Introdução: A rinoplastia é uma cirurgia que muitas vezes apresenta resultados imprevisíveis, mesmo em mãos de cirurgiões experientes. Neste sentido, a rinomodelação com preenchedores é um procedimento não-cirúrgico para corrigir pequenas alterações externas nasais em casos específicos. Métodos: Os autores descrevem a técnica de aplicação de preenchedores (ácido hialurônico ou hidroxiapatita de cálcio) para modelação nasal. Foram incluídos todos os pacientes submetidos ao preenchimento nasal, entre 2009 e 2012. Foi avaliado o edema da ponta nasal, a dor e o grau de satisfação dos pacientes com o resultado. Resultados: Foram incluídos 39 pacientes no estudo. Com relação aos desfechos analisados com ácido hialurônico: 52% apresentaram edema leve; 74% tiveram dor leve; 15% se mostraram muito satisfeitos e 74% satisfeitos com o resultado. Com relação aos pacientes submetidos à rinomodelação com hidroxiapatita de cálcio: 67% apresentaram edema moderado; 50% dor moderada; 17% dor intensa e 84% se mostraram satisfeitos com o resultado. Conclusão: A rinomodelação com preenchedores reabsorvíveis é um procedimento simples, com resultados estéticos aceitáveis, sendo necessário um profundo conhecimento anatômico para diminuir o risco de complicações.


Introduction: Rhinoplasty often leads to unpredictable results, even in the hands of experienced surgeons. However, in specific cases, rhinomodelation with fillers, a non-surgical procedure to correct minor nasal external changes, can be used. Methods: This study describes the application technique of fillers (hyaluronic acid or calcium hydroxyapatite) for nasal modeling. Patients undergoing nasal filling between 2009 and 2012 were included. Edema of the nasal tip, pain, and the degree of patient satisfaction with the outcome were assessed. Results: Thirty-nine patients were included in the study. Regarding the outcomes of rhinomodelation with hyaluronic acid, 52% patients presented with mild edema, 74% had mild pain, 15% were very satisfied, and 74% were satisfied with the result. For the outcomes of rhinomodelation with calcium hydroxyapatite, 67% patients presented with moderate edema, 50% had moderate pain; 17% had severe pain, and 84% were satisfied with the result. Conclusion: Rhinomodelation with resorbable fillers is a simple procedure with acceptable esthetic results. However, a deep anatomical knowledge is necessary to decrease the risk of complications.

PURPOSE: It is essential to fabricate a best-fit three-dimensional (3D) facial prosthesis model capable of facial expressions. In order for the facial prosthesis to remain in position, especially around marginal areas subject to movement, a new method of making 3D facial expression models using time-series data allowing changes in facial expression by morphing technique was developed. METHODS: Seven normal subjects and seven patients with nasal defects or nasal deformities participated in this study. Three distinct facial expressions (i.e., a neutral expression, smiled, and open mouthed) were digitally acquired with a facial scanner. Prepared template models were transformed to homologous models, which can represent the form as shape data with the same number of point cloud data of the same topology referring to the scanning data. Finally, 3D facial expression models were completed by generating a morphing image based on two sets of homologous models, and the accuracy of the homologous models of all subjects was evaluated. RESULTS: 3D facial expression models of both normal subjects and patients with nasal defects were successfully generated. No significant differences in shape between the scanned models and homologous models were shown. CONCLUSIONS: The high accuracy of this 3D facial expression model in both normal subjects and patients suggests its use for fabricating facial prostheses.

Introdução: A reconstrução nasal é a mais antiga das cirurgias plásticas. A anatomia nasal é complexa e necessita de uma associação de técnicas para a restauração da função e estética nasal adequada. Pereira et al. descreveram uma técnica que possibilita a reconstrução nasal total da cartilagem alar, com o uso de um enxerto da cartilagem auricular, com mínima deformidade auricular secundária à retirada do enxerto. O objetivo deste trabalho é apresentar uma modificação da técnica acima descrita, que possibilita reconstruir mais uma região anatômica do nariz, sem aumentar a morbidade, realizada por Collares et al., e a sua inserção no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre. Métodos: Foi realizado um estudo retrospectivo. Avaliou-se a inserção da modificação da técnica em 10 pacientes que realizaram reconstrução nasal total. Resultados: Após a análise dos 10 casos, utilizando a modificação da técnica inserida no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre, encontramos uma melhoria da forma do nariz, a válvula nasal interna com preservação da função e sem sequelas secundárias à retirada do enxerto auricular. Conclusão: Nesta série de casos, a modificação da técnica de Max Pereira resultou em tratamento estético funcional adequado quando implementada no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre, sem aumentar a morbidade na área doadora.


Introduction: Nasal reconstruction is the oldest plastic surgery technique. The nasal anatomy is complex and requires an association of techniques for the restoration of function and adequate nasal esthetics. Pereira et al. described a technique that allows total nasal reconstruction of the alar cartilage through the use of an auricular cartilage graft, with minimal deformity secondary to the donor site. The objective of the present study is to present a modification, by Collares et al., of the technique described above, which allows the reconstruction of another anatomical region of the nose without increasing morbidity, and its insertion into the total nasal reconstruction protocol of Hospital de Clínicas of Porto Alegre. Methods: A retrospective study was conducted. We evaluated technique modification in 10 patients who underwent total nasal reconstructions. Results: After examining the 10 patients who were treated with the modified total nasal reconstruction protocol at the Hospital de Clínicas of Porto Alegre, we observed an improvement in the nose shape and internal nasal valve with preservation of function, without sequelae secondary to auricular graft removal. Conclusion: In this case series, the modification of the Max Pereira technique resulted in adequate aestheticfunctional treatment when implemented in the total nasal reconstruction protocol of the Hospital de Clínicas of Porto Alegre, without increasing the morbidity in the donor area.

A mutilação da face pode ocorrer devido à remoção cirúrgica de cânceres ou traumas na região, com importante prejuízo estético e funcional. Tais mutilações provocam alterações morfológicas, funcionais e psicossociais podendo levar o indivíduo ao isolamento social e familiar, ou mesmo deixa-lo estigmatizado. Assim, é imperativo que os profissionais da saúde, especialmente os cirurgiões-dentistas, participem de sua reabilitação. Nesse aspecto, as técnicas de osseointegração têm se destacado na reabilitação desses pacientes como método de fixação, entretanto, nem todos os pacientes podem se beneficiar delas, seja devido a obstáculos financeiros ou a contraindicação inerente à sua condição. O objetivo deste trabalho é descrever o caso da reabilitação de um paciente com perda de grande parte do apêndice nasal devido a um cirurgia oncológica. Paciente do sexo feminino, com 77 anos de idade compareceu apresentando perda de grande parte do apêndice nasal devido a uma cirurgia para ressecção de lesão de câncer de pele seguida de radioterapia adjuvante. A condição sistêmica da paciente contraindicava a realização de cirurgia reparadora e o uso de implantes osseointegráveis. Assim, optou-se pela reabilitação com prótese nasal de resina acrílica óculo-suportada. Em conclusão, com o procedimento empregado, a paciente apresentou melhora em sua respiração, estética e qualidade de vida, favorecendo usa reinserção à vida social(AU)


Face mutilation may occur due to surgical removal of cancer or trauma in the region, with an important aesthetic and functional impairment. Such mutilations cause morphological, functional and psychosocial changes that lead the individual to the social and family isolation, in addition to acquiring the status of stigmatized individual. Therefore, it is an imperative that health professionals, especially dentists, participate in their rehabilitation. Bone integration techniques have excelled in the rehabilitation of these patients as a fixation method; however, not all patients can benefit from it, either because of financial obstacles inherent in their condition or because of contraindication. The objective of this work is to describe the case of a rehabilitated patient with loss of much of the nasal appendage due to cancer surgery. It is a 77-year-old female patient, with loss of much of the nasal appendage due to surgery for resection of a skin cancer lesion, which was subjected to auxiliary radiation therapy and rehabilitation. Surgery and the use of bone-integrable implants were contraindicated due to the patient's overall health. So we opted for the use of a prosthetic acrylic resin nose sustained by eyeglass frames. In conclusion, by means of the procedure used, the patient experiences improvement in breathing, aesthetic appearance, quality of life, which favors her reintegration into the social life(AU)


La mutilación de la cara puede ocurrir debido a la extirpación quirúrgica del cáncer o trauma en la región, con un importante deterioro estético y funcional. Tales mutilaciones causan cambios morfológicos, funcionales y psicosociales que llevan al individuo al aislamiento social y familiar, además de la adquisición de la condición de individuo estigmatizado. Por tanto, es imperativo que los profesionales de la salud, especialmente los dentistas, participen en su rehabilitación. Técnicas de osteointegración se han destacado en la rehabilitación de estos pacientes como método de fijación, sin embargo, no todos los pacientes se pueden beneficiar de ella, ya sea debido a obstáculos financieros o contraindicación inherente a su condición. Constituye objetivo de este trabajo describir el caso de un paciente rehabilidtado con pérdida de gran parte del apéndice nasal debido a una cirugía de cáncer. Se trata de un paciente del sexo femenino, de 77 años de edad, con la pérdida de gran parte del apéndice nasal debido a una cirugía para la resección de la lesión del cáncer de piel, el cual se sometió a radioterapia auxiliar y rehabilitación. La cirugía y el uso de implantes de hueso integrable fueron contraindicados debido a la salud general del paciente. Así que optamos por el uso de una prótesis nasal de resina acrílica sostenida por armaduras de gafas. En conclusión, con el procedimiento empleado, la paciente experimenta mejora en la respiración, la estética, la calidad de vida, lo que favorece su reinserción a la vida social(AU)

Traditionally, patients with maxillofacial defects have been challenging to treat. A multitude of challenges associated with maxillofacial prosthetic treatment are not typically seen with patients who need conventional prosthodontic treatment. These types of patients generally require replacement of significant amounts of hard and soft tissues than do conventional prosthodontic patients. Most maxillofacial patients also warrant more emotional support than do conventional prosthodontic patients. Successful maxillofacial prosthetics still need to embrace the traditional goals of prosthodontic treatment: stability, support, retention, and esthetics. It is unlikely that a maxillofacial prosthesis will exactly duplicate the anatomy and function of missing or damaged structures. Although craniofacial implants (CFI's) have lower cumulative survival rates (CSR's) than intraoral endosseous implants, osseointegrated CFI's have proven to be significant adjuncts to improving retention of maxillofacial prostheses. However, CSR's of CFI's have been reported to be lower than CSR's for intraoral endosseous implants. Lately, computer-assisted design and computer-assisted machining (CAD/CAM) has been used in dentistry to facilitate fabrication of implant-supported frameworks. CAD/CAM protocols have numerous advantages over conventional casting techniques, including improved accuracy and biocompatibility, and decreased costs. The purpose of this paper is to review the literature on cumulative survival rates (CSR's) reported for CFI's and to illustrate the treatment of a maxillofacial patient using CFI's and a CAD/CAM copy-milled framework for retention and support of a nasal prosthesis.

This is a synopsis of the presentation delivered at the Society of Otorhinolaryngology and Head-Neck Nurses' Annual Congress and Nursing Symposium, September 11, 2012 in Washington, DC. We all have imperfections in our faces and bodies that we would like to change. Consider, though, the cancer patient or the trauma victim who must deal with a facial disfigurement that completely changes their appearance, and often, their ability to function normally. Anaplastology, which combines art and science through creativity and functionality, can make monumental improvements in the quality of a person's life by giving acceptable appearance back to the patient. Custom-made appliances and prosthetic creations can bring hope and confidence back to the patient. Individualized adaptations can make the prosthetic a true work of art. Nurses work closely with patients who are benefitting from this creative process. Arriving at a successful and pleasing outcome is extremely satisfying for the entire team, the patient and family.

Extraoral implants have been used for many years to anchor silicone nasal prostheses. This report describes the design and use of a specially engineered bifunctional implant, which is placed via an intraoral approach, to simultaneously anchor nasal and oral prostheses for an edentulous patient who has undergone a complete rhinectomy. The bifunctional implant was designed and milled from commercially pure titanium using computer-aided design/computer-assisted manufacturing (CAD/CAM) technology. The nasal part of the implant was designed to fit through the prepared site and protrude into the piriform aperture. A hex attachment was orientated perpendicular to the axis of the implant on this extension. The intraoral head of the implant was provided with a standard Branemark hex configuration. Implants were placed using the guide and associated instrumentation. This case demonstrates the potential for CAD/CAM technology to produce bespoke implantable components at low cost. In this report, the implant greatly facilitated the surgical and prosthetic management for the simultaneous provision of nasal and oral prostheses.

Nasal defects after tumor excision can leave a patient functionally and esthetically impaired. Loss of nasal septal cartilage support causes the soft tissue to collapse or undergo stenosis, further compounding the problem. Intranasal stents can be used to maintain the patency of such nasal defects. This clinical report describes the use of an acrylic resin nasal stent bonded to a silicone nasal prosthesis to rehabilitate a patient with a nasal defect.

Maxillofacial trauma refers to any injury to the face or jaw caused by physical force, trauma, the presence of foreign objects, animal or human bites, and burns. Facial defects can be devastating in their impact on physical structure and function of the affected individual, leading to potential compromises in quality of life. Restoration of facial defects, especially nasal defects, is a difficult challenge for both the surgeon and the prosthodontist. Here is a case report of partial nasal defect caused by trauma, rehabilitated with a magnetic nasal prosthesis made with silicone elastomers using mechanical and anatomical retentive aids.

Nasal septal perforation is a common complication of many nasal diseases such as congenital, infectious, trauma, and iatrogenic, and rarely may occur as a complication of systemic diseases, etc. The symptoms of uncorrected nasal septal perforations include crusting, epistaxis, difficulty in breathing, nasal twang in speech, postnasal discharge, foul-smelling, rhinorrhea, and hyposmia. Large nasal defects cannot be closed by hard acrylic resin nasal septal obturators because of the problems in access and path of insertion. However, obturation can be achieved by fabrication of a nasal stent that engages one of the nasal cavities. This clinical report describes prosthetic management of a patient with large nasal septal defect following septal surgery complication with an intranasal heat-processed acrylic resin stent. The stent is rendered patent for comfortable breathing, improves speech, is esthetically acceptable, dense and hygienic. These nasal stents indirectly separate the two nasal cavities with effective obturation of large nasal septal defects.

RESUMO

A new protocol for making an immediate provisional eyeglasses-supported nasal prosthesis is presented that uses laser scanning, computer-aided design/computer-aided manufacturing procedures, and rapid prototyping techniques, reducing time and costs while increasing the quality of the final product. With this protocol, the eyeglasses were digitized, and the relative position of the nasal prosthesis was planned and evaluated in a virtual environment without any try-in appointment. This innovative method saves time, reduces costs, and restores the patient's aesthetic appearance after a disfiguration caused by ablation of the nasal pyramid better than conventional restoration methods. Moreover, the digital model of the designed nasal epithesis can be used to develop a definitive prosthesis anchored to osseointegrated craniofacial implants.

RESUMO

The computer-aided design of a nasal prosthesis based on pre-operative virtual laser scanning of the affected site was virtually adapted to the post-operative laser-scanned surface. The designed volume of the nose was rapidly prototyped and used to fabricate a provisional prosthesis and a computed tomography diagnostic template to check the available premaxilla bone for implants. The mould for the nasal prosthesis was prototyped using a computer-aided design and manufacturing (CAD-CAM) procedure. In addition, the mesiostructure of an eyeglasses-supported provisional prosthesis was also designed and prototyped using CAD-CAM procedures.

A facial defect results in cosmetic, functional, and psychologic problems and acts as a difficult and challenging procedure for the maxillofacial surgeon and the prosthodontist. Retention of a facial prosthesis is an important key in the patient's satisfaction and acceptance. The introduction of extraoral implant applications have been accepted as an advanced retention method. The purpose of this article is to report the results from our clinical experience with patients wearing facial prostheses anchored to extraoral ITI implants (Straumann, AG, Basel, Switzerland).

Prosthetic rehabilitation of the midfacial defects has always perplexed maxillofacial prosthodontists. These midfacial defects lead to functional and cosmetic deficiencies. One of the causes of such defects is midline lethal granuloma, which is a destructive granulomatous lesion of uncertain aetiology, involving the nose, paranasal sinuses and the palate. Presented here is a clinical report of a 22-year-old male patient diagnosed to have lethal midline granuloma, who was referred to the Department of Prosthodontics, SDM College of Dental Sciences Dharwad. The patient had a severe midfacial defect involving the nose, the paranasal sinuses, the palate and the soft tissues of the face. Although the lesion was not of the rapidly progressive type at the time of rehabilitation, the non-specific inflammatory process was persisting. The patient was treated with chemotherapy and palliative therapy. The primary objective of the prosthetic rehabilitation was to provide closure of the severe defect to protect the soft tissues from environmental exposure. Secondarily the prosthesis also provided acceptable aesthetics and psychological benefit to the patient.

RESUMO

The immediate prosthetic rehabilitation of a patient with erosive cutaneous cytomegalovirus (CMV) is presented. Although CMV is a major cause of morbidity and mortality in immunocompromised patients, skin lesions are rarely discussed in the literature. Recognition of the susceptibility of medically compromised patients to infections that can cause extraoral and intraoral deformity is essential to prevention, early diagnosis, and prompt intervention of a life-threatening destructive debilitation. A method for rapidly fabricating a nasal prosthesis is described, and the importance of prompt esthetic and psychological rehabilitation is reviewed.

RESUMO

Carcinoma of the nasal vestibule often requires radical surgical excision, which results in total or partial loss of the nose followed by radiation therapy. A noninvasive, cost-effective technique for rehabilitation of a patient with squamous cell carcinoma of the nasal vestibule following partial rhinectomy and radiation therapy using heat-polymerizing clear acrylic resin is presented.

RESUMO

PURPOSE: This article presents a novel manufacturing approach that integrates laser surface digitizing/scanning and computer-aided design (CAD) and manufacturing (CAM) to achieve automated fabrication of spatially and anatomically accurate extraoral facial prostheses. MATERIALS AND METHODS: Topologic data of the patient's face, including the site of deformity and the anatomy of a healthy "donor" organ, were acquired using laser surface digitizing. Two different CAD data manipulation techniques were developed to alleviate problems encountered in laser-digitized data sets. To reduce patient involvement, fitting of the prosthesis was conducted using a computer model of the patient's face. Once finalized, rapid prototyping techniques were employed to fabricate a master pattern to cast the final prosthesis. RESULTS: Three case studies involving different facial prostheses were conducted to evaluate the integrated manufacturing system. The results demonstrated the many advantages of the system for facial prosthesis production. CONCLUSION: With the new manufacturing approach, reduced patient discomfort, minimal dependence on the artistic skills of the prosthetist, and short turnaround times for prosthesis production can be expected.
