RESUMO

PRACTICAL RELEVANCE: Thoracoscopy provides a minimally invasive means of diagnosis and offers many important benefits when compared with open thoracotomy. CLINICAL CHALLENGES: The expense of the equipment, the steep learning curve required to gain proficiency in thoracoscopic surgery, and the limitations imposed by the feline thoracic cavity, in terms of working and viewing space, are some of the challenges that have limited its uptake to date. However, it is envisaged that it will increase as a technique in feline medicine, in much the same way as laparoscopy has. AUDIENCE: This article is directed at veterinarians currently performing open thoracic surgery and the associated aftercare who concurrently are adept at endoscopic surgery. EVIDENCE BASE: The article draws on the small body of literature that is available on thoracoscopy in cats, which includes reports of its use for evaluation and management of undiagnosed pleural effusion, lung lobe torsion, persistent right aortic arch and chylothorax.

RESUMO

Thoracoscopic surgery offers an exciting method for treating a variety of thoracic disease processes. To date, several thoracoscopic procedures have been described in veterinary patients. This article discusses the basic principles of thoracoscopic surgery and thoracic access, anesthetic concerns, and required instrumentation. A companion article discussing the surgical techniques of thoracoscopic pericardial window creation, subphrenic pericardectomy, thoracoscopic lung biopsy, lung lobectomy, thoracic duct ligation, and cranial mediastinal mass excision will be published in the February 2013 issue.

RESUMO

Exploratory thoracoscopy in small animal veterinary medicine is, compared with an open exploratory thoracotomy, a minimally invasive diagnostic procedure with several benefits. Specific indications, patient positioning, and anesthetic considerations are discussed, as well as instrumentation and general techniques for endoscopic intrathoracic exploration and biopsy.

Six normal, healthy horses age 3-10 years underwent left and right thoracoscopic examination using a rigid telescope. A minimum of 30 days was allowed between procedures. Horses were restrained in stocks and sedated with a continuous detomidine infusion. After surgical preparation of the hemithorax elected for surgery, and administration of local or regional anaesthesia of the surgery sites, thoracoscopy was completed during two 15 min pneumothorax periods. During the procedures, the thoracic structures were viewed using a 57 cm, 10 mm diameter, 30 degrees rigid telescope connected to a digital camcorder to allow computer capture of digital images. The telescope was inserted into the thoracic cavity via 3 different intercostal spaces. The 8th, 10th and 12th intercostal spaces were randomly selected and used among horses. The exploration of each hemithorax started from the dorsal-caudal quadrant continued toward the cranial thorax and was completed by observing the diaphragmatic and caudal pulmonary region. Collapsed lung, aorta, oesophagus and diaphragm were viewed readily in either hemithorax. On exploration of the right hemithorax, the azygos vein, thoracic duct and pulmonary veins were also identified. Horses tolerated thoracoscopy well. Signs of discomfort, such as increased respiratory rate, coughing and decreased level of sedation, were associated with lung collapse in one horse, with pneumothorax on 2 occasions, and when the thorax was approached through the 8th intercostal space. Surgery performed via the 8th intercostal space was hindered by the rigidity of the 8th and 9th ribs, and by the presence of a greater musculature, which did not allow easy cranial and caudal movements of the telescope.

RESUMO

Equine surgery continues to benefit from advances in endoscopic surgical techniques. Much of what we have learned in equine endoscopy comes from techniques used in humans. Descriptions of the normal anatomy of the standing horse and advances in instrumentation have improved our ability to perform laparoscopic surgery in standing and dorsally recumbent horses. This article describes the history of laparoscopy and available instrumentation.

With the recent development of video-assisted thoracic surgery, visual inspection of the thoracic cavity has been used to provide a more accurate diagnosis and prognosis of thoracic diseases and to better manage these diseases. Equipment, techniques, and complications for standing thoracoscopy in horses are described.

RESUMO

Thoracoscopy is a minimally invasive diagnostic technique that provides access to the thoracic cavity for evaluation of intrathoracic pathology without surgical intervention. Intrathoracic structures can be visualized better with thoracoscopy than with an open thoracotomy. Indications for thoracoscopy include pleural effusion, pericardial effusion, intrathoracic masses, pneumothorax, primary pulmonary disease, and trauma. Thoracoscopy is technically similar to laparoscopy, using the same basic instrumentation and principles, but is easier to perform than laparoscopy. Patient preparation, anesthesia, and patient positioning are essentially the same for thoracoscopy as for a standard open thoracotomy. Thoracoscopy provides minimally invasive access to important diagnostic information with a very low incidence of complications.
