ABSTRACT

This study aimed to determine the absolute and relative reliability of concentric and eccentric flexion, extension, horizontal abduction, and adduction movements of the shoulder using a functional electromechanical dynamometer (FEMD). Forty-three active male university students (23.51 ± 4.72 years) were examined for concentric and eccentric strength of shoulder flexion, extension, horizontal abduction, and horizontal adduction with an isokinetic test at 0.80 m·s-1. Relative reliability was determined by intraclass correlation coefficients (ICCs) with 95% confidence intervals. Absolute reliability was quantified by the standard error of measurement (SEM) and coefficient of variation (CV). Reliability was very high to extremely high for all movements on concentric and eccentric strength measurements (ICC: 0.76-0.94, SEM: 0.63-6.57%, CV: 9.40-19.63%). The results of this study provide compelling evidence for the absolute and relative reliability of concentric and eccentric flexion, extension, horizontal abduction, and horizontal adduction shoulder isokinetic strength tests in asymptomatic adults. The mean concentric force was the most reliable strength value for all tests.

ABSTRACT

PA-X is a nonstructural protein of influenza A virus (IAV), which is encoded by the polymerase acidic (PA) N-terminal region that contains a C-terminal +1 frameshifted sequence. IAV PA-X protein modulates virus-induced host innate immune responses and viral pathogenicity via suppression of host gene expression or cellular shutoff, through cellular mRNA cleavage. Highly pathogenic avian influenza viruses (HPAIV) of the H5N1 subtype naturally infect different avian species, they have an enormous economic impact in the poultry farming, and they also have zoonotic and pandemic potential, representing a risk to human public health. In the present study, we describe a novel bacterium-based approach to identify amino acid residues in the PA-X protein of the HPAIV A/Viet Nam/1203/2004 H5N1 that are important for its ability to inhibit host protein expression or cellular shutoff activity. Identified PA-X mutants displayed a reduced shutoff activity compared to that of the wild-type A/Viet Nam/1203/2004 H5N1 PA-X protein. Notably, this new bacterium-based screening allowed us to identify amino acid residues widely distributed over the entire N-terminal region of PA-X. Furthermore, we found that some of the residues affecting A/Viet Nam/1203/2004 H5N1 PA-X host shutoff activity also affect PA polymerase activity in a minigenome assay. This information could be used for the rational design of new and more effective compounds with antiviral activity against IAV. Moreover, our results demonstrate the feasibility of using this bacterium-based approach to identify amino acid residues important for the activity of viral proteins to inhibit host gene expression. IMPORTANCE Highly pathogenic avian influenza viruses continue to pose a huge threat to global animal and human health. Despite of the limited genome size of Influenza A virus (IAV), the virus encodes eight main viral structural proteins and multiple accessory nonstructural proteins, depending on the IAV type, subtype, or strain. One of the IAV accessory proteins, PA-X, is encoded by the polymerase acidic (PA) protein and is involved in pathogenicity through the modulation of IAV-induced host inflammatory and innate immune responses. However, the molecular mechanism(s) of IAV PA-X regulation of the host immune response is not well understood. Here, we used, for the first time, a bacterium-based approach for the identification of amino acids important for the ability of IAV PA-X to induce host shutoff activity and describe novel residues relevant for its ability to inhibit host gene expression, and their contribution in PA polymerase activity.

ABSTRACT

The pursuit of high-power solar laser systems with high efficiency and capacity for large tracking error compensation is determinant for the applicability of this renewable technology. A side-pumped dual-rod Ce:Nd:YAG solar laser was developed and tested at the focus of a 2 m diameter parabolic concentrator. Maximum continuous-wave total solar laser power of 58 W was measured. To the best of our knowledge, this is the highest laser power from a Ce:NdYAG solar laser. Moreover, wide tracking error compensation width of 5.1° in the azimuthal direction was reached, being 4.25 times higher than the previous measurement without solar tracking assistance.

ABSTRACT

OBJECTIVES: A variety of factors are known to mediate on the intraocular pressure (IOP) response to resistance training. However, the influence of the body position adopted during resistance training on IOP remain unknown. The objective of this study was to determine the IOP response to the bench press exercise at three levels of intensity when performed in supine and seated positions. METHODS: Twenty-three physically active healthy young adults (10 men and 13 women) performed 6 sets of 10 repetitions against the 10-RM (repetition maximum) load during the bench press exercise against three levels of intensity (high intensity: 10-RM load; medium intensity: 50% of the 10-RM load; and control: no external load) and while adopting two different body positions (supine and seated). A rebound tonometer was employed to measure IOP in baseline conditions (after 60 s in the corresponding body position), after each of the 10 repetitions, and after 10 s of recovery. RESULTS: The body position adopted during the execution of the bench press exercise significantly affected the changes in IOP (p < 0.001, ηp2 = 0.83), with the seated position providing lower increases in IOP levels compared to the supine position. There was an association between IOP and exercise intensity, with greater IOP values in the more physically demanding conditions (p < 0.001, ηp2 = 0.80). CONCLUSIONS: The use of seated positions, instead of supine positions, for the execution of resistance training should be prioritized for maintaining more stable IOP levels. This set of findings incorporates novel insights into the mediating factors on the IOP response to resistance training. In future studies, the inclusion of glaucoma patients would allow to assess the generalizability of these findings.

ABSTRACT

OBJECTIVE: Functional capacity is 1 of the main risk factors for falls among older adults. The aim of this systematic review and meta-analysis was to determine the effect of power training on functional capacity test (FCT) related to fall risk in older adults. DATA SOURCES: Systematic searches were conducted in 4 databases, including PubMed, Web of Science, Scopus, and SPORTDiscus, from inception to November 2021. STUDY SELECTION: Randomized controlled trials (RCTs) assessing the effect of power training on functional capacity compared with another type of training program or control group in older adults with the ability to exercise independently. DATA EXTRACTION: Two independent researchers evaluated eligibility and used the PEDro scale to assess risk of bias. The information extracted was related to article identification (authors, country and year of publication), participant characteristics (sample, sex, and age), strength training protocols (exercises/intensity/weeks), and the outcome of the FCT used related to fall risk. The Cochran Q statistic and I2 statistics was used to assess heterogeneity. Random-effects model were conducted to pool the effect sizes expressed as mean differences (MD). DATA SYNTHESIS: Twelve studies (478 subjects) were selected for systematic review. A meta-analysis comprised 6 studies (217 subjects) where the outcome measure was the 30-second Sit to Stand (30s-STS) test, and another comprised 4 studies (142 subjects) where the outcome measure was the timed Up and Go (TUG) test. There was an improvement in performance in favor of the experimental group in both the TUG subgroup (MD -0.31 s; 95% CI -0.63, 0.00 s; P=.05), and the 30s-STS subgroup (MD 1.71 reps; 95% CI -0.26, 3.67 reps; P=.09). CONCLUSIONS: In conclusion, power training increases functional capacity related to fall risk further than other types of exercise in older adults.

ABSTRACT

Solar laser technology typically requires a highly accurate solar tracking system that operates continuously, which increases energy consumption and reduces the system's lifetime. We propose a multi-rod solar laser pumping approach to enhance solar laser stability under non-continuous solar tracking conditions. Using a heliostat, solar radiation is redirected toward a first-stage parabolic concentrator. At its focus, an aspheric lens further concentrates the solar rays onto five Nd:YAG rods positioned within an elliptical-shaped pump cavity. Numerical analysis using Zemax and LASCAD software showed that the tracking error width at 10% laser power loss for the five 6.5 mm diameter and 15 mm length rods was 2.20°, which is 50% higher than that of the solar laser in previous non-continuous solar tracking experiments. 2.0% solar-to-laser conversion efficiency was also attained.

ABSTRACT: Barrera-Domínguez, FJ, Martínez-García, D, Jerez-Mayorga, D, Chirosa-Ríos, LJ, Almagro, BJ, and Molina-López, J. Vertical versus horizontal training for improving the change of direction speed in adult basketball players: a systematic review and meta-analysis. J Strength Cond Res XX(X): 000-000, 2023-The ability to perform changes of direction (COD) is a complex skill that involves the application of multiple force-orientations, and its execution at maximum speed is crucial in basketball players. The present study aimed to synthesize findings from previous interventions classified according to force-orientation (vertical, horizontal, or mixed) and determine their magnitude of the effect on COD performance in basketball players. A systematic review of the literature was performed in several databases (Web of Science, Scopus, SPORTDiscus, and PubMed) following the PRISMA statement and reviewed the quality of the included papers according to the Cochrane Collaboration Guidelines Assessment. Sixteen articles with a total of 21 reports were included to analyze the role of force-orientations in COD performance. For the meta-analysis, the standardized mean differences with 95% confidence intervals (CIs) were calculated to determine the chronic changes induced by training and performed an effect size (ES) analysis with a random-effects model. The results showed that all force-orientations generated improvements in COD performance (ES = -0.47 [95% CI -0.57, -0.36], Z = 8.74 [p < 0.01]). It was a mixed force-orientation that produced the greatest changes (ES = -0.91 [95% CI -1.27, -0.55], Z = 4.96 [p < 0.01]), followed by vertical training (ES = -0.45 [95% CI -0.70, -0.20], Z = 3.51 [p < 0.01]). Horizontal training was the least studied and showed the smallest change (ES = -0.10 [95% CI -0.14, -0.07], Z = 5,71 [p < 0.01]). This meta-analysis demonstrates that mixed vertical and horizontally oriented training may be the most optimal because it offers a greater variety of multidirectional stimuli that better prepares the athlete to deal with complex COD in real-game situations.

ABSTRACT

The purposes were to synthesize as much scientific evidence as possible to determine the effect of core training on balance, throwing/hitting velocity or distance, and jumping in healthy subjects, identify the possible differences between isolated and combined core training on performance and study training and sample variables related to performance. PRISMA guidelines were followed, and a systematic search was performed in the Scopus, Web of Science, Sports Discuss, and PubMed databases with no date restrictions until November 2022. The studies were considered for this meta-analysis following PICO; a) randomized control trials and randomized allocation studies with healthy subjects and > 12 years old b)isolated or combined core training programs with a minimum of 4 weeks in length; c) athletic performance outcomes for balance, throw/hit, and jump variables should be measured; d) sufficient data to calculate effect sizes. The Cochrane Collaboration Risk of Bias Tool and the Grading of Recommendations Assessment, Development, and Evaluation approach were used for assessing methodological quality. A total of 3223 studies were identified, 22 studies were included in the systematic review and 21 for the meta-analysis. We observed that core training improved balance outcomes (ES = 1.17; p < 0.0001), throwing/hitting velocity (ES = 0.30; p = 0.14), throwing/hitting distance (ES = 3.42; p = 0.03), vertical jumping (ES = 0.69; p = 0.0003), and horizontal jump (ES = 0.84; p = 0.01). Our findings indicate that core training improved different variables of performance such as balance, throw/hit, and vertical and horizontal jump.

AIM: To compare the effectiveness of two xenografts for maxillary sinus floor augmentation in terms of clinical, radiographical, histologic, and molecular outcomes. MATERIALS AND METHODS: A split-mouth randomized clinical trial was conducted at the University of Granada. Ten consecutive patients in need of bilateral two-staged maxillary sinus floor augmentation were included. Each patient received both biomaterials (porcine bone mineral and anorganic bovine bone), which were randomly assigned for bilateral sinus augmentation. The maxillary autogenous bone scraped from the sinus access window was mixed with each xenograft at a 20:80 ratio. After a healing period of 6 months, bone biopsies were collected with a trephine during the implant placement in the regenerated area. Histologic, histomorphometrical, immunohistochemical, and molecular outcomes were analyzed. Clinical and radiographical data throughout the treatment phases were also evaluated. RESULTS: The resulting anatomic features were similar between both groups. After six months of graft consolidation, the graft resorption rates were similar between both biomaterials. The histologic, histomorphometrical, and immunohistochemical results showed no statistical differences between groups. CONCLUSION: Anorganic bovine bone and porcine bone mineral combined with maxillary autogenous cortical bone show similar biologic and radiologic features in terms of biomaterial resorption, osteoconduction, and osteogenesis when used for maxillary sinus floor augmentation.

BACKGROUND: Coronaviruses usually cause mild respiratory disease in humans but as seen recently, some human coronaviruses can cause more severe diseases, such as the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), the global spread of which has resulted in the ongoing coronavirus pandemic. RESULTS: In this study we analyzed the potential of using iron oxide nanoparticles (IONPs) coated with biocompatible molecules like dimercaptosuccinic acid (DMSA), 3-aminopropyl triethoxysilane (APS) or carboxydextran (FeraSpin™ R), as well as iron oxyhydroxide nanoparticles (IOHNPs) coated with sucrose (Venofer®), or iron salts (ferric ammonium citrate -FAC), to treat and/or prevent SARS-CoV-2 infection. At non-cytotoxic doses, IONPs and IOHNPs impaired virus replication and transcription, and the production of infectious viruses in vitro, either when the cells were treated prior to or after infection, although with different efficiencies. Moreover, our data suggest that SARS-CoV-2 infection affects the expression of genes involved in cellular iron metabolism. Furthermore, the treatment of cells with IONPs and IOHNPs affects oxidative stress and iron metabolism to different extents, likely influencing virus replication and production. Interestingly, some of the nanoparticles used in this work have already been approved for their use in humans as anti-anemic treatments, such as the IOHNP Venofer®, and as contrast agents for magnetic resonance imaging in small animals like mice, such as the FeraSpin™ R IONP. CONCLUSIONS: Therefore, our results suggest that IONPs and IOHNPs may be repurposed to be used as prophylactic or therapeutic treatments in order to combat SARS-CoV-2 infection.

Throwing velocity is one of the most important factors for scoring goals in handball. This study aimed to identify the type of throw and procedure for selecting the final test outcome that provide throwing velocity with the greatest reliability. Fifteen experienced handball players and 33 non-experienced participants were tested in two sessions. Each session consisted of 4 trials of 3 different throwing tests (unspecific, 7-meters, and 3-steps). The maximum value of 4 trials, average value of 4 trials, and average value of the 3 best trials were considered. Throwing velocity was highly reliable (coefficient of variation [CV]≤3.3%, intraclass correlation coefficient≥0.89) with the exception of the unspecific throw for the non-experienced group (CV≥5.9%, intraclass correlation coefficient≤0.56). The 3-steps throw (CV=1.7%) was more reliable than the 7-meters throw (CV=2.1%) (CVratio=1.19) and unspecific throw (CV = 3.8%) (CVratio=2.18), the 3 procedures provided a comparable reliability (CV range=2.4-2.6%; CVratio≤1.07), and the experienced group (CV=1.0%) presented a higher reliability than the non-experienced group (CV=4.0%) (CVratio=3.83). These results support the 3-steps throw to maximise the reliability of throwing velocity performance.

ABSTRACT

Musashi-1 (MSI1) is an RNA-binding protein that regulates progenitor cells in adult and developing organisms to maintain self-renewal capacities. The role of musashi-1 in the bone healing environment and its relation with other osteogenic factors is unknown. In the current study, we analyze the expression of MSI1 in an experimental model of rat femoral bone fractures. We also analyze the relation between MSI1 expression and the expression of two osteogenic markers: periostin (POSTN) and runt-related transcription factor 2 (RUNX2). We use histological, immunohistochemical, and qPCR techniques to evaluate bone healing and the expression of MSI1, POSTN, and RUNX2 over time (4, 7, and 14 days). We compare our findings with non-fractured controls. We find that in bone calluses, the number of cells expressing MSI1 and RUNX2 increase over time and the intensity of POSTN expression decreases over time. Within bone calluses, we find the presence of MSI1 expression in mesenchymal stromal cells, osteoblasts, and osteocytes but not in hypertrophic chondrocytes. After 14 days, the expression of MSI1, POSTN, and RUNX2 was significantly correlated. Thus, we conclude that musashi-1 potentially serves in the osteogenic differentiation of mesenchymal stromal cells and bone healing. Therefore, further studies are needed to determine the possibility of musashi-1's role as a clinical biomarker of bone healing and therapeutic agent for bone regeneration.

ABSTRACT

We propose the use of the 1-minute sit-to-stand test (1STST) to evaluate the physical capacity and exertional desaturation one month after discharge in a sample of patients who survived COVID-19 pneumonia. This was a cross-sectional study that collected routine data from consecutive patients admitted to the outpatient program in a public hospital in Chile. Patients were asked to complete a 1STST. Data were analyzed according to those with and without a prolonged hospital stay of >10 days. Eighty-three percent of the patients were able to complete the test (N = 50). The median age was 62.7 ± 12.5 years. The average number of repetitions in the 1STST was 20.9 ± 4.8. Thirty-two percent of patients had a decrease in pulse oxygen saturation (SpO2) ≥ 4 points. The prolonged hospital stay subgroup had a significant increase in exertional desaturation (mean difference = 2.6; 95% CI = 1.2 to 3.9; p = 0.001) and dyspnea (mean difference = 1.1; 95% CI = 0.4 to 2.1; p = 0.042) compared to the group of length of stay ≤10 days. In-hospital survivors of COVID-19, the 1STST showed a decrease in physical capacity at one month in those 90% who were able to complete it. The 1STST was able to discriminate between those with and without a prolonged hospital stay and was able to detect exertional desaturation in some patients.

This study examined the reliability and validity of three methods of estimating the one-repetition maximum (1RM) during the free-weight prone bench pull exercise. Twenty-six men (22 rowers and four weightlifters) performed an incremental loading test until reaching their 1RM, followed by a set of repetitions-to-failure. Eighteen participants were re-tested to conduct the reliability analysis. The 1RM was estimated through the lifts-to-failure equations proposed by Lombardi and O'Connor, general load-velocity (L-V) relationships proposed by Sánchez-Medina and Loturco and the individual L-V relationships modelled using four (multiple-point method) or only two loads (two-point method). The direct method provided the highest reliability (coefficient of variation [CV] = 2.45% and intraclass correlation coefficient [ICC] = 0.97), followed by the Lombardi's equation (CV = 3.44% and ICC = 0.94), and no meaningful differences were observed between the remaining methods (CV range = 4.95-6.89% and ICC range = 0.81-0.91). The lifts-to-failure equations overestimated the 1RM (3.43-4.08%), the general L-V relationship proposed by Sánchez-Medina underestimated the 1RM (-3.77%), and no significant differences were observed for the remaining prediction methods (-0.40-0.86%). The individual L-V relationship could be recommended as the most accurate method for predicting the 1RM during the free-weight prone bench pull exercise.

BACKGROUND: Musashi-1 (MSI1) is a negative regulator of mesenchymal stromal cell (MSC) differentiation which in turn favors cell proliferation. However, little is known about its expression by MSC from the oral cavity and in the context of osteogenic differentiation. AIM: The aim of this study was to analyze the expression of MSI1 in the context of osteogenic differentiation of MSC derived from the oral cavity. MATERIAL/METHODS: For this in vitro study, MSC were isolated from six different origins of the oral cavity. They were extensively characterized in terms of proliferative and clonogenicity potential, expression of stemness genes (MYC, NANOG, POU5F1, and SOX2), expression of surface markers (CD73, CD90, CD105, CD14, CD31, CD34, and CD45) and adipo-, chondro- and osteogenic differentiation potential. Then, osteogenic differentiation was induced and the expression of MSI1 mRNA and other relevant markers of osteogenic differentiation, including RUNX2 and Periostin, were also evaluated. RESULTS: Cell populations from the alveolar bone (pristine or previously grafted with xenograft), dental follicle, dental germ, dental pulp, and periodontal ligament were obtained. The analysis of proliferative and clonogenicity potential, expression of the stemness genes, expression of surface markers, and differentiation potential showed similar characteristics to those of previously published MSC from the umbilical cord. Under osteogenic differentiation conditions, all MSC populations formed calcium deposits and expressed higher SPARC. Over time, the expression of MSI1 followed different patterns for the different MSC populations. It was not significantly different than the expression of RUNX2. In contrast, the expression of MSI1 and POSTN and RUNX2 were statistically different in most MSC populations. CONCLUSION: In the current study, a similar expression pattern of MSI1 and RUNX2 during in vitro osteogenic differentiation was identified.

ABSTRACT

OBJECTIVES: To validate in Spanish (Spain) a self-administered questionnaire on the quality of life (QoL) specific to nocturia in an older adult male population. MATERIAL AND METHODS: We analysed the Spanish version of the ICIQ-Nqol questionnaire (provided by ICIQ who owns the copyright), which meets the translation criteria required by the validation protocol. The study included 69 male patients older than 60 years of age with 1 or more nocturia episodes. The participants completed the questionnaire on two occasions, along with other Spanish-validated questionnaires that measure the QoL related to urinary symptoms. We analysed the internal reliability and consistency, reproducibility, convergent validity and discriminant validity. RESULTS: The study included 69 men, with a mean age of 70.64 years (range, 60-87 years). Both the overall scale and the energy/sleep subscale met the minimum criteria for internal reliability and consistency. A good correlation has been demonstrated between the overall index of the ICIQ-Nqol questionnaire and the other questionnaires included in the study. The reproducibility (test-retest reliability) of both the overall scale and subscales of the ICIQ-Nqol and the other questionnaires was demonstrated. Conclusions The ICIQ-Nqol questionnaire has been shown to be an appropriate instrument for assessing the symptomatic intensity and impact on the QoL of patients with nocturia. ABSTRACT

A simple high-power, high-brightness Nd:YAG solar laser pumping approach is presented in this paper. The incoming solar radiation is both collected and concentrated by four Fresnel lenses and redirected toward a Nd:YAG laser head by four plane-folding mirrors. A fused-silica secondary concentrator is used to compress the highly concentrated solar radiation to a laser rod. Optimum pumping conditions and laser resonator parameters are found through ZEMAX and LASCAD numerical analysis. Solar laser power of 96 W is numerically calculated, corresponding to the collection efficiency of 24 W/m². A record-high solar laser beam brightness figure of merit of 9.6 W is numerically achieved.

OBJECTIVES: Blood cultures are ordered in emergency departments for 15% of patients with suspected infection. The diagnostic yield varies from 2% to 20%. Thirty-day mortality in patients with bacteremia is high, doubling or tripling the rate in patients with the same infection but without bacteremia. Thus, finding an effective model to predict bacteremia that is applicable in emergency departments is an important goal. Shapiro's model is the one traditionally used as a reference internationally. The aim of this systematic review was to compare the predictive power of bacteremia risk models published since 2008, when Shapiro's model first appeared. MATERIAL AND METHODS: We followed the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement, searching in the following databases for articles published between January 2008 and May 31, 2023: PubMed, Web of Science, EMBASE, Lilacs, Cochrane, Epistemonikos, Trip Medical Database, and ClinicalTrials.gov. No language restrictions were specified. The search terms were the following Medical Subject Headings: bacteremia/bacteraemia/blood stream infection, prediction model/clinical prediction rule/risk prediction model, emergencies/emergency/emergency department, and adults. Observational cohort studies analyzing diagnostic yield were included; case-control studies, narrative reviews, and other types of articles were excluded. The Newcastle-Ottawa Scale was used to score quality and risk of bias in the included studies. The results were compared descriptively, without meta-analysis. The protocol was included in the PROSPERO register (CRD42023426327). RESULTS: Twenty studies out of a total of 917 were found to meet the inclusion criteria. The included studies together analyzed 33 182 blood cultures, which detected 5074 cases of bacteremia (15.3%). Eleven studies were of high quality, 7 of moderate quality, and 2 of low quality. The area under the receiver operating characteristic curve (AUC) of Shapiro's model varied from 0.71 to 0.83. Sensitivity was as high as 98%, and specificity ranged from 26% to 69%. Three models with high scores for quality were also supported by both internal and external validation studies: Lee's model (AUC, 0.81; sensitivity 68%; specificity, 81%), the 5MPB-Toledo model (AUC, 0.906 to 0.946), and the MPB-INFURG-SEMES model (AUC, 0.924; sensitivity, 97%; specificity, 76%. CONCLUSION: The 5MPB-Toledo and MPB-INFURG-SEMES are useful for assessing the true risk of bacteremia in patients attended in emergency departments.


OBJETIVO: La obtención de hemocultivos (HC) se realiza en el 15% de los pacientes atendidos con sospecha de infección en los servicios de urgencias (SU) con una rentabilidad diagnóstica variable (2-20%). La mortalidad a 30 días de estos pacientes con bacteriemia es elevada, doble o triple que el resto con el mismo proceso. Así, encontrar un modelo predictivo de bacteriemia eficaz y aplicable en los SU sería muy importante. Clásicamente, el modelo de Shapiro ha sido la referencia en todo el mundo. El objetivo de esta revisión sistemática (RS) es comparar la capacidad para predecir bacteriemia en los SU de los distintos modelos predictivos publicados desde el año 2008 (fecha de publicación del modelo de Shapiro). METODO: Se realiza una RS siguiendo la normativa PRISMA en las bases de datos de PubMed, Web of Science, EMBASE, Lilacs, Cochrane, Epistemonikos, Tripdatabase y ClinicalTrials.gov desde enero de 2008 hasta 31 mayo 2023 sin restricción de idiomas y utilizando una combinación de términos MESH: "Bacteremia/Bacteraemia/Blood Stream Infection", "Prediction Model/Clinical Prediction Rule/Risk Prediction Model", "Emergencies/Emergency/Emergency Department" y "Adults". Se incluyeron estudios de cohortes observacionales (analíticos de rendimiento diagnóstico). Para valorar la calidad del método empleado y el riesgo de sesgos de los artículos incluidos se utilizó la Newcastle-Ottawa Scale (NOS). No se incluyeron estudios de casos y controles, revisiones narrativas y en otros tipos de artículos. No se realizaron técnicas de metanálisis, pero los resultados se compararon narrativamente. El protocolo de la RS se registró en PROSPERO (CRD42023426327). RESULTADOS: Se identificaron 917 artículos y se analizaron finalmente 20 que cumplían los criterios de inclusión. Los estudios incluidos contienen 33.182 HC procesados con 5.074 bacteriemias (15,3%). Once estudios fueron calificados de calidad alta, 7 moderada y 2 baja. El ABC-COR conseguida por el modelo de Shapiro varía de 0,71 a 0,83, con sensibilidad (Se) hasta del 98%, con especificidad (Es) (26% a 69%). Para los tres modelos que tienen validación interna y externa y una buena calidad metodológica, el modelo de Lee consigue un ABC-COR de 0,81 con Se: 68% y Es: 81%, el modelo 5MPB-Toledo consigue un ABC-COR entre 0,91 y 0,95, y el MPB-INFURG-SEMES obtiene una ABC-COR de 0,92 con una Se: 97% y Es: 76%. CONCLUSIONES: Los modelos 5MPB-Toledo y MPB-INFURG-SEMES representan herramientas útiles para la estratificación del riesgo real de bacteriemia en los pacientes atendidos en los SU.

ABSTRACT

Background Wrist arthroscopy has become a commonly used tool for the management of adults with distal radius fractures (DRFs), although its implementation requires technical competence. This systematic review and meta-analysis appraised the available evidence concerning the contribution of wrist arthroscopy to the functional and radiological outcomes of patients with DRFs operated with arthroscopic assistance. Methods Randomized control trials were identified through five principal electronic databases on May 31, 2022: Web of Science, Scopus, EBSCO, Embase, and PubMed. Two authors independently performed the search and data extraction and assessed risk of bias (RoB) using the Cochrane RoB tool. Results A total of 1,780 relevant abstracts and citations were extracted in the preliminary search, which yielded 6 trials that met the eligibility criteria, and 3 studies were included in the quantitative synthesis. The overall pooled mean difference (MD) estimate showed no significant difference in the Disabilities of the Arm, Shoulder, and Hand scores between surgical groups with and without arthroscopic assistance (MD = 0.77 points, 95% confidence interval = -6.58 to 5.03, p = 0.79), with substantial heterogeneity (I 2 = 85%). Conclusion There was low-quality evidence that wrist arthroscopy conveyed not clinically or statistically significant difference to functional outcomes of patients with DRFs. However, our findings are limited by the numbers of studies included and lack of long-term follow-up, although they do provide a good starting point for future quality research.

BACKGROUND: The strength of the trunk musculature plays an essential role in performance. The aims of this study were to analyze the relationship between the strength of the trunk rotation muscles with rotational medicine ball throwing velocity and distance, to study differences between sex in rotational medicine ball throwing and to compare the asymmetries of trunk rotational strength and rotational medicine ball throwing. METHODS: Thirty physically active college students participated on the study. Horizontal and low cable woodchop exercises were assessed with a functional electromechanical dynamometer and throwing was measured with a radar and a measuring tape. A Pearson's correlation coefficient was calculated for the relation between trunk rotational strength and rotational medicine ball throwing. Independent samples t-test was performed to determine the differences between sex in rotational medicine ball throwing and a paired samples t-test was performed to study the asymmetries. RESULTS: Large and very large correlations were obtained between trunk rotational strength and rotational medicine ball throwing (r=0.68-0.79). Significant differences and a large effect size were found between males and females for all throwing velocity and throwing distance variables (P<0.05; ES>0.90) and asymmetries in performance of mean velocity throwing (P=0.003; ES=-0.60) and peak velocity throwing (P=0.025; ES=-0.43). No significant differences were found between dominant and non-dominant side in any of the trunk rotator strength assessment conditions (P>0.05). CONCLUSIONS: There is a large or very large correlation between the STRM and the RMBT in all its modalities. Differences in strength were found between sexes, with no significant differences between dominant and non-dominant sides.
