RESUMO

Little literature exists documenting how K-12 American Sign Language-English interpreters are supervised or evaluated, or how supervision and evaluation affect interpreters and deaf and hard of hearing students. To address this gap, the author asked 230 educational interpreters about their experiences of supervision. Using a critical policy analysis framework and transformational leadership lens, the author conducted a Qualtrics online survey consisting of 42 open- and closed-ended items. Descriptive and statistical results are reported. Chi-square tests showed a significant difference in supervisory activities based on whether or not the supervisor was a credentialed interpreter. Supervisors who were interpreters conducted more job evaluations, assessed interpreting skills more often, and organized professional development more frequently than non-interpreters.

BACKGROUND: Accreditation of transition to practice (TTP) programs is on the rise across the United States; however, few studies have investigated the effect of program recognition on health care organizations. Even less is known about the organizational value of residency/fellowship program accreditation and which measures support the long-term sustainability of these programs. The goal of this retrospective content analysis is to revisit the effect of accreditation by replicating a previous analysis of TTP programs. METHOD: An administrative survey was distributed in 2019 to TTP programs recognized by the American Nurses Credentialing Center (ANCC) Practice Transition Accreditation Program® (PTAP). Directed content analysis methods were used to compare thematic results on the perceived effect of accreditation with those in the original analysis. RESULTS: A coding matrix was used to categorize responses into broad concept categories with subthemes. All themes from the original publication were replicated in the current analysis; additionally, new categories and subthemes emerged. CONCLUSION: Implications from this analysis may be used by nursing leaders, educators, and policymakers to increase TTP funding and recognition, engage stakeholders in organizational improvement, and expand residency/fellowship science through professional dissemination. RESUMO

Considerando a legislação e as atribuições regimentais no âmbito estadual, a Escola de Saúde Pública da Bahia (ESPBA) tem como finalidade desenvolver ações de qualificação, formação e educação permanente para os trabalhadores de saúde, em consonância com os princípios e diretrizes do Sistema Único de Saúde (SUS) e a Política Estadual de Gestão do Trabalho e Educação em Saúde. Com o cenário adverso durante a pandemia da Covid-19, a ESPBA teve que redobrar os esforços para atender, além das ações já planejadas, as necessidades de ações contingenciais impostas pelo novo coronavírus (SARS-CoV-2). Este artigo tem como objetivo discutir as mudanças do processo de trabalho da ESPBA e as ações realizadas no contexto da pandemia. As informações foram coletadas dos registros de trabalho de cada área técnica (dados secundários oriundos de fichas de inscrição, relatórios dos cursos e relação de certificados), que fazem parte da sistematização de dados, utilizando o software Microsoft Excel, para as devolutivas quadrimestrais nos relatórios. A ESPBA realizou um número significativo de ações educativas a partir da reorganização e reestruturação do processo de trabalho, com a adequação das ações presenciais para modalidades de ensino a distância e remoto, sendo possível descentralizá-las para trabalhadores de outros municípios. Dessa forma, proporciona articulação intra e interinstitucional nas ações educativas específicas para enfrentamento do novo coronavírus. Apesar do contexto, a pandemia foi um momento estratégico para a instituição, pois proporcionou oportunidade de se organizar e de identificar fragilidades quanto aos equipamentos e estruturas para incorporação da tecnologia da informação. Mesmo com os desafios e limitações, conclui-se que a ESPBA cumpriu com seu papel de ordenadora da formação no SUS.


Considering the legislation and regimental attributions at the state level, the School of Public Health of Bahia (ESPBA), aims at developing qualification, training and permanent education actions for health workers, in line with SUS principles and guidelines and the State Policy for Labor Management and Health Education. With the adverse scenario during the pandemic, ESPBA had to work harder to meet the needs of contingent actions imposed by the Coronavirus, in addition to the actions already planned. The objective of this article is to discuss changes in the ESPBA work process and actions in the context of the pandemic. The information was collected from the work records of each technical area (secondary data from the registration forms, report of the courses and list of certificates), which are part of the data systematization ­ using Excel ­ for the quarterly returns in the reports. ESPBA conducted a significant number of educational actions based on the reorganization and restructuring of the work process in the adaptation of the face-to-face actions to distance and remote learning modalities, enabling the decentralization of these activities to workers from other municipalities, intra and interinstitutional articulation in specific educational actions to fight the Coronavirus. Despite the context, the pandemic was a strategic moment for the institution, since it provided an opportunity to organize and identify weaknesses in the equipment and structures for incorporating information technology. Even with the challenges and limitations, ESPBA fulfilled its role as organizer of training in SUS.


Considerando la legislación y atribuciones del regimiento a nivel estadual, la Escuela de Salud Pública de Bahía (ESPBA) busca desarrollar acciones de formación, capacitación y educación permanente para los trabajadores de la salud en línea con los principios y lineamientos del Sistema Único de Salud (SUS) y la Política Estadual de Gestión Laboral y Educación para la Salud. Ante el escenario adverso de la pandemia, la ESPBA debió redoblar esfuerzos para atender las necesidades de acciones contingentes impuestas por el coronavirus, además de las acciones ya planificadas. Este artículo tiene como objetivo discutir los cambios en el proceso de trabajo de la ESPBA y las acciones tomadas en el contexto pandémico. La información fue recolectada de los registros de trabajo de cada área técnica (datos secundarios de los formularios de inscripción, informe de los cursos y lista de certificados), los cuales son parte de la sistematización de datos ­usando Excel­ para las declaraciones cuatrimestrales en los informes. La ESPBA realizó un número significativo de acciones educativas, desde la reorganización y reestructuración del proceso de trabajo en la adecuación de las acciones presenciales a modalidades de aprendizaje a distancia, siendo posible la descentralización de estas a trabajadores de otros municipios. De esta manera, proporciona articulación intra e interinstitucional en acciones educativas específicas para combatir el coronavirus. A pesar del contexto, la pandemia fue un momento estratégico para la institución, ya que brindó la oportunidad de organizar e identificar debilidades en los equipos y estructuras para incorporar tecnología de la información. Aun con los desafíos y limitaciones, se concluye que la ESPBA cumplió con su rol de organizadora de la capacitación en el SUS.

TOPIC: The development of the Critical Care Beacon Collaborative to achieve meaningful recognition. CLINICAL RELEVANCE: Recognizing nurses for contributions to their work environment and care delivery is important for their professional and personal fulfillment, job satisfaction, and retention; such recognition can occur at the individual, unit, or organizational level. The American Nurses Credentialing Center's Magnet Recognition Program acknowledges nursing excellence at the organizational level. It would, however, be difficult for an organization to achieve Magnet designation without nursing excellence at the unit level. To recognize excellence at the unit level, the American Association of Critical-Care Nurses developed the Beacon Award in 2003. OBJECTIVE: To describe one academic medical center's journey toward winning Beacon Awards across 8 units within the adult critical care service. CONTENT COVERED: The Critical Care Beacon Collaborative resulted in a Beacon Award for each unit and important staff outcomes. This article describes the organization, the process before the Critical Care Beacon Collaborative convened and the desired state, and the methods used to achieve our goal. It also discusses unit- and service-level stakeholder involvement. The successes, lessons learned, sustainability, and growth of the Critical Care Beacon Collaborative are shared to assist readers who aspire to pursue a Beacon Award.

OBJECTIVE: The purpose of this study was to determine chief nursing officer (CNO) perspectives on how to sustain the infrastructure required for successful American Nurses Credentialing Center (ANCC) Magnet® redesignation. BACKGROUND: American Nurses Credentialing Center Magnet designation is a prestigious achievement reflective of years of dedication, innovation, mentoring, and leadership support. As challenging as the initial attainment of Magnet status can be, sustaining the success and becoming redesignated is considered even more difficult by many CNOs. However, there have been no published reports indicating how to be successful in Magnet redesignation. METHOD: A grounded theory qualitative approach was used, and data were collected through telephone interviews with CNOs who had successfully attained at least 1 redesignation. RESULTS: Fourteen CNOs participated; data were organized into 6 themes and 15 subthemes describing the critical elements for Magnet redesignation. CONCLUSION: Relationships among the 6 themes and subthemes are theorized in the form of a wheel with 6 spokes. When "set in motion," the wheel gathers momentum and all of the model elements become coalesced into the organizational ethos.

AIM: To engage the global nursing community in sharing knowledge, experiences, and lessons learned about credentialing of individual nurses to improve nursing practice, patient safety, and quality of care. BACKGROUND: Although the United States has a long and robust history of credentialing individual nurses, the opportunity exists to expand the dialogue globally, and is supported by globally focused governing bodies who call for increased or expanded investment in nursing certification and credentialing. Foundational work to define and operationalize certification for research purposes and develop an administrative home for stakeholders interested in certification can be leveraged by countries and regions globally. Existing frameworks for credentialing research may be used to drive theory-based research in the future. SOURCES OF EVIDENCE: PubMed, International Council of Nurses, National Academies of Sciences, Engineering, and Medicine, World Health Organization, American Nurses Association, American Nurses Credentialing Center, and the authors' own experiences. DISCUSSION: Credentialing as a form of self-governance, as a method for public protection, and as a foundational element in the increasingly new forms of healthcare delivery is intensifying in importance. Credentials provide an opportunity to evaluate relationships between key elements such as nursing practice, environments where nursing care is delivered, and nursing programs with outcomes of interest for the profession such as safety, quality of care, and patient outcomes. CONCLUSIONS: Establishing rigorous credentials for nurses demonstrates a commitment to excellence. Credentialing frameworks that are universally applied could enable nurses to move seamlessly across geographic boundaries, permit regulators and employers of nurses to have a common set of standards and expectations, and ensure a level of competency for nursing practice that can be interpreted and trusted by various stakeholders. IMPLICATIONS FOR NURSING POLICY: Policymakers have a pivotal role in advancing credentialing in nursing worldwide. Countries developing credentialing programs in nursing need to study their results to help inform how practice might be required to change over time.

Credentialing and certification are essential processes during hiring to ensure that the physician is competent and possesses the qualifications and skill sets claimed. Peer review ensures the continuing evolution of these skills to meet a standard of care. We have provided an overview and discussion of these processes in the United States. Credentialing is the process by which a physician is determined to be competent and able to practice, used to ensure that medical staff meets specific standards, and to grant operative privileges at an institution. Certification is a standardized affirmation of a physician's competence on a nationwide basis. Although not legally required to practice in the United States, many institutions emphasize certification for full privileges on an ongoing basis at a hospital. In the United States, peer review of adverse events is a mandatory prerequisite for accreditation. The initial lack of standardization led to the development of the Health Care Quality Improvement Act, which protects those involved in the peer review process from litigation, and the National Provider Databank, which was established as a national database to track misconduct. A focus on quality improvement in the peer review process can lead to improved performance and patient outcomes. A thorough understanding of the processes of credentialing, certification, and peer review in the United States will benefit neurosurgeons by allowing them to know what institutions are looking for as well and their rights and responsibilities in any given situation. It could also be useful to compare these policies and practices in the United States to those in other countries.

Due to COVID-19, the 2021 symposium was held virtually. With close to 700 participants, the ANCC PTAP team was challenged to provide an engaging virtual conference. PURPOSE: This study aimed to develop a physical geometric phantom for the deformable image registration (DIR) credentialing of radiotherapy centers for a clinical trial and tested the feasibility of the proposed phantom at multiple domestic and international institutions. METHODS AND MATERIALS: The phantom reproduced tumor shrinkage, rectum shape change, and body shrinkage using several physical phantoms with custom inserts. We tested the feasibility of the proposed phantom using 5 DIR patterns at 17 domestic and 2 international institutions (21 datasets). Eight institutions used the MIM software (MIM Software Inc, Cleveland, OH); seven used Velocity (Varian Medical Systems, Palo Alto, CA), and six used RayStation (RaySearch Laboratories, Stockholm, Sweden). The DIR accuracy was evaluated using the Dice similarity coefficient (DSC) and Hausdorff distance (HD). RESULTS: The mean and one standard deviation (SD) values (range) of DSC were 0.909 ± 0.088 (0.434-0.984) and 0.909 ± 0.048 (0.726-0.972) for tumor and rectum proxies, respectively. The mean and one SD values (range) of the HD value were 5.02 ± 3.32 (1.53-20.35) and 5.79 ± 3.47 (1.22-21.48) (mm) for the tumor and rectum proxies, respectively. In three patterns evaluating the DIR accuracy within the entire phantom, 61.9% of the data had more than a DSC of 0.8 in both tumor and rectum proxies. In two patterns evaluating the DIR accuracy by focusing on tumor and rectum proxies, all data had more than a DSC of 0.8 in both tumor and rectum proxies. CONCLUSIONS: The wide range of DIR performance highlights the importance of optimizing the DIR process. Thus, the proposed method has considerable potential as an evaluation tool for DIR credentialing and quality assurance.

Objetivo: Avaliar a percepção dos enfermeiros acerca de sua formação acadêmica e qualificação profissional para a prestação de Cuidados Paliativos. Métodos: Estudo de campo de natureza descritiva, com abordagem qualitativa, realizado com 14 enfermeiros da Estratégia de Saúde da Família do município de Cajazeiras, Paraíba. Os dados foram coletados por meio de entrevista semiestruturada, processados utilizando-se do software IRAMUTEQ® e analisados em uma perspectiva temática e categorial. Resultados: Foram construídas duas categorias temáticas: Categoria 1 ­ Conhecimento dos enfermeiros sobre Cuidados Paliativos; Categoria 2 ­ Formação acadêmica e qualificação profissional dos enfermeiros para a prestação de Cuidados Paliativos. A maioria dos participantes referiu não haver nenhuma disciplina sobre a temática nas instituições de ensino onde cursaram a graduação e nenhum deles participou de Educação Permanente em Saúde sobre o tema, sugerindo conhecimento limitado dos profissionais sobre Cuidados Paliativos, condição que reflete negativamente na qualidade da assistência. Conclusão: Evidencia-se a necessidade de aproximação dos enfermeiros com os Cuidados Paliativos em seu processo formativo e de qualificação profissional, visando o desenvolvimento de competências necessárias para prestar uma assistência eficiente. Objective: To evaluate the nurses' perception of their academic education and professional qualification for the provision of Palliative Care. Methods: Field study of a descriptive nature with a qualitative approach, conducted with 14 nurses from the Family Health Strategy in the municipality of Cajazeiras, Paraíba. Data were collected through semi-structured interviews, processed using the IRAMUTEQ® software and analyzed in a thematic and categorical perspective. Results: Two thematic categories were constructed: Category 1 ­ Nurses' knowledge about Palliative Care; Category 2 ­ Academic education and professional qualification of nurses for the provision of Palliative Care. Most participants reported not having discipline about the theme in the institutions where they finished their degrees and none participated in Permanent Health Education about the theme, being abel to suggest limited knowledge of professionals about Palliative Care, which reflects negatively on the quality of the assistance. Conclusion: The need to approach nurses wiith palliative care is evidente in their education and professional qualification process for devoloping necessary skills to provide efficient assistance. Conclusión: La necesidad de acercar a los enfermeros a los cuidados paliativos es evidente en su proceso de capacitación y calificación profesional para el desarrollo de las habilidades necesarias para proporcionar una asistencia eficiente. RESUMO

Despite the near-universal acceptance of the benefits of a sound peer review process (PRP), the topic of peer review remains a source of controversy among surgeons. The current PRP is plagued by heterogeneity across different hospital and institutional systems. These inconsistencies, combined with a perceived lack of fairness inherent to the PRP in some institutions, led to concerns among practicing surgeons. In this review of the relevant literature on the PRP, we attempted to provide some context and insight into the history of the PRP, its role, its shortcomings, its potential abuses, and some key requirements for its successful execution.

CONTEXT: The transition to practice of newly credentialed athletic trainers (ATs) has become an area of focus in the athletic training literature. However, no theoretical model has been developed to describe the phenomenon and drive investigation. OBJECTIVE: To better understand the lived experience of the transition to practice and develop a theoretical model of transition to practice for ATs. DESIGN: Qualitative study. SETTING: Telephone interviews. PATIENTS OR OTHER PARTICIPANTS: Fourteen professional master's athletic training students (7 men, 7 women, age = 25.6 ± 3.7 years, from 9 higher education institutions) in the first year of clinical practice as newly credentialed ATs. DATA COLLECTION AND ANALYSIS: Participants completed semistructured phone interviews at 3 timepoints over 12 to 15 months. The first interview was conducted just before graduation, the second 4 to 6 months later, and the third at 10 to 12 months. The interviews were transcribed and analyzed using a grounded theory approach. RESULTS: We developed a theoretical model to explain the causal conditions that triggered transition, how the causal conditions were experienced, the coping strategies used to persist through the first year of practice, and the consequences of those strategies. CONCLUSIONS: The model provides a framework for new athletic training clinicians, educators, and employers to better understand the transition process in order to help new clinicians respond by accepting or adapting to their environment or their behaviors.

This article addresses how accredited provider program directors (AP-PDs), as nursing professional development practitioners, can meet structural capacity criterion 2 in the American Nurses Credentialing Center's Nursing Continuing Professional Development accreditation criteria. This article highlights the leadership responsibilities of the APPD and provides strategies to develop a successful accredited provider unit onboarding and professional development plan for its nurse planners. RESUMO

The American Nurses Credentialing Center (ANCC) manages Magnet Recognition Program® certification. Candidates for this certification are required to pass a review comprising six compulsory documents and 84 sets of written documents in five model components as well as an onsite appraisal. The Magnet Recognition Program® for medical institutes is currently regarded as the highest honor in the nursing field. The series of measures necessary to establish a Magnet-recognized hospital is described in this article. Our hospital began preparation for recognition in January 2016, with the application submitted in May 2017. The required measures include establishing a Professional Practice Model, Governance Council, and National Database of Nursing Quality Indicators as well as implementing a mentoring plan. ANCC approved Magnet Recognition Program® recognition for our hospital in May 2020, making ours the first Magnet-recognized hospital in Taiwan. The nursing staff shared their experiences on their Magnet journey. Magnet-recognition demonstrates that a hospital is adequately equipped and trained to provide patients with outstanding nursing care and to operate an excellent nursing practice environment.

STUDY OBJECTIVE: Simulation-based assessment is poised for application in educational promotion and credentialing in gynecologic surgery. With high-stakes assessment, validation necessitates evidence, not just about the trustworthiness of decisions but of beneficial consequences to education and healthcare as well. In this paper, we unpack the modern conceptualization of validity as it pertains to surgical simulation and high-stakes competency assessment.
