RESUMO

In order to improve the comprehensive nursing effect of the hospital emergency treatment, this paper analyzes the process of the hospital emergency treatment. In addition, this paper combines the possible risks to analyze the risk factors of the comprehensive nursing in the hospital emergency treatment and builds an intelligent analysis model based on the actual situation of the hospital emergency treatment. At the same time, this paper conducts a systematic survey of emergency services and gives the composition and structure of the system. In addition, this paper divides the business required by the system into modules, including registration module, doctor workstation, nurse workstation, query statistics module, decision-making module, and maintenance module. Finally, this paper suggests that in the process of the clinical triage, more ideas for improving the existing evaluation model should be proposed, and experience should be transformed into advantages, so as to improve emergency triage skills; establish an objective, quantitative, and scientific concept of emergency classification and triage; and fully realize scientific triage and precise triage.

INTRODUCTION: The safety and quality of patient care are basic guidelines in finding new and improved solutions in nursing. Important and influential factors shape the nurses' work environment in hospitals. PURPOSE: With the study, we intended to investigate whether the perception of nurses' work environment is related to the safety culture and the quality of patient care and whether it differs according to nurses' level of education. METHODS OF WORK: The study with a quantitative research method was conducted at the six clinical departments of the University Medical Centre, Ljubljana in 2019. We used a survey questionnaire from the European survey Nurse forecasting in Europe (RN4CAST). RESULTS: 270 nurses were included in the study. The response rate was 54%. The study confirmed that there is a correlation between the assessment of the nurses' work environment and the general assessment of patient safety (r = 0.36; p <0.001), the general assessment of the quality of nursing care (r = 0.32; p <0.001), the confidence in patient self-care at discharge (r = 0.29; p <0.001) and the quality of patient care in the previous year (r = 0.27; p = 0.001). The results showed frequent verbal abuse of nurses, in 44.9% by patients and their relatives and in 35.4% by staff. Graduate nurses rated the work environment more negatively than healthcare technicians (p = 0.003). DISCUSSION AND CONCLUSION: We confirmed the correlation between the assessment of nurses' work environment and patient safety and the quality of health care, and that employees' education influences the assessment and perception of the work environment.

In this month's Magnet® Perspectives column, we examine the origins of nursing research, its role in creating the Magnet Recognition Program®, and why a culture of clinical inquiry matters for nurses. This column explores how Magnet hospitals have built upon the foundation of seminal research to advance contemporary standards that address some of the challenges faced by healthcare organizations around the world. We offer strategies for nursing leaders to develop robust research-oriented programs in their organizations.

OBJECTIVE: Based on the concept of total quality management, the practice of managing needle-stick and sharps injuries was analyzed to improve nursing quality. METHODS: Using total quality management, an improvement plan was completed. Standard operating procedures for infusion therapy and monitoring of the circulatory system were made to reduce the utilization of winged metal needles and the frequency of needle-stick injuries. From 2015 to 2018, four cross-sectional surveys were conducted on the use of winged metal needles, peripheral intravenous catheters, central venous catheters, peripherally inserted central catheters, and implantable venous access ports and the status of needle-stick and sharps injuries in our hospital during the 4 years. RESULTS: Four cross-sectional surveys showed that the percentage of winged metal needle utilization decreased significantly from 13% to 0.5%, and that of peripheral intravenous catheters increased from 77% to 87%. Zero tolerance of winged metal needles increased from 33 to 60 nursing units, an improvement rate of 81.82%. The number of needle-stick injuries decreased from 71 to 21, a decrease of 70.42%. Needle-stick injuries occurred mainly during waste disposal (34.71%) and needle withdrawal (18.18%) and when recapping needles (9.92%). CONCLUSION: Based on total quality management, the implementation of zero tolerance of winged metal needles is much better. The use of winged metal needles and the incidence of needle-stick injuries are reduced. Total quality management is of great clinical value in preventing needle-stick injuries.

Establishing new ways of achieving high-quality care is the result of transformational leadership, empowering structures and processes, and exemplary professional practice in nursing. From coast to coast, clinical nurses take advantage of these opportunities to change care for the better, improve the patient experience, and drive the science of nursing forward. American Nurses Credentialing Center's National Magnet Nurse of the Year Awards recognize and reward this work. Five outstanding clinical nurses are honored, 1 in each of the Magnet Model components: transformational leadership; structural empowerment; exemplary professional practice; new knowledge, innovations, and improvements; and empirical outcomes. This month's Magnet Perspectives column spotlights the 2019 award winners and their achievements and examines how Magnet environments support and encourage their success.

BACKGROUND: Missed nursing care (MNC) is a significant health care issue that impacts on the quality of health care and patient safety. It refers to delayed or omitted aspects of nursing care (totally or partially). MNC is an under-researched area in the Australian health care context. OBJECTIVE: This research sought to further explore the MNC phenomenon in the context of an acute care hospital and to identify its common elements and the factors influencing its occurrence. DESIGN: A convergent parallel mixed methods design was employed involving secondary analysis of routinely collected hospital data and a survey of 44 nursing staff using the MISSCARE survey instrument. The two sources of data were converged to address the objective. FINDINGS: The study found that the most common elements of missed nursing care include failure of patient ambulation, emotional support for patients and/or family, and the provision of full documentation. These elements are consistent with previous international studies conducted in acute care hospital settings. This study identified that local context impacting on MNC was also important and included interruptions to workflow, "perceived" lack of management support, poor handover, and communication breakdown between the nursing team and medical staff. CONCLUSION: Consideration of the local health care context is foundational in understanding the MNC phenomenon. The findings of this research may help nursing managers mitigate the possible effects of MNC and therefore improve patient safety in their acute care environment. Additional multisite studies are required to further explore factors associated with MNC in both general and local contexts.

Now in its 29th year, the American Nurses Credentialing Center's Magnet Recognition Program stands as the premier international acknowledgment of nursing excellence in healthcare organizations around the world. The program's applicability in general hospitals, community hospitals, and academic medical centers is established, but what about specialty hospitals? In this month's Magnet Perspectives, nursing leaders from 3 specialty sectors, rehabilitation hospitals, cancer hospitals, and children's hospitals, discuss the ways in which the Magnet framework enriches the practice environment and promotes outstanding nurse and patient outcomes. Insights are shared about how the Magnet journey provides the foundation to address current challenges in healthcare, including nurse staffing shortages, burnout, unit effectiveness, safety and quality imperatives, patient experience, and more.

With increasing patient acuity and a significant nursing shortage expected in the near future, understanding and gaining the commitment of the new generation of novice nurses are important for nursing leaders. This article provides a review of recent literature and presents innovative strategies that nurse leaders can use to improve the engagement and commitment of millennials. The benefit of expanding mentoring to 3 years, involvement in department committees, quality initiatives, or unit-based councils and opportunities for lateral movement are also discussed.

PURPOSE: This study examined the effects of intentional nursing rounds based on the care model on patients' perceived nursing quality and their satisfaction with nursing services. METHODS: The study design was a nonequivalent control group pretest-posttest design. The participants were assigned to the intervention group or the control group in two orthopedic wards of a teaching hospital. A total of 9 rounds (at 7 AM, 9 AM, 12 PM, 3 PM, 5 PM, 8 PM, 10 PM, 2 AM, and 5 AM) were performed every 2 or 3 hours to the experimental group. The intentional nursing rounding intervention was developed through a theoretical development program involving six basic principles which are maintaining belief, knowing, being with, doing for, enabling, and patients' wellness. To evaluate the effects of intentional nursing rounds, data were collected through the Perception of Quality Nursing Care Scale and the Patients' Satisfaction with Nursing Care Quality Questionnaire. RESULTS: The participants were 70 patients (experimental group) and 75 patients (control group). Results of difference-in-difference analysis confirmed that the patient-perceived nursing quality level from the experimental group was 0.85 points (out of 5.00) higher (p = .041) and the satisfaction with nursing services level was 8.28 points (out of 110.00) higher (p < .001) than the control group. CONCLUSIONS: These results proposed that intentional nursing rounds based on the care model were effective in improving perception of quality nursing care and patients' satisfaction with nursing care. Structured patient-oriented intentional nursing rounds based on the care model are expected to expand to a variety of clinical settings further.

Background: Many hospitals have defined procedures for a complaint management. A systematic analysis of patient complaints helps to identify similar complaints and patterns so that targeted improvement measures can be derived (Gallagher & Mazor, 2015). Aim: Our three-month, nurse-led practice development project aimed 1) to identify complaints regarding communication issues, 2) to systemise and prioritise complaints regarding communication issues, and 3) to derive clinic-specific recommendations for improvement. Method: We analysed 273 complaints of patients documented by the quality management (secondary data analysis). Using content analysis and applying the coding taxonomy for inpatient complaints by Reader, Gillespie and Roberts (2014), we distinguished communication-related complaints. By further inductive differentiation of these complaints, we identified patterns and prioritised fields of action. Results: We identified 186 communication-related complaints divided into 16 subcategories. For each subcategory, improvement interventions were derived, discussed and prioritised. Conclusions: Thus, patient complaints provided an excellent opportunity for reflection and workplace learning for nurses. The analysis gave impulse to exemplify the subject "person-centered care" for nurses.

The American Nurses Credentialing Center's Magnet Recognition Program® released the 2019 Magnet® Application Manual () in October. The 2019 manual is the 12th application manual in the 27-year history of the program. The manual builds upon the foundation of seminal research conducted in the early 1980s that led to the creation of the program yet incorporates contemporary standards that address challenges faced globally by healthcare organizations today.

It has been more than 30 years since an enterprising team of American Academy of Nursing researchers unearthed groundbreaking findings that led to the creation of the American Nurses Credentialing Center's Magnet Recognition Program®. Dr Muriel Poulin, EdD, RN, FAAN, a coauthor of the original Magnet® hospital study, recalls how she and her colleagues opted to take their research in a different direction and address pressing questions that spoke directly to the needs and desires of staff nurses. She shares her thoughts on why the Magnet Recognition Program continues to resonate today. Dr Poulin was inducted into the American Nurses Association Hall of Fame in 2016.

AIMS: This systematic review aimed to synthesise the existing evidence of working time use and the division of labour among nurses and health-care workers in hospital wards. BACKGROUND: The environment of nursing work is changing. Health systems are becoming more complex and costly, and highly skilled health-care professionals are transferring to new, more demanding tasks. Changes require a division of labour that is based on the efficient use of working time. EVALUATION: Sixteen studies were identified for the final analysis through a systematic search. KEY ISSUE: The use of working time was examined mainly through six categories: direct care, indirect care, documentation, unit-related work, personal time and non-nursing duties. The division of labour was examined from the perspective of different occupational groups. CONCLUSIONS: Despite nurses' different educational backgrounds, certain similarities could be observed. All working groups seem to spend less than half of their working time in direct patient care. IMPLICATIONS FOR NURSING MANAGEMENT: Nurse managers could influence the increasing nursing workload by supporting the right division of labour and focusing the nurses' working time use so that it benefits the patient.
