RESUMO

Increasing seawater temperatures coupled with more intense and frequent heatwaves pose an increasing threat to marine species. In this study, the New Zealand green-lipped mussel, Perna canaliculus, was used to investigate the effect of genetics and ontogeny on thermal resilience. The culturally and economically significant mussel P. canaliculus (Gmelin, 1971) has been selectively-bred in New Zealand for two decades, making it a unique biological resource to investigate genetic interactions in a temperate bivalve species. Six selectively-bred full sibling families and four different ages, from early juveniles (6, 8, 10 weeks post-fertilisation) to sub-adults (52 weeks post-fertilisation), were used for experimentation. At each age, each family was exposed to a three-hour heat challenge, followed by recovery, and survival assessments. The shell lengths of live and dead juvenile mussels were also measured. Gill tissue samples from sub-adults were collected after the thermal challenge to quantify the 70 kDa heat shock protein gene (hsp70). Results showed that genetics, ontogeny and size influence thermal resilience in P. canaliculus, with LT50 values ranging between 31.3 and 34.4 °C for all studied families and ages. Juveniles showed greater thermotolerance compared to sub-adults, while the largest individuals within each family/age class tended to be more heat sensitive than their siblings. Sub-adults differentially upregulated hsp70 in a pattern that correlated with net family survival following heat challenge, reinforcing the perceived role of inducible HSP70 protein in molluscs. This study provides insights into the complex interactions of age and genotype in determining heat tolerance of a key mussel species. As marine temperatures increase, equally complex selection pressure responses may therefore occur. Future research should focus on transcriptomic and genomic approaches for key species such as P. canaliculus to further understand and predict the effect of genetic variation and ontogeny on their survival in the context of climate change.

The concentrations of macro elements (Ca, K, Mg, and Na), essential trace elements (Cr, Cu, Fe, Li, Mn, Ni, and Zn), and nonessential trace elements (Al, As, Cd, Pb, and Ti) in the shell and soft tissues of Perna perna (L. 1758) mussels from Southeast Brazil are presented as a baseline reference for understanding the effects of the COVID-19 pandemic on the quality of coastal environments. For shells, the macro elements load was greater during the pandemic period at all sampling sites; however, for soft tissues, the opposite trend was recorded. On the contrary, the concentrations of trace elements in the shell were below the limit of quantification in most samples, and they tended to decrease in the soft tissues during the pandemic. Thus, the COVID-19 was a short-term conservation event that positively impacted the mussels. The results are relevant for monitoring the coastal environment in a post-COVID-19 scenario.

Harmful algal blooms (HABs) of Alexandrium pacificum have affected the Marlborough Sounds in New Zealand since 2010, posing a threat to green-lipped mussel (GLM, Perna canaliculus) farming. Previous studies have shown A. pacificum has negative effects GLM embryos and larvae. To further investigate these toxic mechanisms, in vitro bioassays were conducted on GLM spermatozoa, hemocytes, and the diatom, Chaetoceros muelleri. The three cell types were exposed to several treatments of A. pacificum for 2 h and responses were measured using flow cytometry and pulse amplitude-modulated fluorometry. Significant spermatozoa mortality was recorded in treatments containing A. pacificum cells or fragments, while hemocyte and C. muelleri mortality was recorded in cell-free treatments of A. pacificum which contained paralytic shellfish toxins (PSTs). Variation in sensitivity between cell types as well as the sublethal effects observed, emphasise the diverse toxic mechanisms of A. pacificum on co-occurring species in the environment.

Pollutants often exist as mixtures in environmental settings, creating a challenge in selecting the most effective combination of biomarkers for routine monitoring. This study was conducted seasonally in Victoria Harbour, Hong Kong, to compare the responses of nine biomarkers in the green-lipped mussel Perna viridis with respect to its tissue levels of persistent organic pollutants and heavy metals. Multivariate statistical techniques were utilised to determine the single best predictor and optimal subset of biomarkers in P. viridis for each of the four scenarios: representing overall biomarker responses in the dry season, and wet season, as well as correlating tissue levels of mixed pollutants in the dry season, and wet season. Our findings recommend lysosomal destabilisation, and the nucleic acid ratio of RNA to DNA, as the core biomarkers in P. viridis for marine pollution monitoring. The non-specificity of these biomarkers allows effective identification of pollution hotspots and guides further detailed assessment.

Microplastics pollution threatens to marine organisms, particularly bivalves that actively ingest and accumulate microplastics of certain sizes, potentially disrupting intestinal homeostasis. This study investigated the microplastic abundance in wild and farmed mussels around Singapore, and examined the size-dependent effects of nano- to micro-scale polystyrene (0.5 µm/5 µm/50 µm) on the mussel intestinal microbiome in the laboratory. The field investigation revealed higher microplastic abundance in farmed mussels compared to wild ones. Experimentally, mussels exposed to 0.6 mg/L of microplastics for 7 days, followed by a 7-day depuration period, showed substantial impacts on Spirochaetes and Proteobacteria, facilitating the proliferation of pathogenic species and differentially affecting their pathogenic contributions. Metagenomics analysis revealed that microplastic exposure reduced Spirochaeta's contribution to virulence and pathogenicity loss, did not affect Vibrio and Oceanispirochaeta's pathogenicity, and increased Treponema and Oceanispirochaeta's contributions to pathogenicity loss. Moreover, microplastics increased transmembrane transporters and impacted oxidative phosphorylation enzymes, impairing energy metabolism. These effects persisted after depuration, indicating lack of resilience in the microbiome. Nano- and micro-scale plastics perturbed the mussel microbiome composition and functions in a size-dependent manner, with nano-plastics being the most disruptive. The increasing use and sale of aquaculture equipment of plastic may exacerbate the intestinal dysbiosis in bivalves, which threatens consumers' health.

The presence and persistence of microplastics (MPs) in diverse aquatic environments are of global concern. Microplastics can impact marine organisms via direct physical interaction and the release of potentially harmful chemical additives incorporated into the plastic. These chemicals are physically bound to the plastic matrix and can leach out. The hazards associated with chemical additives to exposed organisms is not well characterized. We investigated the hazards of plastic additives leaching from plastic. We used the common plasticizer dibutyl phthalate (DBP) as a chemical additive proxy and the New Zealand green-lipped mussel (Perna canaliculus) as a model. We used early-adult P. canaliculus exposed to combinations of virgin and DBP-spiked polyvinyl chloride (PVC), MPs, and DBP alone for 7 days. Whole transcriptome sequencing (RNA-seq) was conducted to assess whether leaching of DBP from MPs poses a hazard. The differences between groups were evaluated using pairwise permutational multivariate analysis of variance (PERMANOVA), and all treatments were significantly different from controls. In addition, a significant difference was seen between DBP and PVC MP treatment. Transcriptome analysis revealed that mussels exposed to DBP alone had the most differentially expressed genes (914), followed by PVC MP + DBP (448), and PVC MP (250). Gene ontology functional analysis revealed that the most enriched pathway types were in cellular metabolism, immune response, and endocrine disruption. Microplastic treatments enriched numerous pathways related to cellular metabolism and immune response. The combined exposure of PVC MP + DBP appears to cause combined effects, suggesting that DBP is bioavailable to the exposed mussels in the PVC MP + DBP treatment. Heavy infestation by Perna viridis has been observed in the sub-seabed seawater intake tunnel and CWS of a tropical coastal power station in-spite of continuous low dose chlorination regime (0.2 ± 0.1 mg L-1) (CLDC), indicating periodical settlement and growth. Continuous arrival of mussels (colonized in the sub seabed tunnel intake section) at the pump house indicated that the mussels were able to tolerate and survive in a chlorinated environment, for varying time periods and were dislodged when they become weak and subsequent death, leading to flushing out of the system. In the present study, effect of continuous chlorination [0.2 mg L-1 (in-plant use); 0.5 mg L-1 (shock dose) & 1.0 mg L-1 (high levels)] was evaluated on mussels to assess; (a) time taken for mortality, (b) action of chlorine on physiological, genetic, metabolic and neuronal processes. 100% mortality of mussels was observed after 15 (0.2 mg L-1); 9 (0.5 mg L-1) and 6 days (1.0 mg L-1) respectively. Extended valve closure due to chlorination resulted in stress, impairing the respiratory and feeding behavior leading to deterioration in mussel health. Pseudofaeces excretion reduced to 68% (0.2 mg L-1); 10% (0.5 mg L-1) and 89% (1.0 mg L-1) compared to controls. Genotoxicity was observed with increase in % tail DNA fraction in all treatments such as 86% (0.2 mg L-1); 76% (0.5 mg L-1) and 85% (1.0 mg L-1). Reactive Oxygen Species (ROS) stress biomarkers increased drastically/peaked within the first 3 days of continuous chlorination with subsequent quenching by antioxidant enzymes. Gill produced highest generation of ROS; 38% (0.2 mg L-1); 97% (0.5 mg L-1); 98% (1.0 mg L-1). Additionally, it was shown that 84% (0.2 mg L-1), 72% (0.5 mg L-1), and 80.4% (1.0 mg L-1) of the neurotransmitter acetylcholinesterase activity was inhibited by chlorine at the nerve synapse. The cumulative impact of ROS generation, neuronal toxicity, and disrupted functions weakens the overall health of green mussels resulting in mortality.

The New Zealand Greenshell™ mussel (Perna canaliculus) is an economically important aquaculture species. Prolonged increases in seawater temperature above mussel thermotolerance ranges pose a significant threat to mussel survival and health, potentially increasing susceptibility to bacterial infections. Using challenge experiments, this study examined the combined effects of increased seawater temperature and bacterial (Photobacterium swingsii) infection on animal survival, haemocyte and biochemical responses of adult mussels. Mussels maintained at three temperatures (16, 20 and 24 °C) for seven days were either not injected (control), injected with sterile marine broth (injection control) or P. swingsii (challenged with medium and high doses) and monitored daily for five days. Haemolymph and tissue samples were collected at 24, 48, 72, 96, 120 h post-challenge and analysed to quantify bacterial colonies, haemocyte responses and biochemical responses. Mussels infected with P. swingsii exhibited mortalities at 20 and 24 °C, likely due to a compromised immune system, but no mortalities were observed when temperature was the only stressor. Bacterial colony counts in haemolymph decreased over time, suggesting bacterial clearance followed by the activation of immune signalling pathways. Total haemocyte counts and viability data supports haemocyte defence functions being stimulated in the presence of high pathogen loads at 24 °C. In the gill tissue, oxidative stress responses, measured as total antioxidant capacity and malondialdehyde (MDA) levels, were higher in infected mussels (compared to the controls) after 24h and 120h post-challenge at the lowest (16 °C) and highest temperatures (24 °C), indicating the presence of oxidative stress due to temperature and pathogen stressors. Overall, this work confirms that Photobacterium swingsii is pathogenic to P. canaliculus and indicates that mussels may be more vulnerable to bacterial pathogens under conditions of elevated temperature, such as those predicted under future climate change scenarios.

Plastics ranging from nano-scale to micron-scale are frequently ingested by many marine animals. These particles exhibit biotoxicity and additionally perform as vectors that convey and amass adsorbed chemicals within organisms. Meanwhile, the frequency of detection of the benzophenone-3 and ciprofloxacin can be adsorbed on plastic particles, then accumulated in bivalves, causing biotoxicity. To understand their unknown accumulative kinetics in vivo affected by different plastic sizes and toxic effect from co-exposure, several scenarios were set up in which the mode organism were exposed to 0.6 mg/L of polystyrene carrying benzophenone-3 and ciprofloxacin in three sizes (300 nm, 38 µm, and 0.6 mm). The live Asian green mussels were chosen as mode organism for exposure experiments, in which they were exposed to environments with plastics of different sizes laden with benzophenone-3 and ciprofloxacin, then depurated for 7 days. The bioaccumulation and depuration kinetics of benzophenone-3 and ciprofloxacin were measured using HPLC-MS/MS after one week of exposure and depuration. Meanwhile, their toxic effect were investigated by measuring the changes in six biomarkers (condition index, reactive oxygen species, catalase, glutathione, lipid peroxidation, cytochrome P450 and DNA damage). The bioconcentration factors in mussels under different exposure conditions were 41.48-111.75 for benzophenone-3 and 6.45 to 12.35 for ciprofloxacin. The results suggested that microplastics and nanoplastics can act as carriers to increase bioaccumulation and toxicity of adsorbates in mussels in a size-dependent manner. Overproduction of reactive oxygen species caused by microplastics and nanoplastics led to increased DNA damage, lipid peroxidation, and changes in antioxidant enzymes and non-enzymatic antioxidants during exposure. Marked disruption of antioxidant defenses and genotoxic effects in mussels during depuration indicated impaired recovery. Compared to micron-scale plastic with sizes over a hundred micrometers that had little effect on bivalve bioaccumulation and toxicity, nano-scale plastic greatly enhanced the biotoxicity effect.

The green-lipped mussel Perna viridis was utilised for pollution biomonitoring in Victoria Harbour and its adjacent aquaculture area in Hong Kong. P. viridis was collected from a reference site and redeployed at five study sites for five weeks during the dry and wet seasons of 2019. Our study found various polycyclic aromatic hydrocarbons (PAHs) and heavy metals in the mussel tissue, while polychlorinated biphenyls (PCBs) and organochlorine pesticides (OCPs) were not detected. P. viridis at the reference site generally displayed lower levels of pollutants. Comparing with previous research in the 1980s and 2000s, we observed substantial reduction in the tissue levels of PAHs, PCBs, OCPs and heavy metals in P. viridis. The human health risks associated with consuming these mussels were determined to be insignificant. Our findings imply that the Harbour Area Treatment Scheme has been effective in improving the water quality in Victoria Harbour and its adjacent aquaculture area.

Ocean acidification causes severe shell dissolution and threats the survival of marine molluscs. The periostracum in molluscs consists of macromolecules such as proteins and polysaccharides, and protects the inner shell layers from dissolution and microbial erosion. Moreover, it serves as the primary template for shell deposition. However, the chemical composition and formation mechanism of the periostracum is largely unknown. In this study, we applied transcriptomic, proteomics, physical, and chemical analysis to unravel the mysteries of the periostracum formation in the green mussel Perna viridis Linnaeus. FTIR analysis showed that the periostracum layer was an organic membrane mainly composed of polysaccharides, lipids, and proteins, similar to that of the shell matrix. Interestingly, the proteomic study identified components enriched in tyrosine and some enzymes that evolved in tyrosine oxidation, indicating that tyrosine oxidation might play an essential role in the periostracum formation. Moreover, comparative transcriptomics suggested that tyrosine-rich proteins were intensively synthesized in the periostracum groove. After being secreted, the periostracum proteins were gradually tanned by oxidation in the seawater, and the level of crosslink increased significantly as revealed by the ATR-FTIR. Our present study sheds light on the chemical composition and putative tanning mechanism of the periostracum layer in bivalve molluscs. SIGNIFICANCE: The periostracum layer, plays an essential role in the initiation of shell biomineralization, the protection of minerals from dissolution for molluscs and especially ocean acidification conditions in the changing global climate. However, the molecular mechanism underlying the periostracum formation is not fully understood. In this study, we revealed that the oxidation and cross-link of tyrosine-rich proteins by tyrosinase are involved in periostracum formation in the green mussel Perna viridis. This study provides some insights into the first step of mussel shell formation and the robust adaptation of P. viridis to diverse habitats. These findings also help to reveal the potential acclimation of bivalves to the projected acidifying seawater.

Greenshell™ mussels (Perna canaliculus) are endemic to New Zealand and support the largest aquaculture industry in the country. Photobacterium swingsii was isolated and identified from moribund P. canaliculus mussels following a mass mortality event. In this study, a challenge experiment was used to characterise, detect, and quantify P. swingsii in adult P. canaliculus following pathogen exposure via injection into the adductor muscle. A positive control (heat-killed P. swingsii injection) was included to account for the effects of injection and inactive bacterial exposure. Survival of control and infected mussels remained 100% during 72-hour monitoring period. Haemolymph was sampled for bacterial colony counts and haemocyte flow cytometry analyses; histology sections were obtained and processed for histopathological assessments; and adductor muscle, gill, digestive gland were sampled for quantitative polymerase chain reaction (PCR) analyses, all conducted at 12, 24, 48 h post-challenge (hpc). The most profound effects of bacterial injection on mussels were seen at 48 hpc, where mussel mortality, haemocyte counts and haemolymph bacterial colony forming were the highest. The quantification of P. swingsii via qPCR showed highest levels of bacterial DNA at 12 hpc in the adductor muscle, gill, and digestive gland. Histopathological observations suggested a non-specific inflammatory response in all mussels associated with a general stress response. This study highlights the physiological effects of P. swingsii infection in P. canaliculus mussels and provides histopathological insight into the tissue injury caused by the action of injection into the adductor muscle. The multi-technique methods used in this study can be applied for use in early surveillance programs of bacterial infection on mussel farms.

This study aimed to identify the presence of microplastics in green mussels (Perna viridis), surface seawater, and beach sediment on the North Coast of Vietnam. The average concentration of MPs in mussels was 3.67 ± 1.20 MPs/g wet weight and 25.05 ± 5.36 MPs/individual. Regarding surface seawater and beach sediments, the MPs concentration was found at 88.00 ± 30.88 MPs/L and 4800 ± 1776 MPs/kg dry weight, respectively. The dominant microplastics shape was fragment with the fractions ranging from 69.86 to 82.41 %. In addition, the size distribution of MPs was mostly in the range of smaller than 50 µm and 1-150 µm (34.17 % and 45.62 % in mussels; 29.65 % and 43.20 % in surface seawater and 40.22 % and 39.40 % in beach sediment, respectively). Polyethylene terephthalate was the major polymer types 49.93-58.44 % of the detected MPs. The risk assessment results based on the polymer types indicated a warning level in several sites.

Vibrio species are ubiquitous in aquatic environments, including coastal and marine habitats. Vibrio alginolyticus is an opportunistic pathogen for fish, crustaceans and mussels and their identification by biochemical tests may be impaired due their nutritional requirements. The study used Matrix-Assisted Laser Desorption/Ionization Time-of-Flight Mass Spectrometry (MALDI-TOF MS) to identify 49 Vibrio spp. isolates associated with mussels (Perna perna) from different locations along the Rio de Janeiro coast. The rpoA gene was used as a genus-specific marker of Vibrio spp. and was positive in all 209 isolates. MALDI-TOF MS confirmed 87.8% of V. alginolyticus when compared to the results of the biochemical tests. Four isolates were identified as Shewanella putrefaciens (8.16%) and one was identified as V. parahaemolyticus (2.0%). Just one isolate was not identified by this technique (2.0%). The pyrH sequencing confirmed 75% of the proteomic technique results. MALDI-TOF MS is an excellent option for characterization of bacterial species, as it is efficient, fast and easy to apply. In addition, our study confirms its high specificity and sensitivity in these marine bacteria identification.(AU)


Espécies de Vibrio são ubiquitárias em ambientes aquáticos, incluindo habitats costeiros e marinhos. A espécie Vibrio alginolyticus é oportunista para peixes, crustáceos e moluscos e a sua identificação através de testes bioquímicos pode ter a qualidade prejudicada devido às suas exigências nutricionais. O presente estudo utilizou Espectrometria de Massa por Tempo de Vôo de Ionização/Desorção por Laser Assistida por Matriz (MALDI-TOF MS) para identificar diferentes espécies de Vibrio provenientes de mexilhões (Perna perna) coletados em diferentes locais ao longo da costa do Rio de Janeiro. O gene rpoA foi utilizado como um marcador gênero-específico de Vibrio spp. sendo positivo em todos os 209 isolados. MALDI-TOF MS confirmou 87,75% de V. alginolyticus quando comparados com os resultados dos testes bioquímicos. Quatro isolados foram identificados como Shewanella putrefaciens (8,16%), um como V. parahaemolyticus (2,0%) e apenas um (2,0%) não foi identificado pela técnica proteômica. E o sequenciamento do pyrH confirmou 75% dos resultados da técnica proteomica. MALDI-TOF MS tem sido considerada uma excelente opção para a caracterização bacteriana, por ser eficiente, rápida, de fácil aplicação e este estudo confirmou a sua elevada especificidade e sensibilidade na identificação de bactérias marinhas.(AU)

The octopus Octopus cf. vulgaris is a potential species to diversify aquaculture. Due to absence of balanced commercial diet, growth of the O. cf. vulgaris is based on natural diet with local and low-cost inputs. In Brazil, studies on experimental octopus ongrowing are recent and there is little available data. We evaluated the performance, survival and food consumption of O. vulgaris fed on mussel Perna perna for 20 days. Six octopuses with initial weight of 415±12.73g (mean±standard deviation) were divided into two groups (n=3 octopuses/group) according to the diet: MC Group (frozen mussels) and MV Group (live mussels). The Weight Gain of octopuses was 273.33±94.52g and 340.00±26.46g; the Absolute Growth Rate was 13.67±4.73 and 17.00±1.32g.dia-1 and the Specific Growth Rate of 2.95±0.58 and 2.64±0.37%.day-1 to MC and MV groups, respectively. There was no significant difference in performance between groups and the survival rate was 100%. Octopuses well accepted both diets and despite the amount of frozen mussels (129±31) was higher than in live mussels (100±19), there was no significant difference regarding the consumption between groups. Our results demonstrate that the mussel Perna perna can be used frozen or live as monodiet in O. cf. vulgaris ongrowing.(AU)

A mitilicultura no Brasil se baseia no mexilhão Perna perna (Linnaeus, 1758), que é encontrado em todo o litoral brasileiro, sendo especialmente abundante do Espírito Santo a Santa Catarina. Nos últimos anos, foi lançada a hipótese de que P. perna seja uma espécie exótica no litoral brasileiro. A hipótese se baseia na análise da malacofauna presente em sítios arqueológicos. Todas as contestações levantadas não remetem à uma conclusão, pois precisam de estudos específicos, com metodologias claras, aliando arqueologia, ecologia e biologia molecular. O objetivo deste trabalho foi estudar a condição de nativo ou exótico do mexilhão Perna perna no Brasil, a partir de levantamento dos resultados em sítios arqueológicos, de técnicas moleculares e de datação de conchas com C14. A datação indicou que as amostras de Perna perna do sítio arqueológico Rio do Meio/Jurerê, Florianópolis/SC, têm idade de 720±30 e 780±30 anos. O cálculo do tempo de divergência indicou que a separação das populações brasileiras e africanas ocorreu por volta de 200 mil anos. Os resultados apontam a presença da espécie no território brasileiro muito antes do descobrimento do Brasil pelos portugueses no ano de 1500, indicando que P. perna é de fato uma espécie nativa.


Mussel farming in Brazil is based on brown mussel Perna perna (Linnaeus, 1758), which is found throughout the Brazilian coast, especially abundant from Espírito Santo to Santa Catarina coast. In recent years it was suggested that Perna perna is an exotic species for the Brazilian coast. The hypothesis is based on the analysis of the zooarchaeology studies in archaeological sites in Brazil. All objections raised do not offer a conclusion, because they need specific studies with clear methodology, combining archeology, ecology and molecular biology. The aim of this work was to study the condition of the brown mussel Perna perna in Brazil if native or exotic, from survey results in archaeological sites, molecular techniques and dating of shells with C14. The dating indicated that the shells were 720±30 and 780±30 years, respectively. The calculation of divergence time indicated that the separation of the African and Brazilian mussel populations occurred around 200 thousand years ago. The results indicate the presence of the P. perna species in Brazilian territory long before the discovery of Brazil by the Portuguese in 1500, indicating that P. perna is actually a native species of Brazil.

Introdução: As úlceras crônicas de perna (UCP) são um sério problema de saúde que atinge diversas faixas etárias, etnias, sexos, com reflexos nos gastos públicos e interferência na qualidade de vida das pessoas acometidas e de seus familiares. O presente estudo parte da concepção de que há necessidade de uma atenção integral que possa ser estabelecida por meio da utilização e construção de uma Linha de Cuidado. Objetivo: Elaborar coletivamente uma Linha de Cuidado (LC) do indivíduo com UCP em um município do Vale do Paraíba Paulista. Objetivos específicos: discutir os elementos constitutivos do fluxograma da LC e analisar suas dificuldades e potencialidades a partir deste. Metodologia: Pesquisa qualitativa, utilizando-se de 4 oficinas de trabalho realizadas nos meses de novembro de 2015 à março de 2016. Contou com a participação da Coordenação da Atenção Básica, enfermeiros e médicos de Unidades de Saúde da Família, enfermeira responsável pela assistência de indivíduos com úlceras crônicas de perna do ambulatório do município, e enfermeira responsável pela Atenção Domiciliar da Equipe Multiprofissional de Atenção Domiciliar (EMAD). Adotou-se a Integralidade como base conceitual da produção da LC e para a análise dos resultados, o fluxograma analisador. Resultado e Análise: Como resultado principal teve-se a produção coletiva de uma LC do indivíduo com UCP. Foram elementos essenciais na LC: recepção pró-ativa, acolhimento, ações de enfermagem desde a avaliação até os cuidados do indivíduo com UCP, presença de recursos materiais - em especial, na Atenção Primária à Saúde (APS), trabalho multidisciplinar no desenvolvimento do plano terapêutico e contrarreferência. Foram dificuldades e potencialidades: necessidade de programas de educação permanente, fortalecimento da APS para o cuidado, necessidade de articulação entre os serviços para garantir complementariedade e integralidade por meio de instrumentos pactuados, recursos materiais e efetivação do trabalho interdisciplinar. Conclusão: Evidenciou-se que há necessidade de se fortalecer a APS como centro de uma rede, ampliar as atribuições do enfermeiro acerca dos cuidados do indivíduo com UCP, fomentar espaços de diálogo entre os pontos da rede, ou seja, entre aqueles que estariam envolvidos na LC e, ainda, apoio e investimento dos gestores na produção do cuidado presente na LC. É necessário valorizar o potencial de mudança que a LC pode possuir quando existe comprometimento do profissional envolvido no processo do cuidar e a necessária garantia do itinerário terapêutico na rede de atenção em saúde à lógica dos serviços locais


Introduction: Chronic leg ulcers (CLU) is a serious health problem that affects different age groups, ethnicities, genders, reflected in public spending and interference in quality of life of the affected people and their relatives. This study comes from the conception that there is need of an integral attention, which can be established through the use and building of a Line of Care. Objective: To elaborate collectively a Line of Care (LC) of the individual with CLU in a city of the Metropolitan Region of Paulista Paraíba Valley. Specific objectives: To discuss the components of the LC flow chart and analyze the difficulties and potentialities from this. Method: Qualitative research, using 4 workshops held from November of 2015 to March of 2016. They had the participation of the Coordination of Primary Care, nurses and doctors from Family Health Units, the nurse responsible for the care of individuals with chronic leg ulcers of the municipal clinic, and the nurse responsible for the Attention Homecare of the Multiprofessional Team of Attention Homecare (MTAH). This study adopted the Integrality as a conceptual basis for the production of the LC and for the analysis of the results, the analyzer flow chart. Results and Discussion: As a main result it was the collective production of a LC of the individual with CLU. The essential elements in the LC were: proactive reception, user embracement, nursing actions from evaluation to the care of the individual with CLU, presence of material resources - especially in Primary Health Attention (PHA), multidisciplinary work in developing the therapeutic plan and counter reference. The difficulties and potentiality were: the need for continuing education programs, strengthening PHA for care, the need of articulation between the services for ensuring complementarity and integrality through agreed instruments, material resources and realization of the interdisciplinary work. Conclusion: It was evident that there is need to strengthen PHA as the center of a network, expand nurse assignments about the care of the individual with CLU, foster dialogue among network points, that is, among those who would be involved in the LC and also support and investment by managers in the production of the care in the LC. It is necessary to valorize the potential of change that the LC can have when there is impairment of the professional involved in the care process and the necessary guarantee of the therapeutic route in the network of health attention to the logic of the local services.

Introduction: Covering complex lower limb wounds is a major challenge. The medial gastrocnemius muscle flap is usually indicated for reconstruction of loss of substance of the knee and proximal third of the leg. The objective of this study was to evaluate the results using gastrocnemius muscle flaps in lower limb wounds. Methods: A total of 28 patients with loss of substance in the lower limbs were treated with medial gastrocnemius muscle flaps. Data were obtained from clinical records of patients who underwent surgery from 2003 to 2012. Results: In 28 patients who underwent surgery, the age range was 4 to 57 years, with a mean of 30.6 years. The etiology of the lesions was a motorcycle accident in 16 cases (57.1%), being run over in 8 cases (28.6%), car crash in 2 cases (7.2%), meningococcemia in 1 case (3.6%), and snake bite in 1 case (3.6%). After an average follow-up of 15 months (range: 6 to 26 months), all patients had stable coverage. The mean number of preoperative debridements was 2.4 (range: 1 to 8). In 22 patients, the flaps were made without the release of the muscle origin and in 6 patients the muscle flaps were separated from their origin in the femur. Conclusions: Transfer of a gastrocnemius muscle flap is a simple and safe procedure in the treatment of lower limb injuries. The application of the medial gastrocnemius muscle flap was demonstrated in different situations, with satisfactory results and easy reproducibility.


Introdução: A cobertura de feridas complexas de membros inferiores é um grande desafio. O retalho muscular do gastrocnêmio medial é comumente indicado para reconstrução de perdas de substância do joelho e terço proximal da perna. O objetivo desse trabalho é avaliar os resultados da utilização do retalho muscular do gastrocnêmio nos casos de ferida em membros inferiores. Métodos: Um total de 28 pacientes com perda de substância de membros inferiores foi tratado com retalho muscular do gastrocnêmio medial. Os dados foram obtidos em registros clínicos dos pacientes operados no período de 2003 a 2012. Resultados: Vinte e oito pacientes foram operados, com idades variando de 4 a 57 anos, e média de 30,6 anos. A etiologia das lesões foi acidente por motocicleta em 16 casos (57,1%), atropelamentos em oito casos (28,6%), acidente automobilístico em dois casos (7,2%), meningococcemia em um caso (3,6%) e acidente ofídico em um caso (3,6%). Depois de um seguimento médio de 15 meses (variação de 6 a 26 meses), todos pacientes mostraram cobertura estável. O número médio de desbridamentos pré-operatórios foi 2,4 (variação de 1 a 8). Em 22 pacientes, os retalhos foram confeccionados sem a liberação da origem muscular e em seis pacientes os retalhos musculares foram separados de sua origem no fêmur. Conclusões: A transferência do retalho muscular do gastrocnêmio é um procedimento simples e seguro no tratamento de lesões de membros inferiores. Foi demonstrada a aplicação do retalho muscular do gastrocnêmio medial em diferentes situações, com resultados satisfatórios e fácil reprodução.

Introdução: Úlceras crônicas em membros inferiores podem apresentar diferentes etiologias, sendo as mais frequentes: venosa, arterial, traumática, infecciosa e diabética. O tratamento dessas feridas é dinâmico e depende da evolução da reparação tecidual. Esse tratamento inclui métodos clínicos e cirúrgicos, sendo o curativo o método não cirúrgico mais frequentemente utilizado. Curativos podem ser desde de coberturas inertes até veículos para atuação de princípios ativos no leito da ferida. A principal indicação de ativos está relacionada a efeitos de desbridamento e controle da população bacteriana, possibilitando o preparo desses leitos para resolução cirúrgica ou espontânea. Método: Esse estudo é observacional, longitudinal, retrospectivo, de amostra randomizada no qual pretendemos analisar o atendimento prestado aos portadores de úlceras crônicas em membros inferiores no Ambulatório de Feridas Crônicas da Divisão de Cirurgia Plástica do HCFMUSP entre 2011 e 2013. Resultados: Foram analisados prontuários de indivíduos de ambos os gêneros, idade média 60 anos, portadores de úlceras crônicas em diferentes estágios evolutivos. Foram pesquisadas comorbidades, doença de base, tamanho da lesão, tratamentos utilizados e evolução das feridas. Notouse predomínio das causas vasculares (69,2%) como doenças de base. Todos os pacientes foram primeiramente tratados com curativos contendo princípios ativos, para preparo do leito das feridas. Desses, 84% foram encaminhados para resolução cirúrgica das feridas. Houve fechamento espontâneo em 1,5% dos casos. Os demais pacientes (14,5%) apresentaram piora das lesões com tratamento tópico, necessitando outras formas de preparo desse leito. Conclusão: Agentes tópicos podem ser uma forma ambulatorial/domiciliar efetiva de preparo do leito de úlceras crônicas para resolução cirúrgica.


Introduction: Chronic ulcers of the lower limbs may have different etiologies, with the most frequent being venous, arterial, traumatic, infectious, and diabetic. The treatment of these wounds is dynamic and depends on the evolution of tissue repair. This treatment includes clinical and surgical methods, and dressings are the most frequently used. Dressings can range from inert covers to vehicles for actuation of active substances in the wound bed. The main indication for these substances is related to the effects of debridement and control of the bacterial population, enabling the preparation of wound beds for surgical or spontaneous resolution. Method: This study is an observational, cross-sectional, retrospective study, with random sampling, aimed at assessing the care provided to patients with chronic ulcers of the lower limbs in the Outpatient Clinic for chronic wounds of the Plastic Surgery Division, HCFMUSP, between 2011 and 2013. Results: The clinical charts of 481 patients of both sexes, with a mean age 60 years, were analyzed; all had chronic ulcers in different stages of evolution. Comorbidities, underlying disease, size of the lesion, treatments, and evolution of wounds were evaluated. A predominance of vascular disease (69.2%) was the underlying cause. All patients were initially treated with dressings containing active agents, for preparation of the wound bed. Of these, 84% were referred for surgical management of wounds. There was no spontaneous closure in 1.5% of cases. The remaining patients (14.5%) showed worsening of lesions with topical treatment, and required other forms of preparation of the wound bed. Conclusion: Topical agents may be an effective outpatient/home method to prepare the wound bed of chronic ulcers for surgical management.
