OBJECTIVE: To describe the surgical technique and clinical outcome of small ruminants treated for obstructive urolithiasis using a modified tube cystostomy (MTC) technique. ANIMALS: 15 goats and 2 sheep treated with an MTC between March 2018 and February 2023. CLINICAL PRESENTATION: Animals were diagnosed with obstructive urolithiasis on the basis of history, physical examination, and ultrasonographic examination. An MTC was performed with sedation and a local block. Postoperative medical management was instituted to help reestablish urethral patency, and Foley catheters were removed after successful urination. RESULTS: Animals were hospitalized an average of 3 nights (range, 0 to 14 nights). Complications included urine spillage in the abdomen and accidental deflation of the Foley balloon. Six animals were euthanized due to poor prognosis or failure to regain urethral patency. Foley catheters were removed an average of 15.7 days postoperatively in animals that regained urethral patency. Long-term (> 1-month) follow-up was available for 8 animals, with an average postoperative survival time of 19.4 months (range, 1 to 58 months). Four animals were lost to long-term follow-up. CLINICAL RELEVANCE: This MTC technique is an effective means of catheterizing the urinary bladder in small ruminants. It can be performed under field conditions and serve as a standalone procedure for providing temporary urine egress. Patient size is limited by the length of the introducer, and an intact, distended urinary bladder and plan for reestablishing urethral patency are important considerations.

Urolithiasis is a multifactorial disease of male ruminants causing significant economic loss and compromise of animal welfare. Known risk factors include anatomic factors, urine pH, water intake, dietary composition, and genetic factors. Clinical cases of obstructive urolithiasis may be treated using a variety of medical and surgical interventions, including tube cystostomy, perineal urethrostomy, urinary bladder marsupialization, and modifications of these procedures designed to optimize patient outcome.

Data on the foot lesions of housed dairy goats, especially those that are not lame, is sparse. In this study, visits were made to four farms for whole herd locomotion scoring after milking and to observe routine foot trimming. During trimming, the amount of trimming required for each hoof was recorded, alongside the presence of lesions. The process was repeated at the next routine foot trim on three of the four farms (on Farm 1 whole herd locomotion scoring after milking was not feasible). Across both visits, 3445 locomotion scores were recorded, alongside 3850 trimming and lesion records. There were large differences between farms and visits in the proportion of lame goats (6.7 to 25.5%) and in the proportion of goats which needed a substantial trim of all four feet (15.1-33.1%). Hoof lesions were observed in 65.5% of trimmed goats. We defined lesions by their site and presence of separation and/or granuloma, with white line separation being the most commonly recorded lesion (41.5% of goats). On all visits on the three farms which were locomotion scored, the number of goats recorded as lame was lower than the number observed with white line lesions, but the relative proportion ranged from 17% to 95.4%. Separations were much more common than granulomas, as granulomas were usually accompanied by separation. Toe lesions were much more likely to be associated with granulomas than white line lesions, with 91.5% of toe separations having granulomas but only 10.6% of white line separations. This study provides useful baseline data for comparing lesions and locomotion scoring between goat herds but also identifies areas where we need more data and more consensus.

Urolithiasis is the most common cause of urinary tract disease in small ruminants and has significant economic and production impacts worldwide. Urolithiasis is multifactorial in origin and generally begins with the formation of cystoliths followed by urethral obstruction. The condition is most common in males. Clinical signs are variable depending on the severity of the obstruction. Uroliths can be calcium, struvite, or silicate based; however, struvite and amorphous magnesium calcium phosphate are the most common urolith types observed in small ruminants. Although urethral process (vermiform appendage) amputation is widely considered the first line of treatment, reobstruction is common within the first 36 hours. Surgical interventions such as temporary tube cystostomy, perineal urethrostomy (PU), modified proximal perineal urethrostomy, vesico-preputial anastomosis (VPA), and urinary bladder marsupialization (BM) are reported to carry an improved prognosis for long-term survival. PU carries a lower proportion of long-term success (> 12-month survival time) when compared with VPA and BM. Stoma stricture and urine scald are the most commonly observed surgical complications. Currently, the literature provides minimal direction for clinician decision-making in managing these cases while accounting for patient history, client financial ability, composition of calculi, and potential treatment complications. Small ruminant urinary obstructions are challenging and complicated conditions to treat, due to their multifactorial etiology, ruminant urogenital anatomy, and the variety of imperfect treatment options available. The purpose of this article this article is to provide veterinary practitioners with decision trees to guide management and treatment of urolithiasis in small ruminants.

Reduced welfare and productivity of dairy goats have often been associated with poor claw health, especially conditions such as claw overgrowth and deformations. It is known that periodic claw trimmings have prophylactic and therapeutic effects on these problems, and this study aimed to evaluate if the additional use of an angle grinder to finish trimming overgrown and deformed goat claws, after the usual trimming using hoof shears, could provide further changes in these claws. For this, twelve Saanen goats (57.29 ± 11.15 kg of body weight, 3.08 ± 1.78 years old) were selected by presence of severe claw overgrowth, and absence of claw alterations of other nature. Their claws were trimmed in two steps, first using hoof shears and then using an angle grinder. Morphometric, baropodometric, and conformational aspects of all claws were assessed before claw trimming and after each trimming step. To analyse the effects of the trimming steps in each claw, the Tukey's test was used on parametric data, with 5% probability, and descriptive statistics were used on non-parametric data. Although this is a small pilot study, results suggest that using an angle grinder after the use of hoof shears, could further reduce heel length and sole width of claws, as well as reduce the number of deformed claws. The incorporation of the second trimming tool, could also further increase the frequency with which the point of maximum pressure was found in the toes, rather then in the heels of the claws as seen in deformed claws.

BACKGROUND: Urolithiasis in small ruminants has a poor long-term prognosis, and long-term clinical outcomes are variable and unpredictable. OBJECTIVES: To assess the accuracy of preoperative and postoperative blood l-lactate concentrations in predicting a negative outcome in goats undergoing tube cystostomy. ANIMALS: Thirty-four male goats undergoing tube cystostomy. METHODS: Retrospective study. Medical records of goats undergoing tube cystostomy from 2015 to 2020 were reviewed. Clinical variables recorded included signalment, procedures before surgery, urolith location and type, duration of hospitalization, and heart rate. PCV, plasma total protein, potassium, preoperative and postoperative blood l-lactate concentrations, preoperative and postoperative creatinine concentrations, and relative changes in blood l-lactate and creatinine concentrations over time were measured using heparinized blood. A negative outcome was defined as death or euthanasia from urolithiasis complications at 6 months after discharge. Negative outcomes as a function of independent clinical variables were evaluated using χ2 or Fisher's exact tests, and multivariate logistic regression. P < .05 was considered significant. RESULTS: Median (95% confidence interval) preoperative, postoperative, and the relative change over time of blood l-lactate concentrations were 3.3 mmol/L (2.2, 4.8), 1.0 mmol/L (0.7, 1.3), and 0.4 mmol/L (-3.5, 3.2), respectively. Preoperative (P = 1), postoperative (P = .14), and the relative change over time (P = .63) of blood l-lactate concentrations were not significant predictors of a negative outcome. Furthermore, all other clinical variables measured were not significant predictors of a negative outcome (P > .05). CONCLUSIONS AND CLINICAL IMPORTANCE: Veterinarians should advise clients that clinical outcomes after tube cystostomy in goats are likely unpredictable.

A 10-year-old Boer goat wether presented for unilateral exophthalmos of 2- to 3-week duration. Ocular ultrasonography and computed tomography (CT) were utilized in the diagnosis of the patient's orbital disease and surgical planning. Exenteration was performed under the same general anesthetic event as CT. Cytology, culture, and histopathology were performed after exenteration. Cytology was consistent with a mixed bacterial infection. Culture confirmed the presence of Streptococcus ovis. Histopathology on the enucleated globe and mass revealed no evidence of tumor and confirmed intraocular extension of retrobulbar inflammation. Histopathologic diagnosis was consistent with severe chronic orbital pyogranuloma and fibrinosuppurative endophthalmitis confined to the subretinal space. The abscess recurred in the orbital space 2 weeks postoperatively; the orbit was explored. Repeat culture was consistent with S. ovis, Staphylococcus schleigeri subspecies coagulans, and Fusobacterium necrophorum. Complete resolution was obtained after drainage and lavage of the orbit. Abscess is cited as a cause of exophthalmos in small ruminants, but no individual case reports exist. Advanced imaging allowed presumptive diagnosis and surgical planning. Histopathology confirmed intraocular extension of retrobulbar disease.

OBJECTIVE: To describe a caudal paramedian approach to cryptorchidectomy in small ruminants. STUDY DESIGN: Retrospective case series. ANIMALS: Sheep (n = 20) and goats (n = 9) with cryptorchidism. METHODS: Medical records from January 2011 to July 2019 of small ruminants that underwent caudal paramedian cryptorchidectomy at our institution were retrospectively reviewed. Data from animal signalment, operative and postoperative complications, and client satisfaction were evaluated. RESULTS: Twenty sheep and nine goats underwent caudal paramedian cryptorchidectomy during the study period. The median age of rams was 2 months, and the median age of bucks was 3 months; median weights for rams and bucks were 20.5 kg (range, 14.5-41.3) and 28.1 kg (range, 12.9-82), respectively. Cryptorchidism was bilateral in 27.6% (8/29) of cases and unilateral in 72.4% (21/29). Among unilateral cases, 85.7% (18/21) were right sided and 14.3% (3/21) were left sided. Twenty-four of 29 (82.8%) cases were performed under sedation. One operative complication occurred in a 4-year-old 82 kg buck (rate, 3.4% [95% CI: 0.1%-17.2%]). Postoperatively, there were two minor and one major complications (rates, 6.9% [95% CI: 0.8%-22.1%] and 3.4% [95% CI: 0.1%-17.2%], respectively). Long-term follow-up (range, 8-117 months) reports described owner satisfaction and all animals doing well at the time of follow-up telephone call. CONCLUSION: Caudal paramedian approach to cryptorchidectomy was safely performed in small ruminants less than 4 months old. CLINICAL SIGNIFICANCE: Ease of surgical technique, minimal operative and postoperative complications, and owner satisfaction make this a suitable method for cryptorchidectomy.

OBJECTIVE: To describe the treatment of goats with urethral obstruction secondary to urolithiasis by combining tube cystostomy and urethrotomy. ANIMALS: Eight male goats. STUDY DESIGN: Short case series. METHODS: Medical records (September 2012-September 2017) of male goats treated for obstruction secondary to urolithiasis with tube cystostomy and urethrotomy were reviewed. Data collected included signalment, history, physical examination findings, diagnostic results, perioperative treatments, operative details, hospitalization duration, intraoperative and postoperative complications, urolith analysis, and time to restoration of urethral patency. Long-term follow-up (>12 months) was obtained by email or telephone interviews of owners or by clinical examination. RESULTS: Seven of eight goats were castrated males of various of breeds. All goats were tachycardic with urethral pulsation at admission. Uroliths were composed of calcium carbonate in four goats and silica in one goat. All goats regained urethral patency during hospitalization, and all were discharged alive from the hospital. Seven goats were alive at long-term follow-up. Postoperative complications included persistent urethral obstruction requiring a second urethrotomy 2 days postoperatively, premature dislodgement of the bladder catheter and jejunal obstruction secondary to adhesions, and recurrence of obstructive urolithiasis within the proximal perineal urethra requiring a second surgery 8 months later (1 each). Long-term outcome was good, with urethral patency beyond 12 months in six of eight goats. CONCLUSION: Combining tube cystostomy and urethrotomy restored urethral patency in goats with urethral obstruction from uroliths. Although none of the complications seemed directly related to the urethrotomy, formation of abdominal adhesions and recurrence of urolithiasis affected long-term outcomes. CLINICAL SIGNIFICANCE: Uroliths that do not dissolve in acidic urine may be more frequent in some practices. The combined approach of tube cystostomy and urethrotomy appears to successfully restore urethral patency with promising long-term outcomes.

OBJECTIVE: To report the clinical findings, outcome, and complications associated with urethrotomy performed in combination with or after temporary tube cystostomy for treatment of obstructive urolithiasis in male goats. STUDY DESIGN: Retrospective case series. SAMPLE POPULATION: Castrated male goats (n = 15). METHODS: The medical records of male goats admitted for obstructive urolithiasis were reviewed. Goats that underwent aurethrotomy performed in combination with or after a temporary tube cystostomy were included in the study population. Follow-up information was obtained through hospital records and telephone communication. RESULTS: Thirteen of 15 goats were discharged from the hospital a median of 13 days (range, 7-25) after urethrotomy. Complications related to the urinary system occurred in 5 goats and included recurrent obstruction, urination from open urethrotomy, uroabdomen and intra-abdominal adhesions after removal of the Foley catheter, and incisional infection. Five of 7 goats with long-term follow-up (2-24 months after surgery) were healthy, with normal urinary function. The remaining 2 goats returned to the hospital with signs of reobstruction that were attributed to uroliths at necropsy in 1 goat. CONCLUSION: Goats treated with urethrotomy for persistent urethral obstruction after temporary tube cystostomy generally survived the procedure. Normal long-term urinary function was obtained in some goats, although recurrence was the most common complication. CLINICAL SIGNIFICANCE: Urethrotomy is a surgical option for male goats with persistent urethral obstruction due to urethral calculi after temporary tube cystostomy.

BACKGROUND: Obstructive urolithiasis commonly affects male goats. Perineal urethrostomy (PU) can be a permanent treatment option but is generally considered undesirable because of the risk of stricture of the urethral stoma. Limited information exists regarding long-term outcome and complications in goats undergoing PU for treatment of obstructive urolithiasis. OBJECTIVES: To determine short-term and long-term outcome and complications in goats undergoing PU for treatment of obstructive urolithiasis. ANIMALS: Twenty-five client-owned goats. METHODS: Multi-institutional retrospective case series. RESULTS: Of the 25 goats, 13 (52%) were alive at the time of follow-up. Mean time from surgery to follow-up was 34 months (range, 4-65). Nine goats (36%) died between discharge and follow-up with a mean survival time of 46 days (range, 5-120). Cause of death in 7 of 9 (78%) goats was related to urolithiasis. Goats treated by use of a modified proximal perineal urethrostomy (MPPU) were significantly more likely to survive at least 150 days postoperatively (P < .01). The most common postoperative complications were hemorrhage (10/25 [40%]) and surgical site infection (3/25 [12%]). Hemorrhage was significantly associated with MPPU (P < .0001). Stricture of the surgical stoma occurred in 7 of 22 (32%) discharged goats. Mean time to stricture was 65 days (range, 10-240). CONCLUSIONS AND CLINICAL IMPORTANCE: Perineal urethrostomy can provide effective long-term resolution of obstructive urolithiasis in goats. Re-obstruction or stricture seems most likely within the 1st 2 months after surgery. MPPU may provide better long-term results but should be approached cautiously because it can be associated with life-threatening hemorrhage.

Carcinosarcomas are biphasic malignant tumors composed of 2 distinct neoplastic cell populations, epithelial cells and mesenchymal cells. A 13-y-old, female, mixed-breed goat was presented with a 1-wk history of anuria and lethargy. Transabdominal ultrasonography showed an irregular and heterogeneous structure in the region of the bladder and uterus and changes in the echogenicity of both kidneys. Given the poor prognosis, euthanasia was elected. Autopsy revealed a large mass within the uterine cervix and confirmed the changes in the urinary tract. Histopathology and immunohistochemistry revealed a mixed, anti-cytokeratin AE1/AE3-positive epithelial, and vimentin-positive mesenchymal neoplasm consistent with a homologous carcinosarcoma, also called malignant mixed Müllerian tumor, with areas of double-labeling. We highlight the complexity of the diagnosis of uterine neoplasms in domestic animals and in goats in particular.

OBJECTIVE To characterize indications for and clinical outcomes of limb amputation in goats and sheep. DESIGN Retrospective case series and observational study. ANIMALS Goats (n = 15) and sheep (7) that underwent partial or complete limb amputation at the University of California-Davis Veterinary Medical Teaching Hospital from January 1, 1985, through December 31, 2015. PROCEDURES Medical records of qualifying goats and sheep were reviewed and data extracted regarding signalment, use of animal, characteristics of the amputated limb, duration of hospitalization, outcome, and surgery-associated complications. The Fisher exact test was performed to compare postsurgical complication rates between various groups. RESULTS Reasons for limb amputation included trauma (ie, fracture, dog bite, and wire-fence injury) and infectious or degenerative disease (ie, osteomyelitis and osteoarthritis). Median (range) hospitalization period for goats and sheep was 7.5 days (1 to 63 days) and 8 days (3 to 20 days), respectively. Most (7/9) of the postsurgical complications were noticed > 2 months after amputation and included uncoordinated gait, tendon breakdown and laxity of the contralateral limb, chronic lameness, surgical site infection, chronic intermittent pain, and angular limb deformity. No association was identified between postsurgical complication rate in goats and body weight category (< 20 vs ≥ 20 kg [44 lb]), age category (< 2 vs ≥ 2 years), and amputated limb type (forelimb vs hind limb). CONCLUSIONS AND CLINICAL RELEVANCE Clinical outcomes for goats and sheep following limb amputation varied and were seemingly case dependent. Amputation should be considered when preservation of the whole limb is not possible. Veterinarians should advise owners that limb amputation may result in various complications and recommend aftercare treatment such as physiotherapy.

OBJECTIVE To determine the anatomic location and clinical signs of thymoma in goats and long-term outcomes in a subset of goats treated by tumor excision. DESIGN Retrospective case series. ANIMALS 13 goats with a histologic diagnosis of thymoma at the Cornell University Hospital for Animals between 1990 and 2014. PROCEDURES Medical records of goats with thymoma were reviewed and data were evaluated regarding signalment, clinical signs, diagnostic imaging results, thymoma size, treatment, and outcome. Follow-up information was obtained via contact with the owners and review of medical records. RESULTS 8 goats had a mediastinal mass, 4 had a palpable ventral cervical mass, and 1 had both types of masses. Median age at the time of diagnosis was 9.5 years (range, 3 to 12 years). Goats with a mediastinal mass had respiratory distress or marked tachypnea. Six goats were treated surgically, including all 5 with a ventral cervical mass. All 5 goats with a ventral cervical mass survived with no tumor recurrence for ≥ 1 year after excision. Only 2 goats with a mediastinal mass survived to hospital discharge. CONCLUSIONS AND CLINICAL RELEVANCE Results suggested that the prognosis for goats following excision of ventral cervical thymomas was favorable, whereas goats with mediastinal thymomas appeared more likely to have severe clinical signs and a guarded prognosis.

BACKGROUND: Cerebral coenurosis is a disease of the central nervous system in sheep and goats, and is usually fatal unless surgical relief is provided. Information regarding neuroanaesthesia in veterinary medicine in goats is scant. CASE PRESENTATION: We describe anaesthetic management of an intact female goat (2 years; 16 kg) presented for craniectomy. The goat was sedated with xylazine (0.05 mg kg-1, i.m.) and morphine (0.05 mg kg-1, i.m.). General anaesthesia was induced 20 min later with propofol and maintained with a constant rate infusion of propofol (0.2 mg kg-1 min-1). A cuffed endotracheal tube was placed and connected to a rebreathing (circle) system and mechanical ventilation with 100% oxygen was initiated. A bolus of lidocaine (1 mg kg-1), midazolam (0.25 mg kg-1) and fentanyl 2.5 µg kg-1 was delivered via the intravenous route followed immediately by a constant rate infusion of lidocaine (50 µg kg-1 min-1), midazolam (0.15 mg kg-1 h-1) and fentanyl (6 µg kg-1 h-1) administered via the intravenous route throughout surgery. Craniectomy was undertaken and the goat recovered uneventfully. CONCLUSION: Total intravenous anaesthesia with propofol, lidocaine, fentanyl and midazolam could be an acceptable option for anaesthesia during intracranial surgery in goats.

Tube cystostomy is a surgical method used for managing obstructive urolithiasis and involves placement of a Foley catheter into the urinary bladder. We identified and evaluated the antibiotic resistance patterns of bacteria isolated from indwelling Foley catheters following tube cystostomy in goats with obstructive urolithiasis. Urine samples collected over a 10-y period from catheter tips at the time of removal were submitted for bacteriologic culture and antibiotic susceptibility testing. Resistance patterns to antibiotics, trends in the resistance patterns over the study period, and the probability of a bacterial isolate being resistant as a function of the identity of the isolate and antibiotic tested were determined. A total of 103 urine samples from 103 male goats with obstructive urolithiasis managed surgically with tube cystostomy were included in the study. Aerococcus (36.9%) and Enterococcus (30.1%) were isolated most frequently. The susceptibility patterns of all bacteria isolated did not change over the study period ( p > 0.05). Proportions of isolates resistant to 1, 2, and ≥3 antibiotics were 36.9%, 18.5%, and 23.3%, respectively. Thus, 41.8% of bacterial isolates were resistant to 2 or more antibiotics tested. The probability of Aerococcus spp., Escherichia coli, and Pseudomonas aeruginosa isolates to be resistant to ampicillin, ceftiofur, erythromycin, penicillin, or tetracycline ranged from 0.59 to 0.76.

OBJECTIVE: To describe a novel surgical technique for the correction of recurrent obstructive urolithiasis in male goats. STUDY DESIGN: Clinical case series. ANIMALS: Castrated male goats (n = 4). METHODS: Medical records of male goats having undergone vesicopreputial anastomosis (VPA) as a treatment for obstructive urolithiasis were reviewed for history, signalment, clinical signs, and intraoperative and postoperative complications. Long-term follow-up (≥12 months) was obtained by telephone interview with owners or by clinical examination. RESULTS: All goats had undergone at least one surgical procedure (median, 2.5, range 2-4) to correct obstructive urolithiasis before undergoing VPA. Postoperative complications included premature removal of the tube from the bladder (1 goat), bacterial cystitis (2), and abscess formation (1). One goat suffered stricture of the anastomosis site 3 months following the original procedure and underwent a second VPA and 1 goat died 7 months after surgery due to severe, acute hydronephrosis and renal failure. Long-term survival ≥12 months was good with 3/4 goats (75%) or 3/5 VPA procedures (60%) having unobstructed urine flow at 12 months. CONCLUSION: Vesicopreputial anastomosis is a feasible surgical procedure for the correction of recurrent obstructive urolithiasis in male goats and one that can result in a favorable clinical outcome. Further investigation in a larger population of goats is warranted for the evaluation of the suitability of VPA in male goats with obstructive urolithiasis.

This survey describes the evolution in vivo of Coenurus cerebralis in small ruminants. At presentation, neurological signs and cerebrospinal fluid (CSF) features were suggestive of multifocal or diffuse inflammatory reaction. Magnetic resonance imaging (MRI) captured the transition between the invasive and quiescent phase of the infection, revealing the concurrent presence of meningitis and small cysts. During the quiescent phase, in all animals, neurological symptoms disappeared, and cerebrospinal fluid was unremarkable while cysts grew progressively. Subsequently, the onset of neurological symptoms coincided with MRI signs of diffuse or localized increase of intracranial pressure, as confirmed by direct intracranial pressure measuring. All the animals had an excellent post-surgical recovery. This is the first report describing the evolution of coenurosis in vivo. Sequential imaging allowed describing interesting such as the death of some coenuri and different parasite growth rate in the same host.

Nine goats were treated for squamous cell carcinoma (SCC) of the perineal and/or tail region. This case series is the first detailed description of clinical presentation and treatment of caprine SCC in North America and characterizes the potential risk factors and outcomes.
