BACKGROUND: Intracranial venous thrombosis (ICVT) accounts for around 0.5% of all stroke cases. There have been no previously published studies of the International Classification of Diseases, Tenth Edition (ICD-10) validation for the identification of ICVT admissions in adults. OBJECTIVE: The aims of this study were to validate and quantify the performance of the ICD-10 coding system for identifying cases of ICVT in adults and to derive an estimate of incidence. METHOD: Administrative data were collected for all patients admitted to a regional neurosciences centre over a 5-year period. We searched for the following ICD-10 codes at any position: G08.X (intracranial and intraspinal phlebitis and thrombophlebitis), I67.6 (non-pyogenic thrombosis of intracranial venous system), I63.6 (cerebral infarction due to cerebral venous thrombosis, non-pyogenic), O22.5 (cerebral venous thrombosis in pregnancy) and O87.3 (cerebral venous thrombosis in the puerperium). RESULTS: Sixty-five admissions were identified by at least one of the relevant ICD-10 codes. The overall positive predictive value (PPV) for confirmed ICVT from all of the admissions combined was 92.3% (60 out of 65) with the results for each code as follows: G08.X 91.5% (54 of 59), O22.5 100% (4 of 4), I67.6 100% (1 of 1), I63.6 100% (1 of 1) and O87.3 100% (1 of 1). There were 40 unique cases of ICVT over a 5-year period giving an annual incidence of ICVT of 5 per million. CONCLUSIONS: All codes gave a high PPV. IMPLICATIONS FOR PRACTICE: As demonstrated in previous studies, the incidence of ICVT may be higher than previously thought.

Cerebral venous thrombosis (CVT) is uncommon, representing approximately 0.5% of all cases of cerebrovascular disease worldwide. Many factors, alone or combined, can cause CVT. Although CVT can occur at any age, it most commonly affects neonates and young adults. CVT is difficult to diagnose clinically because patients can present with a wide spectrum of nonspecific manifestations, the most common of which are headache in 89%-91%, focal deficits in 52%-68%, and seizures in 39%-44% of patients. Consequently, imaging is fundamental to its diagnosis. MRI is the most sensitive and specific technique for diagnosis of CVT. The different MRI sequences, with and without the use of contrast material, have variable strengths. Contrast material-enhanced MR venography has the highest accuracy compared with sequences without contrast enhancement.Online supplemental material is available for this article.©RSNA, 2019.

BACKGROUND: The aim of our study was to evaluate in patients with acute ischemic stroke the relationship between immuno-inflammatory variables, clinical outcome and infarct site. MATERIALS AND METHODS: We evaluated plasma levels of IL-1beta, TNF-alpha, IL-6 and IL-10, E-selectin, P-selectin, sICAM-1 ,sVCAM-1 vWF, TPA and PAI-1. RESULTS: Patients with cardioembolic subtype showed significantly higher median plasma levels of TNF-alpha, IL-6, IL-1beta whereas the lacunar subtype showed significantly lower median plasma levels of TNF-alpha, IL-6 and IL-1beta. CONCLUSIONS: A significant association was noted between the severity of neurological deficit at admission, the diagnostic subtype and some inflammatory variables.

OBJECTIVE: Thrombotic aneurysms are a diverse collection of complex aneurysms characterized by organized intraluminal thrombus and solid mass. Consequently, their treatment often requires techniques other than conventional clipping, such as thrombectomy with clip reconstruction or bypass with parent artery occlusion. A single-surgeon experience with thrombotic aneurysms was analyzed to determine optimal treatment strategies. A classification scheme was devised on the basis of aneurysm, thrombus, and lumen morphology to relate these anatomic features to surgical therapy. METHODS: Sixty-eight patients with thrombotic aneurysms were managed during a period of 6.25 years. Thrombotic aneurysms were classified into six types: concentric (n = 17, 25%), eccentric (n = 14, 21%), lobulated (n = 2, 3%), complete (n = 2, 3%), canalized (n = 17, 25%), and coiled (n = 16, 24%). RESULTS: Aneurysm management consisted of direct clipping (n = 22, 32%), thrombectomy-clip reconstruction (n = 18, 26%), bypass-occlusion (n = 20, 29%), other (n = 6, 9%), or observation (n = 2, 3%). Complete angiographic obliteration was achieved in 97% of patients, and 47% of aneurysms were thrombectomized. The surgical mortality rate was 6%, and the permanent neurological morbidity rate was 7%. Overall, 87% of patients were improved or unchanged at follow-up, with 79% reaching a Glasgow Outcome Scale score of 5 or 4. Management strategy was influenced by thrombotic aneurysm type, but patient outcome was not. The best results were observed in patients treated with direct clipping and bypass-occlusion. CONCLUSION: Despite their solid mass, one-third of thrombotic aneurysms can be treated surgically with conventional clipping. Direct clipping is associated with the best surgical results, and the proposed classification scheme identifies thrombotic aneurysms that may be clippable. Patients with unclippable thrombotic aneurysms had more favorable results when treated with bypass and aneurysm occlusion than with thrombectomy and clip reconstruction. The classification scheme may provide conceptual clarity and therapeutic guidance with preoperative and intraoperative decision making.

BACKGROUND: Arterial recanalization precedes clinical improvement or may lead to hemorrhage or reperfusion injury. Speed of clot lysis was not previously measured in human stroke. METHODS AND RESULTS: Transcranial Doppler (TCD) and the National Institutes of Health Stroke Scale (NIHSS) were used to monitor consecutive patients receiving intravenous tissue plasminogen activator (tPA), before tPA bolus and at 24 hours. Patients with complete or partial recanalization of the middle cerebral or basilar artery on TCD were studied. Recanalization was classified a priori as sudden (abrupt appearance of a normal or stenotic low-resistance signal), stepwise (flow improvement over 1 to 29 minutes), or slow (>/=30 minutes). Recanalization was documented in 43 tPA-treated patients (age 68+/-17 years; NIHSS score 16.8+/-6, median 15 points). tPA bolus was given at a mean of 135+/-61 minutes after symptom onset. Recanalization began at a median of 17 minutes and was completed at 35 minutes after tPA bolus, with mean duration of recanalization of 23+/-16 minutes. Recanalization was sudden in 5, stepwise in 23, and slow in 15 patients. Faster recanalization predicted better short-term improvement (P=0.03). At 24 hours, 80%, 30%, and 13% of patients in these respective recanalization groups had NIHSS scores of 0 to 3. Symptomatic hemorrhage occurred in only 1 patient, who had stepwise recanalization 5.5 hours after stroke onset. Slow or partial recanalization with dampened flow signal was found in 53% of patients with total NIHSS scores >10 points at 24 hours (P=0.01). Complete recanalization (n=25) occurred faster (median 10 minutes) than partial recanalization (n=18; median 30 minutes; P=0.0001). CONCLUSIONS: Rapid arterial recanalization is associated with better short-term improvement, mostly likely because of faster and more complete clot breakup with low resistance of the distal circulatory bed. Slow (>/=30 minutes) flow improvement and dampened flow signal are less favorable prognostic signs. These findings may be evaluated to assist with selection of patients for additional pharmacological or interventional treatment.
