary work), (2) Commitment to ethics (honesty, confidentiality, and trustworthiness), (3) Respecting dignity of victims (respecting cultural values, maintaining privacy, having humanistic perspective, and effective communication), and (4) Spiritual support (helping patients to do religious rituals Psychological support). CONCLUSION: The results showed the nurses' experience with providing care to earthquake victims. The findings underlined ethics and ethical values in providing nursing care during disasters. It is suggested that special courses on the importance of nursing ethics in critical situations be incorporated into nursing curriculums and in-service educations.

BACKGROUND: In research on co-creation in nursing, a caring manner can be used to create opportunities for the patient to reach vital goals and thereby increase the patient's quality of life in palliative home care. This can be described as an ethical cornerstone and the goal of palliative care. Nurses must be extra sensitive to patients' and their relatives' needs with regard to ethical and existential issues and situations in home care encounters, especially at the end of life. AIM: The aim of this study was to explore nurses' experiences of dealing with ethical and existential issues through co-creation at the end of life in palliative home care. RESEARCH DESIGN, PARTICIPANTS, AND RESEARCH CONTEXT: The material consisted of texts from interviews with 12 nurses in a home care context. A hermeneutical approach was used, and the method was inspired by a thematic analysis. ETHICAL CONSIDERATIONS: Informed consent was sought from the participants regarding study participation and the storage and handling of data for research purposes. Ethical permission to conduct the study was given from organizations that participated in this study. FINDINGS: A main theme and four subthemes emerged. The main theme was "Deep co-creative relationships are needed to manage ethical and existential issues at the end of life." A model was created to display the findings and relations between ethical issues and situations and the need for a deep trustful caring relationship to solve problems in palliative home care. DISCUSSION: Together, the themes can be considered as a tool for learning and dealing with ethical and existential issues at the end of life in home care. The themes can also be seen as a part of nurses' ethical competence within this context. CONCLUSION: The quality of life at the end of life can be improved through co-creation, despite difficult ethical and existential issues. Future research should focus on co-creation from the patients' perspective.

This article explores emerging ethical questions that result from knowledge development in a complex, technological age. Nursing practice is at a critical ideological and ethical precipice where decision-making is enhanced and burdened by new ways of knowing that include artificial intelligence, algorithms, Big Data, genetics and genomics, neuroscience, and technological innovation. On the positive side is the new understanding provided by large data sets; the quick and efficient reduction of data into useable pieces; the replacement of redundant human tasks by machines, error reduction, pattern recognition, and so forth. However, these innovations require skepticism and critique from a profession whose mission is to care for and protect patients. The promise of technology and the new biological sciences to radically and positively transform healthcare may seem compelling when couched in terms of safety, efficiency, and effectiveness but their role in the provision of ethical nursing care remains uncertain. Given the profound moral and clinical implications of how today's knowledge is developed and utilized, it is time to reconsider the relationship between ethics and knowledge development in this new uncharted area.

BACKGROUND: Since "dignity" is one of the fundamental rights of every patient, consideration for patients' dignity is essential. Unfortunately, in many cases, especially in cancer patients, dignity is not fully respected. Dignity is an abstract concept, and there are only a few comprehensive studies on the dignity of cancer patients in Iran. RESEARCH OBJECTIVE: This study aimed to evaluate the perception of Iranian cancer patients on human dignity. RESEARCH DESIGN: A qualitative research approach was used as the study design. The data were collected through individual semi-structured interviews and analyzed using the qualitative content analysis method. PARTICIPANTS AND RESEARCH CONTEXT:  This study was conducted on cancer patients in internal medicine wards in Iran. The data were gathered through semi-structured interviews from May 2017 to February 2018. ETHICAL CONSIDERATIONS: The study protocol was approved by the Research Ethics Committee of medical universities located in Southwest of Iran. The ethical principles were carefully followed throughout the study. FINDINGS: Based on the results of the interviews, 3 main themes and 11 categories were determined. The main themes were identified as the "personal space and privacy," "respect for human values," and "moral support." DISCUSSION: The results of the present study showed the necessity of care for cancer patients in a respectful manner. The key elements in such care were the preservation of their personal space and privacy, respect for their values, and the provision of adequate moral support. These measures will have a positive effect on the perception of such patients on human dignity. CONCLUSION: Considering the special care required by cancer patients, the Iranian healthcare and hygiene managers should design and implement a care plan that includes the ethical principles related to human dignity.

BACKGROUND: The concept of conscientious objection is well described; however, because of its nature, little is known about real experiences of nursing professionals who apply objections in their practice. Extended roles in nursing indicate that clinical and value-based dilemmas are becoming increasingly common. In addition, the migration trends of the nursing workforce have increased the need for the mutual understanding of culturally based assumptions on aspects of health care delivery. AIM: To present (a) the arguments for and against conscientious objection in nursing practice, (b) a description of current regulations and practice regarding conscientious objection in nursing in Poland and the United Kingdom, and (c) to offer a balanced view regarding the application of conscientious objection in clinical nursing practice. DESIGN: Discussion paper. ETHICAL CONSIDERATIONS: Ethical guidelines has been followed at each stage of this study. FINDINGS: Strong arguments exist both for and against conscientious objection in nursing which are underpinned by empirical research from across Europe. Arguments against conscientious objection relate less to it as a concept, but rather in regard to organisational aspects of its application and different mechanisms which could be introduced in order to reach the balance between professional and patient's rights. DISCUSSION AND CONCLUSION: Debate regarding conscientious objection is vivid, and there is consensus that the right to objection among nurses is an important, acknowledged part of nursing practice. Regulation in the United Kingdom is limited to reproductive health, while in Poland, there are no specific procedures to which nurses can apply an objection. The same obligations of those who express conscientious objection apply in both countries, including the requirement to share information with a line manager, the patient, documentation of the objection and necessity to indicate the possibility of receiving care from other nurses. Using Poland and the United Kingdom as case study countries, this article offers a balanced view regarding the application of conscientious objection in clinical nursing practice.

BACKGROUND: Medical Assistance in Dying, also known as euthanasia or assisted suicide, is expanding internationally. Canada is the first country to permit Nurse Practitioners to provide euthanasia. These developments highlight the need for nurses to reflect upon the moral and ethical issues that euthanasia presents for nursing practice. PURPOSE: The purpose of this article is to provide a narrative review of the ethical arguments surrounding euthanasia in relationship to nursing practice. METHODS: Systematic search and narrative review. Nine electronic databases were searched using vocabulary developed from a stage 1 search of Medline and CINAHL. Articles that analysed a focused ethical question related to euthanasia in the context of nursing practice were included. Articles were synthesized to provide an overview of the literature of nursing ethics and euthanasia. ETHICAL CONSIDERATIONS: This review was conducted as per established scientific guidelines. We have tried to be fair and respectful to the authors discussed. FINDINGS: Forty-three articles were identified and arranged inductively into four themes: arguments from the nature of nursing; arguments from ethical principles, concepts and theories; arguments for moral consistency; and arguments from the nature of the social good. Key considerations included nursing's moral ontology, the nurse-patient relationship, potential impact on the profession, ethical principles and theories, moral culpability for acts versus omissions, the role of intention and the nature of the society in which euthanasia would be enacted. In many cases, the same assumptions, values, principles and theories were used to argue both for and against euthanasia. DISCUSSION: The review identified a relative paucity of literature in light of the expansion of euthanasia internationally. However, the literature provided a fulsome range of positions for nurses to consider as they reflect on their own participation in euthanasia. Many of the arguments reviewed were not nursing-specific, but rather are relevant across healthcare disciplines. Arguments explicitly grounded within the nature of nursing and nurse-patient relationships warrant further exploration.
